In Russia, Hemorrhagic Fever with Renal Syndrome (HFRS) takes the leading place among all natural focal infections, the incidence rate in 2018 was 3,99 per 100 thousand population. In the Northwestern Federal District of the Russian Federation, HFRS occurs much less frequently – 1,46 cases per 100 thousand population. The disease is more susceptible to men aged 16–50 years, children rarely get sick. In 2019, 73 cases of HFRS (1,36 per 100 thousand population) were registered in St. Petersburg, among them only 2 episodes – in children under 18 years of age. A clinical case of HFRS in a child aged 1 year and 8 months is presented. The diagnosis was established on the basis of clinical and epidemiological data (acute onset of the disease, the presence of fever, thrombohemorrhagic syndrome, oliguria, anuria, information on cases of HFRS at the place of residence of the child) and laboratory data (detection of specific IgM antibodies to the Hantaan virus) in the later stages of the disease. The described case should alert pediatricians regarding possible natural focal diseases, including HFRS. A thorough analysis of the anamnestic, epidemiological and clinical data of the patient will help in the early stages of the disease to suggest a diagnosis, conduct laboratory diagnostics and prescribe appropriate therapy.

Aim. To lay down criteria for the differential diagnosis of Crimean-Congo hemorrhagic fever with a number of natural focal diseases prevalent in the Astrakhan region. Methods. A retrospective analysis of clinical, epidemiological and laboratory data of patients with Crimean-Congo hemorrhagic fever (74 cases), Astrakhan rickettsial fever (75), Q fever (75) and leptospirosis (25) was carried out in 2000-2013. Results. The basic clinical symptoms characteristic of the Crimean-Congo hemorrhagic fever for the differential diagnosis of Astrakhan rickettsial fever, Q fever and leptospirosis were distinguished. The early diagnosis of Crimean-Congo hemorrhagic fever was based on the epidemiological data taking into account spring-summer seasonality of the disease, patient’s visits to countryside, contact with animals, and data about tick bites and contact with ticks. Timely prescription of adequate antiviral and pathogenetic therapy helped to reduce the rate of severe forms and mortality from Crimean-Congo hemorrhagic fever. Conclusion. Differential diagnostic features of Crimean-Congo hemorrhagic fever were: the presence of the primary affect on the site of the tick bite, high body temperature, two-wave fever, significant signs of intoxication, the relative and absolute bradycardia, hemorrhagic rash, abdominal bleeding, changes of peripheral blood - leukopenia and thrombocytopenia.

The conflict may be solved by suggestinga) that the »thrombotic proneness« in A compared with 0 causes a poorer prognosis in CHD among the former, leaving a disproportionate excess of 0 among longterm CHD survivors, and b) that AB0-related factors have had an insignificant, independent impact on the evolution of preclinical coronary artery disease in our 71 men with latent CHD.

Eosinophilic enteritis, an increasing recognized condition, is rare and often presents with non-specific symptoms. We report a case of a 46-year old female who presented with acute onset abdominal pain and nausea associated with ascites, small bowel thickening and peripheral eosinophilia. Diagnosis was confirmed by biopsies taken at esophagogastroduodenoscopy demonstrating diffuse infiltration by inflammatory cells, mainly eosinophils. Appropriate therapy was instituted. The patient recovered well and was symptom-free at 1-month follow up. In this report, we discuss the clinical presentation and the diagnostic criteria of the eosinophilic enteritis, and examine the pathophysiological theories and therapeutic strategies. The relevant literature on eosinophilic enteritis is summarized.

The paper presents analysis of epidemiological situation on hemorrhagic fever with renal syndrome around the world and in the Russian Federation over the period of 2009–2018. 5855 cases of hemorrhagic fever with renal syndrome were registered in Russia in 2018 (3.99 per one hundred thousand of the population). Downward trend is observed as compared to 2017 (by 29.6 %). Cases of cluster infection were not reported. It is established that the highest level of morbidity, exceeding the average level across Russia 3.9 times, was noted in the Volga Federal District, where 77.5% of the total cases occurred. The territory of the Russian Federation was differentiated by the HFRS incidence rates. The areas with high levels of morbidity included the entities of the Russian Federation with intensive index range between 9.08 and 41.39 per one hundred thousand of the population, among them Republics of Bashkortostan, Mari El, Tatarstan, and Mordovia, Udmurt and Chuvashi Republics, Kirov, Nizhny Novgorod, Penza, Samara, Ulyanovsk, Kostroma, Yaroslavl, and Jewish Autonomous Regions. The forecast for preservation of tense epidemiological situation on HFRS incidence in the territory of the Volga Federal District was substantiated.

The criterionof inclusion in literature search is the year of article publishing starting in 2010until 2018, and the English full article. Search keywords are Progressive MuscleRelaxation, breast cancer, quality of life and chemotherapy. Literature analysisuses PICOT (Population, Intervention, Comparison, Outcome, and Time).Based on the analysis found 5 themes, namely the quality of life characteristicsof patients undergoing chemotherapy, PMR as a relaxation therapy, PMR as apsychological stress reduction intervention, PMR as an intervention to increasemuscle metabolism, and PMR as a pain reduction intervention.Patients who undergoing chemotherapy will experience a decrease in QOL. PMRis a recommended intervention to improve patient QOL.

