Objective: Comprehensive mental and social health services is the new benefit package which had been aimed to provide mental health services to people who suffer from mental disorders. The aim of this study was to estimate the cost of plan and its drivers to provide evidence for decision-making by national policymakers. Method: We used the bottom-up costing approach to estimate the cost of plan. We identified the cost centers, services delivery process, and facilities. Data were collected via different sources and tools such as the new financial system, registration forms, and performance reporting forms. We categorized the cost into 4 groups and selected appropriate measures to estimate the cost. We estimate the total and unit cost for 3 levels in 2 scenarios by considering the 2017 prices. Results: Screening resulted in 8.9% new detection with a different incidence in urban and rural areas (urban: 16.5%; rural: 2.7%). Also, 61 842 million IRR was spent for the screening, diagnose, treatment, and rehabilitation of detected people in 2017. Personal cost is responsible for 90.6% and primary screening for 66.4% of the total cost. Conclusion: For the development of the program (from screening to rehabilitation) 530 513 IRR should be spent per capita. The cost of detection per client can vary due to differences in disease prevalence, especially treatment and rehabilitation costs. It is suggested to consider the variation of the prevalence in expanding the plan to the whole country. Integrating the services in primary health care lead to huge cost saving.

Introduction: Implementation of the Federal Public Health Promotion Project should raise awareness and develop skills of healthy nutrition in children, thus contributing to disease prevention. Our objective was to evaluate the results of pilot nutrition monitoring in school-aged children of the Sverdlovsk Region as part of the Federal Public Health Promotion Project and the National Demography Project. Results: We established that school meals were generally satisfactory: the rations complied with physiological needs of children in terms of their nutritional value, basic nutrients, energy, and distribution of calories by main meals. We noted differences in the cost and nutritional value of meals and the variety of dishes and foodstuffs used between urban and rural areas. As a rule, pupils have one or two school meals a day. Outside of school, their consumption of dairy products and fruit is limited. Conclusions: Our findings may promote the elaboration of municipal programs aimed, inter alia, at changing the amount of sugar and salt used in the manufacture of public catering products, the cost of dishes with a high content of sugar, saturated fats, and salt, and subsidies on healthy nutrition.

Drawing on ethnographic fieldwork with Greek Cypriot elementary school children in urban and rural areas of the Republic of Cyprus, the author describes and analyses the ways in which national musical identity is constructed in and out of school in connection with Cypriot traditional music. Findings reveal the development of fluid and often insecure, ambiguous and contradictory national musical identities as a result of the ideological messages children receive from their musical enculturation contexts. In addition public music education not only fails to assist pupils to become familiar with the tradition's inherent meanings and processes of creation and performance, but enhances children's contradictory ideological understanding and construction of an ambiguous national musical identity.

Abstract The need for broadband data has increased speedily but in underserved rural areas, the mobile connectivity of 3G and LTE is still a significant challenge. By looking at the historical trend, the data traffic and the internet are still expected to grow in these areas [1]. The next generation of satellites is trying to decrease the cost per MB having the advantage of higher throughput and availability. To maintain the performance of the link, choosing an appropriate frequency is evident. A multi-beam satellite system can fulfill the demand and performance over a coverage area. The high throughput satellites (HTS) fulfill this requirement using C and Ku bands. In this paper, we present the benefits of using Ku-band on the user site and the composite of C and Ku bands on the gateway site. This configuration has proved to be a cost-efficient solution with high performance over the traditional straight configuration. The data rate is improved five times both on upstream and downstream as compared to the existing available FSS system. Moreover, it has got an advantage to Ku-band user that they would enjoy the significant improvement in the performance without upgrading their systems.

Performance-based financing (PBF) is a mechanism to improve the quality and the utilisation of health benefit packages. There is a dearth of economic evaluations of PBF in the ‘real world’. Afghanistan implemented PBF between 2010 and 2015 and evaluated the programme using a pragmatic cluster-randomised control trial. We conducted a cost-effectiveness analysis of the PBF programme in Afghanistan, compared with the standard of care, from the provider payer’s perspective. The incremental cost-effectiveness ratio of PBF compared with the standard of care was US$1242 per disability-adjusted life year averted; not cost-effective when compared with an opportunity cost threshold of US$349. Incentive payments were the main contributor to PBF financial cost (70%) followed by data verification (23%), staff time (5%) and administration (2%). The unit cost per case of antenatal care (ANC), skilled birth attendance (SBA) and postnatal care (PNC) services in the standard of care was US$0.96 (95% CI 0.92–1.0), US$4.8 (95% CI 4.1–6.3) and US$1.3 (95% CI 1.2–1.4), respectively, whereas the cost of ANC, SBA and PNC services per case in PBF areas were US$4.72 (95% CI 4.68–5.7), US$48.5 (95% CI 48.0–52.5) and US$5.4 (95% CI 5.1–5.9), respectively. To conclude, our study found that PBF, as implemented in the Afghan context, was not the best use of funds to strengthen the delivery of maternal and child health services. The cost-effectiveness of alternative PBF designs needs to be appraised before using PBF at scale to support health benefit packages. PBF needs to be considered in the context of funding the range of constraints that inhibit health service performance improvement.

