Title: Weight and waist-to-hip ratio change pattern during the first five years of survival: data from a longitudinal observational Chinese breast cancer cohort

Abstract Background: Body weight management was an important component in breast cancer survivorship care. The present study described the change patterns of body weight and waist-to-hip ratio (WHR) during the first five years of survival, and investigated potential factors associated with very substantial changes.Patients and methods: Based on a longitudinal cohort with 1462 Chinese women with breast cancer, anthropometric measurements including body weight, height, waist and hip circumferences were measured by trained interviewers following standard protocol at four time-points: baseline at study entry, 18-, 36- and 60-months follow up assessments (termed as T0, T1, T2 and T3, respectively). Body height was measured at baseline and body weight at cancer diagnosis were retrieved from medical record.Results: Compared to weight at breast cancer diagnosis, the median weight change was -0.5kg, 0kg, +0.5kg, and +1kg at T0, T1, T2 and T3, respectively. During the first five years of survival, the proportion of women who were obese have slightly increased. At 60-months after diagnosis, only 14.3% of women had weight gain by > 5kg; and the percentage of women who had weight gain by > 10% was 10.7%. Nearly half of patients had abdominal obesity at study entry, and this proportion were gradually increased to nearly 70% at 60-months follow-up. Multivariate analysis indicated that older age, frequent sports participation and having vegetables and fruits intake ≥ 400g/day were related to lower risk of very substantial weight gain (> 10%) at 60-months follow-up; patients aged 40-49 years, having ≥ 2 comorbidities, ER negative and having vegetables and fruits intake ≥ 400g/day were associated with less likelihood of very substantial WHR substantial increase (> 10%) at 60-months follow-up. Conclusion: Weight gain was modest in Chinese breast cancer survivors during the first five years of survival, while central adiposity has become a contemporary public health issue. The incorporation of healthy weight and abdominal circumference patient education and management has a potential to improve cancer survivorship.

12076 Background: Weight gain is common after a breast cancer diagnosis. The incidence of and risk factors for weight gain during adjuvant endocrine therapy (ET) are poorly described. Limited data support an association between emergent symptoms and weight gain after a breast cancer diagnosis. Methods: We enrolled women with stage 0-III breast cancer initiating ET in a prospective clinic-based cohort. We assessed symptoms with the FACT-ES and PROMIS pain interference, depression, anxiety, fatigue, sleep disturbance and physical function measures at baseline (BL), 3, 6, 12, 24, 36, 48 and 60 months (mo). We defined emergent symptoms at 3 and/or 6 mo as worsening of 4 points from BL on PROMIS measures and 5 points from BL on the FACT-ES. We abstracted weight and menopausal status from charts. The primary outcome of this secondary analysis was weight gain (dichotomized as ≥5% vs < 5% of body weight compared to BL) through 60 mo. We evaluated the association between weight gain during ET and menopausal status. We also evaluated the associations between clinicodemographic factors and emergent symptoms with weight gain and if these associations differed by menopausal status. We performed logistic regression modeling with GEE to account for the longitudinal design. We identified a multivariable model for the set of factors associated with weight gain among pre-menopausal women taking ET. Results: 309 of 321 participants with BL and ≥1 follow-up (FU) weight were included. 263 (85%) had stage I-II disease, 99 (32%) were pre-menopausal, 259 (84%) were White and 32 (10%) were Black. Prior to ET, 45% had mastectomy, 66% had radiation, and 28% received chemotherapy. 4% of pre- and 82% of post-menopausal participants initiated an aromatase inhibitor (AI); all others initiated tamoxifen (Tam). 17% of pre-menopausal participants received ovarian suppression. At BL, 75% of Black and 59% of White participants were overweight/obese. With a median FU of 56 mo, 51% of pre- and 34% of post-menopausal participants gained ≥5% body weight (OR 1.09, 95% CI 1.07-1.13, p < 0.001). For each PRO measure, > 20% of participants had emergent symptoms. Worsening of physical function and pain interference scores at 3 and/or 6 mo were differentially associated with weight gain according to menopausal status (interaction p-values ≤0.05). On multivariate analysis, factors associated with weight gain among pre-menopausal participants were ET (AI vs Tam) (OR 2.8, 95% CI 0.90- 8.77, p = 0.08), prior mastectomy (OR 2.06, 95% CI 0.89-4.77, p = 0.09), emergent pain interference (OR 2.49, 95% CI 0.99-6.24, p = 0.05) and race (White vs other) (OR 7.13, 95% CI 1.29-39.4], p = 0.02). Conclusions: Weight gain during ET for breast cancer is more frequent among pre-menopausal than post-menopausal women. Worsening pain soon after ET initiation, receipt of AI, prior mastectomy and race may identify pre-menopausal women at risk for weight gain for whom prevention strategies are a priority.

