OBJECTIVE: The purpose of this study was to ascertain the extent to which the ocular surface of women with polycystic ovary syndrome is impacted. STUDY DESIGN: Included in this case-control study were 23 eyes of 23 patients (Group I) with polycystic ovary syndrome and 10 eyes of 10 healthy subjects (Group II). Polycystic ovary syndrome was diagnosed when two of the following conditions were met: oligo- or anovulation, clinical and/or biochemical signs of hyperandrogenism, and ultrasonographic documentation of polycystic ovaries. Ocular surface disease grading was performed using the Ocular Surface Disease Index questionnaire, slit lamp examination, meibomian gland secretion scoring, tear film breakup time, staining scores of ocular surfaces, the Schirmer's test, and tear osmolarity assessment. RESULTS: The study observed no significant difference in age (p=0.896). Group I was found to have a significantly higher median luteinizing hormone level (p=0.027). The Ocular Surface Disease Index scores in group I were significantly higher than those in group II (p=0.031). The prevalence of anterior blepharitis was significantly greater in group I (p=0.05). Tear film breakup time was higher in group I than in group II (p=0.026). The ocular surface disease severity was found to be significantly higher in group I (p=0.03). In group I, Ocular Surface Disease Index scores were significantly positively correlated with free testosterone, while tear osmolarity was significantly positively correlated with estradiol levels but significantly negatively correlated with dehydroepiandrosterone sulfate levels (p<0.05). CONCLUSIONS: Daily activities appear to be slightly impaired in patients with polycystic ovary syndrome due to ocular surface problems. However, environmental factors (humidity, temperature, etc.) contribute to the aggravation of ocular symptoms. Patients with polycystic ovary syndrome may be referred to an ophthalmologist to preserve ocular surface health.

