Hypertension is highly prevalent in the elderly population and microinfarcts are the most common vascular brain pathology identified in older adults at autopsy. We investigated the associations between systolic and diastolic blood pressures measured antemortem and the presence of microinfarcts at autopsy. Study subjects (n=302; age range=71-95) were participants in the population-based Mayo Clinic Study of Aging autopsy study, who had blood pressure measurements recorded during life. We investigated both cross-sectional systolic and diastolic blood pressure measurements at the baseline visit and the change in blood pressure (slope). Presence and location (subcortical or cortical) of chronic microinfarcts was abstracted from the autopsy reports. Of the 302 study subjects, 47 (16%) had cerebral microinfarcts, and, of those, 18 (38%) had subcortical microinfarcts and 29 (62%) had only cortical microinfarcts. The baseline blood pressures were not different between subjects with no microinfarcts, subcortical microinfarcts, and only cortical microinfarcts. In a logistic regression model including time between last blood pressure measurement and death, a greater decline in systolic [OR= 1.06 (1.01, 1.11); p=0.02]) and greater decline in diastolic [OR= 1.11 (1.02, 1.20); p=0.01] blood pressures were predictors of the presence of subcortical microinfarcts at autopsy. However, these variables were not associated with the presence of cortical microinfarcts. In conclusion, microinfarcts are common in the older adult population, and most of them are located in the cortex. A greater decline in both systolic and diastolic blood pressures and their association with subcortical microinfarcts, but not with cortical microinfarcts, may have implications for aggressive lowering of blood pressure in the elderly population.

Idoso. Controle. Abstract: Hypertension is defined as an elevation of systolic and diastolic blood pressure above 140/90mmHg. More than 50% of patients over 60 years have hypertension, which increases the risk of cerebrovascular, kidney and heart diseases in this population. The anti-hypertensive therapy reduces risks according several studies. This cross sectional hypertension study of the elderly patients over 60 years old analyzed blood pressure in two opportunities separated by two days. Twenty-four patients had the blood pressure measured among 70 patients. Results: the systolic blood pressure were between 120-190mmHg, the diastolic blood pressure between 60-100mmHg. Eighteen (75%) were women, 17 (70.83%) does not have other chronic diseases, four (16.22%) had diabetes type II, three (12.50%) were dislipidemic. In ten (41.67%) patients the systolic blood pressure was over 140mmHg and four (16.67%) had the diastolic blood pressure over 90mmHg. The hypertension was uncontrolled in two fifty of elderly patients. Keywords: Hypertension. Elderly. Control.

The present cross-sectional observational study was made in family medicine offices of Timi County, Romania. The aim of the study was to investigate the prevalence of urinary microalbumin excretion (MAU) in resistant systemic arterial hypertension (RH), to analyze patients biochemical and clinical characteristics, and the predictive factors for MAU. From a total number of 347 patients, MAU was detected in 76 cases (21.9%). The microalbuminuria positive patients were older, with significant higher office systolic blood pressure (BP) (155 13.50 vs 148 12.40 mmHg, p [ 0.0001) and diastolic blood pressure (94 12.20 vs 88 14.6 mmHg, p = 0.0013), higher prevalence of left ventricular hypertrophy, diabetes mellitus, obesity, ischemic and peripheral arterial disease. MAU positive patients presented statistical significant differences in biochemical data concerning: fasting plasma glucose (FPG) (118.80 32.02 vs 108.01 26.01 mg/dL, p = 0.003), impaired glucose tolerance (IGT) (10.52 % vs 4.94 %), glycated hemoglobin (HbA1c) (6.56 0.98% vs 5.96 0.91%, p [ 0.001), reduced estimated glomerular filtration rate (eGFR) (56.10 15.4 vs 69.30 17.5 ml/min/1.73m2, p [ 0.001) and higher potassium levels (4.71 0.43 vs 4.59 0.44 mg/dL, p = 0.0378). No significant differences were noticed regarding LDL- and HDL-cholesterol, triglycerides, uric acid and serum creatinine. In a logistic multivariate analysis independent predictors for MAU were: systolic BP (odds ratio, OR = 1.024, 95% confidence interval, CI:1.011-1.039, p [ 0.001), HbA1c (OR = 1.324, 95% CI: 1.078-1.724, p = 0.008) and eGFR (OR = 0.989, 95% CI: 0.977-0.999, p = 0.01). Our findings suggest that an important part of RH patients have microalbuminuria and highlight the importance of controlling its predictors, in order to improve patients outcome.

