From January 2003 to December 2017, the authors report five cases of acute intestinal obstruction during pregnancy. Two of them were received in digestive and proctological surgery emergencies. The other three patients were received urgently from the obstetrics and gynecology department. This association is a rare event. Clinical polymorphism delays the therapeutic management. In 1/5 cases only the diagnosis of occlusion on pregnancy was made. In 2/5 of the cases the diagnosis of occlusion was made and that of the pregnancy which at the 1st trimester was unknown. In last 2/5 of cases the pregnancies were at the 3rd trimester, the diagnosis of occlusion during pregnancy was made intraoperatively; Caesarean section being indicated for the fetal in distress on probable urinary tract infection. Authors did not have any maternal-fetal deaths.  Through five cases of this association and a review of the literature, authors highlight the diagnostic and therapeutic difficulties and the therapeutic attitudes likely to improve the feto-maternal prognosis. This fetal prognosis depends essentially on the age of the pregnancy and the speed of the management.

The direct inguinal hernia has a wider neck and thus usually doesn’t present as strangulation or incarceration in comparison to the indirect component. When direct inguinal hernias are untreated for a longer duration, they may get strangulated and incarcerated. Hence such long-standing direct hernias with features of intestinal obstruction and /or peritonism should be promptly seen and diagnosed to prevent massive and unwanted intestinal resection. We are reporting a case of 83-year-old male presented to Surgical Emergency Department of Dhulikhel Hospital, Kathmandu University hospital with complaints of swelling in the right inguinoscrotal region for 12 years and progressed to become irreducible and painful for 12 hours. Clinically he had an acute intestinal obstruction. Intra-operatively we found a direct hernia containing congested small bowel loops and toxic fluids. The toxic fluid was suctioned and after confirming viability, modified Bassini’s repair was done with reinforcement of the posterior wall. Even direct inguinal hernia of longer duration can cause acute or sub-acute intestinal obstruction with or without features of peritonism. This complication is more common in elderly patients.

Internal hernia is a relatively uncommon condition and is a rare type of intestinal obstruction. Paraduodenal hernia is considered the most common type of internal hernias. The rare prevalence and the variable symptoms make the clinical diagnosis of paraduodenal hernia a diagnostic challenge. We present the case of acute intestinal obstruction by left para-duodenal internal hernia treatedat the general surgery department of the Mohammed VI University Hospital Center in Marrakech, Morocco inorder to contribute to the knowledge of the clinical particularities of this entity.

Intussusception is a common finding in children accounting for approximately 25% of all abdominal emergencies in patients below five years of age. This, however, is a rare observation in adults accounting for only 5% of all intussusceptions. Among them, those leading to acute intestinal obstruction are about 1-5%. Diagnosis is based on radiographic findings seen on ultrasonography or CT scan as the presentation in adults is often difficult to diagnose clinically due to the variable presentation. We present here a case of intussusception in a 24 year old male presenting as acute intestinal obstruction. Exploratory laparotomy with resection of the bowel segment was done followed by end to end anastomosis. Post-operative course was uneventful.

Background: Uterine rupture is an obstetric emergency. It involves the life of the mother and the fetus. The main objective of this study was to describe the epidemiological-clinical profile of uterine rupture in Befelatanana Obstetrics and Gynecology University Hospital from January 2016 to April 2018.Methods: This is a retrospective and descriptive study conducted from January 1, 2016 to April 30, 2018 at the Befelatanana Hospital Centre of Gynecology-Obstetrics. All pregnant patients with uterine rupture were included.Results: We recorded 35 cases of uterine rupture in 13184 deliveries with a prevalence of 2.65%. The average age was 27.69±6.21 years old. More than half of the pregnant women attended at least four prenatal care consultations. Uterine rupture occurred after a vaginal delivery attempt outside the centre in 62.86%. There were 77.14% cases occurring on non-scarred uterus and 14.29% on mechanical dystocia. A case of fetal macrosomia has been reported. There were 28.57% maternal deaths and 51.43% perinatal deaths, half of which were fetal deaths in utero and the other half were early neonatal deaths.Conclusions: Uterine rupture remains high in Madagascar. Quality antenatal care and training of the personal would be needed.

Abstract Introduction: Ingestion of sharp objects is a rare event. We report our experience in the care of prisoners who ingested razor blades as well as the in-time evolution of our management. Material and Methods: This is a descriptive study including all detainees ingesting razor blade, transferred from the prison to Farhat Hached University Hospital of Sousse, from January first,2014 to December 31st ,2015. Thus, we have counted 19 cases of swallowed razor blades in 16 prisoners; indeed, one prisoner had ingested a razor blade thrice and another had ingested a blade twice. Results: Our study includes 19 episodes of swallowed razor blades. In two cases, these events were secondary to suicide attempts. In the other cases, it was an expression of discontent and rejection of the situation in the prison. Three patients had a psychiatric history. The mean age of patients is 24 years. The initial clinical examination had not objectified signs of gastrointestinal perforation. Thoraco-abdomino-pelvic CT had allowed specifying the blade location in all cases. It had objectified, also, a pneumo-mediastinum in two cases. The blade location was esophageal in one case, gastric in six cases and in the small bowel in the other cases. The first four patients had surgical removal of the blade. In subsequent cases, the surgical abstention was the rule, and the spontaneous blade expulsion was observed. The average time of expulsion was four days. Conclusion: In our experience, surgical removal of the sharp foreign bodies should not be performed systematically without complications.