The objective: to study the effectiveness and safety of modern antiandrogen therapy in patients with polycystic ovary syndrome (PCOS). Materials and methods. Materials and methods. The first (main) study group included 39 patients with PCOS who received combination nonsteroidal antiandrogen therapy: antiandrogen drug containing flutamide (125 mg 3 times a day) and phyto drug Ovarimedin 1 capsule 2 times a day after meals for 6 months . The II (control) group included 37 patients who received a combination drug containing 35 μg of ethinylestradiol and 2 mg of cyproterone acetate for 6 months. Results. After the course of treatment, biphasic MC was preserved in 25 (64.1%) patients of group I, and in 14 (37.8%) of group II. In group I, before the treatment of stage III hirsutism was in 23 patients (58.9%), after treatment – in 6 patients (15.3%); II stage in 10 patients (25.6%) before treatment, after treatment – in 18 patients (46.1%); I stage was found in 6 patients before treatment (15.5%) and in 15 patients (38.6%) after treatment. In group II patients, stage ІІІ hirsutism was detected in 21 patients (56.7%) before treatment, and in 7 patients (18.9%) after treatment; II stage in 9 patients (24.3%) before treatment, after treatment – in 13 patients (35.1%); I stage was found in 7 patients before treatment (19.0%) and in 17 patients (45.9%) after treatment. Conclusions. Antiandrogenic nonsteroidal therapy with multicomponent herbal drug (Ovaremidine) and nonsteroidal antiandrogens may be recommended for the treatment of women with functional hyperandrogenism accompanied by hirsut syndrome, menstrual irregularities and / or infertility. Our study confirmed that the use of this therapy contributes to the effective treatment of hirsutism, the restoration of menstrual disorders and fertility in women. Differential algorithm for the treatment of hyperandrogenism of various etiologies, which can be recommended in the clinical practice of obstetricians and gynecologists. Purpose:  To determine the role of 0.05% topical cyclosporine A (CsA) in treatment of symptomatic dry eye after LASIK (Laser-Assisted in Situ Keratomileusis). Study Design:  Interventional Case Series. Place and Duration of Study:  Ophthalmology Department, Sir Syed Hospital, Karachi, from July 2018 to Dec. 2019. Methods:  Seventy-five patients who underwent LASIK for myopic refractive error and presented with symptomatic dry eye and showed no response to artificial tears therapy after 1 month of LASIK were selected. The patient had ablation zone diameter of 6mm and flap diameter was 8.5mm to 9mm. Cyclisporine A 0.05% ophthalmic preparation was added to treatment regimen and response was observed at 1, 3 and 6 months. OSDI (Ocular Surface Disease Index) scores, Tear film break up time (TBUT), Schirmer's test (ST) values and Best Corrected Visual Acuity (BCVA) were recorded to analyze the treatment response at each visit. The statistical interpretation was done by using SPSS version 21. To interpret the correlation between pre-treatment and 6 month post treatment, Paired sample T test was applied with 95% Confidence Interval; CI and p-value of ≤ 0.05. Results:  The mean post-LASIK OSDI score of enrolled patients was 54.25 ± 10.81. After 6 months of treatment mean OSDI score was improved to 21.05 ± 5.13 (p < 0.001). Post-LASIK mean ST value was 5.2 ± 1.2 mm and mean TBUT value was 5.6 ± 1.3 seconds, which changed to 9.8 ± 1.0 mm and 8.9 ± 1.1 seconds respectively. BCVA improved from Log MAR 0.14 ± 0.09 to 0.01 ± 0.03 (p < 0.001). Conclusion:  Topical CsA 0.05% was effective in alleviating the symptoms of post-LASIK dry eye. Purpose: The aim of this study is to evaluate the difference between reflex and basal tear osmolarity among healthy normal subjects. Method: The right eyes of 20 healthy normal male subjects aged 20 to 40 years were recruited for this study. The inclusion criteria for the subjects were the Ocular Surface Disease Index questionnaire score of less than 12 and wetting length of the phenol red thread of more than 10 mm. Tear film osmolarity was assessed using TearLab osmometer. Basic tear osmolarity was measured normally without inducing any irritation to the eye. In order to stimulate reflex tear, subjects were asked to open their eye as long as they can till they feel ocular surface irritation (minimum 20 s). Results: The mean score on the Ocular Surface Disease Index questionnaire was 5.5 ± 3. The mean value obtained from the phenol red thread was 21 ± 4.5 mm. There were no statistically significant differences between the osmolarity readings of basal and reflex tear osmolarity ( p > 0.05). The mean value was 308 ± 12 and 306 ± 9 mOsm/l for basic and reflex tear osmolarity, respectively. Conclusion: This study found that the osmolarity of the basal and reflex tears fell within the same range. The values found in this study are in agreement with published results for normal subjects.

Abstract The purpose of this study was to present a new procedure to treat symptomatic conjunctivochalasis (CCh) and to evaluate its efficacy. Forty-two patients with symptomatic CCh refractory to medical management were included on this study. Twenty-two patients (n  =  32, eyes; n  =  14 women and n  =  8 men) underwent the new electrocoagulation procedure (Group I). Twenty patients (n  =  27 eyes; n  =  11 women and n  =  9 men) underwent crescent-shaped conjunctiva resection (Group II). Ocular Surface Disease Index (OSDI) was used to evaluate ocular symptoms. There was a significant difference in mean and SD operation time between Group I (8.67 ± 2.07 minutes) and Group II (20.45 ± 3.98 minutes; P &lt; 0.0001). OSDI scores (mean ± SD) were significantly lower in Group I (28.38 ± 3.14) than Group II (31.62 ± 3.17) at postoperative week 2 (P  =  0.0004). No differences in OSDI scores were found between the 2 groups at postoperative week 4 (P  =  0.1749) or 8 (P  =  0.1483). OSDI scores were significantly lower at postoperative week 8 than at baseline in both Group I (P  =  0.0002) and Group II (P  =  0.0011). Electrocoagulation of the conjunctiva can successfully treat symptomatic CCh with earlier symptomatic attenuation and less operation time than traditional conjunctiva resection.

