Estudar o perfil do idoso portador de Hipertensão Arterial Sistêmica (HAS) inserido em um Grupo de Apoio a Terceira Idade (GATI), sua adesão ao tratamento medicamentoso e o controle dos níveis pressóricos. Estudo transversal, descritivo e comparativo em 72 idosos portadores de HAS divididos em dois grupos: participantes e não participantes de um GATI na cidade de Jequeri (MG). Os dados foram coletados por entrevistas através do questionário de adesão a medicamentos auto referido, aferição da pressão arterial e coleta de dados do prontuário. A maioria dos indivíduos era do sexo feminino, casados e praticavam atividades físicas regulares. Os participantes do GATI apresentaram maior escolaridade, maior adesão ao tratamento medicamentoso da HAS, maior proporção de praticantes de atividades físicas, menor IMC e menor proporção de fumantes e melhores índices pressóricos. A implementação de GATI pode auxiliar no controle dos níveis pressóricos em pacientes portadores de HAS.Descritores: Hipertensão Arterial Sistêmica, Idoso, Envelhecimento. Effectiveness of a support program for the elderly in the control of Systemic Arterial HypertensionAbstract: To study the profile of elderly patients with Systemic Arterial Hypertension (SAH) included in a support program for elderly people (SPEP) at Jequeri city (MG), as well as their drug treatment adherence and control of blood pressure levels. Cross-sectional, descriptive and comparative study performed in 72 elderly patients with SAH divided into two groups: participants and non-participants of a SPEP. The data were collected by interviews through a self-reported adherence questionnaire, blood pressure measurement, as well as the collection of relevant information to the patient’s medical record. Most of the individuals were female, married, and engaged in regular physical activity. SPEP participants presented higher schooling, greater adherence to the medical treatment of hypertension, greater proportion of physical activity practitioners, lower BMI, a lower proportion of smokers and better blood pressure indexes. OBJECTIVE: To describe the socio-demographic characteristics and quality of life of elderly patients with systemic arterial hypertension; to correlate the quality of life with the time of diagnosis and number of medication, as well as to compare the quality of life with the type of medication. METHOD: In this cross sectional home survey design, 460 elderly people from rural areas were involved. The data was collected with the use of the following instruments: World Health Organization Quality of Life-bref and World Health Organization Quality of Life Olders. A descriptive analysis, Pearson correlation and t-Student test (p<0.05) were undertaken. RESULTS: Predominant patient characteristics were: women between the age of 60 and 70, married, four to eight years of formal education, income of one minimum wage, living with their partners. The time of diagnosis was associated with lower quality of life in the physical domain. Elderly patients who used Inhibitors and Angiotensin Converting Enzyme and who did not use AT1 blocker had lower scores in relation to functioning of the senses. Those who used calcium channel blocker scored higher in relation to autonomy. CONCLUSION: there is a need for actions to control systemic arterial hypertension and its associated complications, with the purpose of improving quality of life.

Introduction : High prevalence of vitamin D deficiency has been described in more than 1 billion people around the world, es pecially in Middle - East and Asia. In Thailand, the largest study of vitamin D status in normal population (N=2641) identified overall 45.2 percent of vitamin D inadequacy in normal population. However, there was no study focusing on Vitamin D inadequacy in traumatic elderly patients. The purpose of this study is going to define the prevalence of vitamin D inadequacy in traumatic hip fracture among elderly patients and to identify a clinical correlation. Materials and Methods : A prospective study of elderly patients (more than 60 years old) with low energy trauma who underwent orthopaedic surgery at Phramongkutklao Hospital between the period of January 2016 and December 2016. All hip fracture patients underwent orthopedic surgery were collected for demograph ic data, comorbidities including Charlson Comorbidity Index. Primary outcome was to identify a prevalence of vitamin D inadequacy (<30 ng/ml) and vitamin D deficiency (<20 ng/ml) in hip fracture underwent orthopedic surgery. Secondary outcome was to define association of demographic data, comorbidity with vitamin D level. Additionally, comparison the ambulatory status and mechanical failure between normal vitamin D and vitamin D inadequacy Results : The serum 25(OH)D levels for this study was normally distri buted, with a mean of 25.0±12.2 ng/ml in all patients, Prevalence of vitamin D inadequacy was 70.5 percent (n=103) and the prevalence of vitamin D deficiency was 37.7 percent (n=55). There was no significant difference of ambulatory status between vitamin D inadequacy and sufficient vitamin D groups (p=0.958). Additionally, there was not different in mechanical failure between groups (4.5% and 0%, p=1.000). Conclusion : Vitamin D inadequacy is so common in traumatic elderly patients. We should raise attentiveness among physician to determine vitamin D status. Public health strategy should add knowledge about diet, supplements or adequate sun exposure to elderly population . Vitamin D inadequacy did not affect the outcome including time to union and ambulatory status at 1 year follow up.

