Abstract Background Type 2 diabetes mellitus poses a major health challenge worldwide and in low-income countries such as Bangladesh, however little is known about the care-seeking of people with diabetes. We sought to understand the factors that affect care-seeking and diabetes management in rural Bangladesh in order to make recommendations as to how care could be better delivered. Methods Survey data from a community-based random sample of 12,047 adults aged 30 years and above identified 292 individuals with a self-reported prior diagnosis of diabetes. Data on health seeking practices regarding testing, medical advice, medication and use of non-allopathic medicine were gathered from these 292 individuals. Qualitative semi-structured interviews and focus group discussions with people with diabetes and semi-structured interviews with health workers explored care-seeking behaviour, management of diabetes and perceptions on quality of care. We explore quality of care using the WHO model with the following domains: safe, effective, patient-centred, timely, equitable and efficient. Results People with diabetes who are aware of their diabetic status do seek care but access, particularly to specialist diabetes services, is hindered by costs, time, crowded conditions and distance. Locally available services, while more accessible, lack infrastructure and expertise. Women are less likely to be diagnosed with diabetes and attend specialist services. Furthermore costs of care and dissatisfaction with health care providers affect medication adherence. Conclusion People with diabetes often make a trade-off between seeking locally available accessible care and specialised care which is more difficult to access. It is vital that health services respond to the needs of patients by building the capacity of local health providers and consider practical ways of supporting diabetes care. e18223 Background: Cancer survivor rates have increased over the past few decades leading to a growing interest in research related to quality of life. The American society of clinical oncology’s updated guidelines of 2013 recommend that health care providers discuss the possibility of infertility with patients and present fertility preservation options to those who express interest. We attempted to explore the unique barriers that might prevent adult male cancer patients from accessing sperm cryopreservation in Pakistan Methods: We conducted semi-structured interviews of male cancer patients aged 18-45 years, diagnosed with cancer of any stage or type. The interviews were audio-recorded in Urdu and translated to English, following which they were transcribed ad verbatim. The topics included quality of information received regarding the risk of infertility following chemotherapy, future reproductive choices and barriers to sperm cryopreservation Results: Out of the 25 patients interviewed, there were n = 10 cases of Leukemia, n = 3 of Lymphoma, n = 2 cases each of colorectal carcinoma and Multiple Myeloma, n = 1 case each of Neuroblastoma and Osteosarcoma, and solitary cases involving the lung, breast, thymus, brain, jaw and testis. Only n = 4 patients knew about the potential for infertility due to cancer chemotherapy, all of whom were also aware of the option of sperm cryopreservation. Two patients had their sperm preserved prior to the initiation of chemotherapy. Perceived treatment related expenses appeared to be the biggest barrier to sperm cryopreservation for n = 9 patients (36%). This was followed by lack of information which was cited by n = 8 patients (32%) and religious reasons (n = 2 patients, 8.3%). Other barriers were identified as family wishes, female gender of the doctor and patient’s preferences. Four patients stated there are no barriers. Conclusions: There is a significant lack of awareness among male cancer patients regarding threat to fertility following cancer treatment. It is imperative that physicians inform them of this and discuss treatment options, along with addressing potential barriers.

Abstract Aging is a global phenomenon, and the population of elderly persons continues to increase in many communities around the world. Ghana is not an exception. As the demographics are changing, it is vital that health care providers are trained to effectively diagnose, care for, and manage aging-related conditions to improve the quality of life of the elderly. The aim of this study is to explore the underlying reasons for medical students’ interest in geriatrics and their intentions, likelihood of selecting geriatrics as a specialty for future practice. This exploratory study involved (n =12) fourth, fifth and sixth year medical students in a Ghanaian public university. Semi-structured interviews guided by the theory of planned behavior were conducted. The findings show a lack of geriatric education or training for students in this institution and region. Students’ interest and preferences for geriatrics were mainly influenced by the lack of knowledge and exposure to the specialty, as well as what it entailed to be a geriatrician. Although some students expressed interest in working with older patients, none had the intention to pursue geriatrics as a career option. Creating positive exposure to geriatrics through educational and vocational opportunities are necessary. The findings from this study can be instrumental in developing a knowledge-base of aging and caring for the elderly. Findings can also serve as the foundation for a more comprehensive study, leading to effective geriatrics education interventions in Ghana and Africa at large.

Little is known about the coping strategies used by elite athletes suffering from mental health issues. Therefore, this study examined coping strategies implemented by elite athletes facing clinical depression. Semi-structured interviews were conducted with four elite athletes and analysed using Interpretative Phenomenological Analysis (IPA). Results present a broad picture of how elite athletes tried to cope with depression using a range of coping strategies. Among the different strategies highlighted, talking, seeking professional help and social support were particularly emphasized by the participants. Surprisingly, however, only one participant reported transferring the skills and strategies learned on her way to the top to many other aspects of her everyday life such as coping with her depression. Findings, therefore, suggest that athletes should be encouraged to transfer and make the most of the skills learned throughout their sport career to deal with their daily life. Future research perspectives and implications are discussed.
