Abstract Background: The coagulation test is used to evaluate in various diseases. Little is known about correlation between the coagulation test and neonatal hyperbilirubinemia. The aim of the study was to determine the neonatal coagulation analysis value in infants with neonatal hyperbilirubinemia as reliable markers. Methods: All newborns were born and inpatients in the newborn wards of the Chengdu Second People’s Hospital. Measurement of serum bilirubin was done to diagnose hyperbilirubinemia. All neonates involved were full term of either gender without any significant illness or major congenital malformations. Data were collected from newborns who had neonatal hyperbilirubinemia and healthy in Chengdu Second People’s Hospital. Prothrombin time(PT), Thrombin time(TT), Fibrinogen(Fbg), activated partial thromboplastin time(APTT), and calculated international normalized ration(INR) values were recorded. Linear relationship between INR and level of total bilirubin and indirect bilirubin were analyzed by linear regression. The receiver operating curve(ROC) and the area under the curve(AUC) were made evaluated. Results: In this case-control study, the mean PT, APTT level were significant higher in the infants with hyperbilirubinemia group compared to the healthy infants. We found the positive correlation between INR and total bilirubin(R=0.3327; P<0.0001). Furthermore, there was also significant positive correlation between INR and indirect bilirubin(R=0.3402; P<0.0001). Meanwhile, INR in neonates with hyperbilirubinemia significantly achieved an AUC of 0.800(95% CI=0.6288-0.9712, cutoff value 1.060, specificity71.43%, and sensitivity 80.00%).Conclusions: PT, APTT were significant high in neonatal hyperbilirubinemia group. Correlation were observed between the INR and neonatal hyperbilirubinemia. Importantly, the study revealed INR as novel biomarkers for diagnosis of neonatal hyperbilirubinemia in full term newborn.

To the Editor.— Osborn et al1 have reported a positive association between breast-feeding and neonatal hyperbilirubinemia in full-term infants. To give further support to the findings of Osborn et al, we wish to report the results of two similar studies that have been completed in two different hospitals. The first study was carried out in the nursery of the Second School of Medicine of Naples.2 Rooming-in was practiced from 9 am to 12 pm, and during the day, breastfed babies were only offered a supplement of 5% dextrose in water when appropriate.

Abstract: Hyperbilirubinemia is an increase of blood bilirubin level due to physiological or non-physiologic factors, which is clinically characterized by jaundice. In neonates, their livers have not worked optimally, therefore, the process of glucuronidation of bilirubin does not occur optimally. This situation causes the predominance of unconjugated bilirubin in the blood. This study was aimed to obtain the profile of neonatal hyperbilirubinemia cases in the Pediatrics Department of Prof. Dr. R. D. Kandou Hospital Manado. We used medical record data of neonatal hiperbilirubinemia in aterm and premature neonates at the Pediatrics Department of Prof. Dr. R. D. Kandou Hospital di Bagian Ilmu Kesehatan Anak RSUP Prof. Dr. R. D. Kandou Hospital during 2014-2015. The results showed that there were 54 cases, consisted of 32 males (59.3%) and 22 females (40.7%). Among aterm neonates, mean direct bilirubin was 3.97 mg/dL; mean indirect bilirubin was 11.76 mg/dL; and mean total bilirubin was 15.69 mg/dL. Meanwhile, among premature neonates, mean direct bilirubin was 0.87 mg/dL; mean indirect bilirubin was 12.48 mg/dL; and mean total bilirubin was 13.35 mg/dL. In conclusion, at the Pediatrics Department of Prof. Dr. R. D. Kandou Hospital Manado neonatal hyperbilirubinemia was more common in aterm neonates compared to premature neonates. Abstract Introduction Acute coronary syndrome (ACS) is a leading cause of mortality and morbidity worldwide. Despite being the gold standard biomarkers, cTn and CK-MB have a major drawback as they are less sensitive in the first 3 hours of the onset of symptom. So, there is still a need for novel biomarkers, which can reliably rule in or rule out this disease immediately on admission. Aim of the work To evaluate the role of copeptin, miRNA-499 and miRNA-208 as novel biomarkers for early detection of unstable angina (UA) and non-ST-segment elevation myocardial infarction (NSTEMI) Patients and Methods: A total of 65 patients presenting within 4 h of onset of chest pain suggestive of ACS were enrolled in the study. They included 23 UA, 42 NSTEMI. Also 25 apparently healthy controls were included. Blood samples (first set within the first 3 hours and second set at 6 hours) were taken for estimation of copeptin by ELISA and miRNA-499 and miRNA-208 expression levels by real time PCR. Results Copeptin, miRNA-499 and miRNA-208 expression levels were significantly increased in UA and NSTEMI patients compared to controls (P&lt;0.001 each). Also these biomarkers were significantly increased in NSTEMT compared to UA (P&lt;0.001 each). They also significantly elevated in UA and NSTEMI patient in the first 3 hours who had negative cardiac troponin (p&lt;0.001 each). ROC curve analysis revealed that the area under curve (AUC) for prediction of ACS was 0.96 for copeptin, 0.97 for miRNA-499 and 0.0.97 for miRNA-208. Interestingly, combining copeptin with miRNA-499 and miRNA-210 significantly improved the diagnostic value by increasing the AUC to 0.98, P&lt;0.001. The sensitivity and specificity within the first 3 hours were 90%, 86% for copeptin, 95%, 94% for miRNA-499 and 93%, 98% for miRNA-208. The sensitivity and specificity were 81% and 86% for cardiac troponin within 6 hours. There was a positive correlation between copeptin and miRNA-499 and miRNA-208 (r=0.75, P&lt;0.001 and r=0.76, P&lt;0.001 respectively) Also, there was a positive correlation between these biomarkers and cTn (r=0.7. P&lt;0.001, r=0.64, P&lt;0.001 and r=0.68, P&lt;0.001 respectively). Conclusions Copeptin, miRNA-499 and miRNA-208 expression might be novel biomarkers as they are associated with UA and NSTEMI presented in the first 3 hours of onset of pain. The combination of copeptin and miRNA with cTn accelerate the diagnosis of ACS and avoiding the gray zone of cTn. Copeptin and miRNAs representing a potential aid in early diagnosis as they have different pathogenesis and site of liberation. Funding Acknowledgement Type of funding source: None

To estimate the effect of neonatal hyperbilirubinemia on long-term cognitive ability in full-term newborns with a negative Coombs test, we performed a 17-year historical prospective study of 1948 subjects. Intelligence tests and medical examinations performed at the military draft board were stratified according to serum bilirubin concentration. A logistic regression analysis was used to adjust for the confounding effects of gestational age, birth weight, Apgar score, ethnic origin, socioeconomic class, paternal education, birth order, and the administration of phototherapy and exchange transfusion. No direct linear association was shown between neonatal bilirubin levels and intelligence test scores or school achievement at 17 years of age. However, the risk for low intelligence test scores (IQ score &lt;85) was found to be significantly higher (P = .014) among full-term male subjects with serum bilirubin levels above 342 µmol/L (20 mg/dL) (odds ratio, 2.96; 95% confidence interval, 1.29-6.79). This association was not observed among female subjects. We conclude that severe neonatal hyperbilirubinemia, among full-term male newborns with a negative Coombs test, could be associated with lower IQ scores at 17 years of age.
