The article presents the results of a study of the dynamics of recurrent disability due to malignant neoplasms (ZNO) of the adult population in the Chechen Republic for the period 2008-2017. in comparison with the figures for the Russian Federation. In the Russian Federation, the dynamics of re-disability of the adult population due to the ZNO in the period 2008-2017 was characterized by an increase in the number of repeatedly recognized disabled (PPI), the level of which increased from 19.9 to 27.0 per 10 thousand people. Compared to 2008 (taken as 100%), the indicator of visibility of PPI was 135%. In the PPI contingent, the elderly prevailed with a negative trend of growth in their numbers. The proportion of young and middle-aged PPI decreased. In the Chechen Republic, the epidemiological picture of the disability of the adult population due to the ZNO was characterized by different dynamics. The number of PPI due to ZNO in the country decreased. Compared to 2008 (taken as 100%), the visibility index of the PPI was 57.0%; In the structure of recurrent disability due to ZNO, middle-aged and old-aged PPI prevailed, however, their proportion declined. There is a negative growth trend in the proportion of young people.

Introduction: Disability resulting from diseases of the respiratory system is a complex problem for the health care and social security systems. The prevalence of chronic lung diseases keeps growing. Our objective was to analyze the rates of initial and recurrent respiratory disability in the adult population of the Rostov Region in 2014–2018 and to compare them with those registered in the Southern Federal District and the Russian Federation. Material and methods: We used information about initial and recurrent respiratory disability in adults from the electronic database of the unified information and analytical system of medical and social expertise of the Rostov Region and digests of the Federal Bureau of Medical and Social Expert Examination of the Ministry of Labor and Social Protection of the Russian Federation for 2014–2018. We applied methods of data copying, descriptive statistics (estimation of extensive and intensive indicators, significance tests (p)), analytical and comparative analysis. Results: The analysis of adult respiratory disability in the Rostov Region in 2014–2018 showed a decrease in the number of adults with initial and recurrent disability. The proportion of initial cases tended to decrease and was similar to the Russian average rate but exceeded that in the Southern Federal District while the proportion of recurrent disability cases increased. The rates of initial respiratory disability in the adult population of the Rostov Region and the Russian Federation were comparable. The regional proportion of young people with respiratory disability was 42.2 % in contrast to the Southern Federal District and the Russian Federation where elderly cases prevailed (42.0 % and 48.7 %, respectively). In terms of severity of disability, grade II disability cases prevailed (50.4 %) in the Rostov Region as opposed to the Southern Federal District and the Russian Federation with higher proportions of persons with disability grade III (62.3 % and 72.0 %, respectively). The recurrent disability rate showed a rising trend and was higher than the intensive indicator in the Southern Federal District but lower than that in the Russian Federation. The majority of regional cases with recurrent respiratory disability were young and middle-aged (40.5 % and 40.2 %, respectively); the proportion of young cases tended to increase while that of the middle-aged showed a downward trend. In the Southern Federal District and the Russian Federation, most recurrent cases were middle-aged or elderly (37.8 % and 35.3 %) and elderly or middle-aged (39.0 % and 35.1 %), respectively. The rate of recurrent respiratory disability among young people in the Rostov Region was twice as high as in the Southern Federal District and the Russian Federation. In terms of severity of recurrent respiratory disability, grade III disability was most prevalent in all territories but in the Rostov Region its proportion was lower than in the areas of comparison. The proportion of people with grade II disability tended to decrease but was still higher than in the Southern Federal District and the Russian Federation. Conclusions: Our findings will allow a more accurate assessment of severity of impairments and vital activity limitations in adults with respiratory disability and help plan appropriate age and sex-specific rehabilitation measures contributing to effective rehabilitation and habilitation actions and successful social integration of a disabled person.

