Peculiarities of the clinical course and complex therapy of peritonitis after cesarean section

We observed 23 women who developed peritonitis after cesarean section. The average age of the patients was 28.9 years. There were 7 first-pregnant women and 16 second-pregnant women. The anamnesis revealed the presence of endogenous foci of infection in all the patients. Thus, flu, sore throat, acute respiratory infections were diagnosed in 20 women, pneumonia in 3; chronic pyelonephritis in 5, inflammatory uterine appendages in 6, rheumatism with mitral valve insufficiency in 1, Botkin disease in 1. Genuine pregnancy was preceded by abortions in 19 women, of whom 11 had formal abortions, 3 had criminal abortions, and 5 had spontaneous abortions at 10 to 25 weeks' gestation against a background of chronic pyelonephritis and inflammatory diseases of the uterine appendages.

Respiratory diseases persistently lead in the structure of general morbidity in both children and adolescents. Acute respiratory infections are the most common among them. Interest of pediatricians to phytotherapy is not accidental and is due to the fact that it has a mild therapeutic effect, enhances the effectiveness of complex therapy, has few side effects. Of particular interest are herbal remedies containing ivy extract (Hedera helix), the active ingredients are mainly found in ivy leaves, they are biologically active substances: Triterpene saponins, glycosides, also contain carbohydrates, essential oils, steroids, phenolcarboxylic acids, coumarins, flavonoids, carotenoids, tocopherols (vitamin E), B and C vitamins, tannins, phytoncides. Ivy preparations are prescribed as an expectorant in the treatment of infectious and inflammatory diseases of the upper and lower respiratory tract due to their proven multidirectional effects: secretolytic, mucolytic, bronchospasmolytic. They also have anti-inflammatory, reparative, antioxidant effects.This article presents a clinical example of an officinal ivy-based preparation used in the treatment of a 3.5 year old patient diagnosed with acute respiratory viral infection. Temperature increased to 38 °C, runny nose; on the second day of illness the temperature remained subfebrile and a dry cough appeared. Nasal vasoconstrictor drops and oral acetylcysteine were prescribed, but there was no relief. On the third day of the illness, against the background of subfebrile temperature, the cough persisted, single dry rales were heard in the lungs, wheezing appeared during physical exertion. Acetylcysteine was withdrawn and replaced by ivy-based drops (16 drops 3 times a day orally after meals in a small amount of water). After 2 days the cough was moist, the sputum was easily expectorated, the wheezing in the lungs was almost gone, there was no more wheezing and the temperature was normal. By the sixth day of treatment the cough was gone and the girl had almost recovered. Nevertheless, Gedelix was continued for another 4 days. Prospective results and detailed study of the effects of herbal preparations on the basis of ivy extracts served as the basis for their recommendation as a complex therapy of cough in children.

Annotation. The aim of this work is to analyze the course of pregnancy in women with mitral pathology, the task is to identify preclinical criteria for worsening cardiovascular pathology that can lead to the development of complications of pregnancy and functioning of the fetal CVS, to evaluate the effectiveness of preventive measures. The article presents the analysis of changes in ultrasound characteristics of fetoplacental blood flow in women with mitral valve prolapse and mitral valve insufficiency, as well as cardiovascular status in these women. Statistical processing was performed by calculating Student coefficients (+) using the Microsoft Excel PC program. Doppler examination of fetal umbilical cord blood flow during pregnancy was studied. The correlation between the level of maternal blood flow compensation and pathological changes in the fetoplacental complex was established. As a result of our work, the prevalence of the disease was estimated. As of 2018, it was found that of the total number of extragenital pathology in 1676 pregnant women with CVSD; respectively for 2017 — 1699, for 2016 — 1925, 2015 — 1857 in the region. Of these, 331 (2018), 313 (2017), 326 (2016), 306 (2015) had mitral valve defects, including prolapse. A retrospective assessment of pregnancy and childbirth indicates the possibility of complications of pregnancy and childbirth. In the group of patients with mitral valve prolapse and mitral valve insufficiency, compared to the control group, they did not show significant changes in the dynamics of the Doppler metrics of cardiovascular pathology, which indicates the compensation of the cardiovascular system against the background of changes in the valve. At the same time, the tendency to decrease of CF in women with mitral valve prolapse and insufficiency of the mitral valve against the background of increasing pregnancy is revealed, indicates a violation of CVD adaptation to changes inherent in pregnancy, which provoke hemic and hemodynamic changes in the body, which will cause pregnancy and fetal status. FPC status in women with mitral valve prolapse and mitral valve insufficiency worsens in the third trimester of pregnancy, especially indicators such as the ratio of systolic volume ratio (SVR) to diastolic volume ratio (DVR) and IR. The administration of 40 mg of ƅ-blockers for a 2-week period improved the FPC, was safe and effective.