Since the outbreak of COVID-19, billions of people have gone into lockdown, facing pandemic related challenges that engender weight gain, especially in the obese. We report the results of an online survey, conducted during Israel’s first quarantine, of 279 adults treated in hospital-based obesity clinics with counseling, medications, surgery, endoscopic procedures, or any combination of these for weight loss. In this study, we assessed the association between changes in dietary and lifestyle habits and body weight, and the benefits of receiving weight management care remotely through telemedicine during lockdown. Compared to patients not receiving obesity care via telemedicine, patients receiving this care were more likely to lose weight (OR, 2.79; p = 0.042) and also to increase participation in exercise (OR, 2.4; p = 0.022). While 40% of respondents reported consuming more sweet or salty processed snacks and 33% reported less vegetables and fruits, 65% reported more homemade foods. At the same time, 40% of respondents reported a reduction in exercise and 52% reported a decline in mood. Alterations in these eating patterns, as well as in exercise habits and mood, were significantly associated with weight changes. This study highlights that lockdown affects health behaviors associated with weight change, and advocates for the use of telemedicine to provide ongoing obesity care during future quarantines in order to promote weight loss and prevent weight gain.

Abstract Purpose: Short and long sleep duration and poor sleep quality are risk factors for weight gain and cancer mortality. The purpose of this study is to investigate the relationship between sleep and weight change among postmenopausal breast cancer survivors. Methods: Women participating in the Women’s Health Initiative who were diagnosed with incident breast cancer between year 1 and year 3 were included. Self-reported sleep duration was categorized as ≤5 hours (short), 6 hours, 7-8 hours (optimal), and ≥9 hours (long). Self-reported sleep quality was categorized as poor, average, and above average. Post-diagnosis weight change was the difference of weight closest to, but preceding diagnosis, and year 3 weight. We used linear regression to evaluate sleep duration and sleep quality associations with post-diagnosis weight change adjusted for potential confounders. Results: Among 1,156 participants, 63% were weight stable after diagnosis; average weight gain post cancer diagnosis was 3.2 kg. Six percent of women reported sleeping ≤5 hours, 26% reported 6 hours, 64% reported 7-8 hours, and 4% reported ≥9 hours. There were no differences in adjusted estimates of weight change among participants with short duration (0.37kg; 95%CI -0.88, 1.63), or long duration (-0.56kg; 95% CI -2.03, 0.90) compared to optimal duration, nor was there a difference among poor quality (-0.51kg; 95% CI -1.42, 0.41) compared to above average quality. Conclusion: Among postmenopausal breast cancer survivors, sleep duration and quality were not associated with weight change after breast cancer diagnosis. Future studies should consider capturing change in adiposity and to expand beyond self-reported sleep.

