Purpose. In experiment in vivo to study the features of regeneration of the conjunctiva and sclera of rats after surgery with intraoperative application of a 0.05% Ciclosporin A.Materials and methods. Еxperimental animals (rats) (n = 48) were divided into the main group, including the subgroups a (n = 16) and b (n = 16) and the comparison group (n = 16). Performed a through cut of the conjunctiva and damage to the surface layers of the sclera one of the eyes of all animals. Further on the surgical trauma zone in the main group, the intraoperative application of the cytostatic was performed. In the subgroup a with a duration of 3 minutes, in the subgroup b – 6 minutes. In the comparison group a hemostatic sponge without a cytostatic was used intraoperatively.Results. In the comparison group postoperative period proceeds with a stereotyped dynamics of cell phase changes in damaged tissues. In the end the development of dense conjunctival-scleral fusion in the area of surgical trauma was noted. Intraoperative application of 0.05% Cyclosporine A leads to a slowing of regeneration, preventing formation of rough conjunctival-scleral scar.Conclusions. Intraoperative applications of 0.05% Cyclosporin A change the stereotyped dynamics of the inflammatory-reparative regeneration in the surgical intervention zone, inhibiting the migration of cells almost in 3 times and significantly (in 2 times) prolonging the duration of the macrophage phase. This causes a slowdown of reparative regeneration, prevents excessive scarring in the operating area.

Goal. Improving the scheme of treatment and prevention of periodontal diseases in workers of the woodworking industry of Chernivtsi region, who in the process of professional activity have long-term contact with unfavorable factors of the production environment. Materials and methods. 70 workers of the woodworking industry with diagnosed chronic generalized periodontitis of the I degree were treated (35 people – the main group (A); 35 people – comparison group (B)), age – from 25 to 45 years. To compare the results of laboratory studies, an additional survey of 25 healthy individuals of the same age with an intact periodontium were conducted. They formed the control group (C). Clinical examination of patients was performed according to standart methods: subjective (complaints, medical history, life history) and objective (examination, periodontal indices, determination of the level of gingival attachment). Laboratory methods of research included determination of urease and lysozyme activity in saliva, degree of dysbiosis of oral cavity. As maintenance therapy, patients of the main group were prescribed the proposed composition DEPE. Results. After the treatment, a significant improvement in the hygienic status of patients in both groups was observed, but difference between groups A and B in the indicators of oral hygiene after treatment was no statistically significant (pA2–B2 > 0.05). There was a significant improvement in periodontal indices after treatment in patients of both groups (PMA index according to C. Parma, bleeding index according to Muhlemann in Cowell I. modification, Russell periodontal index, PSR-test), but the indicators in patients of the main group were significantly better – pA2–B2 < 0.05. In patients of the main group (A) the level of attachment loss decreased by 1.83 times, comparison group (B) – by 1.71 times. The difference in levels of attachment loss after treatment between groups A and B is statistically significant (pA2–B2 < 0.05). The activity of the enzyme urease in patients of the main group (A) and comparison group (B) after treatment was significantly reduced (pA1–A2 < 0.001, pB1–B2 < 0.001), but only in the main group it reached the level in the control group (C) (pA2–C > 0.05). The level of lysozyme after treatment was significantly increased in patients of both observation groups (pA1–A2 < 0.001, pB1–B2 < 0.001), and reached the level of lysozyme in patients of the control group (C) (pA2–C > 0.05, pB2–C > 0.05). The degree of oral dysbiosis in patients of the main group (A) was significantly reduced by 5.43 times (pA1–A2 < 0.001), and its difference from the degree of dysbiosis in patients of the control group (C) is statistically insignificant (pA2–C > 0.05 ). The degree of oral dysbiosis in patients of the comparison group (B) was significantly reduced by 3.04 times (pB1–B2 < 0.001), but still remained significantly different from oral dysbiosis in patients of the control group (C) (pB2–C < 0.05). Conclusion. The proposed pharmacological composition DEPE is an effective antiseptic solution and can be used in the phase of maintenance therapy to improve the treatment and prevention of periodontal disease in workers of woodworking industry. Введение. Эндотелиально-эпителиальная дистрофия (ЭЭД) роговицы - тяжелое, прогрессирующее заболевание, сопровождающееся хроническим отеком всех слоев ткани роговой оболочки, болевыми ощущениями и значительным снижением