Introduction: The plate manufacturing production unit is one of the work units in PT. INKA (Persero), which involves the interaction between humans and machines in its activities, heavy equipment, and materials, all of which can cause possible hazard impacts that can impact the safety and health of workers. The purpose of this study is to conduct risk assessment on occupational safety and health aspects by identifying risks, assessing risks, identifying control efforts and assessing residual risk as a form of efforts to prevent occupational accidents and occupational diseases, using existing resources effectively and efficiently. Method: This research is a type of qualitative research, through interviews and observations, with cross-sectional studies and descriptive analysis. The interviewees for this study were K3LH management managers, steel managers, and machine operators in the plate production unit (PPL). The tools in this study werean interview guide, Job Safety Analysis (JSA) and Hazard Identification Risk Assessment Determining Control (HIRADC) using the AS / NZS 4360: 2004 Risk Management Worksheet Standard Risk Matrix. Results: From the research, it was found that there are 94 hazards for 11 different machines. Regarding the risk levels, there are 9 extreme risk levels, 46 high risk levels, 33 medium risk levels and 6 low risk levels. Conclusion: There are still 61 risks with medium risk level and 6 remaining risks with high risk level that still need control. Control efforts have been implemented by PT. INKA (Persero) in accordance with the hierarchy of control, such as the use of PPE and the provision of work SOPs. Keywords: hazard identification, risk management, risk assessment, risk control, residual risk

ABSTRACTProblems that arised when employees works at height are worker did not wear full body harness, lanyard did not hanged perfectly to handrail, did not works according to the procedure, and using unsafe scaffolding. Hazard identification and risk assessment used as prevention for accident when operating scaffolding. This study was an observational study with cross sectional design and analyzed descriptively. The design used for the study carried out by observing cause and effect within a period of time and space. Objects of this study were frame scaffolding and scaffolding PCH, while subjects of this study were SHE Officer, SHE supervisor, workers section structure, finishing, plumbing hydrant, and mechanical engineering. Results of hazard identification using JSA method founds 43 potential hazards originating from 4 different types of work in this project. The results of the risk assessment of 43 potential hazards have been found that 1 hazard have low risk, 40 hazards have moderate risk , and high risk hazard are 2. The dominant risk from the operation of the scaffolding was 40 potential hazards or 93% of the total potential hazards and high-risk hazard eventhough only 5% of all potential hazards remains a top priority and should be controlled soon to reduce the high and medium risk becomes low risk. The likelihood-based control is more possible to reduce risk level than severity-based control.Keywords: risk assessment, scaffolding operation

Introduction: Risk management is a management activity that aims to reduce risk to prevent negative impact. Calculation of the amount of risk was carried out to then determine whether the risk was acceptable. In printing activities, contact between humans and machines is unavoidable. The purpose of this study was to evaluate the risk of occupational safety management in the activity of print checking on stamping stage of plastic sacks production in Sidoarjo, Indonesia by conducting hazard identification, risk assessment which includes risk analysis and risk evaluation, control assessment, and residual risk assessment. This is an observational study with a cross sectional design. Methods: Data included primary and secondary data. Primary data were taken by interview and observation. Secondary data were taken from work accident data. Results: A total of 6 hazards were identified. Risk assessment recorded 2 low hazards category  2 medium hazards category, and 2 high risk hazards. Administrative control in the form of inspection and the use of  PPE (Personal Protective Equipment) were implemented. Residual risk assessment shows that the implementation of controls was not optimal. The results of the residual risk assessment indicate that several risk categories fell into a lower category i.e  4 low hazards category and 2 high risk category hazards. Conclusion: The applied risk management has not been optimally implemented. It is recommended that the implementation of inspections is scheduled and a system for recording and reporting inspection findings is established. Keywords: hazard, printing checks, risk management

