Burden of Care and Its Relationship with Sleep Quality of Cancer Patients’ Caregivers

Abstract Purpose: This study aimed to assess the burden of care and its relationship with sleep quality of cancer patients’ caregivers.Methods: This descriptive-correlational study was conducted in a referral center of cancer in Zanjan, northwest of Iran. 135 caregivers of cancer patients were recruited through convenience sampling method. The data were collected using a demographic characteristics questionnaire, Novak and Guest’s caregiver burden inventory and Pittsburgh sleep quality index (PSQI). The collected data was analyzed using descriptive and inferential statistics.Results: The mean (± SD) age of the participants was 39.71 (± 10.74) years. The mean (± SD) burden of care and sleep quality of the participants was 45.22 (± 17.75) and 8.88 (± 4.21), respectively. It was found that there is a significant positive relationship between burden of care and quality of sleep scores (r = 0.65, P < 0.001).Conclusion: cancer patients’ caregivers endure a remarkable burden of care and their sleep quality is undesirable. Results indicated that an increase in the burden of care reduces the caregiver’s quality of sleep. High burden of care and poor sleep quality may reduce the quality of the provided care that increase the costs and weakens the disease prognosis. According to the study results, reducing burden of care is an effective strategy regarding improving the caregivers’ quality of sleep that can improve the quality of provided cares by caregivers.

Background: Breast cancer survivors make up a growing population facing treatment that poses long – standing adverse effects including chemotherapy- related sleep disorders and fatigue. There is limited knowledge of patients' lived experiences of chemotherapy- induced sleep disorders and fatigue. The aim of this study was to explore sleep quality and fatigue among breast cancer patients undergoing chemotherapy. Materials and Methods: One hundred fifteen patients were included in this census-based cross-sectional study. Data were collected through the Pittsburgh Sleep Quality Index and Brief Fatigue Inventory four days after the chemotherapy session. Statistical analysis was carried out using SPSS software version 13 and P<0.05 was considered statistically significant in all tests. Results: The mean hours of sleep were 5.6±1.83 in the range of 2 to 10 hours. The mean score of fatigue of participants was 5.59±1.67. Based on the cutting point, 57.4%, 20.9%, and 21.7% of participants had a moderate (4-6.9), mild (0.1-3.9), and severe (7-9.9) level of fatigue, respectively. The mean score of sleep quality among the participants was equal to 14.06±3.06, with a maximum and minimum of 7 and 21. The results of Spearman correlation coefficient showed that there is a significant relationship between fatigue and quality of sleep )0.210). Conclusion: Although the study findings revealed that patients with breast cancer undergoing chemotherapy experience different degrees of sleep disorders and fatigue, there is a need for more detailed studies to improve the quality of sleep and reduce fatigue in these patients due to the little attention paid to this issue in the medical field.

Objective: This was a descriptive study with the objective to determine the sleep disturbances and the quality of sleep among patients with cardiovascular diseases (CVDs).Methods: Quantitative approach with non-experimental descriptive design was used. A semi-structured sleep disturbance questionnaire was used to determine the sleep disturbances, and a standardized Pittsburgh Sleep Quality Index tool was used to assess the sleep quality among 100 subjects in Amrita Institute of Medical Sciences, Kochi.Results: The study results showed that majority of the subjects (51%) were of the age group between 56 and 65 years, and 71% were male with CVD. Sleep disturbances were present in 80% of the subjects with CVD, among which 56% subjects had mild disturbances, 20% subjects with moderate sleep disturbances, and 4% subjects with severe sleep disturbances. The result also indicated that 48% of subjects had poor sleep quality. Findings also showed a highly statistically significant association between cardiomyopathy and sleep disturbances (p<0.001). Total cholesterol and low-density cholesterol levels were significantly associated with sleep disturbances (p<0.05). There is a highly significant association between sleep disturbances and quality of sleep (p<0.001), indicating that sleep disturbances can be one factor in decreasing the quality of sleep in patients with CVD.Conclusion: The results of the study indicated that sleep disturbances were prevalent in patients with CVD and often hazardous to heart health. As health practitioner and other health-care providers have not focused major attention on the importance of sleep to health, the need for providing information to them and patients regarding the importance of improving the quality of sleep and identifying the sleep disturbances will call for action to bring sleep to the forefront of public health.