Background: Docetaxel is an important chemotherapy-agent for breast cancer treatment. One of its side-effects is weight gain, which increases the all-cause mortality rate. Considering gut microbiota is one important factor for weight regulation, we hypothesized that probiotics could be potentially used to reduce the docetaxel-related weight gain in breast cancer patients.Methods: From 10/8/2018 to 10/17/2019, 100 breast cancer (Stage I-III) patients underwent four cycles of docetaxel-based chemotherapy were enrolled and randomly assigned to receive probiotics (Bifidobacterium longum, Lactobacillus acidophilus, and Enterococcus faecalis) or placebo (supplementary material of the probiotics capsule) treatment for 84 days with three capsules per time, twice/day. The primary outcome: the changes in body weight and body-fat percentage of the patients were measured by a designated physician using a fat analyzer, and the secondary outcomes: the fasting insulin, plasma glucose, and lipids were directly obtained from the Hospital Information System (HIS); The metabolites were measured using liquid chromatography coupled with tandem mass spectrometry (LC-MS/MS); The fecal microbiome was analyzed using bacterial 16S ribosomal RNA (rRNA) gene sequence. All indicators were measured 1 day before the first cycle of docetaxel-based chemotherapy and 21 days after the last cycle of docetaxel-based chemotherapy.Results: Compared with the placebo group, the probiotic group showed significantly smaller changes in body weight (Mean [SD] 0.77 [2.58] vs. 2.70 [3.08], P = 0.03), body-fat percentage (Mean [SD] 0.04 [1.14] vs. 3.86 [11.09], P = 0.02), and low density lipoprotein (LDL) (Mean [SD]−0.05[0.68] vs. 0.39 [0.58], P = 0.002). Moreover, five of the 340 detected plasma metabolites showed significant differences between the two groups. The change of biliverdin dihydrochloride (B = −0.724, P = 0.02) was inverse correlated with weight gain. One strain of the phylum and three strains of the genus were detected to be significantly different between the two groups. Also, the changes of Bacteroides (B = −0.917, P &lt; 0.001) and Anaerostipes (B = −0.894, P &lt; 0.001) were inverse correlated with the change of LDL.Conclusions: Probiotics supplement during docetaxel-based chemotherapy for breast cancer treatment may help to reduce the increase in body weight, body-fat percentage, plasma LDL, and minimize the metabolic changes and gut dysbacteriosis.Clinical Trial Registration:http://www.chictr.org.cn/showproj.aspx?proj=24294, ChiCTR-INQ-17014181.

CONTEXT: Patients receiving adjuvant chemotherapy for breast cancer have a tendency to gain weight. This tendency has determining factors not completely defined and an unknown prognostic impact. OBJECTIVE: To evaluate weight change during chemotherapy for breast cancer in a defined population and to identify its predisposing factors and possible prognostic significance. DESIGN: Observational, retrospective cohort study. SETTING: Private clinical oncology service. PARTICIPANTS: 106 consecutive patients with breast cancer treated between June 1994 and April 2000, who received neoadjuvant (n = 8), adjuvant (n = 74) or palliative (n = 24) chemotherapy. INTERVETION: Review of medical records and gathering of clinical information, including patients’ body weights before treatment and at follow-up reviews. MAIN MEASUREMENTS: Body weight change, expressed as percentage of body weight per month in treatment; role of clinical data in weight change; and influence of weight change in overall survival and disease-free survival. RESULTS: There was a mean increase of 0.50 ± 1.42% (p = 0.21) of body weight per month of treatment. We noted a negative correlation between metastatic disease and weight gain (r = -0.447, p < 0.0001). In the adjuvant and neoadjuvant therapy groups there was a mean weight gain of 0.91 ± 1.19 % (p < 0.00001) per month, whereas in the metastatic (palliative) group, we observed a mean loss of 0.52 ± 1.21% (p = 0.11) of body weight per month during the treatment. We did not observe any statistically significant correlation between weight changes and disease-free survival or overall survival. CONCLUSIONS: Women with breast cancer undergoing adjuvant or neoadjuvant chemotherapy gain weight, whereas metastatic cancer patients will probably lose weight during palliative chemotherapy. Further studies are needed in order to evaluate the prognostic significance of weight changes during chemotherapy.

This study aimed to analyze associations between weight variation patterns and changes in cognitive function and hippocampal volume among non-demented, community-dwelling elderly. Sample was formed of 1394 adults >70 years (63.9% female), all volunteers from the Multidomain Alzheimer Preventive Trial (MAPT). Weight loss was defined as ≥5% of body weight decrease in the first year of follow-up; weight gain as ≥5% of weight increase; and stability if <5% weight variation. Cognition was examined by a Z-score combining four tests. Measures were assessed at baseline, 6, 12, 24, 36, 48, and 60 months of follow-up. Hippocampal volume was evaluated with magnetic resonance imaging in 349 subjects in the first year and at 36 months. Mixed models were performed. From the 1394 participants, 5.5% (n = 76) presented weight loss, and 9.0% (n = 125) presented weight gain. Cognitive Z-score decreased among all groups after 5 years, but decline was more pronounced among those who presented weight loss (adjusted between-group mean difference vs. stable: −0.24, 95%CI: −0.41 to −0.07; p = 0.006). After 3 years, hippocampal atrophy was observed among all groups, but no between-group differences were found. In conclusion, weight loss ≥5% in the first year predicted higher cognitive decline over a 5 year follow-up among community-dwelling elderly, independently of body mass index.