Background Orthohantaviruses, causing hemorrhagic fever with renal syndrome (HFRS) and hantavirus cardiopulmonary syndrome, pose a significant public health threat worldwide. Despite the significant mortality and morbidity, effective antiviral therapeutics or vaccines for orthohantavirus infections are currently unavailable. This study aimed to investigate the prevalence of HFRS-associated orthohantaviruses and identify the etiological agent of orthohantavirus outbreaks in southern Republic of Korea (ROK). Methodology/Principal findings We collected small mammals on Jeju Island during 2018–2020. We detected the Hantaan virus (HTNV)-specific antibodies and RNA using an indirect immunofluorescence assay test and reverse transcription-polymerase chain reaction on Apodemus agrarius chejuensis (A. chejuensis). The prevalence of anti-HTNV antibodies among rodents was 14.1%. A total of six seropositive mice harbored HTNV RNA. The amplicon-based next-generation sequencing provided nearly full-length tripartite genomic sequences of six HTNV harbored by A. chejuensis. Phylogenetic and tanglegram analyses were conducted for inferring evolutionary relationships between orthohantaviruses with their reservoir hosts. Phylogenetic analyses identified a novel distinct HTNV genotype. The detected HTNV genomic sequences were phylogenetically related to a viral sequence derived from HFRS patient in southern ROK. Tanglegram analysis demonstrated the segregation of HTNV genotypes corresponding to Apodemus spp. divergence. Conclusions/Significance Our results suggest that A. chejuensis-borne HTNV may be a potential etiological agent of HFRS in southern ROK. Ancestral HTNV may infect A. chejuensis prior to geological isolation between the Korean peninsula and Jeju Island, supporting the co-evolution of orthohantaviruses and rodents. This study arises awareness among physicians for HFRS outbreaks in southern ROK.

In the European part of Russia, the highest number of hemorrhagic fever with renal syndrome (HFRS) cases are registered in the Volga Federal District (VFD), which includes the Republic of Tatarstan (RT). Puumala orthohantavirus (PUUV) is the main causative agent of HFRS identified in the RT. The goal of the current study is to analyze the genetic variations of the PUUV strains and possible presence of chimeric and reassortant variants among the PUUV strains circulating in bank vole populations in the Trans-Kama area of the RT. Complete S segment CDS as well as partial M and L segment coding nucleotide sequences were obtained from 40 PUUV-positive bank voles and used for the analysis. We found that all PUUV strains belonged to RUS genetic lineage and clustered in two subclades corresponding to the Western and Eastern Trans-Kama geographic areas. PUUV strains from Western Trans-Kama were related to the previously identified strain from Teteevo in the Pre-Kama area. It can be suggested that the PUUV strains were introduced to the Teteevo area as a result of the bank voles’ migration from Western Trans-Kama. It also appears that physical obstacles, including rivers, could be overcome by migrating rodents under favorable circumstances. Based on results of the comparative and phylogenetic analyses, we propose that bank vole distribution in the Trans-Kama area occurred upstream along the river valleys, and that watersheds could act as barriers for migrations. As a result, the diverged PUUV strains could be formed in closely located populations. In times of extensive bank vole population growth, happening every 3–4 years, some regions of watersheds may become open for contact between individual rodents from neighboring populations, leading to an exchange of the genetic material between divergent PUUV strains.

Objective: Epidemiological data analysis concerning the spread of brucellosis in Russia and Primorsky Region, study of the manifestations of the epidemic process and the current state of brucellosis issue.Methods: Descriptive epidemiological analysis of materials on infectious morbidity based on state statistical reporting forms No. 1, No. 2 “Information on infectious and parasitic morbidity”.Results: Brucellosis affects workers in the main occupational groups of livestock breeders, workers involved in the processing of livestock, who contact sick animals and other sources of brucellosis infection. Primorsky Region is a territory with an unstable epidemiological situation on brucellosis. A different morbidity rates were registered over the years. However usually it does not exceed the national average rate. In 2009, the morbidity rate counted 27.5% less than in Russian Federation (0.21 and 0.29 per 100,000 respectively), compared to previous years, the average long-term morbidity rate in the region increased by 3.5 times. The disease rate for the period from 2009 to 2019 ranged from 0.05 to 0.21%. In 2012, it exceeded the index in the Far Eastern Federal District (0.1 per 100,000 population). Brucellosis cases were detected among livestock breeders during an extraordinary medical examination due to the bad situation on brucellosis among animals.Conclusions: In recent decades, the epizootic and epidemiological situation on brucellosis remains tense due to the decrease in the number of serological and bacteriological studies among animals and humans, the weakening of veterinary-sanitary control and the formation of new private farms.