остроты зрения. Радикальным способом лечения ЭЭД является кератопластика, однако, любая операция сопряжена с высоким риском развития гнойных осложнений и отторжения трансплантата в послеоперационном периоде. Альтернативой радикальной хирургии является клеточная терапия. Среди клеточных популяций наиболее широко используются мононуклеарные лейкоциты крови, вырабатывающие свыше 120 видов биологически активных веществ - цитокинов. Цель исследования - изучение особенностей развития экспериментально индуцированной эндотелиально-эпителиальной дистрофии роговицы и оценка эффективности лечения заболевания методом наслоения суспензии аутологичных мононуклеарных лейкоцитов на внутреннюю поверхность патологически измененной роговой оболочки. Методика. Исследования выполнены на 54 крысах-самцах Wistar. На 1-м этапе у всех животных моделировали ЭЭД путем повреждения и удаления эндотелиального слоя роговицы. На 2-м этапе у 6 крыс энуклеировали пораженный глаз для морфологического исследования. Остальных животных разделяли на основную группу и группу сравнения в зависимости от метода лечения: 24 крысам основной группы наслаивали суспензию аутологичных мононуклеарных лейкоцитов на поврежденную внутреннюю поверхность роговой оболочки, 24 крысам группы сравнения проводили инстилляции препаратов Корнерегеля 3 раза и Баларпана 2 раза в сутки. Результаты. Развитие ЭЭД роговицы in vivo подтверждалось при наружном осмотре и данными оптической когерентной томографии роговой оболочки и световой микроскопии. Наблюдалось увеличение толщины роговой оболочки за счет диффузного отека (до 382±53 мкм), развитие баллонной дистрофии переднего эпителия, гидратация и местами расслоение отечной жидкостью коллагеновых волокон собственного вещества роговицы. В эксперименте in vivo также установлено, что механическое повреждение и удаление эндотелия роговицы сопровождается вторичной альтерацией всех ее слоев с развитием ЭЭД в течение 2 нед. На модели ЭЭД роговицы апробирован новый метод лечения заболевания, показавший значительную эффективность. Установлено, что наслоение суспензии способствует быстрому (в 13,0 раз) уменьшению гидратации основного вещества роговицы к 21-м сут от начала лечения, нормализации строения переднего эпителия и уменьшения его толщины (в 1,3 раза) на 21-е сут наблюдения Заключение. Результаты морфологического и инструментального исследования подтверждают эффективность использования метода наслоения суспензии аутологичных мононуклеарных лейкоцитов на поврежденную внутреннюю поверхность роговой оболочки больного глаза при лечении ЭЭД роговицы.Introduction. Bullous keratopathy is a serious, progressive disease, which involves chronic edema of the corneal tissue associated with a significant decrease in visual acuity and severe pain. Current treatment of corneal edema includes conservative and surgical methods. Keratoplasty is a radical treatment; however, this method is associated with a high risk of purulent complications and graft rejection in the postoperative period. Cell therapy is an alternative to the radical surgery. Among cell populations, the most widely used are mononuclear white blood cells producing more than 120 types of biologically active substances, such as cytokines. The aim was to study features of the development of experimentally-induced bullous keratopathy and to evaluate the effectiveness of treatment by layering a suspension of autologous mononuclear leukocytes on the inner surface of pathologically altered cornea. Methods. The study was performed on 54 Wistar male rats. At the 1st stage of experiment, bullous keratopathy was modeled on all animals by damaging and removing the corneal endothelium. At the 2nd stage of experiment the animals were divided nto the main group and the comparison group based on the treatment method. In the main group (n=24), a suspension of autologous mononuclear leukocytes was layered on the damaged surface of the cornea; in the comparison group (n=24, Korneregel was instilled three times daily and Balarpan was instilled twice a day. Results. The development of experimental bullous keratopathy was confirmed by external examination, optical coherence tomography of the cornea, and light microscopy. Mechanical damage and removal of the corneal endothelium was associated with secondary alterations in all corneal layers with a rapid (within 2 weeks) development of bullous keratopathy. The new therapy tested on the model of corneal bullous keratopathy proved to be highly effective. The suspension layering provided a rapid (13.0 times faster than in comparison group) decrease in corneal hydration by day 21 of the treatment, improved the structure of anterior corneal epithelium, and decreased its thickness (1.3 times) by day 21 of follow-up. Conclusion. The study results obtained with morphological and instrumental methods confirmed the effectiveness of layering the suspension of autologous mononuclear leukocytes on the damaged inner surface of the cornea in the treatment of bullous keratopathy.

