Background: Despite numerous reports about temporal changes in semen quality from all over the world, the debates continue. The latest systemic review has shown an overtime decrease in semen quality worldwide. Objective: To assess the temporal changes in the semen quality among Iranian population referred to an infertility center. Materials and Methods: In this retrospective cross-sectional study, semen parameters including concentration, motility, and morphology were compared between Iranian men reffered to Research and Clinical Center for Infertility, Yazd between 1990 to 1992 (group 1, n = 707) and 2010 to 2012 (group 2, n = 1108). Demographic characteristics and semen analysis were collected from the records. The effect of age on semen parameters was also investigated. Results: Despite the increase in sperm concentration l in group 2, sperm with normal morphology decreased significantly (p < 0.001). Grade-A motility decreased (p < 0.001), grade B motility increased (p < 0.001), and grade C and D motile sperm remained constant (p = 0.303 and p = 0.315, respectively). Also, no significant correlation between the age and semen parameters were observed. Conclusion: This study showed inconsistent temporal changes in the participant semen quality. Significant temporal decline were obtained between various semen parameters, sperm morphology and grade A motility. These results should be further evaluated by larger studies in the future. Abstract Background The aim of this study was to provide information on the semen quality pattern of infertile men and age thresholds for semen parameters in China. Methods This was a retrospective cross-sectional study investigating 71,623 infertile men from the Reproductive and Genetic Hospital of CITIC Xiangya in Hunan, China, from 2011 to 2017. The Kruskal-Wallis test, Mann-Kendall test, linear regression model and joinpoint regression were used. Results Although erratic changes were observed in the median semen parameters (sperm concentration 40.1–52.1 × 106/ml, total sperm count 117.8–153.1 × 106, sperm progressive motility 33.4–38.1%) during the 7 years of observation, no significant decrease in semen quality was found, and 47.88% of infertile men showed normal semen parameters according to the World Health Organization (WHO) criteria. According to the joinpoint regression analysis, sperm progressive motility appeared to decrease earlier than the sperm concentration and total sperm count (at 28, 58, and 42 years of age, respectively). Conclusions There is no evidence of a deterioration in semen quality among infertile men in Hunan, China. Semen parameters decreased with increasing age, with turning points noted at different ages. Semen parameters are not absolute evidence for the assessment of male fertility potential. Therefore, we believe that, among semen parameters, the sperm concentration is the best predictor of fertility for ART, followed by motility. Decreased sperm motility may affect natural pregnancy, but it is not necessary for successful IVF.

Background: In India, the prevalence of primary infertility ranges from 3.9% to 16.8%. Male factor contributes 40-50% of this. Male factor infertility is indicated by decreased sperm concentration, reduced motility, vitality or abnormal sperm morphology. Semen analysis is the single most important investigation to detect male factor infertility. The aim of this study was to analyse the prevalence of abnormal semen parameters among males in infertile couples and their association with contributing factors.Methods: This cross-sectional hospital-based study was carried out in the Department of Pathology at Lady Hardinge Medical College and Smt. Sucheta Kriplani Hospital. A total of 400 cases were analyzed during a period of 6 months. Detailed history of the couple was taken. Semen analysis was done using automated semen analyzer (SQA-vision) after 3 days of abstinence according to the WHO 5th edition 2010 guidelines. The results were analysed using excel sheet and SPSS software.Results: In the present study, 122 cases (30.5%) out of 400 cases had abnormal semen parameters. Most common abnormality detected was asthenozoospermia (14.3%) followed by oligozoospermia (13.8%), azoospermia (10.5%) and teratozoospermia (10.5%). There was significant association of alcohol intake, obesity and trauma with abnormal semen parameters.Conclusions: Asthenozoospermia was the most common abnormality noted in this study. Lifestyle modifications along with timely medical attention in male partners of infertile couples can improve the semen quality.

The influence of individual lifestyle factors is widely described in studies on semen quality. However, their synergistic effect is often neglected. The aim of the study was to examine the association between semen quality and dietary approaches to stop hypertension (DASH) diet, physical activity (PA), and the two separately and in combination. A cross-sectional study was carried out among 207 men aged 20–55. Dietary data were collected by a food frequency questionnaire (FFQ) and analysed according to the DASH scoring index. Physical activity was evaluated by the International Physical Activity Questionnaire. Semen parameters were assessed via the computer-aided semen analysis (CASA). Adherence to the DASH diet was associated with higher sperm count (ΔT3-T1 = 82.1 mln/ej) and concentration (ΔT3-T1 = 24.6 mln/mL). Higher PA was related to higher sperm count (ΔT3-T1 = 69.4 mln/ej), total (ΔT3-T1 = 11.9%), and progressive motility (ΔT3-T1 = 8.5%) and morphology (ΔT3-T1 = 2.8%) in the crude model and remained significant after adjustment. The combination of the DASH diet and PA, was significantly positively associated with sperm count (ΔT3-T1 = 98.1 mln/ej), sperm concentration (ΔT3-T1 = 17.5 mln/mL), total (ΔT3-T1 = 11.8%), and progressive motility (ΔT3-T1 = 10.0%) and morphology (ΔT3-T1 = 3.3%) in both models. Adherence to the DASH diet was related to higher sperm count and concentration, whereas after its combination with physical activity it was also positively correlated with sperm motility and morphology.

