Endometriosis usually presents as cyclical pain in the pelvis but may also present as painful cutaneous lesions. Many diseases may mimic hidradenitis suppurativa (HS) since HS, a chronic inflammatory painful skin disease, is a clinical diagnosis. A 32-year-old woman presented with painful lumps on her vulva and groin accompanied with bloody discharge during menstruation. She was followed up with preliminary diagnosis of endometriosis. Previous excision without definitive diagnosis resulted in temporary relief. She was prescribed oral contraceptive pills that alleviated her symptoms but quitted due to headache. Surgical excision was performed for definitive diagnosis and therapeutic relief. Pathology report revealed HS. She was prescribed oral doxycycline. She responded well to the therapy and has symptom-free for the last 2 years. She delivered one healthy girl via cesarian section 2 years after the treatment. Endometriosis and HS should be included in the differential diagnosis when women present with menstrual painful lesions around external genitalia for diagnosis and treatment.

Abstract Objectives Mindfulness-based interventions have shown effectiveness in reducing risk factors for disordered eating; however, little is known about mechanisms. This online study evaluated two isolated metacognitive components of mindfulness, adopting a decentered or non-judgemental stance towards internal experiences, respectively, for reducing body dissatisfaction and negative affect. Methods Women (N = 330, Mage = 25.18, SD = 4.44) viewed appearance-ideal media images before listening to a 5-min audio recording that guided them to (a) distance themselves from their experience (decentering), (b) accept their experience without judgement (non-judgement), or (c) rest (active control). Participants reported state body dissatisfaction and negative affect at baseline, post-media exposure, and final assessment. Trait measurements (weight and shape concerns, mindfulness, emotion regulation) were assessed as potential moderators. Participants self-reported engagement and acceptability. Results All groups reported significant reductions in body dissatisfaction and negative affect following the recording (d = 0.15–0.38, p < 0.001), with no between-group differences. Trait measurements did not moderate effects. Conclusions The results suggest rest was as effective as the metacognitive components in ameliorating immediate negative impacts of appearance-related threats. Alternatively, coping strategies spontaneously adopted by the control group may have supplied temporary relief. Findings highlight the importance of including suitable control; further research should investigate when and for whom specific aspects of mindfulness-based interventions may be particularly helpful.

Abstract Background Chronic urticaria is a common disease. Plasmapheresis is an alternative treatment that can be appropriate for patients who are resistant to treatment with 2nd generation antihistamines or for whom treatment with omalizumab is unsuitable. Objective To investigate the effect of plasmapheresis treatment in chronic urticaria. Methods A retrospective analysis was performed based on the data of 98 patients suffering from refractory chronic urticaria who received plasmapheresis as an alternative treatment in Vilnius University’s Hospital Santaros Clinics from 2000 to 2020. The efficiency of the treatment was evaluated by clinical judgment. Results 58.2% of the patients exhibited a complete or significant response; of these, 37.8% had temporary relief of symptoms and 20.4% achieved disease remission; 41.8% showed no response to the plasmapheresis. Men (34.8%) had a tendency to achieve disease remission more often than women (16%) (p < 0.05). One patient did not finish the plasmapheresis treatment due to the symptoms’ exacerbation and treatment with omalizumab was initiated. Conclusion Plasmapheresis is a safe and effective alternative treatment when traditional treatment is unavailable or does not relieve symptoms completely. Our data showed that plasmapheresis was effective in more than half of our patients.

In December 2019 the deadly pandemic COVID-19 traumatized mankind through its lethal impact. To seize the outbreak, nationwide/region-based lockdown strategies were adopted by most of the COVID-19 affected countries. This in turn resulted in restricted transportation via surface, water, and air, as well as significantly reduced working hours of the industry sectors, so on and so forth. The obvious outcome was a sudden discernible decline in atmospheric adulteration. Accordingly, the anthropogenic emissions at the global and regional/local scales were examined during the lockdown period by several researchers using both or either satellite-based and ground-based monitoring. Among several other air-contaminants, carbon has a dominant toxicological profile causing adverse health effects and thereby attracting researches interest in carbon-release probing during the systematic confinement period imposed by the ruling authorities across the globe. The results of those studies indicated a confirmed decline in carbon emission/concentration making the air more breathable for the period. In this review, the studies related to anthropogenic emissions of carbon during the lockdown period are accounted for by compiling the recently reported data from published articles.

