The aim of this work was to increase the effectiveness of treatment in patients with osteoarthritis of the knee by including in the treatment complex procedures of electromagnetic radiation of the terahertz range. This study was prospective and randomized. 65 patients were examined and treated. They were divided into 2 groups comparable to sex, age, disease type, and severity. All patients received basic therapy, including drugs and exercise therapy. In addition to the basic treatment, the patients of the main group underwent a course of terahertz therapy using the Emir device with a ceramic coating of the emitter. The study revealed that the procedures of terahertz therapy are well tolerated by patients, do not cause exacerbation and are combined with other methods of treatment of gonarthrosis. Repeated studies at the end of the course of treatment showed that the inclusion of terahertz radiation in the therapeutic complex has a positive effect on the main clinical symptoms of the disease, reducing the intensity of pain in the knee joints and improving their functional state, having a favorable effect on the state of local hemodynamics.

Abstract Background: Hip involvement is an important cause of disability and poor prognostic in patients with spondyloarthritis (SpA). TNF-α inhibitor treatment had been demonstrated to be effective in SpA patients with hip arthritis, however, quantitative assessment with magnetic resonance imaging (MRI) in the long-term follow-up still needs further application.Methods: A total of 239 patients were involved in this study, MTX and SSZ were given as basic treatment. 165 patients received TNF-α inhibitor plus basic treatment and 74 patients received basic treatment only as control. The clinical symptoms were assessed at baseline, week 12, week 24, and week 52. The MRI performances of hip arthritis including bone marrow edema (BME) and synovitis were quantitatively assessed using Hip MRI Inflammation Scoring System (HIMRISS).Results: The clinical symptoms of ESR, CRP, BASDAI, Harris, and ASDAS-ESR of both groups got significant clinical remission at week 52 (p<0.001). However, the disease activity change levels at week 52 in control group were significantly worse than TNF group. In week 52, MRI performance showed significant remission trend in TNF-α inhibitor group versus baseline of which total HIMRISS scores decreased significantly ( 26.49±10.37 vs 20.59±9.41, p<0.001), but the control group only had slight improvement trend (p<0.05).Conclusions: TNF-α inhibitor could significantly and better improve the clinical and MRI manifestation of hip involvement in SpA patients than controls. Quantitative MRI assessment combined with clinical assessment could accurately evaluate the treatment effect of TNF-α for SpA patients with hip involvement, and help to guide targeted treatment.Trial registration: Chinese Clinical Trial Registry, ChiCTR-ONRC-11001846, Registered 28 November 2011. The palliative care patient is definitely a unique type of patient. Due to the complexity of the symptoms requires a holistic therapeutic approach. Modern methods of treatment in palliative and hospice care underline an important role of physio, kinesiotherapy and pharmacological treatment coexistence. The rehabilitation reduces clinical symptoms, accompanying the basic disease and increases the quality of life of palliative patients and their families. It becomes an inseparable element of treatment, both in outpatient care as well as in stationary care and home care. Due to the high dynamics onset of cancer in the group of geriatric patients there is a need for a broader analysis of the topic. The goal of palliative care is to achieve the best possible quality of life for patients and their families.

Objective. To increase the effectiveness of treatment of placental insufficiency in pregnant women with the inclusion of L-carnitine (Inestom) in complex therapy. Materials and methods. A total of 76 pregnant women aged 18 to 39 years were examined at 2240 weeks of gestation. All the examined patients were divided into two groups. Patients of the first group (n = 37) received only basic treatment, which included the use of a complex of vasodilators, tocolytics, drugs that improve microcirculation and rheological properties of blood. Patients of the second group (n = 39) received L-carnitine along with basic therapy (Inestom, Help SA Pharmaceuticals, Greece). Results. The use of the drug Inestom as a part of the complex treatment of placental insufficiency led to the normalization of utero-placental-fetal blood flow and intrauterine fetal development in 94.7 % of pregnant women. After the course of drug therapy, there was an increase in the total score of cardiotography, which in the first group of patients was 7.08 0.06 before treatment and 7.13 0.11 after treatment (p 0.05), and in the second group 7.12 0.11 before treatment and 7.95 0.05 after (p 0.05). The results of antenatal dopplerometry showed that in pregnant women who received Inestom as a part of the complex treatment, there was a decrease in the mean values of all vascular resistance indices. Conclusions. The use of Inestom in the complex treatment of placental insufficiency has a positive effect on the state of hemodynamics in the "mother-placenta-fetus" system, and affects the intrauterine state of the fetus.