Sleep quality (Pittsburgh Sleep Quality Index), severity of anxiety and depression (Patient Health Questionnaire - Anxiety and Depression Scale) were accessed in the present study. Linear regression analysis was used to investigate the association between sleep quality and its related factors. Results: The mean age of study population was 59.6 ± 11.7. The average PSQI score was 10.6 ± 5.2. 79.8% cancer patients had poor sleep quality (PSQI score > 5). 50% patients had difficulty falling asleep and 46.2% patients reported sleep of < 5 hours per day. There were statistically associations between quality of sleep and gender, depression/anxiety (p < 0.05). Conclusion: The majority of cancer patients had poor sleep quality. The quality of sleep is required by breast cancer patients to regenerate and repair the body cells. When patients’ sleep is disturbed, it may be affected to the physiological conditions, such as decreased appetite, weight loss, anxiety, irritability, and changes in natural and cellular immune functions. Breast cancer therapies can affect the quality of sleep. This study aimed to determine the sleep quality of breast cancer patients that had treatments in a public hospital in Bandung. This research used the quantitative descriptive approach. The sample was selected using the consecutive sampling technique (n = 31). Data were collected using the Pittsburgh Sleep Quality Index (PSQI) questionnaire. Sleep quality was categorized as good when the total score was ≤ 5, and sleep quality was poor when the total score was > 5. The results of this study indicated that all respondents (100%) had poor sleep quality. The components that contributed to the assessment were sleep latency, sleep duration, sleep efficiency, and daytime dysfunction. The worst sleep quality with the highest score (18) occurred in respondents who had radiotherapy and mastectomy (2 persons), and respondents in advanced stage (3 persons). This study concludes that the pharmacological therapies and the stages of cancer contributed to the sleep quality of breast cancer patients. It is expected that health professionals and hospitals notice the patient’s sleeping needs during treatment at the hospital.

e20544 Background: Poor sleep is common, and is linked to impaired cognitive, psychological, and physical functioning and a lower quality of life. The purpose of this study is to measure quality of sleep in cancer patients and determine the relationship between quality of sleep and performance status. Methods: Patients with cancer (n=139) were recruited at the public hospital between November 2011 and January 2012 and asked to complete the Pittsburgh Sleep Quality Index (PSQI) and the Eastern Cooperative Oncology Group (ECOG) performance status (PS) questionnaires. Twenty patients repeated the questionnaires after 3 months. For the purpose of this study PSQI global score >5 was considered insomnia. PS score 0-1 was considered good, and PS 2-4 was considered poor. Results: The mean age of participants was 55±11 years, 56% were women, 78% were ethnic minority, 77% had advanced cancer, and 71% were undergoing cancer treatment. Among all patients, 73% reported good PS. Median PSQI score was 7 (IQR 4; 11) and 62% had insomnia. Sleep medication was used by 25%. Patients with poor PS had worse PSQI score (Mann-Whitney test, p< 0.0001), and 7.34 (95%CI 2.11-25.49) higher odds of insomnia (p=0.0002) compared to patients with good PS.(Table1) No significant correlation was found between insomnia and gender, ethnicity, primary tumor type, tumor stage, or treatment status. Conclusions: The incidence of insomnia among cancer patients is high. There is a higher incidence of sleep disturbance and worse sleep quality among patients with poorer performance status. e23100 Background: Sleep disturbances are highly prevalent in cancer patients and may be attributable to factors including pain, treatment side effects and psychological factors. However, the relationship between sleep and cancer is bidirectional. There is substantial evidence that sleep disturbance is associated with many aspects of cancer treatment, morbidity, mortality and quality of life. This study evaluated sleep quality in adult Filipinos with cancer and determined the association of demographic characteristics and clinical features to sleep quality. Methods: This was a cross-sectional surveillance study conducted among adult Filipinos with cancer seen at the Ambulatory Care Unit of a tertiary hospital. Questionnaires were answered to assess subjective sleep quality, pain score and quality of life. Frequency and percentages were determined. Determination of factors affecting sleep quality was analyzed using univariate and multivariate statistics. Results: Of the 406 cancer patients studied, 80% were women and the mean age was 53 years old. Majority (62%) had breast cancer. Seventy-nine percent (79%) of the patients had a PSQI score of 5 or greater, indicating poor sleep quality. Sleep among participants was characterized by prolonged time to fall asleep and shortened sleep duration. Majority reported some degree of daytime dysfunction due to poor sleep. Age, gender and marital status did not appear to affect the quality of sleep. Among disease-related factors, presence of more advanced disease, increasing pain severity and treatment with chemotherapy and/or radiotherapy were demonstrated to lead to poorer sleep quality. Surprisingly, even patients who were not receiving any active therapies (on-going surveillance) complained of sleep disturbance. Upon further investigation, poor sleep quality was shown to predict a worse quality of life among the study participants. Conclusions: Overall, the quality of sleep among adult Filipinos with cancer is significantly impaired. In particular, presence of more advanced disease, treatment with chemotherapy and radiotherapy and moderate to severe pain significantly increase the risk of having poor sleep. In turn, disturbed sleep predicted a poorer quality of life. These findings support the need to include assessment of sleep quality for a more holistic approach to cancer care.

