Currently, there are still no perfect clinical methods for early diagnosis of miscarriage, as a result of which the final diagnosis is usually made only after long-term observation of patients. The disappearance of reliable signs of pregnancy does not always solve the problem. The difficulty of diagnosis lies in differentiating a miscarriage from a normal pregnancy, from a threatening and incipient miscarriage.

“Hridorogo Hridrogaha" means the impairment in the function of the heart is known as Hridroga. Cardiovascular diseases are the most common diseases which affect the circulatory system including the heart and its vasculature. Darshana, Sparshana and Prashna Pariksha holds relevance in the current clinical methods like history taking, general examination and systemic examination. Trividha pariksha helps to attain an appropriate knowledge regard- ing the Roga and Rogavastha more accurately in Hridroga. The prevailing diagnostic techniques like haematolog- ical, biochemical, invasive, and non-invasive methods aids up in the early diagnosis of cardiovascular diseases. The diagnostic approach in a cardiac patient assists to improve the effectiveness of treatment and avoiding long term complications. The possibilities for diagnosing prion diseases have shifted significantly over the last 10 years. The RT-QuIC assay option has been added for neuropsychiatric symptoms, supporting biomarkers and final post-mortem confirmation. Samples of brain homogenates used for final diagnosis, archived for many years, provide the possibility for retrospective studies. We used a second-generation RT-QuIC assay to detect seeding activity in different types of sporadic and genetic prion diseases in archival brain homogenates and post-mortem CSF samples that were 2 to 15 years old. Together, we tested 92 archival brain homogenates: 39 with definite prion disease, 28 with definite other neurological disease, and 25 with no signs of neurological disorders. The sensitivity and specificity of the assay were 97.4% and 100%, respectively. Differences were observed in gCJD E200K, compared to the sporadic CJD group. In 52 post-mortem CSF samples—24 with definite prion disease and 28 controls—we detected the inhibition of seeding reaction due to high protein content. Diluting the samples eliminated such inhibition and led to 95.8% sensitivity and 100% specificity of the assay. In conclusion, we proved the reliability of archived brain homogenates and post-mortem CSF samples for retrospective analysis by RT-QuIC after long-term storage, without changed reactivity.

Lung hepatoid adenocarcinoma (HAC) is a rare primary lung carcinoma pathologically characterized by hepatocellular carcinoma-like tumor cells, the majority of which produce alpha-fetoprotein (AFP). The clinical prognosis of lung HAC is generally poor, and effective therapeutic regimens for inoperable or recurrent cases have not been established. Here, we report a case of AFP-producing lung HAC with brain metastasis with long-term disease control, treated with the 5-fluorouracil-derived regimen S-1. The patient was a 66-year-old male admitted to the hospital with alexia. Chest X-ray revealed a massive tumor in the left upper lobe, and a head CT scan revealed a metastasis in the left parietal lobe. The laboratory data showed a remarkably elevated AFP level (97,561 ng/mL). Pathological assessment of the resected brain tumor revealed HAC, which was compatible with the lung biopsies. Together with the absence of other metastatic lesions, a final diagnosis of primary lung HAC, stage IV T4N3M1b, was given. The patient first underwent non-small cell lung cancer chemotherapy regimens (carboplatin and paclitaxel as the first line, and pemetrexed as the second line), but had clinical progression. After third-line oral S-1 (tegafur/gimeracil/oteracil) administration, the serum AFP level significantly dropped and the patient achieved long-term disease control without relapse, surviving more than 19 months after disease presentation. The autopsy result was consistent with the diagnosis of primary lung HAC, and immunohistochemical staining was AFP+, glypican 3+, and spalt-like transcription factor 4+. Here, we report the case of a rare primary lung HAC with apparent disease control on S-1 therapy, together with a literature review.

DBA/2 mice are a well-known animal model for hearing loss developed due to intrinsic properties of these animals. However, results on the phenotype of hearing loss in DBA/2 mice have been mainly reported at an early stage in mice aged ≤7 weeks. Instead, the present study evaluated the hearing ability at 5, 13, and 34 weeks of age using DBA/2korl mice. Auditory brainstem response test was performed at 8–32 KHz at 5, 13, and 34 weeks of age, and hearing loss was confirmed to be induced in a time-dependent manner. In addition, histopathological evaluation at the same age confirmed the morphological damage of the cochlea. The findings presented herein are the results of the long-term observation of the phenotype of hearing loss in DBA/2 mice and can be useful in studies related to aging-dependent hearing loss.

We report a case of posterior uveal effusion (UE) with a long-term follow-up that has occurred following cataract surgery. A 64-year-old woman presented with diminished vision of the right eye (RE) 3 weeks after an uneventful phacoemulsification and intraocular lens implantation. Complete ophthalmic examination including fluorescein angiography (FA), indocyanine green angiography (ICGA), echography and optical coherence tomography (OCT) were performed. Best corrected visual acuity (BCVA) of the RE was 20/50. Anterior segment and intraocular pressure were unremarkable. OCT revealed prominent folds of the choroid and retina, subretinal fluid and darkening of the choroid with reduced visibility of the choroidal vessels and the scleral border. The left eye (LE) was unremarkable. BCVA of the LE was: 20/20. After topical anti-inflammatory and systemic corticosteroid therapy for 5 months, no morphological change of the macula was seen. The patient was observed without any treatment. Forty-three months after the cataract surgery and 38 months after cessation of the corticosteroid therapy, OCT revealed a normal macular morphology and the BCVA improved to 20/25. Even though rare, UE at the posterior pole may occur after modern cataract surgery. OCT examination is a reliable tool in monitoring the macular morphology. Since morphological and functional improvement can be seen in long-term, observation may be considered for some cases of posterior UE with resistance to the therapy.

It is an important to study atmospheric thermal and dynamic vertical structures over the Tibetan Plateau (TP) and their impact on precipitation by using long-term observation at representative stations. This study exhibits the observational facts of summer precipitation variation on subdiurnal scale and its atmospheric thermal and dynamic vertical structures over the TP with hourly precipitation and intensive soundings in Jiulong during 2013–2020. It is found that precipitation amount and frequency are low in the daytime and high in the nighttime, and hourly precipitation greater than 1 mm mostly occurs at nighttime. Weak precipitation during the daytime may be caused by air advection, and strong precipitation at nighttime may be closely related with air convection. Both humidity and wind speed profiles show obvious fluctuation when precipitation occurs, and the greater the precipitation intensity, the larger the fluctuation. Moreover, the fluctuation of wind speed is small in the morning, large at noon and largest at night, presenting a similar diurnal cycle to that of convective activity over the TP, which is conductive to nighttime precipitation. Additionally, the inverse layer is accompanied by the inverse humidity layer, and wind speed presents multi-peaks distribution in its vertical structure. Both of these are closely related with the underlying surface and topography of Jiulong. More studies on physical mechanism and numerical simulation are necessary for better understanding the atmospheric phenomenon over the TP.
