Adaptive capabilities under extreme conditions, depending on motivational orientation of Anesthesiologist

The aim is to identify the level of adaptive capacity anesthesiologists depending on the motivational orientation. Presented survey data 149 anesthesiologists. Testing was conducted on a multi-level personality questionnaire «Adaptability» and on the test Kuhl. The results showed that most physicians have high adaptive capacity, which is highly correlated with the motivational oriented under extreme conditions. The necessity of developing a program of psychological support to the development of non-motivational deficit.

Does civic participation lead to a large social network? This study claims that high levels of civic participation may obstruct individual social embeddedness. Using survey data from the German Survey on Volunteering (Deutscher Freiwilligensurvey; 1999–2009), this study conducts macro- as well as multi-level regressions to examine the link between civic participation and social embeddedness. Findings reveal that civic participation on the sub-national regional level is not generally associated with social embeddedness, but it affects the participants’ and non-participants’ possibilities for friendships differently. This holds especially true in urban areas, but the effect cannot be found in rural areas. The analysis has implications for further research to enhance the social embeddedness of the excluded.

The study is devoted to assessing the psychophysiological adaptive capabilities of students of higher medical institutions. There was conducted a screening study on the basis of which a correlation analysis of a group of 42 students was performed. The main inclusion criterion was good physical activity. Exclusion criteria - the presence of chronic diseases, acute respiratory viral infections at the time of examination, taking antidepressants or psychoactive substances. The study was aimed to identify the peculiarities of changes in psychophysiological functions and the state of adaptive capacity of students of higher medical institutions being in stressful conditions. The following methods from the software and hardware complex "Psycholot-1" were used for the study: "Functional mobility of nervous processes according to Khilchenko" and "Memory." The survey was conducted under micro-stress, it means all tasks had to be completed in a limited period. Statistical analysis of the data was performed to establish correlations between psychophysiological indicators of short-term memory and functional mobility of nervous processes  (Spearman's rank correlation index). It was found that the faster the stimulus appears in conditions of time deficit, the lower the mobility of nervous processes in the student, which is directly reflected in the indicators in increasing the minimum exposure time of the figure on the screen (0.27±0.007 s). In addition, a relationship was found between the rate of reaction of the left hand to a stimulus and the amount of information that a person can store in short-term memory. The information obtained will help to determine the initial data of the adaptive capacity of a potentially healthy contingent of people and predict their success in further professional activities in the sphere of medicine.



The biopsychosocial model of therapy for endogenous mental disorders involves a flexible combination of psychopharmacotherapy with psychosocial interventions. Psychoeducation is one of the most important components of psychosocial interventions in a multifaceted system of psychosocial rehabilitation. The primary task of psychoeducation is to provide patients and their family caregivers with realistic knowledge about mental disorder, on the basis of which the patient and his or her family members can get more control over the symptoms of the disease. The aim of our work was from the standpoint of a systematic approach, based on the study of clinical and psychological manifestations and risk factors for the development of pathological functioning in a family, where a patient with endogenous mental disorder lives, to develop, substantiate scientifically and introduce a psychoeducational module as an element of psychosocial interventions in complex system of medical -psychological support. To achieve this goal, according to the principles of bioethics and medical deontology, a comprehensive examination of 243 patients with endogenous mental disorders (168 patients with paranoid schizophrenia, 75 patients with affective disorders) and 243 family caregivers was performed. The work was done in three stages: during the the first stage we examined patients and their family caregivers. During the second stage, an in-depth study of psycho-emotional, individual-psychological, interpersonal-communicative and psychosocial predictors of reducing the adaptive capacity of the family was performed. The third stage included scientific substantiation, development and implementation of an appropriate comprehensive system of medical and psychological support for the families, based on the analysis of data obtained during the previous stages. The proposed system of medical and psychological support has shown its effectiveness in improving the adaptive capacity of the family, where patients with endogenous mental disorders live, and can be recommended for further implementation in appropriate medical institutions.

Abstract Background There is growing concern that routine mammography screening is overused among older women. Successful and equitable de-implementation of mammography will require a multi-level understanding of the factors contributing to mammography overuse. Methods This explanatory, sequential, mixed-methods study collected survey data (n = 52, 73.1% Hispanic, 73.1% Spanish-speaking) from women ≥ 70 years of age at the time of screening mammography, followed by semi-structured interviews with a subset of older women completing the survey (n = 19, 63.2% Hispanic, 63.2% Spanish-speaking) and providers (n = 5, 4 primary care, 1 obstetrics and gynecology) to better understand multi-level factors influencing mammography overuse and inform potential de-implementation strategies. We conducted descriptive analysis of survey data and content analysis of qualitative interview data. Survey and interview data were examined separately, compared, integrated, and organized according to Norton and Chambers Continuum of Factors Influencing De-Implementation Process. Results Survey findings show that 87.2% of older women believe it is important to plan for an annual mammogram, 80.8% received a provider recommendation, and 78.9% received a reminder in the last 12 months to schedule a mammogram. Per interviews with older women, the majority were unaware of or did not experience overuse and intended to continue mammography screening. Findings from interviews with older women and providers suggest that there are multiple opportunities for older women to obtain a mammogram. Per provider interviews, almost all reported that overuse was not viewed as a priority by the system or other providers. Providers also discussed that variation in mammography screening practices across providers, fear of malpractice, and monetary incentives may be reasons for overuse. Providers identified potential strategies including patient and provider education around harms of screening, leveraging the electronic health record to identify women who may no longer benefit from screening, customizing system-generated reminder letters, and organizing workgroups to develop a standard process of care around mammography screening. Conclusions Multi-level factors contributing to mammography overuse are dynamic and reinforced. To ensure equitable de-implementation, there is a need for more refined theories, models, and frameworks for de-implementation with a strong patient-level component that considers the interplay between multilevel factors and the larger care delivery process.

Based on the evaluation of the clinical course and the adaptive capabilities of the organism in the postabortion period after manual vacuum aspiration in 128 patients found that disorder of adaptation and the most frequent complications in the postabortion period observed in the production of abortion in the second phase of the suppositive menstrual cycle , especially in first time pregnant patients. In this way, the abortion in the first phase of "suppositive" menstrual cycle is the reserve for increasing the safety of abortion by MVA.

Skeleton-based person re-identification (Re-ID) is an emerging open topic providing great value for safety-critical applications. Existing methods typically extract hand-crafted features or model skeleton dynamics from the trajectory of body joints, while they rarely explore valuable relation information contained in body structure or motion. To fully explore body relations, we construct graphs to model human skeletons from different levels, and for the first time propose a Multi-level Graph encoding approach with Structural-Collaborative Relation learning (MG-SCR) to encode discriminative graph features for person Re-ID. Specifically, considering that structurally-connected body components are highly correlated in a skeleton, we first propose a multi-head structural relation layer to learn different relations of neighbor body-component nodes in graphs, which helps aggregate key correlative features for effective node representations. Second, inspired by the fact that body-component collaboration in walking usually carries recognizable patterns, we propose a cross-level collaborative relation layer to infer collaboration between different level components, so as to capture more discriminative skeleton graph features. Finally, to enhance graph dynamics encoding, we propose a novel self-supervised sparse sequential prediction task for model pre-training, which facilitates encoding high-level graph semantics for person Re-ID. MG-SCR outperforms state-of-the-art skeleton-based methods, and it achieves superior performance to many multi-modal methods that utilize extra RGB or depth features.