Disability of the elderly age group of the population is the most important problem of public health and social services of all developed countries over the world without exception. In the Russian Federation, this age segment is characterized by the gain, reflecting the global process of population aging. The aim of the study was to analyze the nosological structure of the primary and secondary disability in elderly people in comparison with the main age categories of the adult population in the Russian Federation and the evaluation of rehabilitation / rehabilitation activities for disabled people. The volume of the study was 34,840,933 people aged of from 18 years and over, recognized as disabled, including the disabled elderly - 12,971,062. The subject of a special study was the nature of functional disorders, the degree of the disability, the nosological spectrum of causes of the disability of elderly people and the effectiveness of implemented measures of medical and social rehabilitation (rehabilitation and expert characterization). The study was selective, the sample size account of 370 people. The results of the research demonstrate the nosological structure of the pathology, the associated types of impairments and the nature of the disability to determine the severity of the disability in elderly people, the complexity of the formation and implementation of individual programs for the rehabilitation and rehabilitation of disabled people (IPRA). The improvement of the quality of life and integrating disabled people into the society has been and remains a difficult task for rehabilitation and habilitation activities. Rehabilitation and provision of technical means of rehabilitation, to a lesser extent - social and professional rehabilitation are the most feasible for the implementation.

Background. Taking into account the special medical and social significance of oncological diseases, as well as taking into account that patients suffering from neoplasms are a special risk group for developing complications of coronavirus infection, it seems relevant to study the impact of the COVID-19 pandemic on the level of morbidity and hospital lethality of patients with neoplasms. Aims to study the trends of morbidity and hospital lethality due to neoplasms in the Russian Federation before and during the pandemic of the new coronavirus infection COVID-19. Methods. The indicators of primary and general morbidity, hospital lethality from neoplasms of the adult population were calculated on the basis of federal statistical observation data contained in reporting forms No. 12 and No. 14 of all subjects of the Russian Federation for 20152020. Results. During the period from 2015 to 2019, an increase in the general (by 14.8%) and primary (by 4.8%) incidence of neoplasms was noted in the Russian Federation. In 2020, compared with 2019, on the contrary, the indicators of general (by 5.0%) and primary (by 17.4%) incidence of neoplasms decreased. Hospital lethality due to neoplasms also increased during 20152019, in total it increased by 19.2%, in one year (2020) hospital lethality increased by 6.6%. A similar situation has developed in all federal districts and most regions of the country. Conclusions. The decline in the incidence of neoplasms in 2020, with their previous continuous growth, reflects the difficult situation that has developed during the pandemic of a new coronavirus infection in the healthcare of the Russian Federation. The increase in hospital lethality from neoplasms indicates a more severe course of coronavirus infection in adult patients with malignant neoplasms and a more frequent development of fatal complications in them.

The article presents results of the study ofproblems of the repeated disability due to diseases of the musculoskeletal system and connective tissue in retirement age citizens of the Russian Federation in the period of from 2005 to 2014. There were analyzed the number of this category of invalids, their specific weight among re-recognized as disabled cases with account of classes of diseases and the age, structure of the repeated disability in groups, the level of repeated disability and ranking places of subjects of the Russian Federation on this index in 2014. The results of the executed analysis indicate that only in the Russian Federation for 10 years (2005-2014) there were re-recognized as disabled due to diseases of the musculoskeletal system and connective tissue 487.1 thousand people from citizens of retirement age with distinct dynamics of the increase as their absolute number (from 22.1 thousand persons in 2005 to 69.3 thousand people in 2014) and the index of disability level (respectively from 7.6 to 20.5 per 10 thousand of the corresponding adult population). This pathology was established to determine a high level of repeated disability in this age group (20.5 per 10 thousand population). Comparative analysis by age revealed that in the Russian Federation in the structure of the repeated disability due to diseases of the musculoskeletal system and connective tissue in the adult population invalids of the retirement age prevail (42.8% in 2014). Along with diseases of the circulatory system (of rank 1) and malignant neoplasms (rank 2), diseases of the musculoskeletal system and connective tissue form the contingent of cases repeatedly recognized as disabled of retirement age (3th rank place). A ranking ofall subjects of the Russian Federation by the level of repeated disability of citizens of retirement age due to diseases of the musculoskeletal system and connective tissue allowed to select 70 ranking places, that with bearing in mind the present demographic situation, has a practical orientation for planning the organization of medical and social care for the given large category of the population.

