Glaucoma is a socially sensitive disease, being one of the leading causes of irreversible visual impairment and blindness. Refractory glaucoma is one of the most severe forms of the disease as it is resistant to conservative and surgical methods of treatment. Because of pronounced postoperative fibroblastic activity of eye tissues, leading to gross scarring and obliteration of outflow paths, modern materials and methods of drainage surgery are needed. The review presents a variety of drains used today and considers in detail their main characteristics and the biocompatibility of the material used with eye tissues. Various groups of implants are presented, whose effectiveness and safety are compared and assessed. The statistics of postoperative complications and long-term results of surgical treatment are given.

Objective: a comparative analysis of the results of the use of different surgical methods of treatment of patients with formed pancreatic cysts.Material and methods. 108 patients with formed pancreatic cysts were treated and analysis of shortand long-term results of their surgical treatment was performed. Patients were divided into three groups depending on the type of surgical intervention: I – external drainage – 44 (40.7%), II – internal drainage –33 (30.6%), III – resection operations – 31 (28.7%).Results and discussions. Marsupialization of cyst by laparotomy incision was performed in patients of I group (n = 44). 18 (40.9%) complications, 9 (20.5%) lethal cases were after operation. Anastomoses of cysts with the small intestine were mostly performed in II group (n = 33) – 21 (63.6%). 7 (21.2%) complications, 1 (3.0%) lethal case were after operation. Distal resections were performed in patients of III group (n = 31) in 16 (51.6%) cases. Duodenum-preserving resections were introduced for treatment of cyst of pancreas head – 12 (38.7%). When performing this type of operations we proposed nikelid titanium stents for the prevention of anastomosis stenosis and preoperative retrograde stenting of the common bile duct for the prevention of damage. 10 (32.3%) complications and no lethal cases were after operation. Immediate results were worse in patients of I group. 47 (43.5%) patients were analyzed in long-term period. The number of recurrences of the disease (13.3%) and long-term mortality (33.3%) prevailed in the group of patients undergoing internal drainage of cysts. Quality of life, level of mental and physical health, that was assessed using SF-36, were higher in group of patients with reactionary treatment.Conclusion. The best immediate and long-term results were noted after resection operations, that enables to recommend their as the most optimal and radical method for treatment of patients with pancreatic cysts. Introducing of duodenum-preserving resections in case of pancreas head cyst improves the quality of patients life.

The treatment of inflammatory diseases of the female genitalia has long been one of the most important tasks of the gynecologist. With the development of surgery, surgical methods of treatment began to be applied: exudates were removed, the uterus and inflamed appendages were extirpated, etc.; but the danger of surgical intervention on the one hand, and on the other - the severe consequences of removal of organs such as ovaries, especially in young women - forced gynecologists to spend much effort in discovering and improving non-operative methods of treatment of inflammatory diseases of the female genital parts. Among these methods, physical methods such as water, mud, light baths, massage, etc., as well as those related to electricity have long been prominent.

Objective. To increase efficiency of cavitary pulmonary tuberculosis treatment by developing modern indications and technology of extrapleural pneumolysis with filling material.Material and methods. We reviewed 25 patients with cavitary pulmonary tuberculosis and bacterial excretion who had undergone extrapleural pneumolysis with different types of filling material in 2004–2015. In 16 cases, we performed surgery in patients with one lung.Results. 56% of operated patients demonstrated extensively drug-resistant TB, 40% had multidrug-resistant TB. Cavities were closed and bacterial excretion ceased in 92% of patients at discharge. Long-term results with observation period from 1 to 12 years were analyzed in 24 patients, 17 (71%) of them had complete clinical efficacy.Conclusion. Extrapleural pneumolysis is a mini-invasive surgical treatment that can prove positive effect in patients with non-fibrotic changes in a cavity wall and pathological process in upper parts of the lungs.

Object The goal of this report was to describe the safety and effectiveness of nucleus ventralis intermedius (VIM) thalamotomy performed with the Leksell Gamma Knife (GK) for the treatment of essential tremor (ET). Methods One hundred seventy-two patients underwent a total of 214 VIM thalamotomy procedures with the Leksell GK between February 1994 and March 2007 for treatment of disabling ET. Eleven patients were lost to follow-up less than 1 year after the procedures, so that in this report the authors describe the results in 161 patients who underwent a total of 203 thalamotomies (119 unilateral and 42 bilateral). Results There were statistically significant decreases (p < 0.0001) in tremor scores for both writing and drawing. The mean postoperative follow-up duration for all patients was 44 ± 33 months. Fifty-four patients have been followed for more than 60 months posttreatment. There were 14 patients who suffered neurological side effects that were temporary (6) or permanent (8), which accounted for 6.9% of the 203 treatments. All complications were related to lesions that grew larger than expected. Conclusions A VIM thalamotomy with the Leksell GK offers a safe and effective alternative for surgical treatment of ET. It is particularly applicable to patients who are not ideal candidates for deep brain stimulation but can be offered to all patients who are considering surgical intervention for ET.

The introduction of sex therapy two decades ago was accompanied by largely uncritical enthusiasm, with the result that too few careful evaluative studies were conducted. Those that were indicated that a weekly or twice weekly schedule of treatment sessions was best and that treatment by individual therapists was as effective as co-therapy. Some of the major prognostic factors and the long-term results of sex therapy have now been elucidated. Low sexual desire has emerged as a problem for which our now traditional methods of treatment are often inadequate and new therapeutic approaches are required. Current efforts to explore the beliefs and cognitive processes associated with erectile dysfunction are proving rewarding and are likely to enrich therapeutic interventions in the future. Attention should now be paid to the beliefs and cognitions associated with other sexual dysfunctions, both male and female.

Experience in surgical treatment of 112 patients with malunited fractures of distal radius epimetaphysis is presented. Indication to surgical intervention was fragments consolidation with displacement that was accompanied by marked wrist joint deformity and hand function disturbance. In all patients osteotomy and bone plasty were performed followed by application of distraction device. In the postoperative period dosed distraction of bone fragments up to their complete reposition and rehabilitation treatment was performed. Long term results were assessed for all 87 patients: good result was achieved in 67 (77%), satisfactory - in 17 (19.5%) and poor - in 3 (3.5%) patients.

Issues of locally advanced cancers which involve into the pathological process laryngopharynx and cervical part of esophagus remain to be actual till the present time. Unsatisfactory long-term results lead to the absence of common approaches and clear guidelines on the methods of the treatment choice (radiotherapy, chemoradiation therapy, surgery). Many researchers suppose that the foremost decision is to include surgical component into the treatment schemes of such patients. Main surgical method of the treatment is laryngopharyngectomy or laryngopharyngoesophagectomy. It depends on the extension of cancer. Different kinds of grafts (gastric, jejunal and colon) are used for esophageal reconstrution. The high frequency of complications after surgery, low survival rates are main motives for the search of effective methods of treatment.
