A Comprehensive Strategic Analysis of the Environmental Scanning of Iranian Public Hospitals: A Prospective Approach

Abstract Objectives: This study was conducted to interpret the micro and macro environmental issues of public hospitals in order to equip hospital managers to plan and perform proactively and could deal more better with uncertain future. Results: semi-structured in-depth interview and a researcher-made questionnaire were used to determine macro and micro level issues based on PESTLE (Political, Economic, Social, Technological, Legal and Environmental) and Douglas West framework in order to determine the effectiveness and feasibility of factors The key issues at micro level environment were related to prescription and overuse of pharmaceuticals, inequality in distribution of healthcare services and high demands for luxurious services. At macro level: changes in disease patterns, inappropriate hospital budgeting, economic sanctions, government corruption and centralization had a great impact on public hospitals in Iran. following the safety guidelines laid down by the government and better management of bed distribution in order to decrease waiting times at micro level and improve strategic policies in order to have quality-based payment system, enhance the efficiency and effectiveness of services and strategic purchasing are the most important ways at macro level. At this time, the considerable impact of economic sanctions on hospital financial resources should not be ignored.

Abstract Objectives: This study was conducted to provide a strategic direction to public hospitals in Iran via environmental scanning in order to equip hospitals to plan and perform proactively and adapt with the ever-changing environment. Results: A mixed method study including in-depth interview and survey were used to determine influential environmental factors based on PESTLE (Political, Economic, Social, Technological, Legal and Environmental) and Douglas West framework (to determine the effectiveness and feasibility of factors). Issues identified at micro environmental level were over prescription, inequality in distribution of healthcare services and high demands for luxurious health services. Issues identified at the macro environmental level were related to changes in disease patterns, inappropriate hospital budgeting, economic sanctions, government corruption and healthcare centralization. In order to tackle the issues identified, it is paramount to enhance bed distribution management, improve strategic policies for a more equitable payment system, and enhance the efficiency and effectiveness of services by implementing a strategic inventory control. Furthermore, the considerable impact of economic sanctions on financial resources of Iranian hospitals should not be ignored.

Results: structured interview using Douglas West framework and a researcher-made questionnaire were used to determine the effectiveness and feasibility of hospitals’ environmental factors. The key issues at micro level environment of public hospitals were related to prescription and overuse of pharmaceuticals, inequality in distribution of healthcare services and high demands for luxurious services. At macro level: higher fertility rates, hospital services tariffs, changes in the patterns of diseases and inappropriate hospital budgeting have direct impact and economic sanctions, government corruption, centralization and high bank interest rates have indirect impact on public hospitals in Iran. Generally the strategic policies are required to improve quality-based payment system, enhance the efficiency and effectiveness of services, improving fair income generation and urging positive motivations for service providers are the strategies to cope with the future changes. The new challenges of globalisation have made it essential to achieve and sustain a high level of efficiency in the provision of healthcare. Even though the Government of Maharashtra clearly views assuring efficiency and providing public health services to the people as an important policy objective, data reveals that the amount of resources spent on public healthcare especially public hospitals is very meagre as compared to the needs of the population. This study attempts to understand the shift from public to the private health sector by the users of health services in the city of Mumbai. This changing trend towards increasing use of private hospitals for healthcare services was analysed to understand the specific reasons behind the privatisation of health services in this metropolitan city. Using primary survey, the analysis revealed that majority of the users preferred private hospital services due to various reasons like cleanliness, better infrastructure, efficient and competent doctors and other staff. The results also indicated significant inefficiencies in the functioning of the public hospitals as noted from the responses of the users.