Aims: This article aimed to evaluate the use of drugs and to correlate the understanding of the prescription with the elderly lipid profile and bloodpressure. Methods: It was an observational, cross-sectional, quantitative study, which evaluated 97 records of the elderly outpatient from Single HealthSystem of the Institute of Geriatrics and Gerontology (IGG) and the University Extension Center (UEC) Villa Fatima, Porto Alegre/RS from 2008 to 2009.Results: The results revealed that the lack of understanding might be related to the progression of age of the population. The prescription understanding ofthe patients attending the IGG was higher when compared to the elderly who attended the UEC (p<0.05). The parameters of blood pressure and dyslipidemiaare not directly correlated to the understanding of the prescription. Conclusions: Thus, it is necessary to work towardsfollowing of pharmacotherapyfrom elderly patients to improve therapy adhesion in this population.

Hypertension is a health problem that many elderly suffer from. Blood pressure in the elderly with hypertension can be controlled in various ways, namely doing physical activity regularly, performing activities related to cognitive function, and controlling stress levels so as not to affect contractions or pulsations of the heart and nervous system that can cause blood pressure to increase. The purpose of this study was to determine the relationship of partner support with physical, mental, and social activities of the elderly with hypertension. This study used a correlational non-experimental design with a cross-sectional approach. The number of samples were 102 respondents in the Morokrembangan area, where the sampling used Nonprobability Sampling method, namely Purposive Sampling. The research instrument used partner support questionnaires, Global Physical Activity Questionnaire (GPAQ), mental health practices, social activities and demographic characteristics. The analysis used Spearman Rho test. The results of this study were there was a relationship between partner support with physical activity (p = 0.000; r = 0.589 ), mental activity (p = 0.000; r = 0.778 ), and social activity (p = 0.000; r = 0.671) with positive and unidirectional correlation. As you get older, daily activities will decrease too. The better the support provided by the partner, the higher the level of daily activities carried out by the elderly. Therefore, elderly couples are expected to provide mutual support in carrying out physical, mental, and social activities.

This study investigated the physical fitness of elderly hypertensive users of the fitness zones  of the city of Maringá-PR. The subjects were 79 male and female elderly users of 13 fitness zones located in the city of Maringá-PR, with a diagnosis of Systemic Arterial Hypertension (SAH). A semistructured questionnaire was used to identify the sociodemographic and health profile. Resting blood pressure and Body Mass Index (BMI) were measured. The arm flexion and the chair lift and sit tests were used to assess muscle strength and International Physical Activity Questionnaire (IPAQ) was used to identify the physical activity level. The results showed active / very active elderly, with controlled blood pressure, low sedentary behavior index and high limb strength, but with BMI representing overweight and low limb strength. Men presented higher body mass values (p = 0.020); however, other factors related to physical fitness did not show a statistically significant difference between male and female elders. It was concluded that  male and female elderly with a diagnosis of SAH, users of fitness zones presented excellent levels of physical activity, associated with good health behaviors, low sedentary behavior and controlled blood pressure.

Objective – to determine the level of anxiety and depression, to investigate cognitive functions and the status of physical activity in patients with hypertension, depending on age.Materials and methods. We examined 67 patients with hypertension I–III stage (atherosclerotic plaque and/or myocardial infarction in history). According to age classification (WHO, 2012) patients were randomized into 3 groups: group I – 25 middle-aged patients, group II – 22 elderly patients, group III – 20 patients of senile age. The highest level of education, the total number of education years; duration of arterial hypertension; anthropometric data and the office blood pressure were assessed. We used an individual approach to determine the target level of blood pressure according to the recommendations ESC/ESH 2018. Cognitive functions were evaluated using the MoCA-test and the Clock Drawing Test. The level of anxiety and depression was determined by the results of the Hospital scale of anxiety and depression.Results. According to the data obtained, a statistically greater degree of depression was recorded among eldery patients in comparison with other groups. Only in this group, a higher level of depression was associated with less time that patients spent for regular exercise during the week. Among middle-aged patients with arterial hypertension, the absence of anxiety and depression was detected in 56.0 %. At the same time, in the group of senile age, 75.0 % of patients were diagnosed with manifestations of anxiety and depression of varying severity. It is important to notice that in the group of senile age the median level of anxiety was higher, although nonsignificantly, in comparison with other groups and met the criteria for subclinical anxiety. Results of multivariate correlation analysis showed that a high level of anxiety in middle-aged patients was associated with an earlier age of arterial hypertension verification, as well as with waist circumference increase and the waist/hip circumference ratio. Among elderly patients, a higher level of anxiety was determined in the presence of physical inactivity and among patients of senile age, an increase in anxiety level was associated with a shorter duration of education and a decrease in cognitive functions. Conclusions. The highest level of depression was diagnosed among people of senile age, which was associated with the presence of physical inactivity. The groups did not differ statistically in level of anxiety, however, an increase in its severity in the middle-aged group was associated with an earlier age of hypertension verification and an increase in the degree of abdominal obesity, among elderly patients – with hypodynamia, and in the group of senile age – with a low level of education and the presence of cognitive dysfunctions.
