A large proportion of patients with metastatic colorectal cancer (mCRC) experience disease progression after first-line treatment with chemotherapy and bevacizumab, an anti-angiogenic agent, as a result of acquired resistance. However, blocking angiogenesis by targeted therapy towards the vascular endothelial growth factor (VEGF) pathway still forms an essential part of the second-line treatment strategy. Although three approved evidence-based choices for angiogenic agents (continuing treatment with bevacizumab, ramucirumab and aflibercept) are currently available in the second line, making the most effective choice is challenging due to the lack of studies directly comparing these agents. Moreover, despite huge investigational efforts, no predictive biomarker for anti-angiogenic cancer therapies could be identified so far.

BackgroundRecommendations on current clinical guidelines are informed by limited economic evidence.AimsA formal economic evaluation of three modalities of psychotherapy for patients with cluster B personality disorders.MethodA probabilistic decision-analytic model to assess the cost-effectiveness of out-patient, day hospital and in-patient psychotherapy over 5 years in terms of cost per recovered patient-year and cost per quality-adjusted life-year (QALY). Analyses were conducted from both societal and payer perspectives.ResultsFrom the societal perspective, the most cost-effective choice switched from out-patient to day hospital psychotherapy at a threshold of €12 274 per recovered patient-year; and from day hospital to in-patient psychotherapy at €113 298. In terms of cost per QALY, the optimal strategy changed at €56 325 and €286 493 per QALY respectively. From the payer perspective, the switch points were at €9895 and €155 797 per recovered patient-year, and €43 427 and €561 188 per QALY.ConclusionsOut-patient psychotherapy and day hospital psychotherapy are the optimal treatments for patients with cluster B personality disorders in terms of cost per recovered patient-year and cost per QALY.

Analogue aerial-photopraphs external orientation reconstruction based on geoportal data For acquisition of source data for geoinformation analyses is necessary to do some field works. This way of data acquisition is time-consuming. In this case, photogrammetric and remote sensed methods can be more effective choice. Especially orthophotomap extracting is more effective process in creation of geodata. It is good foundation for further analysis and nice extension of existing geographical information systems. Despite fast growth of photogrammetry there are plenty of analogue, archival airphotos which can be used for geoinformation analysis. They are quiet up to date and scanned in very high resolution which means they can be used for really reliable analysis. The problem is very important because many of analogue, archival air photos do not contain photogrammetric warp. The aim of this paper is expression of applicability of geoportal webpage, which is part of INSPIRE directive, that can be used for external orientation reconstruction when there is no other georeference data.

Dysphagia associated with the cricopharyngeus muscle (CPm) dysfunction negatively influences the quality of life. This high-pressure region must relax and the lumen must open for smooth food passage. The CP muscle is therefore a common target of chemodenervation with botulinum toxin (BTX). Here we presented a patient with severe left lateral medullary syndrome and non-relaxation of the CPm. We described how to localize the CPm in the transverse and longitudinal views under ultrasonography and offered a video demonstrating ultrasonography-guided BTX injection. Ultrasonography-guided CPm injection with BTX may serve as a reliable, rapid, and effective choice for treatment of cricopharyngeal dysphagia.

This chapter explores the limits of simulations for university-level historical education. The authors develop an alternative gamic mode more fit for purpose by focusing on epistemology and procedural rhetoric. This chapter will start by examining how history functions as a form of disciplinary knowledge and how this disciplinary way of knowing things is taught at the post-secondary level. The manner in which history is taught will be contrasted with its evaluation in order to better define what students are actually expected to learn. The simulation will be then examined in the light of learning goals and evaluation. This will demonstrate that simulations are in fact a poor fit for most post-secondary history courses. The more appropriate and effective choice is to construct the past via procedural rhetoric, using games that mirror the structure of the historical argument.

The selection of the delivery scheme is one of the most complicated problems and the results of its solution condition the sustainable development of the whole market of transportation services. Freight forwarders should consider numerous random parameters characterizing demand and technological processes to choose the proper structure of the logistics chain. The paper aims to propose a method for choosing the logistics chain structure, based on the analysis of the total expenses as a function from the demand parameters characterizing stochastic variables of the consignment weight, the delivery distance, and the time interval between the requests in the flow of queries for cargo delivery. Four basic logistics chain structures, widely used on road transport, are described to demonstrate the selection process. The areas of the most efficient use of the logistics chain structures can be defined for the flow of requests for cargo deliveries. The paper shows such areas on the example of goods delivery by automobile transport. Determining the areas of the most efficient use of the specific logistics chain structures contributes to the effective choice of correct delivery variants by freight forwarders.