This paper presents the positive characteristics of preoperative autohemochemotherapy associated with the drug &#34;Melaxen” as a synthetic analogue of the epiphysial hormone melatonin, produced by Unipharm-USA in the complex treatment of patients with ovarian cancer on the stage III-IV. The authors emphasize that the &#34;Melaxen&#34; has a powerful antioxidant, anticarcinogenic and adaptogenic action, reduces stress response and regulates neuroendocrine function and biological rhythms. The authors believe that neoadjuvant autohemotherapy by the &#34;Melaxen&#34; with a powerful immunomodulatory and lipoic effects, improves general non-specific resistance of the organism, has a positive effect on the immediate and remote results of complex treatment of patients. In comparison with polychemotherapy, the autohemotherapy in combination with the “Melaxen&#34; has the advantages. This therapy is significant in several basic methods of treatment of malignant tumors of the localization. It was found that with autohemochemotherapy by the &#34;Melaxen&#34; improves efficiency polychemotherapy, reduces its toxicity, consequently, clinically warranted and expedient in this connection, can be used as subsidiary means complex treatment of ovarian cancer recurrence in the spread process. The drug allows to improve the quality of life, to increase its duration in critically ill patients.

Aim. To evaluate the results of complex treatment with application of methods of correction of chronic venous insufficiency in patients with osteoarthritis of the knee joints in combination with varicose disease of the lower extremities. Methods. Examination of a cardio-vascular surgeon, orthopedist or rheumatologist, triplex scanning the veins of the lower extremities, arthrosonography and X-ray imaging of the knee joints, assessment of the severity of chronic venous insufficiency according to a CEAP scale [Clinical signs, Etiologic classification, Anatomic distribution, Pathophysiologic Dysfunction (1984)], and the degree of gonarthrosis according to the special Leken’s index and WOMAC index (Western Ontario and McMaster Universities Arthritis Index). The investigations were performed before treatment and repeatedly during the year in the course of treatment. According to the indications both conservative therapy and surgical treatment were used. Results. As a result of the complex treatment of patients achieved was a reduction in the severity of symptoms of gonarthrosis, which manifested with a decrease in the total value of the Leken’s index and the WOMAC functional index. Positive results were obtained in the group of patients who underwent surgery, and in the group of patients who received conservative treatment. Conclusion. Inclusion in the treatment of the methods aimed at eliminating manifestations of chronic venous insufficiency leads to improved results of treatment of osteoarthritis of the knee joints.

AbstractGaucher disease (GD) is a multi-systemic disease with a low population frequency. It is a lysosomal storage disorder (LSD) that causes accumulation of glucocerebroside in the so called Gaucher cells predominantly in areas like the spleen, liver and bone marrow. Type I GD (GDI) is the most common form and usually does not involve the brain and the spinal cord. The symptoms can range from mild to severe and may appear anytime from childhood to adulthood. Diagnostics can often be challenging and imposes looking at person’s medical history, symptoms, physical exam, and laboratory test results. We present a difficult to diagnose case of a 34-year-old woman admitted to the Clinic of Hematology, “Sv. I. Rilski” hospital with splenomegaly, normal laboratory findings and non-enlarged liver. She didn’t show focal neurological symptoms. A series of tests were assigned including genetic targeted analysis. The case is an example of a rare genetic disease with mild clinical symptoms. Diagnosis of Gaucher disease, type I was confirmed by measurement of a GBA enzyme activity and identification of mutations in the GBA gene inherited in an autosomal recessive manner. Thanks to the efforts of the clinical team, the assignment of adequate clinical and laboratory tests and their correct interpretation, the patient was subjected to enzyme replacement therapy (ERT). Although the diagnosis was settled relatively late (at 34 years of age), the correct therapy slowed down the invalidation and improved the quality of life of the patient.
