Pleomorphic lipoma is a rare neoplasm that is considered as a variant of spindle cell lipoma. It predominantly occurs in the dermis or subcutis of the posterior neck, upper back, and shoulders. Pleomorphic lipoma may clinically present as a slow-growing and well-circumscribed subcutaneous mass. Though it is a benign tumour it may contain atypical cells for which it may mimick sarcoma or other malignant soft tissue tumours. So histopathological diagnosis is vital for preventing unnecessary surgery. Here we report a case of a pleomorphic lipoma on upper back in a 55yr old patient.

ABSTRACT Spindle Cell Lipoma (SCL) is an uncommon benign tumor of adipose tissue that is usually super􀃶cially located in the posterior neck, back and shoulder region. We report a rare case of SCL in a 39 years old male presenting as neck swelling with c/o stridor and dysphagia. The contrast enhanced computed tomographic scan of neck showed an external and internal part of the swelling traversing through the thyrohyoid membrane suggestive of Mixed Pyolaryngocele. The tumor was excised by a trans-cervical approach but the post operative histopathology was found to be spindle cell Lipoma. We report this case due to its atypical presentation and location. Lipomas are among the most commonly encountered soft tissue tumors in clinical practice, though they are rare in the foot. Although a presumptive diagnosis is typically made clinically, those tumors with atypical clinical features may require radiological consultation. Difficulty arises when radiographic features are not typical of lipoma. We present a fatty soft tissue tumor of the foot with nonadipose elements on magnetic resonance imaging evaluation. Differentiation of lipoma variants (e.g., spindle cell lipoma, atypical lipoma, pleomorphic lipoma, lipoblastoma, angiolipoma) from liposarcoma based on imaging features is not possible, necessitating surgical resection for definitive histological diagnosis.

Spindle cell lipoma (SCL) is a rare distinct variant of lipoma, which presents as a painless, circumscribed, slow-growing, superficial lesion on the lip and can mimic a minor salivary gland tumour. We present a slow growing lower lip lesion and its management.Case Report. A 38-year-old female gave an eight-year history of a slow-growing mass on her lower lip with intermittent change in size. She presented with a submucosal nodule and thin overlying mucosa adjacent to the vermilion border. Surgical excision was carried as the diagnostic and therapeutic approach.Conclusion. Lip SCL is rare, and surgical excision is advocated in order to exclude underlying pathology and minor salivary gland tumours.

Spindle cell lipoma (SCL) in the scrotum is a very rare presentation with only a few cases reported in the literature. The most common sites for this benign lesion are the neck, shoulder, and back. We present a 72-year-old male patient with painless swelling on the left half of the scrotum with a histopathological diagnosis of SCL.

Spindle cell lipomas (SCL) are slow-growing benign adipocyte tumours that are most commonly seen in (1) the upper back, posterior aspect of the neck, and shoulders . Both genders are affected, but it is more (1) prevalent in males between the ages of 40 and 70 . Spindle cell lipoma is a benign tumour that is frequently confused with (1) liposarcoma. SCLs account for approximately 1.5 percent of all lipomatous tumours reported, making them uncommon . They (2) have a morphology similar to other benign and malignant fatty/spindle cell or myxoid lesions . Microscopically, it is a complex mixture of lipocytes and uniform spindle cells embedded in a mucinous matrix and traversed by (2) a variable number of birefringent collagen bres . Due to the unusual presentation and similar morphology of tumours, early diagnosis is critical; thus, diagnosis is based on clinical examination and conrmed by histopathological ndings. Because SCL has a favourable prognosis, wide local (1) excision is the treatment of choice . This is a case report of a 52-year-old male with a similar presentation in the right upper limb.

Purpose/Methods:The aP2 gene product (aP2 protein) is known to be expressed by preadipocytes and other immature fat cells in vitro. A mouse monoclonal antibody raised against an 18 amino acid segment of the aP2 protein was found to react with lipoblasts and fetal fat cells in paraffin sections of soft tissue tumours of adipose differentiation. In this immunohistochemical study, we have further examined the diagnostic utility of aP2 expression in distinguishing tumours of adipose differentiation from other benign and malignant soft tissue tumours.Result and discussionaP2 was strongly expressed by lipoblasts in lipoblastomas and all types of liposarcoma as well as brown fat cells in hibernomas. Optimal conditions for immunohistochemical identification of lipoblasts in tumours of adipose differentiation was noted when the antibody was diluted 1:30 to 1:50. Small lipoblast-like fat cells in pleomorphic lipoma and spindle cell lipoma also showed variable staining for aP2 at this dilution of the antibody. Most benign and malignant soft tissue tumours were distinguished by their absence of staining for aP2 protein, but some cases of myxoma, malignant fibrous histiocytoma, synovial sarcoma and leiomyosarcoma contained tumour cells which reacted for aP2. aP2 protein expression is likely to prove a useful means of distinguishing lipoblasts in liposarcoma but it should be used as part of a tumour panel to exclude expression in other forms of mesenchymal tumour.

Pleomorphic lipoma/ spindle cell lipoma accounts for 15 % of lipoma within extremities, confining to the superficial plain comprising lower dermis and subcutaneous tissue. Intramuscular presentation in extrimities is a rare presentation. Here is a case of deep seated pleomorphic lipoma, of forearm, both on fine needle aspiration, biopsy and IHC confirmation. 100% of benign cases of lipoma are positive for CD 34, as stated in many studies and was also positive in our case.