Background. Sleep disturbance is particularly common among medical students worldwide and affects their wellbeing and academic performance. However, little is known about this issue in Yemen. This study looks at sleep quality and its association with personal and life-style factors and self-reported academic performance among medical students at the largest Yemeni university. Methods. A cross-sectional survey was conducted at Sana’a University, Yemen, in 2017. The Pittsburgh Sleep Quality Index (PSQI), consisting of 19 items and 7 components ( score   range = 0 ‐ 3 ), was used to assess sleep quality. The summation of the components’ scores yields the global PSQI score ( range = 0 ‐ 21 ). A global PSQI value higher than 5 indicates poor quality of sleep. Logistic regression was applied to look at relationships. Results. 240 male (41%) and female (59%) medical students took part in the study with 54% being preclinical and 46% clinical with an average age of 23.3 years ( SD = 1.7 ). The mean global score (SD) was 6.85 (2.8), and 68% of the students ( N = 163 ) were identified as poor sleepers. The mean global PSQI score (SD) and proportion of poor sleepers were higher among males (7.7 (2.8) and 81%, respectively) than females (6.27 (2.42) and 59.2%, respectively), p ≤ 0.001 . Good sleep quality was more likely (OR (95% CI)) among females (3.4 (1.3-8.8)), the unmarried (2.8 (1-7.8)), those in good health (2.3 (1.1-4.5)), and nonkhat chewers (4.9 (1.4-17.1)). Nonsmokers were less likely to have good quality sleep compared to occasional smokers (0.185 (0.071-.485)). Stress (30%) and academic workload (21%) were the most commonly reported causes of poor sleep quality. Almost two-thirds of the students (65%) mentioned that disturbed sleep undermined their academic performance. Conclusions. Poor sleep quality is common among Sana’a medical students and impacts their academic performance. Specific stress management and sleep hygiene promoting programs should be incorporated early on in medical education.

AimsThe main objective was to determine whether quality of sleep is associated with burnout among undergraduate medical students at the University of Nairobi.The null hypothesis in our study population was; 'There is no significant association between poor sleep quality and burnout'.BackgroundIn a pressure prevailing environment, medical students find themselves in a vicious cycle of cutting down on sleep in attempts to cope and adjust to increasing workloads. Students with poor sleep quality have been found to perform worse in their board exam and have strained social engagements. Ultimately, this chronic sleep deprivation may lead to burnout which may cause diminished sense of accomplishment and impaired professional conduct, that may be carried on to the career as a physician. High levels of burnout have been associated with suicides.MethodThe sample size obtained was 384 and participants were selected by a mixed sampling method. Data collection was through self-administered questionnaires. Scales used for this study were the Pittsburg Sleep Quality Index(PSQI) and the Oldenburg Burnout Inventory(OLBI).Ethical considerations were adhered to and approval obtained from the Kenyatta National Hospital-University of Nairobi(KNH-UON) Ethics Board. Data entry and analysis was by SPSS v23. Data from 336 questionnaires were deemed fit for analysis.ResultWith a response rate of 87.5%, the prevalence of poor sleep quality and burnout were 69.9% and 74.7% respectively. There was a significant positive association between poor sleep quality and female gender, clinical years of study, living with family, poorly perceived socio-economic state and poor subjective academic performance. In addition, being female, younger, pre-clinical years, living independently off-campus and poor subjective academic performance were significantly associated with higher levels of burnout.Burnout had a significant correlation with poor sleep quality. Daytime functioning, a component of sleep quality had the highest correlation with components of burnout, disengagement and exhaustion. Overall, 57% of the respondents had both poor sleep quality &burnout, while only 12% were good sleepers with no burnout. Furthermore, having poor sleep increased the risk of having burnout by 2.8times. It is crucial that students adopt better sleeping habits to reduce the risk of burnout.ConclusionWith the high prevalence of poor sleep quality and burnout, peer-support groups and peer-led mentorship programs are recommended within this population to help deal with expectations, challenges and difficulties encountered within the course of medical education, in addition to preparing for the early future careers.