Mata kering adalah penyakit multifaktorial yang ditandai dengan hilangnya homeostasis tear film yang diikuti gejala okular akibat penurunan kuantitas tear film. Pada jaringan mata, hormon seksual berperan dalam mengatur struktur dan fungsi dari kelenjar lakrimal. Penggunaan kontrasepsi hormonal merupakan salah satu terapi sistemik yang dapat dihubungkan dengan penurunan kuantitas tear film. Penelitian ini merupakan penelitian analitik dengan pendekatan observasional pada 40 orang wanita dengan rentang usia 21-44 tahun. Kelompok sampel adalah wanita yang menggunakan kontrasepsi hormonal minimal dalam 1 tahun dan kelompok kontrol adalah kelompok yang tidak menggunakan kontrasepsi hormonal. Hasil penelitian dan analisis data mendapatkan rerata uji Schirmer kelompok sampel dan kontrol adalah (21,3±12,7) dan (23,9±10). Rerata terkecil uji Schirmer adalah pada kontrasepsi jenis injeksi 3 bulan, yaitu (18,2 ± 11,6). Pada tear meniscus terjadi penurunan pada kelompok sampel (40%) dan kontrol (30%). Hasil kuesioner ocular surface disease index (OSDI) kelompok sampel didapatkan dry eye ringan (20%), sedang (20%), dan berat (5%). Sementara itu kelompok kontrol hanya mengalami dry eye ringan (45%). Dalam penelitian ini menunjukkan tidak terdapat perbedaan hasil yang bermakna (p > 0,05) antara kelompok tersebut. Kesimpulan yang didapatkan adalah tidak terdapat hubungan yang bermakna pemakaian kontrasepsi hormonal dengan kuantitas tear film.

Abstract Purpose: The purpose of the present study is to determine the effects of type 2 diabetes mellitus on tear osmolarity using a TearLab Osmolarity system. Moreover, the relationship between tear film osmolarity and ocular surface discomfort in controlled and uncontrolled diabetic patients was assessed.Patients and Methods: This study included 40 male type 2 diabetic patients aged 20 to 70 years (mean + SD 49±12). A control group (18–43 years; 32.2 ± 6.5 years) consisting of 40 male subjects was also enrolled for comparison. The tear osmolarity was measured using the TearLab Osmolarity System. The ocular surface disease index questionnaire (OSDI) was used to assess ocular discomfort symptoms.Results: The mean tear osmolarity was 297 + 8.00 and 296 + 11 mOsm/L for controlled and uncontrolled diabetic subjects, respectively. While the average osmolarity in the control group was 299 + 8.00 mOsm/L. No significant differences were detected in tear osmolarity between the control and diabetes groups. The ocular surface disease index questionnaire (OSDI) score was significantly higher in the diabetic patient group. No significant correlation was found between tear osmolarity and OSDI scores.Conclusion: The ocular discomfort symptoms score in diabetic patients was significantly higher compared to normal eye subjects. Tear osmolarity was not significantly different in diabetic patients. This finding may be explained by a lack of relationship between tear film parameters and diabetic severity; tear film parameters may correlate more with diabetic duration rather than severity. Therefore, studies focused on diabetes duration and tear film parameters are recommended.

Objective: The aim of this study was to evaluate the serum lipid status in patients with PCOS and to compare the lipid status between PCOS patients and woman without PCOS. Methods: This cross sectional analytical study was carried out in 50 women diagnosed as polycystic ovary syndrome on the basis of Rotterdam Criteria (group I) and 50 women of reproductive age group without polycystic ovary syndrome (group II) attending the outpatient department of Obstetrics and Gynaecology of Dhaka Medical College Hospital, Dhaka during the period of July 2013 to June 2015. Results: The mean total cholesterol, triglycerides and LDL were significantly (p<0.005) higher in group I but mean HDL cholesterol was not significantly (p>0.05) associated with PCOS. Patients with raised total cholesterol : HDL ratio having the risk of developing dyslipidemia estimated to be 11.16 (95% CI = 3.9-33.1) times higher in PCOS patients than that in the group II. In multivariate logistic regression analysis of lipid profile, only raised LDL-C (>130 mg/dl) was found to be significantly associated with PCOS (p<0.05). Conclusion: High LDL level was more associated with PCOS followed by TC, TG and TC:HDL ratio. This study demonstrated a higher level of dyslipidemia specially in PCOS with higher BMI.