Broadband is a public utility with positive effects on competitiveness, employment and growth. Although evidence shows that broadband development is increasing, this increment is not homogeneous between urban and rural areas. The main factor is the high cost for deploying next generation networks in rural areas, and especially the cost linked to civil engineering works. In order to overcome the investment challenges arising in the context of broadband deployment, the European Commission made mandatory the sharing of existing telecommunication and non-telecommunication infrastructures and facilities. This manuscript proposes and presents a novel tool that enables the estimation of the expected savings from exploiting existing infrastructures when deploying a broadband network. This is followed by a number of general principles and recommendations that policy makers and national authorities could embrace to diminish deployment costs and promote broadband deployment.

In this study, exergoeconomic analysis and performance evaluation of selected gas turbine power plants in Nigeria were carried out. The study was conducted using operating data obtained from the power plants to determine the exergy efficiency, exergy destruction, unit cost of electricity and cost of exergy destruction of the major components of a gas turbine engine in the selected power plants. The results of exergy analysis confirmed that the combustion chamber is the most exergy destructive component compared to other cycle components as expected. The total efficiency defects and overall exergetic efficiency of the selected power plants vary from 38.64 to 69.33% and 15.66 to 30.72% respectively. The exergy analysis further shows that the exergy improvement potential of the selected plants varies from 54.04 MW to 159.88 MW. The component with the highest exergy improvement potential is the combustion chamber and its value varies from 30.21 MW to 88.86 MW. The results of exergoeconomic analysis show that the combustion chamber has the greatest cost of exergy destruction compared to other components. Increasing the gas turbine inlet temperature (GTIT), both the exergy destruction and the cost of exergy destruction of this component were found to decrease. The results of this study revealed that an increase in the GTIT of about 200 K can lead to a reduction of about 29% in the cost of exergy destruction. From exergy costing analysis, the unit cost of electricity produced in the selected power plants varies from cents 1.99 /kWh (N3.16 /kWh) to cents 5.65 /kWh (N8.98 /kWh).

In developed countries, along with urbanization, the opposite process - ruralization or deurbanization - is becoming increasingly noticeable. Ruralization is not only the movement of townspeople to the countryside, but also the development of villages and the increasing importance of rural areas as a place to live and work. Townspeople make the decision to move by comparing the advantages and disadvantages of living in urban and rural areas, and non-economic motives (the desire to get closer to nature, to find ones roots, to live in silence, without haste, to eat natural products, etc.) play an important role in this decision. Among economic motives, the difference in the cost of urban and rural real estate and of life in general is the most important motive, i.e., families, especially young and large, can improve their living conditions by moving to the countryside. The study, the results of which are presented in the article, aimed at identifying those groups of townspeople that tend to resettlement, their motives, and factors pushing people to leave cities and facilitating/hindering resettlement to rural areas. The research is based on the study of special literature on the topic and on the data of the survey of resettled townspeople and experts in ruralization. Unlike most publications on ruralization, the author focuses on the positive aspects of the resettlement of townspeople to the countryside and insists on the removal of administrative barriers that prevent ruralization, because the resettlement of townspeople to the countryside does not have a negative impact on the city and is compensated by the influx of people from the countryside, who want to get education or a new profession.

This essay discusses British discourses and efforts to regulate social policy in both urban and rural areas in late colonial Tanzania. It focuses mainly on questions of social security and especially on the vague concept of social welfare and development, which after the Second World War became a favoured means of expressing a new imperial commitment to colonial people. The British were very reluctant about implementing international standards of social security in Tanganyika, mainly due to the insight that the cost of providing European-scale benefits could not be borne by the colonial regime in such a poor territory. They were far more enthusiastic in pursuing a policy of social development, embodied in social welfare centres and various other schemes. It is argued that in Tanzania, this policy remained focused on peasantization rather than on proletarianization and was characterized by a disconnection between Colonial Office mandarins in London, attempting to create bourgeois, respectable African middle classes, and colonial officials in Tanganyika, seeking to maintain the political legitimacy of the chiefs and headmen. Most Africans ignored rather than challenged many of these state efforts. However, the nationalist party, the Tanganyika African National Union (TANU) under Julius Nyerere believed in these programmes and continued such dirigiste and poorly financed improvement schemes after independence.