Despite wide legislation basis of regulating relations in work safety and workers’ health, one third of workplaces demonstrate exceeded allowable normal levels of workers’ exposure to occupational hazards and present occupational risk for health disorders.In accordance to national legislation acts, evaluation should cover factors of occupational environment and working process, and occupational risk is understood in context of mandatory social insurance. This approach has been formed due to mostly compensatory trend in legal principles of work safety in Russia by now. Implementation of new preventive concept of work safety, based on idea of risk management for workers, necessitates development of legal acts that regulate requirements to evaluation of occupational risk and its reports with consideration of changes in Federal Law on 30 March 1999 №52 FZ “On sanitary epidemiologic well-being of population”.Those acts can include Sanitary Rules and Regulations “Evaluation of occupational risk for workers’ health”, that will contain main principles of risk assessment, requirements to risk assessment, including its characteristics which can serve as a basis of categorizing the risk levels with acceptability.To standardize requirements for informing a worker on the occupational risk, the expediency is specification of sanitary rules “Notifying a worker on occupational risk”. These rules should contain requirements: to a source of data on occupational risk level at workplace, to informational content and to ways of notifying the worker. Specification and implementation of the stated documents enable to fulfil legal requirements completely on work safety — that will provide preservation and increase of efficiency in using work resources.

BACKGROUND: The COVID-19 global pandemic brought several challenges to occupational safety and health practice. One of these is the need to (re)assess the occupational risks, particularly, biological risks. OBJECTIVE: The purpose of this work is to promote guidance to occupational safety and health practitioners when conducting a biological risk assessment in this context. METHODS: The main steps of the biological risk assessment are explained with some inputs regarding the novelty posed by SARS-CoV-2 and an example of a qualitative risk assessment method is presented. Also, its application to two different activities was exemplified. RESULTS: In both cases, the assessment considered that vulnerable workers were working from home or in medical leave. The results showed low or medium risk level for the assessed tasks. For medium risk level, additional controls are advised, such maintain social distancing, sanitize instruments/equipment before use, use proper and well-maintained PPE (when applicable), and promote awareness sessions to spread good practices at work. Employers must be aware of their obligations regarding biological risk assessment and OSH practitioners must be prepared to screen and link the abundance of scientific evidence generated following the outbreak, with the technical practice. CONCLUSIONS: This paper could be an important contribution to OSH practice since it highlights the need to (re)assess occupational risks, especially biological risk, to ensure a safe return to work, providing technical guidance.

ABSTRACTRevolution in the industry sector has been rapidly grown to fill up all the needs of the consumer products. One involves  supporting advanced machinery such as “Cutting, Skiving, Stitching, Emboss Logo, Roving, Punch Hole, Juki, BrushingEdge, Hammer Over Lapping and Two Molding”. In the factory production process, there are various types of high-risk activities, especially on line upper. The main of this research is to study the risk assessment on export companies line the upper part of the shoes export company using Job Safety Analysis. This research was conducted observational crosssectional design. Observations made to the hazards and control measures. Interviews were conducted to 12 employees. Variables in this research is production activity, hazard identification, risk assessment, risk control and residual risk. The results of hazard identification has been done, there are 91 known potential hazards, for risk assessment found 7 high risk and low risk 5. Machine classified as high risk on the risk assessment is roving machine, whereas low-risk is two molding machine. Control efforts on the upper line in accordance with the hierarchy of controlling a number of 91 controls, whereas for the residual risk still remains as much as 30 residual risk. Control has been applied quite well by pressing the consequences of hazards and risk management.Keywords: risk assessment, controlling, residual risk


The rapid development of industrial field giving a huge impact to the society or even in organizations however total number of accident is currently increase due to the lack awareness about the important of safety and health issue. This study focusing on safety and health issue and safety improvement at Palm Oil Mill at East Malaysia. The mill is produce two products which are crude oil and kernels. In order to identify the hazards in this mill, several methods had been used such as questionnaires distribution, interview session, observations, safety audit and body discomfort survey. Hazard Identification, Risk Assessment, and Risk Analysis (HIRARC) model were used in this study. The data collected and recorded for the risk and ergonomic assessments. Risk and ergonomic assessment is conducted after the hazards being identified in all 6 stations of Palm Oil Mill with total 33 activities were analyzed. Quantitative risk assessment was used by considering the likelihood and severity of the hazards. The values of risk were determined based on the table of likelihood and severity. All the hazards identified is sort into station and level of risk. The hazards with high value of risk is considered as the problem identification. Comprehensive discussion and analysis were done on the top three (3) high risk activities. The ergonomic assessments which are Rapid entire body assessment and body discomfort survey were used as a tools to support finding. HIRARC model has successful being used to analyze the hazard and risk exposure on the palm oil mill and action need to be taken on the high risk activities to prevent from an accident.