Abstract Background Plaque Psoriasis is very embarrassing condition of skin as it produces itchy rashes and scaly lesions which may not respond well to the treatment. It can be correlated with Kitibhakushta which is one among the 18 types of Kushta (skin diseases). Several corticosteroids for skin applications are used now a day’s which gives only temporary relief. Case presentation A 36 years old moderate built male patient complains of blackish skin rashes with itching, pain and scaling all over the body for three months. Based on the symptoms, Dosha (Biological humors) involved were assessed as Kapha (binding factors) and Vata (vital force of life) and suggestive of Kitibhakushta. Diagnosis of the Plaque psoriasis was further confirmed by biopsy. Classical Kushta treatment was adopted here. As Poorvakarma (pre-operative procedures), Rukshana (Dehydrating therapy) was done and then Snehapana (intake of medicated ghee) was administered. Then classical Vamana (emetic therapy) and Virechana (purgative therapy) were performed. This took almost one month. After Samsarjana (special diet) patient was administered Manibhadragula as Shodhana Rasayana for a period of one month. Results At the end of Manibhadragula intake, remarkable changes were observed in all the symptoms. There were significant changes seen in PASI, DLQI and photographs taken before and after treatment. Conclusions Here the special mode of administration of Manibhadragula as mentioned in Ashtanga Hridaya helps for Shodhana and the Shodhana itself has Rasayana action. Drugs also have Rasayana property. This made the authors to propose a new concept called Shodhana Rasayana. Classical treatment by considering the condition of Roga (disease) and Rogi (patient) helps for management of Kushta.

This article examines a humorous meme that emerged on Chinese TikTok during the COVID-19 pandemic in China. Using #workfromhomewithchildcare, Chinese working mothers shared humorous clips of their experience of working from home with their children who were also at home during the pandemic lockdown. By analysing the themes, protagonists, and humour techniques of a sample of 85 videos, we ask why the mood of these clips is so strongly marked by humour, and what this tells us about contemporary Chinese society, particularly about the position of women and mothers. We show that these memetic clips consist of three distinct genres of mothers working from home: (1) ‘balancing mothers’ who balance between work and childcare, (2) ‘pedagogic mothers’ who give childcare tips, and (3) ‘commercially oriented’ mothers who offer tutorials by means of product placement and advertisement. While these memes express what Mary Douglas called ‘a joke in the social structure’ without offering either relief or critique, they do create an online joking culture that offers temporary relief as well as awareness that others are in the same position. Our analysis tempers enthusiastic claims about both the critical potential of humour and the new ‘liberating’ affordances offered by digital platforms to produce liberating female spaces.

Hydropneumopericardium is defined as the presence of air and water in the pericardial cavity. Several causes have been postulated which can lead to hydropneumopericardium including trauma, infections secondary to gas-producing bacilli, fistula formation, positive pressure ventilation or even spontaneously without an underlying cause in healthy adults and rarely after pericardiocentesis. We report an uncommon instance of hydropneumopericardium after pericardiocentesis in a 35-year-old man, which developed due to a leaky drainage system. It was immediately drained through the subxiphoid approach under echocardiographic guidance, and the patient was relieved. Hydropneumopericardium is an uncommon but easily diagnosable and avoidable complication of pericardiocentesis. It should be suspected whenever the patient develops increasing dyspnoea following a temporary relief by pericardiocentesis.

Purpose: Topical steroids are considered to be the first line of treatment for some forms of idiopathic punctal stenosis causing epiphora. We present a case of epiphora, which was unresponsive to traditional management, but responded to one week of systemic steroids. Observations: A 48-year-old male with no significant medical history, was referred to us for excessive tearing. He was seen earlier by an ophthalmologist for redness in one of his eyes. He was prescribed Pataday (Olopatadine) eye drops and Zylet (Tobramycin, Lotepriol) eye drops for both eyes. The patient was seen again after a week of treatment without much improvement. His treatment was changed to Alrex (Loteprednol) and Azasite (Azithromycin) eye drops. The redness of the eye disappeared in a week but patient noticed excessive tearing of his eyes. He was referred to reconstructive ophthalmologist for treatment of possible punctal stenosis. On examination the height of tear meniscus was increased and diagnostic lacrimal probing and syringing of the lacrimal drainage system demonstrated both lacrimal drainage system patent. Punctal stenosis was diagnosed. Over the next couple of weeks, patient underwent multiple syringing of the lacrimal duct system with temporary relief of symptoms. He was also prescribed Tobradex (Tobramycin+Dexamethasone) eye drops to be used for a month. Eye drops helped for the duration of treatment but symptoms returned after a couple of days of discontinuing eye drops. Patient also received ductal probing and punctal dilation in the office without much relief. Mini perforated plugs were placed in both puncta resulting in temporary relief. At the same time the intra ocular pressure started to rise and Tobradex eye drops were discontinued immediately. After almost three months of treatment and continuation of debilitating symptoms of epiphora, patient decided to consider surgical intervention and consulted an ENT surgeon for second opinion. Since he was responding to topical steroids, which was causing an increase in his intra ocular pressure, the ENT surgeon suggested Medrol Pack (Prednisolone for a week). Patient was symptom free after 4 days of treatment and by 7th day, his eyes were back to normal with complete disappearance of epiphora symptoms. Conclusions and importance: Systemic steroids can be beneficial in patients suffering from Idiopathic/Iatrogenic punctal stenosis unresponsive to topical steroids and conventional treatments.