Summary. The purpose of the study is to establish the indications and contraindications for the removal of foreign bodies of lungs of gunshot origin. Materials and methods. In the surgical department of the surgical clinic of the Military Medical Clinical Center of the Northern Region, 207 wounded who received gunshot wounds penetrating the chest were examined. The wounded were divided into 2 groups depending on the operational tactics used. The main group of 97 wounded who were treated using modern video endoscopic technology and magnetic surgical instruments. The comparison group consisted of 110 wounded who received traditional treatment. Results and their discussion. Using video endoscopic technologies and magnetic surgical instruments during surgery, 42 (54.5%) in the main group were removed, in the comparison group — 23 (27.4%) foreign bodies. The amount of surgery should not exceed the amount of damage that minimizes surgical trauma. The article presents indications and contraindications to the removal of foreign bodies of pulmonary origin. The differential approach allows to determine the surgical tactics depending on the location of the foreign body, its size and the fit of foreign bodies to the vessels, bronchi, mediastinum. Adherence to the rules of removal of foreign bodies of the lung, depending on the location of its location and size leads to a decrease in inappropriate surgical interventions and postoperative complications. Conclusions. 1. The distribution of foreign bodies by location and size allows you to clearly determine the tactics of treatment and avoid postoperative complications. 2. Surgical interventions to remove foreign bodies should be performed in the first days after stabilization of the patient’s condition using minimally invasive technologies.

Abstract Immunosuppressive agents have been recognized as factors that induce changes and modifications in bone metabolism. The purpose of this study was to evaluate the effect of ozonated plant extracts (herein termed ozonated oil) under the influence of Cyclosporin A (CsA) on osseointegration. A total of 20 dental implants were placed in 20 rabbit tibiae assigned to Group A or B. CsA was injected at an immunosuppressive dose in Groups A and B as a single-dose treatment. At the day of surgery, Group A received a single topical ozonated oil treatment (0.55 mL) around dental implants; Group B, the control group, received no ozonated oil. Animals were sacrificed after 8 weeks. Radiographs were obtained at implant surgery and on the day of sacrifice. Bone quality was compared between the 2 groups. Radiographically, osseointegration was microscopically evaluated using scanning electron and light microscopies. In ozonated Group A specimens, light microscopic examination demonstrated evidence of more organized mature bone compared with Group B. Within the limits of this study, the results suggest that short-term administration of CsA, when administered with topical ozonated oil, may influence bone density and the quality of dental implant osseointegration. Therefore, topically applied ozonated oil may influence bone density and the quality of osseointegration around dental implants.