In 2012, the Korean Occupational Safety and Health Agency developed Chemical Hazard Risk Management (CHARM) as a risk assessment tool. This study aims to reorganize the CHARM technique by complementing its logical loopholes, while evaluating the risk to enterprises and verifying this technique by applying it to some enterprises in Korea. The optimized technique changed the method of quantitative assessment and evaluation criteria, matched the risk level with the required control level, and specified the use of control practice. For the target enterprises, for several assessment methods, risk levels, hazard bands, exposure bands, and the risk assessment results were derived, and the same types of options were compared. Fewer informational methods resulted in more conservative results of risk levels and hazard bands. Since the control status of the enterprises could not be confirmed and the substances handled at the target enterprises were limited in this study, a follow-up study should be performed with more target materials and additional information on the current control status of the enterprises.

Every practical activity carried out by student at Aircraft Engineering Departement, then these activities have risks that need mitigation measures. In safety risk management requires departement to conduct an assessment of the risks that can occur from an activity using hazard identification, risk assessment and mitigation. By identifying types of hazards, assessing risks, getting results in the form of risk matrix and risk level and identifying mitigation actions that can be taken and accepted. Furthermore, it produces hazard identification and risk assessment & mitigation (HIRAM) documents, and controls risks to acceptable and tolerable levels with mitigation measures. Safety risk management which is one of the standards in implementing safety management system in Aircraft Engineering Departement can be fulfilled.

Abstract Background Falls in hospital are adverse events with serious consequences for the patient. Fall risk assessment requires easy tools that are suitable for the specific clinical context. This is important to quickly identify preventing measures. The aim of the study is to identify an appropriate scale for assessing fall risk in patients from an emergency department. Methods For the fall risk assessment in the emergency department, three scales were identified in literature: Kinder 1, MEDFRAT, and Morse. MEDFRAT and Morse classify the patient in high, moderate, and low risk; Kinder 1 split patients “at risk” (also when there is only one positive item) and “non-risk” (in which all items are negative). The study was carried out in July 2019 in an Italian emergency department. Patients who arrived in triage were assessed for the fall risk using the three scales. Results On a sample of 318 patients, the used scales show different levels of fall risk. For Kinder 1, 83.02% is at risk and 16.98% is not at risk; for MEDFRAT, 14.78% is at high risk, 15.09% moderate, and 70.13% low risk; for Morse, 8.81% is at high risk, 35.53% moderate, and 56.66% low risk. As Kinder 1 implies as “high risk” that all items of the questionnaire are positive, to compare Kinder 1 to the other scales with three measurements, we assumed only one positive response as “moderate risk”, all negative responses as “low risk”. Thus, Kinder 1 shows no cases at high risk, 83.02% moderate risk, and 16.98% low risk. All the scales show that the moderate-high risk increases with age. MEDFRAT and Morse have concordant percentages for young (13.6%), elderly (61.2%), and long-lived (66.6%) people. Kinder 1, 59%, 96.7%, and 100%, respectively. Conclusions The comparison between scales shows inhomogeneity in identifying the level of risk. MEDFRAT and Morse appear more reliable and consistent. Key messages An appropriate assessment scale is important to identify the fall risk level. Identifying accurate fall risk levels allows for implementing specific prevention actions.

Background: Forklift is one of the most used craning plane in the manufacture and logistics industry. The operational of forklift can cause fatality work accident. PT SIGK uses forklift in the daily production activities, so it is necessary to identify hazard and risk assessment. Purpose: This study was to identify and assess the risk of physical and mechanical hazard at every stage of forklift operation. Methods: The type of this research was descriptive observational research, the object of the study were potential hazard and risk of all forklift operation stages in the AAC production area, consisted of 4 stages which were initial inspections stage, operational preparation stage, operational stage and post operations stage. Results: The result showed that there were 25 hazards, which were 4 hazards at the initial inspections stage, 2 hazards at the operational preparations stage, 14 hazards at the operational stage and 5 hazards at the post operations stage. Initial risk assessment obtained 2 hazards with high risk level, 22 hazards with medium risk level and 1 hazard with low risk level. The most prevalent hazard controls performed by PT SIGK were administrative (SOP, for klift training, safety sign) and PPE (helmet, gloves, mask), the controls got the value of effectiveness between 50% to 75%. Residual risk assessment, obtained 5 hazards with medium risk level and 20 hazards with low risk level. Conclusion: Additional control should be done by the company to reduce the residual risk level of medium category to be low, if the control recommendation is implemented well then the overall level of residual risk with low category will be obtained.