Abstract Background: The geographic variation in semen quality has been reported in many countries. However, the situation in China is still unknown. To answer this question, we examined the variation in semen parameters between two cities in south China: Guangzhou (GZ) and Hong Kong (HK).Methods: Semen analysis data of 13507 subjects from GZ and 1106 subjects from HK were retrospectively analysed. In order to control for referral bias, we selected two well-defined subpopulations, namely, subpopulation Ⅰ: male partners of women with an underlying cause of subfertility, which is assumed to be similar to the general male population; subpopulation Ⅱ: normozoospermic men, which is assumed to represent men with normal spermatogenesis, sperm maturation and sperm transportation. In addition, we have also chosen to control for two well-recognized confounding variables, namely, age and abstinence time by using a case matching design and generalised additive models (GAM).Results: In using the case matching design, subjects from different cities with similar age and abstinence time were matched. The results of the paired T test showed that sperm concentration and motility of subjects in HK were significantly lower than that of subjects in GZ in both subpopulation Ⅰ and Ⅱ (P < 0.05); on the other hand, the sperm morphology of subjects in HK was significantly lower than that of subjects in GZ in subpopulation Ⅰ (P < 0.05) but not in subpopulation Ⅱ (P > 0.05).The results of GAM model showed that the sperm parameters were variated by age and/or abstinence time, however, the sperm concentration, motility and morphology of subjects in HK were significantly lower than that of subjects in GZ in both subpopulation Ⅰ and Ⅱ (P < 0.05). Conclusions: The semen quality of subjects in HK was significantly worse than that of subjects in GZ, which may be caused by environmental and lifestyle factors, given that the study subjects in both cities of the present study are of the same ethnic origin.Trial registration: Not applicable.

Among the possible risk factors for male reproduction, exposure to phthalates and alkylphenols is widely documented. This study evaluated the possible association between chemical exposure and the quality of the seminal fluid of 105 subjects in a fertility clinic. The urinary levels of seven phthalate metabolites (monoethylphthalate, MEP; monobenzylphthalate, MBzP; mono n-butylphthalate, MnBP; mono-(2-ethylhexyl) phthalate, MEHP; mono(2-ethyl-5-hydroxyhexyl) phthalate, MEHHP; mono-n-octylphthalate, MnOP; mono-isononylphthalate, MiNP) and bisphenol A (BPA), were analysed by high performance liquid chromatography/tandem mass spectrometry HPLC/MS/MS. The regression analysis showed that the semen volume was positively associated with MnBP, MnOP and BPA levels while was negatively associated with MiNP levels. The sperm concentration had a significant inverse relationship with MEP levels. A negative association was found between the use of plastic containers for food storage (p = 0.037) and semen volume (3.06 vs. 2.30 mL as average values, never vs daily). A significant positive correlation emerged (p < 0.005) between the consumption of canned food and the levels of BPA (2.81 vs. 0.14 µg/g creat as average values, daily vs. never) and between the use of perfumes and levels of MEP (389.86 vs. 48.68 µg/g creat, as average values, daily vs. never). No further statistically significant associations were found, even considering the working activity. Some evidence emerged about the possible link between exposure and seminal fluid quality: further case/control or prospective studies will allow us to confirm this causality hypothesis.

Background: Varicocele is a condition characterized by elongation, dilatation and tortuousity of spermatic vein in pampiniform plexus. Approximately 50% of infertility cases among men are caused by varicocele. The varicocele may affect the components of sperm. This study aimed to determine the association between varicocele grade based on ultrasound Doppler examination and sperm concentration, sperm motility, and sperm morphology based on semen analysis examination.Methods: This was a descriptive, cross-sectional study which used secondary data from 85 patients that visited Department of Urology, Cipto Mangunkusumo Hospital, then underwent ultrasonography examination at Department of Radiology, Cipto Mangunkusumo Hospital and semen analysis examination at Department of Obstetrics and Gynecology, Cipto Mangunkusumo General Hospital.Results: Varicocele grade was significantly associated with sperm morphology, concentration and motility (all p<0.05). Significant association was found between maximum condition and semen analysis component.Conclusion: Varicocele grade may affect semen analysis component.

