Ayurvedic Management of Dry Eye Sydrome - A Case Study

Dry eye syndrome is a common condition that results from reduced tear production or excessive tear evaporation or an abnormality in the production of mucus or lipids normally found in the tear layer or combination of these. If blinking is decreased or if the eyelids cannot be closed, they may dry out leading to dry eye. It is accompanied by increased osmolarity of the tear film and inflammation on the surface of the eye may occur of left untreated leading to pain, ulcers or scars on the cornea and loss of vision. Prevalence of dry eye range from 5% to 35% worldwide while in India it is 29.25%. Tear substitute are the only treatment modality with modern medicine, only providing symptomatic relief. Ayurveda describes similar condition called Sushkakshi Paka and this patient was treated with Tarpana and Nasya with Jeevantyadi Ghrita.

Introduction:Avascular necrosis is also known as aseptic, osteonecrosis and ischaemic necrosis of bone. There are a multitude of risk factors but over 80% of cases are attributed to glucocorticoid treatment or alcohol excess. Avascular Necrosis of femoral head is the most common type of necrosis, because the artery supplying to neck of femur is very narrow which easily gets injured. In modern medicine there is no specic treatment rather than surgery. In Ayurveda avascular necrosis can be correlated with Asthimajjagatvata. Aim: To assess the efcacy of Dashmooladi Majja Sneha in the management of avascular necrosis. The objective of the treatment includes the preservation of structure and function of hip joint with symptomatic relief. Materials & Methods:The present case study is upon a 32 year old, diagnosed case of avascular necrosis of femoral head with complaints of pain in bilateral sacroiliac joint since one and half years which was associated with difculty in doing normal daily activities such as walking, sitting, squatting along with change in the gait, at the Panchakarma OPD of Govt. Ayurvedic hospital Bhopal. The patient of idiopathic AVN of femoral head was treated with Dashmooladi Majja Sneha, Brihatvata Chintamani Rasa, Amritaristha, Panchtikta Ghrita Guggulu and Shastik Shali Pind Swedan have done as per the classical method for 30 days. Assessment was done after treatment and follow up after 15 days for 2months. Observations: The Ayurvedic therapies and oral medicines yielded complete symptomatic relief from pain, general debility and improvement in the gait. Conclusion: On the basis of the results obtained it can be concluded that Dashmooladi Majja Sneha, Shastik Shali Pind Swedan and some oral medicines can be used as an effective treatment in the management of Avascular Necrosis.

AbstractWe aimed to determine the clinical impact of conjunctivochalasis (CCh) and its correction using high-frequency radiowave electrosurgery (HFR-ES), for signs and symptoms of dry eye disease (DED). Forty patients diagnosed with symptomatic CCh were prospectively enrolled. As a result, patients with CCh had moderate to severe DED and most of them exhibited meibomian gland dysfunction (MGD). Corneo-conjunctival fluorescein staining score (CFS) and all lid-parallel-conjunctival-folds scores (LIPCOFs) were positively correlated. Nasal LIPCOF significantly correlated with symptoms and tear volume. Central, temporal, and total LIPCOF significantly correlated with MG loss, MGD stage, and lipid layer thickness. Independent significant factors associated with total LIPCOF included CFS, tear break-up time, and MGD stage. One month following HFR-ES, CCh was completely resolved in all cases. Patient age and preoperative nasal LIPCOF were determinants of outcomes associated with postoperative improvements in symptoms. Ocular surface parameters significantly improved, but MGD-related signs did not. Collectively, CCh associated with MGD severity deteriorates not only tear film stability and reservoir capacity, leading to DED exacerbation. Therefore, CCh should be corrected in patients with DED and MGD. Younger patients with nasal CCh are likely to experience more symptomatic relief after HFR-ES. Particularly, management for MGD should be maintained after CCh correction.

ABSTRACT : Amavata is composed of two words namely- Ama and Vata. The Ama is the product of incomplete digestion which circulates throughout the body and induces heaviness, loss of taste and appetite, body ache, joint swelling and stiffness, constipation etc like symptoms. The Amavata is described under Vata-Kaphaja vikaras since the period of Madhavkara (16th century). It is a chronic inflammatory systemic disorder affecting mainly the synovial joints in the body. It resembles with Rheumatoid Arthritis in the modern pathology. The Amavata is known to be a crippling, chronic and progressive disorder making it difficult to cure. The symptomatic relief is attained by modern medicine but the root cause and pathology of the disease remains untreated. In Ayurveda, the basic treatment principles of Deepana, Langhana, Swedana, Virechana, Basti, Tikta-Katu dravyas are applied for effective management of the root cause of Amavata. A 38 year old female patient with complaints of Shoola, Shotha and Sthamba of the knee, wrist, ankle and metacarpophalangeal joints of both hands was reported in our hospital OPD an year ago. According to the Lakshanas and the blood investigations, the diagnosis was made as Amavata and treatment was planned. Oral medication with Sinhanad Guggul, Vatavidhwans Ras, Arogyavardhini vati, Dashamularistha and Shunti-Eranda kwath along with Valuka sweda was advised. The assessement was made on the basis of subjective and objective parameters. The results did show a significant improvement in the symptoms of the patient and she was able to perform her routine work without any difficulty.

