Background: Vitamin D deficiency during pregnancy and in newborn period is common in this country. Vitamin D status of the mother is known to influence the vitamin D levels in the neonate, however how closely the maternal vitamin D level correlates with the cord blood Vitamin D is not clearly understood. To study the correlation between maternal and neonatal serum Vitamin D3 levels by as indicated by cord blood 25(OH)D levels and find out if there is a significant variation of cord blood 25(OH)D levels in Vitamin D sufficient and insufficient mothers.Methods: Healthy pregnant women between 18-45 years of age with no known history of chronic disease or long-term medication, consenting for the study were enrolled. Maternal blood sample was collected in peripartum period, cord blood sample was obtained after delivery from the umbilical cord after clamping. Vitamin D3 levels were measured by RIA and paired maternal and cord blood levels were statistically analyzed.Results: 569 paired samples of maternal and cord blood were analyzed. The mean maternal serum 25(OH)D level was 35.63ng/ml (sd 6.18, range 9.2-39.8) as compared to 13.52ng/ml (sd 3.79, range 7.9-27) for the neonates. 457 of the mothers were found to have sufficient, 101(18%) insufficient and 11(2%) deficient Vitamin D levels as per Endocrinological Society guidelines. In comparison, 535(94%) of the neonates had deficient levels, none of the neonates had sufficient Vitamin D levels, 34(5.99%) had insufficient levels. No significant correlation was found between maternal and neonatal serum vitamin 25(OH)D levels (r=0.007, P=0.85).Conclusions: Maternal and Cord blood serum Vitamin D3 levels were found to be poorly correlated in this study.

Background: The association of serum vitamin D levels to clinical outcome in VLBW infants has not been studied. Our objective was to measure the cord blood levels, and the dose response for two doses of vitamin D in preterm infants and correlate the relationship of vitamin D levels to the clinical outcome.Methods: We prospectively obtained cord blood levels in 80 preterm infants under 34 weeks gestation (mean gestation age 29±2 weeks and BW: 1210±350 gms). Infants were supplemented with 400 IU or 800-1000 IU vitamin D daily. Serun vitamin D levels were obtained at 2 - 3 weeks after supplementation and levels were correlated to clinical outcome. Results: The mean cord blood vitamin D level was 12±8.5 ng/ml. Babies who developed sepsis and compared to those who did not develop these morbidities, ROP had vitamin D levels: 13.5±6 (ng/ml) versus 30.5±10 (ng/ml) (p < 0.01) and 15.7±11 (ng/ml) versus 34±18 (ng/ml) (p <0.03) respectively. Supplementation with 400 IU vitamin D resulted in levels of 17±8.6 (ng/ml) and infants given 800-1000 IU vitamin D had levels 46±17(ng/ml) (p <0.001).Conclusions: These data suggest that cord blood vitamin D levels are low in preterm infants and 800-1000 IU vitamin D supplementation is advisable to achieve levels >30 ng/ml. Infants with low levels of vitamin D have higher incidence of sepsis, and ROP.

BACKGROUND: Low Vitamin D levels associated with obesity have reached an epidemic level all over the world. It has been supposed that the low serum level of Vitamin D3 in obese subjects may be due to an increase in the uptake of Vitamin D3 by adipose tissue. AIM: The current study aimed to investigate the effect of a specially designed exercise program for boosting lipolysis on the Vitamin D level in obese children. METHODS: Thirty obese male children participated in the study. Their age was ranged from 9 to 11 years. The participants were assigned to two groups, Group I (GI) who received endurance exercise (ENE) only and Group II (GII) who received the specially designed exercise for increasing lipolysis (ENE preceded by resistance exercise). Free fatty acids (FFA), glycerol, and 25(OH)D were assessed before and immediately after exercise. RESULTS: FFA and glycerol showed a significant increase in both groups following exercise, while 25(OH)D showed a significant increase only in GII. GII showed significantly higher levels of FFA, glycerol, and 25(OH)D following exercise when it was compared to GI. CONCLUSION: The application of resistance training before ENE could improve the Vitamin D status through increasing the lipolytic activities more than the application of endurance exercise alone.

Background: Female pattern hair loss (FPHL) is the most common cause of alopecia in women, characterized by diffuse non-scarring hair loss in frontal, central, and parietal areas of the scalp. Vitamin D is a factor that has recently been considered in dealing with these patients.  Objective: The aim of this work was to evaluate the serum level of 25 hydroxy vitamin D in patients with FPHL to elucidate its possible role in the pathogenesis of the disease. Methods: This study included 45 patients with female pattern hair loss; Evaluation of vitamin D level by Elisa was done for both control and patients. Results: Regarding vitamin D levels, mean ± SD. 18.0 ± 11.97. Conclusion: This study indicated the correlation between FPHL and decreased serum levels of Vitamin D3. It is recommended to evaluate serum Vitamin D3 levels as well as other hormone assays in these patients.