Objective. To investigate the question of whether duration of pain before surgery ultimately affects sperm parameters after varicocelectomy.Methods. Fifty patients with painful grade-3 varicocele were investigated prospectively. The patients were divided into two groups according to their symptom period. The patients having had grade-3 varicocele for less than 1 year were included in Group-1 (Ge,n=25). Twenty-five patients who had painful grade-3 varicocele for more than 1 year (Gs,n=25) were classified in Group-2. Semen analysis was performed after 3 days of sexual abstinence twice a month. Total sperm concentration (TSC), rapidly progressive motility (SPa), and slow or sluggish motility (SPb) rates were noted. Pain was evaluated by using 10 cm visual analogue scale (VAS).Results. Postoperative TSC and %SPb were significantly higher in both groups (P=0.01). There was no difference between two groups for preoperative and postoperative TSC, %SPa, % and SPb values. VAS significantly declined in both groups (P=0.005). This postoperative decline was not significant for intergroup comparison.Conclusions. Our results show that increase in semen quality and decrease in the pain after microsurgery varicocelectomy do not depend on the duration of the preoperative pain.

Objective: The objective of this study was to study the semen parameters and its relation to hormonal status in infertile men. Study Design: Cross sectional study. Period: March 2015 to April 2017. Setting: Jinnah Hospital Fertility Centre; Department of Obstetrics and Gynecology, Lahore. Method: A total of 70 infertile men were included in this study. Semen samples for semen analysis and serum for hormonal analysis were obtained and further tests were performed. Results: The results of our study show that 28.57% were Normospermic, 18.56% were azoospermic and 52.85% were Oligospermic. In Oligospermic and azoospermic patients significant low levels of testosterone and prolactin were noticed. The results indicate that along with semen parameters hormonal levels should also be assessed to diagnose the underlying cause of infertility. Conclusion: It is concluded from our study that both quality of semen and hormonal levels cause infertility in men. Therefore the patients should be properly investigated and should be treated accordingly to the underlying abnormality. This is a regional study and further studies should be conducted to have a conclusive evidence at national level.

OBJECTIVE: To determine the frequency of abnormal semen parameters among patients presenting withvaricocele1METHODS:It was a descriptive cross-sectional study conducted at the Department of Urology Institute ofkidney diseases Hayat Abad Medical Complex Peshawar and Cenna hospital Saidu Sharif Swat. The studywas carried out on 139 human subjects with clinical evidence of varicocele between age range of 15-45years.The diagnosis of varicocele was based on palpable and/or visible scrotal lump of testicular veins(pampiniform plexus) and was diagnosed on the basis of clinical examination. Semen analysis was carriedout in all these patients and information wascollected on pre designed proforma.RESULTS:The study included a total of 139 patients with varicocele. The mean age of patient was 30 years(15-45) among the patients having symptoms of varicocele. The Mean ±SD for duration of varicocelesymptoms was 9.32 ± 9.70 months. 6.5% (n=9) patients were having azoospermia and 20.1% (n=28)patients had oligozoospermia.CONCLUSION: Patients with varicocele have poor seminal parameters in terms of sperm count i.e.oligozoospermia and azoospermia responsible for male factor infertility in majority ofcases.KEYWORDS:varicocele;seminal parameters;sperm count, infertility

Objective: To study the patterns and distribution of various abnormal semen parameters in infertile males. Study Design: Cross sectional study. Place and Duration of Study: Department of Pathology, Combined Military Hospital, Karachi, from Nov 2019 to Oct 2020. Methodology: The study included 364 patients who presented with primary and secondary infertility. Consecutive convenient sampling was done. Semen analysis was performed using World Health Organization latest guidelines. Samples were categorized as normospermia, azoospermia, oligospermia, asthenozoospermia and necrospermia. Results: The study comprised of 364 samples of infertile males. Normal sperm count was observed in 317 (87%) males, azoospermia in 28 (7.6%) and oligospermia in 19 (5.2%) males. Low ejaculated volume and higher non-motile sperms were noted in oligospermia samples in comparison with normospermia samples. Asthenozoospermia was observed in 102 (28%) and oligoasthenospermia was noted in 15 (4.1%) samples. Conclusion: Good quality semen analysis is a corner stone to diagnose the cause of male infertility.Sperm concentration and motility are the important markers of normal male reproductive system and are related to each other.