Dry eye syndrome (DES) is a common condition that causes pain and can lead to vision problems. Modern living has given rise to a massive increase in the incidence of vision threatening dry eye disease, around 10-30% of population living in industrialized countries suffer from dry eye syndrome (DES). According to Second International Dry Eye Workshop (DEWS II) in 2017 offered dry eye as it is a multifactorial ocular surface disorder defined by a loss of tear film homeostasis and ocular symptoms, in which tear film instability and hyperosmolarity, as well as ocular surface inflammation and injury, play a part in the etiology. If it is not controlled with in time it can lead to corneal blindness. Ayurveda proposes a comprehensive systemic & topical strategy in the treatment of dry eye syndrome, in contrast to the available modern medical treatment. In this publication, a case study of a Shushkakshipaka with many complications who was treated with dynamic Ayurvedic treatments & after continuous efforts patient got very significant results is being discussed here.

Vata Vyadhi is one of the most prevailing health problems in our day today clinical practice, Gridhrasi is one among them. Gridhrasi is Shoola Pradhana Nanatmaja Vatavyadhi, affecting the locomotor system and disable from daily routine activity. Gridhrasi the name itself indicates the way of gait shown by the patient due to extreme pain i.e. like Gridhra or Vulture. Gridhrasi is a condition characterized by Ruk, Toda, Stambha, Spandana in Sphik Pradesha and radiates downwards to Kati, Prusta, Uru, Janu, Jangha and Pada. Gridhrasi can be compared with Sciatica. Pain is the chief cause of person to visit a doctor. Although low back pain is a common condition that affects as many as 80 to 90 percent of people during their lifetime. Gridhrasi can be cured by the help of Vaitarana Basti. Hence in the case study of male patient of age 30 yrs presenting with cardinal clinical sign and symptoms of Gridhrasi are Ruka, Toda and Muhu Spandana in the Sphika, Kati, Uru, Janu, Jangha and Pada in order and Sakthikshepanigraha that is restricted lifting of the leg.

Most studies about dry eye disease (DED) chose unilateral eye for investigation and drew conclusions based on monocular results, whereas most studies involving tear proteomics were based on the results of pooling tears from a group of DED patients. Patients with DED were consecutively enrolled for binocular clinical tests, tear biochemical markers of DED, and tear proteome. We found that bilateral eyes of DED patients may have similar but different ocular surface performance and tear proteome. Most ocular surface homeostatic markers and tear biomarkers were not significantly different in the bilateral eyes of DED subjects, and most clinical parameters and tear biomarkers were correlated significantly between bilateral eyes. However, discrepant binocular presentation in the markers of ocular surface homeostasis and the associations with tear proteins suggested that one eye’s performance cannot represent that of the other eye or both eyes. Therefore, in studies for elucidating tear film homeostasis of DED, we may lose some important messages hidden in the fellow eye if we collected clinical and proteomic data only from a unilateral eye. For mechanistic studies, it is recommended that researchers collect tear samples from the eye with more severe DED under sensitive criteria for identifying the more severe eye and evaluating the tear biochemical and proteomic markers with binocular concordance drawn in prior binocular studies.

Malaria is a serious infection affecting millions of people in Africa. Our study investigated the personal preferences and applications of antimalarial medicines in Ghana. Based on over 1000 questionnaires distributed in Ghana from January to May 2019, we noticed that although Western medications to fight this disease are widely available, most patients in Ghana prefer treatment with locally produced herbal remedies. This preference appears to be due to a combination of traditional venues for obtaining medicines “on the street” rather than in licensed pharmacies, trust in local and “green” products, extensive advertisement of such local products, and an inherent distrust of imported and synthetic or unnatural medicines. Going local and natural is a trend also observed in other countries across the globe, and adds to the acceptance or rejection of drugs regardless of their activity or toxicity. In fact, adverse side effects associated with herbal remedies, such as general weakness and swollen, sore mouth, do not seem to deter the respondents of this study in Ghana. We propose a combination of (a) increasing public awareness of the benefits of modern medicine and (b) an improvement and control of the quality of herbal remedies to raise the standard of malaria treatment in countries such as Ghana.

Purpose.To conduct a comparative analysis of the effectiveness of assessing the stability of the tear film using corneotopography and the classic Norn test in patients with myopia and myopic astigmatism before keratorefractive surgery. Material and methods. The study included 26 patients planned for keratorefractive surgery for myopia and myopic astigmatism, aged 18 to 47 years. 7 people there was a mild degree of myopia, in 14 people – medium degree, in 5 people – high degree. The average degree of corneal astigmatism was 2.25 diopters. The stability of the pre-corneal tear film was assessed using computed keratotopography and Norn's test – the tear film rupture time (TFRT) was deter- mined. A standard questionnaire was also used to assess the severity of symptoms of dry eye syndrome. A comparison was made of the indicators of the stability of the tear film obtained by the methods of corneotopography and the Norn's test, with their subsequent comparison with the data of the questionnaire. Results. In 17 patients (65.4%), the TFRT index corresponded to the age norm. In the remaining 9 people (34.6%), indicators of stability of the tear film were reduced both by corneotopography (12±4 sec) and by Norn's test (13.5±4.5 sec), and there were also subjective signs of dry eye syndrome according to data questionnaire. The data turned out to be comparable. Conclusions. 1. Among the total population of patients, 34.6% were persons with impaired tear film stability and subjective manifestations of dry eye syndrome. 2. Assessment of the stability of the tear film, determined using the method of corneal topography, showed a high degree of comparability with the results of Norn's test. A significant advantage of the keratotopography method is its non-invasiveness. 3. It is necessary to pay attention to the condition of the ocular surface and identify risk groups to avoid chronicity or aggravation of existing disorders.