Vitamin D3 is an endogenous fat-soluble secosteroid, either biosynthesized in human skin or absorbed from diet and health supplements. Multiple hydroxylation reactions in several tissues including liver and small intestine produce different forms of vitamin D3. Low serum vitamin D levels is a global problem which may origin from differential absorption following supplementation. The objective of the present study was to estimate the physicochemical properties, metabolism, transport and pharmacokinetic behavior of vitamin D3 derivatives following oral ingestion. GastroPlus software, which is an in silico mechanistically-constructed simulation tool, was used to simulate the physicochemical and pharmacokinetic behavior for twelve vitamin D3 derivatives. The Absorption, Distribution, Metabolism, Excretion and Toxicity (ADMET) Predictor and PKPlus modules were employed to derive the relevant parameters from the structural features of the compounds. The majority of the vitamin D3 derivatives are lipophilic (log P values >5) with poor water solubility which are reflected in the poor predicted bioavailability. The fraction absorbed values for the vitamin D3 derivatives were low except for calcitroic acid, 1,23S,25-trihydroxy-24-oxo-vitamin D3, and (23S,25R)-1,25-dihydroxyvitamin D3-26,23-lactone each being greater than 90% fraction absorbed. Cytochrome P450 3A4 (CYP3A4) is the primary hepatic enzyme along with P-glycoprotein involved in the disposition of the vitamin D derivatives. Lipophilicity and solubility appear to be strongly associated with the oral absorption of the vitamin D3 derivatives. Understanding the ADME properties of vitamin D3 derivatives with the knowledge of pharmacological potency could influence the identification of pharmacokinetically most acceptable vitamin D3 derivative for routine supplementation.

Abstract Background: This study was thus undertaken to study the estimation of vitamin D levels in pregnant women and their newborns in DRC and to study its association with various neonatal outcomes. Methods: Observational study done on 569 pregnant women and their newborn babies. Sample obtained from maternal serum and cord blood from the placental end during peripartum period. The data was analysed to estimate the prevalence of vitamin D levels in mothers and newborns and to identify association, if any, between maternal vitamin D levels and neonatal outcomes.Results: 464 mothers had sufficient vitamin D (VDS) levels (>30 ng/dL) and 105 had vitamin D deficiency (VDD) with mean maternal serum vitamin D level of 35.63ng/ml (SD 6.18, range 9.2-39.8). All the newborns (n=569) were vitamin D deficient (<30ng/ml). The percentage of LBW babies born to VDD mothers was 18.09%, which was very similar to the percentage of LBW babies born to VDS mothers (18.31%) (p=0.76456749). The incidence of caesarean section was 58.09% in the vitamin D deficient (VDD) group as compared to 19.61% in the sufficient (VDS) group (p<0.0005). The incidence of preterm births was 10.77% in the sufficient group (VDS) as compared to 17.14% in the vitamin D deficient (VDD) group (p=0.038). Conclusion: Our findings will help health professionals, policy makers, and the general public in Africa aware of the high prevalence of vitamin D deficiency and the associated health risks.

Alaska Native (AN) children experience one of the highest reported rates of severe early childhood caries (S-ECC). Serum vitamin D concentrations in AN childbearing women in the Yukon Kuskokwim Delta (YKD) region have decreased since the 1960s to currently low levels, related to a decrease in traditional marine diet. Recent studies suggest an association between prenatal vitamin D (25(OH)D) concentrations in mothers and S-ECC in their infants. We used independent t tests to analyze the influence of prenatal 25(OH)D levels in YKD AN mothers on S-ECC in their children using data collected in the Maternal Organics Monitoring Study (MOMS). Maternal 25(OH)D levels were assessed at prenatal visits and in cord blood. We queried electronic dental records to assess early childhood caries (ECC) status using highest decayed, missing, filled, primary teeth (dmft) scores at 12 to 59 mo of age. We examined prenatal and cord blood for 76 and 57 mother/infant pairs, respectively. Children 12 to 35 mo of age with “deficient” cord blood (25(OH)D <30 nmol/L) had a mean dmft score twice as high as children who were “nondeficient” at birth (9.3 vs. 4.7; P = 0.002). There was no significant difference in mean dmft scores for children aged 36 to 59 mo with deficient versus nondeficient cord blood 25(OH)D (10.9 vs. 8.7 P = 0.14). There was no significant difference in mean dmft scores for children aged 12 to 35 mo whose mothers had “sufficient” versus “insufficient” 25(OH)D during prenatal visits (9.0 vs. 7.4; P = 0.48). In this small sample, children with deficient vitamin D levels in cord blood had a dmft score at 12 to 35 mo 2-fold higher than children with nondeficient cord blood. Maternal 25(OH)D may influence the primary dentition, and improving vitamin D status in pregnant women might affect ECC rates in their infants.

Objective: To determine the association of long term sodium valproate monotherapy and vitamin D3 levels in epileptic children Methods: This cross-sectional study was conducted in the Department of Pediatrics, Children Hospital, Pakistan Institute of Medical Sciences, Islamabad for six months from 15th February 2019 to 14th August 2019. A total of one hundred and thirty (n=130) children and adolescents of either gender between age 3-18 years who had a history of two seizures at least 24 hours apart in their life and were on sodium valproate monotherapy for more than one year were enrolled in this study through non-probability, consecutive sampling. Serum vitamin D3 (25-hydroxy vitamin D) levels were measured in all the patients at the time of enrolment into the study. All the demographic data and laboratory investigations were entered on the predesigned proforma and analyzed through SPSS version 17. Results: Vitamin D3 deficiency was found in 47 (36.2%) children which were significantly higher among patients with older age and longer duration of treatment (P<0.05) while gender and BMI of the patients did not show any significant difference (P>0.05). Conclusion: Significant percentage of epileptic children on sodium valproate monotherapy was found to have vitamin D3 deficiency. Therefore we recommend routine screening of vitamin D3 deficiency in all the epileptic children on long-term sodium valproate therapy followed by vitamin D supplementation in deficient patients.