In the insurance industry, the majority of fraud and abuse cases fall into a limited number of patterns, yet false claims normally lead to negative national, local, and organizational effects. Through monitoring the exploitative and abusive behavior commonly found in healthcare services, this paper aims to analyze initiatives implemented by governmental and related healthcare insurance agencies in Saudi Arabia to reduce moral offenses. To accomplish this objective, major governmental health insurance policy documents were analyzed at the macro-level. At the meso-level, semi-structured interviews were conducted with five health insurance professionals on measures undertaken to prevent such incidents. At the micro-level, the critical factors of fraudulent behaviors were analyzed using a retrospective analysis. Data were retrieved from anti-fraud records of ten leading health insurance companies and the focus was mainly on individuals involved in unethical practices between 2014 and 2019. After a full audit was completed, the results concluded that the Saudi healthcare system is composed of twenty-six cooperative health insurance agencies and over 5,202 health services providers. The official documents contain the details of various moral hazard measures. On annual average, more than 196 fraudulent cases were reported with a claim rejection rate of approximately 15%. The majority of fraud cases were reported in dental services with invalid card usage, followed by obstetrics-gynecology services (47 and 113 cases, respectively). Females tended to make up most deceit cases in obstetrics-gynecology with a high level of abuse (95% confidence interval: −83.398 to −24.202; P < .003 and −28 > 638 to −7.362; P < .005, respectively). This study ultimately identifies basic measures employed at the macro-level to reduce moral hazards. However, such measures are not intended to be coherently implemented at the micro-level, especially by health insurance companies and healthcare providers.

Since the 1990s, language-planning interventions have changed the alphabet on car number-plates in Cyprus three times, while a fourth change is expected to take place in line with the parliamentary decision of 2010. An investigation of the alternating adoption of the 24-letter English (Latin) and the 12-letter Helleno-Latin alphabetical system demonstrates that a minor matter, such as the alphabet on the plates, can be a multilevel language planning issue. It was initiated at the micro-level by individuals and small groups who exercised pressure on the government to change this language practice, but through interventions by government officials and politicians it snowballed to the macro-level. Examination of the issue reveals the pragmatic and symbolic roles attributed to the alphabetical systems as well as the beliefs, perceptions and ideologies about language and identity in Cyprus held among Greek-Cypriots who were involved in the controversy.

The article emphasizes the importance of the interdependence of managing the economic efficiencyof economic activity at the macro level and the micro level, considering that management efficiency was firststudied as an independent concept as a result of pressing issues of the present time, such as quality,efficiency, solvency, stability, optimality, reliability. The concept of efficiency in the context of the economiccategory has been considered, the main types of efficiency have been defined. The difference between theconcept of efficiency and effectiveness has been justified. Theoretical and practical aspects of the analysis ofthe effectiveness of the business entity's activity have been stated. Characterization of the fundamentalapproaches and methods of analyzing the effectiveness of the enterprise has been given. As a result of theresearch targeted, systematic, individual and integrated approaches to determining performance have beenemphasized. It has been suggested to use complex and individual approaches to evaluate the effectivenessof the enterprise activity, which should take into account the specificity of the sphere of activity. The systemof criteria and indicators of the analysis of efficiency of the enterprise activity has been presented,generalized and investigated.

Changing global trends have made people more aware and conscious about their health and attainment of quality life by accessing healthcare services. But countries like Pakistan are not able to maintain quality healthcare services for common people. The healthcare service providers are not able to face challenges in this regard. The system of healthcare in Pakistan faces many issues and challenges of imbalance and insufficiency regarding deliverance and access to effective and quality healthcare services. The governments should develop an appropriate health system whose mechanism can be easily accessible for people especially women. For this purpose data from public hospitals has been collected to analyze specific issues within the boundaries of a specific environment and situation, because proper and accurate data availability can assure the effective policy making mechanism. In this study data has been collected by semi-structured interview schedule. Three public hospitals (Civil, Jinnah and Abbasi Shaheed) were selected as a universe of the study, and the respondents were purposively selected for conducting interviews in detail. The findings of this study reveal that in developing countries like Pakistan the healthcare services are not provided adequately in the public sector. People do not have easy access to healthcare opportunities for many reasons among which one reason is poverty, which leads to ill health and low health status. This growing dissatisfaction leads to imbalance societal gap, which demands appropriate measures and policies by the government. Government hospitals have some women specific wards but public private partnership has not been very effective, despite the establishment of Act and collaborations in public-private domain.