Mammary gland prosthesis rotation (MGPR) after augmentation mammoplasty is a poorly studied complication of plastic surgery. The frequency of MGPR is not well known. There are indications that MGPR of 90○ , which is considered clinically significant, ranges from 0.9 % to 14 %. Preventing methods of MGPR are not developed enough. Aim: determination of the effectiveness of the upper pole reinforcement of «neo pocket» with prolen thread for the prevention of MGPR after primary augmentative mammoplasty (PAMP) in three years of postoperative period. Material and methods. The invectigation is performed on the basis of the Surgery Department No. 4 of Bogomolets NMU in the period from 2013 to 2018. A prospective, independent comparison of the primary augmentative mammaplasty results between three independent groups of patients was performed: two groups of comparison and one main groups. The first group of comparison (Group P1) included 94 patients who evaluated the results of augmentative mammaplasty one year after the operation, the second comparison group (Group P3) – 63 patients three years after this operation. The main group (group O) included 76 patients who had a prospective study after one (group O1) and then three years later (group O3) – 62 patients. In the comparison groups, the standard MGPR was performed, in the main group – according to the developed method. The patients did not differ statistically by age, body mass index (BMI), weight of prostheses, percentage of pregnant women and patients with lactation period. In all the cases, textured prostheses with high and medium profile were implanted. The majority of patients were operated with prostheses from 325-335 ml and 375 ml. The MGPR evaluation was carried out in accordance with the developed method using the ultrasound of the ESAOTE firmware model Technos Partner with a linear sensor with frequency of 12.5 MHz. Data analysis was performed using the IBM SPSS Statistics 22 statistical analysis packets. Results. One year after the operation in the P1 group, a slightly lower, but not reliable percentage of prosthesis rotation – 143 (76.1 %) against 103 (81.7 %) in the group P3, p=0.231, was diagnosed. At the same time, in the P1 group only 4 (4.3 %) women did not have the rotation of the prosthetics of both mammary gland (MG) and in the P3 group 2 women (3.2 %), p=0.001. Significant difference of the frequency of patients with clinically significant MGPR: 11 (11.7 %) patients in the P1 group versus 12 (19.0 %) in the P3 group (p = 0.202) and 13 (6.9 %) in the P1 group against 13 (10.3 %) of the prostheses in the group P3 (p=0.284). In patients in the main group, a small (30.9%) frequency of clinically insignificant MGPR was noticed after PAMP and it did not increase after the three years period (32.1%). In the P1 group, 143 (76.1 %) prostheses were diagnosed with rotation, in the group O1 – 46 (30.3 %), p = 0.001. At the same time, in the P1 group, only 4 (1.6 %) women didn`t have the rotation of the of both MG prostheses and in the group – 39 (51.3 %), p = 0.001, which indicates the effectiveness of the developed methodology. A similar trend was observed in the case of comparison of the three years results between the group P3 and the group O3. The absence of rotation of prostheses was diagnosed in 83 (66.9 %) O3 group and 26 (20.6 %) in the P group, and there was no clinically significant rotation of O3 patients versus group P where these patients were observed: 12 (19.0% ), p=0.001. Conclusion: the method of fixation of the submuscular «neo pocket» upper pole with prolen thread, provides stable protective effect of MGPR and gives a possibility to avoid a clinically significant rotation(more than 900) throughout three years after augmentative mammoplasty comparing with a traditional method – 11.7 %, patients in a year – 19 %, in three years after PAMP, р<0,01.

The objective: optimization of treatment of clinical symptoms and prevention of relapse infetsii virus (HSV, HPV) using immunomodulatory therapy alloferon. Patients and methods. The study included 50 patients aged 20 to 42 years with clinical manifestations of HPV in the form of warts of vulva, vagina, cervix, cervical dysplasia. Patients were divided into two groups according to the treatment received – the main group received Allokin-Alpha 1ml subcutaneous 6 injections, the comparison group – a-2b interferon 3 million 10 injections. Results. The group of patients treated with Allokin-аlfa, showed better clinical performance treatment of genital kondilomas lesions, decrease in the number of lesions in 6–12 days after initiation of therapy, decrease in the degree of dysplastic changes in the cervix, and the absence of relapse (in the control group 32% of patients had a relapse). No side effects as a result of Allokin-alpha has not been registered, while 76% of patients receiving a-interferon-2b expressed flu-like symptoms were observed, requiring additional purpose of detoxification drugs. Test results: in the main group had the elimination of HPV in 92% of cases, 78% in the control group; herpes – elimination of 96% of the cases in the main group and 75% in the comparison group. Conclusion The medicine Allokin-alpha is highly effective in the treatment of HPV and HSV infection, demonstrating good results of therapy, no relapse of infection. The recommended regimen – 6 injections of the drug prior to the electrosurgical treatment. Key words: human papillomavirus infection, herpes, Allokin-alpha, a-interferon-2b.