Analysis of re-disability among working age persons due to the nervous system diseases showed that proportion of working age disabled persons in the structure of re-recognized as disabled persons (RRDP) was 73.1 % of which 43.2 % were young people with disabilities, 25.9 % were middle-aged with ones for 2014–2018 in Moscow. The prevalence of young people with disabilities, their share at the level of the indicator in the Russian Federation, but lower than in the Central Federal District is noted in the structure of RRDP. The proportion and the re-disability level of middle-aged persons with disabilities in Moscow are lower than in the Central Federal District and the Russian Federation. There is a decrease in the number of RRDP both in Moscow and in the Central Federal District and the Russian Federation in the dynamics. There is the prevalence of persons with disabilities with third-degree of disability in the RRDP structure both among young people (62.6 %) and middle-aged persons (59.1 %) with a tendency to decrease their share among middle-aged persons and reduce the re-disability level. We noted a decrease in the proportion and re-disability level of disabled persons with the second-degree of disability in dynamics, but the re-disability level among middle-aged persons is in 2 times higher than among young people. There was an increase in the proportion of the first-degree of disability among young people.

The article presents the results of the analysis of the long-term dynamics of indicators of primary disability due to malignant neoplasms of the adult population in the Russian Federation. The study volume was 1919677 people aged 18 years and older. An increase in the share of newly recognized disabled people (VPI) in the structure of the total number of VPI as a result of all diseases from 17.6% in 2008 to 32.4% in 2017 (an average of 24.2% per year) was found. In the structure of the primary disability due to ZNO, elderly and middle age IDDs prevail. The level of primary disability among the middle-aged population is decreasing, while among the young and elderly age segments, is increasing. The structure of primary disability due to ZNO by severity is characterized by the predominance of VPI II and I groups of disability with a lower proportion of VPI III group. It has been established that in the Russian Federation there is a regional differentiation of subjects according to the level of disability due to EIT, which allows us to identify regions with an unfavorable situation and the need to optimize measures for primary and secondary medical and social prevention and rehabilitation.

The article is devoted to the problem of primary retirement due to malignant neoplasms in the adult population in the Russian Federation. There are presented results of a study of the dynamics (2005-2014) of the age-gender structure, level and severity of primary disability due to this socially significant disease. The ranking of subjects of the Russian Federation according to the intensive index, describing the prevalence of primary disability due to malignant neoplasms is presented also. Over the ten-year period the maximum number for the first time recognized as disabled (FTRD) was revealed to be observed in 2005, amounting to 200.4 thousand people, with a subsequent decrease to 170.2 thousand people in 2008. However, later the trend in the dynamics changed to the increase in the total number of FTRD due to malignant neoplasms and the gain in the index accountedfor 13% in comparison with 2008. In the age pattern of disability due to malignant neoplasms there are dominated elderly invalids (60.3 %), among which there are prevailed the invalids of the group I (31.2%) and group II (53.6%). The area with the lowest level of the primary disability due to malignant neoplasms among the population of elderly is the Sakhalin region (15.4 per 10 thousand of the elderly population). The greatest prevalence of malignant neoplasms occurs in the Kurgan region (52.3 at the average rate of 34.4 per 10 thousand of the elderly population). Also the unfavorable situation in the Kaluga region, the Republic of Kalmykia, Novosibirsk, Bryansk, Orenburg and Arkhangelsk regions (46.9-45.1) per 10 thousand of the elderly population), which requires measures for the effective prevention of primary disability due to this socially significant severe disabling pathology.

The analysis of repeated disability of the adult population as a result of diseases of the genitourinary system in Moscow in comparison with the indicators of the Central Federal District and the Russian Federation for the period 2015-2019 revealed that extensive and intensive indicators of repeated disability in Moscow are lower than in the Central Federal District and the Russian Federation. In the age structure of repeated disability among young people as a result of diseases of the genitourinary system, the predominance in the Russian Federation was revealed, significant differences in values and dynamics among young people in Moscow and the Central Federal district were not revealed. The lowest proportion of middle-aged people was recorded in Moscow, which is lower than in the Central Federal district and the Russian Federation. There was a significant predominance of people of retirement age with a tendency to increase their level and share. The severity of disability was dominated by I group of disabled people with a tendency of increasing their specific weight and a relatively stable level. The second place was taken by disabled people of II group with a tendency of increasing their share.
