Iliopsoas Abscess (IA) is a rare condition that predominantly affects young adults. It is a purulent collection that forms inside the iliopsoas muscle for several reasons. The primary form is due to hematogenous or lymphatic dissemination and the secondary form is the direct expansion of an infectious focus near the iliopsoas muscle. It is an insidious disease with a dragged clinical course, presenting with fever, thigh pain associated with abdominal and/or lower back pain, and there may be limitation of movements in the hip joint. The aim of this study is to demonstrate a case of IA in a 32-year-old female patient, admitted to the Palmas General Public Hospital (HGPP). After treatment, the patient evolved with symptomatic improvement and good clinical outcome.

Perforation of descending colon cancer combined with iliopsoas abscess and fistula formation is a rare condition and has been reported few times. A 67-year-old man came to our first aid for an acute pain in the left iliac fossa, in the flank, and in the ipsilateral thigh. Ultrasonography and computed tomography revealed a left abdominal wall, retroperitoneal, and iliopsoas abscess that also involved the ipsilateral obturator muscle. It proceeded with an exploratory laparotomy that showed a tumor of the descending colon adhered and perforated in the retroperitoneum with abscess of the iliopsoas muscle on the left-hand side, with presence of a fistula and liver metastases. A left hemicolectomy with drainage of the broad abscess was performed. Pathologic report findings determined adenocarcinoma of the resected colon.

Introduction: The iliopsoas muscle is a rare place for an abscess to collect. While these abscesses can have high mortality, they are often misdiagnosed. The use of point-of-care ultrasound (POCUS) can aid in earlier diagnosis. Case Report: A 45-year-old male presented to the emergency department (ED) with severe lower back pain. The pain radiated to both of his legs and was associated with fever, weight loss, and malaise. The differential diagnosis for this patient was broad. A POCUS was performed at the bedside and revealed bilateral iliopsoas abscesses. This finding was then confirmed by computed tomography. Conclusion: In this case report we will discuss how to identify an iliopsoas abscess using POCUS in ED patients, and the utility of POCUS to facilitate an expedited diagnosis.

Calf enlargement following sciatica is a rare condition. It is reported the case of a 28-year-old woman who complained of repeated episodes of lower back pain radiating into the left buttock and foot. One year after the beginning of her symptoms, she noticed enlargement of her left calf. X-ray studies disclosed L5-S1 disk degeneration. EMG showed muscle denervation with normal motor conduction velocity. Open biopsies of the gastrocnemius muscles were performed. The left gastrocnemius muscle showed hypertrophic type 2 fibers in comparison with the right gastrocnemius. Electron microscopy showed mildly increased number of mitochondria in these fibers. A satisfactory explanation for denervation hypertrophy has yet to be provided.

One of the diseases leading to chronic end-stage renal disease is membranous nephropathy (MN). The main cause of this disease is the formation of antibodies to foreign and native antigens. Membranous nephropathy can be conventionally divided into 2 types: primary form (when the primary disease is unknown) and secondary form. Detection of appropriate antibodies is one of the methods to recognize and differentiate primary and secondary forms. A large role in non-invasive diagnosis of MN and differentiation of the primary form from the secondary play antinuclear antibodies (ANA), antibodies against granulocyte cytoplasm (ANCA), antiglomerular basement antibodies (anti-GBM) and phospholipase A2 receptor antibodies (anti-PLA2R). Differentiation matters when choosing a treatment choice. In the primary form, it is immunosuppression, and in the form of secondary treatment, it consists in curing or controlling diseases that can cause symptoms of MN. The aim: Analysis of serological methods helpful in immunodiagnosis of membranous nephropathy.

We report a case of necrotising fasciitis caused by metastatic endometrial adenocarcinoma. Metastases to the lumbar spine with local invasion to the iliopsoas muscle led to an iliopsoas abscess, which subsequently progressed to necrotising fasciitis of the flank. This patient lacked common risk factors for necrotising fasciitis. There are no previous reports in the literature of necrotising fasciitis with this aetiology. We discuss the available evidence for aetiology of and risk factors for necrotising fasciitis, and the relation of time to surgery with prognosis.

AbstractThis study investigates the effects of token item frequency in Structured Input activities on both a primary target form (Spanish accusative clitics) and a secondary target form (Spanish dative clitics). Participants included 460 adult learners enrolled in a beginning-level Spanish language course and they were exposed to either 40, 60, 80, 100, 120, or 140 target form tokens. This study included a pretest, immediate posttest, and a delayed posttest measuring interpretation and production of both primary and secondary target forms. Findings reveal that primary form interpretation effects across all frequencies, however, production findings present themselves with the 60 and 80 token groups only. Secondary form interpretation findings reveal themselves across all frequency levels with the exception of the lowest frequency investigated (40 tokens) and secondary form production mirror those found in previous studies on the same forms. As such, we discuss the theoretical and methodological ramifications of these findings as well as directions for future research.

AbstractSeptic arthritis of the facet joint is an extremely rare condition, even more in young immunocompetent patients. There have been approximately only 40 cases of this entity reported worldwide. Here, the authors present a 16-year-old male patient with lower back pain, fever, and cephalea. Blood cultures and cerebrospinal fluid analysis confirmed methicillin-sensitive Staphylococcus aureus infection, and magnetic resonance imaging showed septic arthritis of the lumbar left facet joint L3 and epidural abscess. Intravenous oxacillin was administered, and the patient improved. No other treatment was required.

As an entomopathogenic bacterium, Xenorhabdus nematophila (X. nematophila) can be parasitic in the gut of entomopathogenic nematodes. Compared with the primary form of X. nematophila, its secondary form has more differences in physiological characteristics and metabolic pathways, such as insecticide, antibacterial, anti-cancer and other functions. Many studies about the application of X. nematophila as biological agents in ecological environment have been reported. However, its pathogenic mechanism and specific functional molecules remain to be studied. Therefore, this study is dedicated to research differential proteins between the primary and the secondary bacteria with iTRAQ technique to further explore the functional mechanism and functional proteins of X. nematophila. A total number of 367 differentially expressed proteins were detected with iTRAQ technique. Besides Xenocoumacin and Xnph1 proteins that had been widely studied by some scholars, other specific proteins in primary bacteria excavated are as follows: Xcn and Xnph1 proteins with antibacterial property, proteins related to the attachment of X. nematophila to the host insects including flagellin, fimbrial adaptor, fimbrial adhesion and capsular synthesis regulator component B, glutamate synthase that plays an important role in multiple metabolic pathways in the primary bacteria, multidrug resistance secretion proteins with the property of tumor drug resistance and defense effects against cytotoxic compounds produced by normal cells and malignant cells.

Abstract OBJECTIVE: Intraspinal endometriosis is an extremely rare condition with characteristic symptoms, including lower back pain that increases in severity during each menstrual cycle. METHODS: Here, we report a case of endometriosis involving the conus cauda region. This patient presented with acute deterioration secondary to hemorrhage. We also review the relevant literature. RESULTS: Magnetic resonance imaging scans of the dorsolumbar region showed a mass lesion within the spinal canal at the L1–L2 level with evidence of acute hemorrhage. The patient underwent an emergency D12–L2 laminectomy and microdecompression of the lesion. The histological and immunohistochemical features were characteristic of intraspinal endometriosis. CONCLUSION: Intraspinal endometriosis must be recognized as a potential cause of periodic neurological signs and symptoms in young and middle-aged women.