In obstetric practice, the search for reserves to increase the effectiveness of prevention of gestational complications associated with infectious and inflammatory diseases of the urinaiy system is relevant. The purpose of this study was to assess the results of using the Canephron® N herbal medicine in pregnant women with chronic pyelonephritis. The study included 100 women, randomly divided into four groups, three of which were carried out for prophylactic use of the drug Canephron®N, and were not prescribed phytoprophylaxis in the fourth. It has been established that monotherapy with Canephron®N reduces the risk of exacerbations of chronic pyelonephritis, improves the functional state of the urinary tract, reduces the incidence of bacteriuria. Optimal results are achieved with the use of prophylactic courses of Canephron®N at the gestational period of 10-12, 22-24, and 34-36 weeks.

The clinical features of the course of norovirus infection at various terms ofpregnancy in 65 examined women are presented in the article. In all patients moderate forms of disease prevailed. A characteristic of the clinical course, its peculiarities revealed in gravida, and complex therapy of norovirus infection are described.

The objective: optimization of diagnostics and tactics of conducting pregnancy at women with benign tumors and tumorous formations of ovaries by improvement of diagnostic and treatment-and-prophylactic actions. Patients and methods. 129 patients in various durations of gestation from which 104 were with benign tumors and tumorous formations of ovaries were surveyed. Results. The clinical course of pregnancy at course of pregnancy at women with benign tumors and tumorous formations of ovaries is characterized by the high frequency of threat of discontinuing in І (49.7%) that ІІ trimesters (50.8%). Level of other complications depends on maintaining tactics, and it considerably vichy at refusal of women of expeditious treatment and their conservative maintaining: placental dysfunction – 44.0% and 16.2%; fetus distress – 30.0% and 6.7%; delivery by operation of cesarean section – 33.3% and 10.8%; disturbances of post-natal adaptation of newborns – 9.5% and 6.8% respectively. Conclusion. The improved and introduced algorithm of maintaining women with benign tumors and tumorous formations of ovaries allows to improve obstetric and perinatal outcomes of delivery of women of group of high risk. There are presented data of the analysis of the composition of the oropharynx microbiota and antimicrobial resistance of opportunistic bacteria isolated in sickly children to determine the effectiveness of the preparation Miramistin. There was executed a bacteriological study of the microflora of the oropharynx in 71 children with respiratory recurrent infections (RIRs). There was determined the resistance of isolated species ofpathogens of respiratory infections to antibiotics of different classes. Children suffering from RIRs were found to have species and and quantitative disorders of the pattern ofpharynx microflora in 90,2% of cases. In cultures from oropharyngeal mucosa there were isolated 15 types of potentially pathogenic bacteria, among them there were dominated: S. aureus (31%), Haemophilus influenzae (11,3%),) Strept. pyogenes (11,3%) and Strept. pneumoniae (8,4%). All of the isolated in sickly children strains of S. aureus and Strept. pyogenes (including strains resistant to antibiotics and bacteriophages) were susceptible to Miramistin. There was proved high efficiency and safety of Miramistin in the complex therapy of various forms of inflammatory diseases oa upper respiratory tract: angina, chronic tonsillitis, pharyngitis and nasopharyngitis in children. It is recommended to always include Miramistin in the complex treatment of infectious and inflammatory diseases of the oropharynx.

Background: Cardiovascular complications that can cause severe catastrophic outcomes for both the mother and the fetus are rarely seen during pregnancy. Time to diagnosis is often delayed by a low degree of suspicion and atypical presentation. We report surgical strategies in three pregnant women with cardiovascular complications. Methods: A retrospective search from 2009 to 2016 identified three pregnant women who underwent urgent cardiac surgery. We used extracorporeal circulation (ECC) without cesarean section with careful follow-up of the fetuses during the perioperative and postoperative period. We used levosimendan as a potent inodilator in all patients to increase feto-placental blood flow and fetal heart rhythm.Results: Median time to diagnosis was 23.8 h (range 11.7-120 h) and median time from diagnosis to arrival in the operating theater was 9.8 h (range 7.4-19.8 h). One patient with prosthetic heart valve thrombosis underwent concomitant cesarean section prior to cardiac surgery. In a young pregnant woman who had spontaneous dissection of the left anterior descending artery, on-pump beating heart coronary artery bypass grafting was performed without cross clamping. Two and three months after surgery, cesarean sections were performed without any complication in two pregnant women.Conclusion: Because unusual cardiovascular complications are the main cause of maternal and/or fetus death during pregnancy, prompt and exact diagnosis is very important. Life-saving surgical strategy with the help of appropriate teams are necessary to optimize outcome for both mother and baby.

Several techniques are currently in use for mitral valve reconstruction. We report a mitral repair case in which the use of a combination of different surgical techniques resulted in the necessary correction. A 47-year-old woman underwent surgical intervention to treat severe mitral valve insufficiency due to A1/A2/A3 and P2 prolapsed valve tissue. A combination of quadrangular resection, sliding leaflet, single chordal transposition, "flip-over" leaflet, and ring annuloplasty techniques were applied, and postsurgical correct valve function was documented by results of a left ventricular saline filling test and transesophageal echocardiography control. Complex mitral valve repairing techniques can be combined to reestablish valvular function.