Цель работы - оценка связи дефицита витамина D в сыворотке крови и риска диастолической дисфункции у пожилых пациентов с артериальной гипертензией (АГ). В исследование включены 162 больных пожилого возраста (средний возраст - 65,4±5,2 года) с АГ: 1-ю группу составили 67 пациентов с АГ без дефицита витамина D; 2-ю - 95 пациентов с АГ при наличии дефицита. Выявлено, что при дефиците витамина D в сыворотке крови больные были достоверно старше (Δ7,3 %, р<0,05), имели более высокий ИМТ (Δ9,6 %, р<0,05), меньшую дистанцию при тесте шестиминутной ходьбы (Δ10,4 %, р<0,05), чаще (72,6 %) имели диастолическую дисфункцию ЛЖ (ДД ЛЖ). Больные АГ с ДД ЛЖ при дефиците витамина D в сыворотке имели худший метаболический профиль - у них установлены более высокие значения общего холестерина (Δ15,7 %, р<0,05), триглицеридов (Δ15,2 %, р<0,05), индекса НОМА-IR (Δ12 %, р<0,05) и более низкие значения ЛПВП (Δ11,9 %, р<0,05). У больных АГ с ДД ЛЖ при дефиците витамина D качество жизни ниже, чем у больных с адекватным его уровнем. Больные пожилого возраста с АГ имели незначительную разницу по частоте встречаемости ДД ЛЖ между изучаемыми группами с разным статусом витамина D. Тем не менее, пожилые больные АГ при дефиците витамина D в сыворотке имели более выраженные нарушения диастолической дисфункции ЛЖ, липидного и углеводного обмена. Качество жизни по опроснику SF-36 у пожилых больных АГ при дефиците витамина D в сыворотке крови было достоверно ниже, чем в группе с адекватным его содержанием. The aim of the study was to evaluate the relationship between serum vitamin D deficiency and the risk of diastolic dysfunction in elderly patients with arterial hypertension. The study included 162 elderly patients (mean age 65,4±5,2 years) with arterial hypertension. The first group consisted of 67 patients with hypertension without vitamin D deficiency, the second-95 patients with hypertension with deficiency. It was revealed that the patients with vitamin D deficiency in the blood serum were significantly older (Δ7,3 %, p<0,05), had a higher body mass index (Δ9,6 %, p<0,05), a shorter distance in the 6-minute walk test (Δ10,4 %, p<0,05), and more often (72,6 %) had left ventricular diastolic dysfunction (LV DD). Patients with hypertension with LV DD in the presence of vitamin D deficiency in serum had a worse metabolic profile - they had higher values of total cholesterol (Δ15,7 %, p<0,05), triglycerides (Δ15,2 %, p<0,05), the НOMA-IR index (Δ12 %, p<0,05) and lower HDL values (Δ11,9 %, p<0,05). In patients with hypertension with LV DD in the presence of vitamin D deficiency, the quality of life is lower than in patients with adequate levels of it. Conclusions. Elderly patients with hypertension had a slight difference in the incidence of LV DD between the study groups with different vitamin D status. However, elderly patients with hypertension with serum vitamin D deficiency had more pronounced disorders of LV diastolic dysfunction, lipid and carbohydrate metabolism. The quality of life according to the SF-36 questionnaire in elderly patients with hypertension with vitamin D deficiency in the blood serum was significantly lower than in the group with adequate vitamin D content.