The aim: Evaluation of the effectiveness of the biotherapeutic agent Saccharomyces boulardii in the treatment complex of patients with clinical manifestations of dysbiosis and/or signs of enteric insufficiency refractory to previous therapy, to develop ways for their medical correction. Materials and methods: The study included 209 patients, treated during 2018-2020, grouped into main and the comparison group. The main group was divided into subgroups depending on the nosological form of the underlying disease: surgical – 36, general therapeutic – 58, gynecological – 47 patients. Main group treatment was supplemented with Saccharomyces boulardii for 10-14 days from the first day of inclusion of patients in the observation group. Results: The analysis of motor-evacuation disorders verified flatulence in 89.47%, localized epigastric pain in 22.48%, diarrhea in 55.02%, and constipation in 11.48% of cases. Based on the results of laboratory parameters, an absolute increase in the number of leukocytes in the range from 12.4 ± 1.8 x 109 / l to 14.7 ± 2.8 x 109 / l with a neutrophilic formula shift, bilirubinemia to 54.4 ± 12.2 mmol / l , elevated levels of urea to 14.7 ± 3.9 and creatinine to 0.199 ± 0.07 mmol / L. Conclusions: After completion of the treatment course (main group), a significant (p <0.05) improvement in the general condition (decrease in stool frequency within 1–4 times (2.3 ± 0.28) per day, the absence of rumbling, flatulence and pain in abdomen) in 131 (92.9%) of 141 patients included in the study was stated. Clinical improvement was confirmed by bacteriological studies of copro-culture.

Aim. To analyze the clinical and functional results of the combined technology for laser treatment of patients with the initial stages of idiopathic epiretinal membrane.Methods. This study included 3 groups of patients: the main group (32 patients) after the combined laser technology (subthreshold “grid” laser photocoagulation (LP) and subthreshold micropulse LP); control group (30 patients) which did not receive any treatment; comparison group (30 patients) after subthreshold “grid” LP. The assessment uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), central retinal thickness (CRT) and central retinal sensitivity (CRS) was performed. The follow-up period was 5 years.Results. In the control group a slow decrease of UCVA, BCVA, CRS (p < 0.05) and an increasing CRT (p < 0.05) were observed. In the main group maximal UCVA, BCVA, CRS (p < 0.05) were marked from 3 to 6 months after laser treatment and there was a gradual decrease in the CRT indices. In the comparison group, a short-term positive effect was achieved up to 3 months with an increase of UCVA, BCVA, CRS (p < 0.05). 12 months post op there was an increase in CRT with a decrease of UCVA, BCVA, and CRS until the end of study.Conclusion. The technology of combined laser treatment showed high efficiency in control and comparison groups, with stabilization/increase of UCVA, BCVA, CRS and stabilization/improvement of the morphological parameters of the retina – the decrease of CRT with ERM involution.

This study examines the expression of selflessness – the tendency to ignore one’s own needs and serve others’ needs – in Rorschach protocols of women suffering from anorexia nervosa. The protocols of 35 women suffering from anorexia nervosa were compared to 30 protocols of a psychiatric comparison group. A multivariate analysis of variance over five variables (AG, PER, PHR, COP, and GHR) was significant: Anorexic patients showed higher characteristics of selflessness compared to the psychiatric comparison group. These findings contribute to the validation of the Rorschach technique and to the clinical observation of selflessness in anorexic patients, and they emphasize specific characteristics in the treatment of anorexia nervosa patients.
