Agreement Analysis of Pentacam and Lenstar in Measurement of Keratometry, Anterior Chamber Depth and Lens Thickness

Introduction: To assess the agreement of the Scheimpflug camera system Pentacam with the optical low-coherence reflectometry (OLCR) device LENSTAR LS900 in measuring anterior segment biometry. Methods: This is a prospective, non- randomized, comparative analysis study. Patients with age-related nucleus sclerosis cataract who fulfilled the inclusion criteria and attended the Ophthalmology Clinic in University of Malaya Medical Centre (UMMC) between December 2011 and March 2012 were recruited. The keratometer, anterior chamber (AC) depth and lens thickness were measured with both methods Pentacam and Lenstar. Results: 223 eyes of 125 patients were analysed. There was an agreement in the keratometer, anterior chamber depth and lens thickness measurement between the two devices. There was a significant correlation between K-reading as measured by both devices, at 0.05 significant level, with correlation coefficient of 0.904 and p < 0.001. The correlation coefficient of AC depth measurement between Pentacam and Lenstar at 0.05 significant level was 0.992 (p < 0.001). From the 124 eyes in which the lens thickness were measureable with both Pentacam and Lenstar, a positive correlation was noted (correlation coefficient of 0.585, p-value of < 0.001). There was no significant difference in the three parameters between males and females and among Malay, Chinese and Indian patients. Conclusions: There is a statistical agreement in the anterior segment biometry measurements between the Pentacam and the Lenstar.

Abstract Purpose: To evaluate the anatomical effects of implantable phakic contact lens (IPCL) (Care Group, India) on anterior segment and its visual outcomes .Patients and methods: In a prospective interventional case series study, 60 highly myopic eyes of 32 patients were subjected to IPCL implantation in the Ophthalmology Department of Minia University Hospital, Egypt from January 2019 to June 2021. All patients had complete ophthalmic examination and were followed up for 1 year. Pentacam was used for preoperative and postoperative estimation of anterior chamber depth (ACD), anterior chamber angle (ACA), anterior chamber volume (ACV) and IPCL vault in the 1st, 3rd, and 12th months. Assessment of corneal endothelium was done using specular microscope preoperatively and after 12 months. Preoperative and postoperative refraction and visual acuity were measured. Results: There was a statistically significant decrease in ACD, ACA, and ACV. There was no significant difference between preoperative and postoperative mean intraocular pressure (IOP) by the 12th month (P=0.163). The mean preoperative endothelial cell count (ECD) was significantly reduced from 2929.3±248 cells/mm2 to 2737.9±303 cells/mm2 at the 12th month (P<0.001). with a statistically highly significant improvement of mean Log Mar uncorrected visual acuity (UCVA) from 1.48±0.19 preoperatively to 0.46±0.11 by the end of follow up (P<0.001) with insignificant difference between preoperative best corrected visual acuity (BCVA) and postoperative UCVA (P=0.209). In the 12th month, the mean vault was 240±540 μm. No sight threatening complications occurred.Conclusion: Although IPCL induced anatomical changes, it was safe and effective for correction of high myopia.

Purpose:  To determine the Anterior Chamber depth and Intraocular pressure change following uncomplicated phacoemulsification. Study Design:  Quasi experimental study. Place and Duration of Study:  Layyton Rehmatullah Benevolent Trust Free Eye and Cancer Hospital, Lahore from 16/12/2017 to 15/06/2018. Methods:  One hundred and thirty nine patients who came to Layyton Rehmatullah Benevolent Trust Free Eye for phacoemulsification and IOL implantation were included in the study. Anterior Chamber depth was measured with IOL Master and intraocular pressure was determined using Goldmann Applanation Tonometer one day before and one month after surgery. Data was recorded on a self-designed proforma. Comparison between pre- and post-operative data with respect to change was analyzed by chi-square test. P-value ≤ 0.05 was considered significant. Results:  The mean intra-ocular pressure dropped from 14.36 ± 4.19 mmHg to 12.14 ± 4.26 mmHg 1 month after surgery with a mean drop of 2.21 ± 0.65 mmHg while the mean anterior chamber depth increased from 2.31 ± 0.08 mm to 3.59 ± 0.37 mm with a mean increase of 1.29 ± 0.36 mm. There was no statistically significant difference in the mean change of intra-ocular pressure and anterior chamber depth across various subgroups based on patient’s age, gender and duration of cataract. Conclusion:  Phacoemulsification and IOL implantation not only improves the visual acuity by removing the cataract but also deepens the anterior chamber and decreases intra-ocular pressure, which can have a beneficial effect in glaucoma patients. Purpose. To evaluate anterior segment’s structures by Pentacam in patients with tilted disc syndrome (TDS).Methods. Group 1 included forty-six eyes of forty-six patients who have the TDS. Group 2 including forty-six eyes of forty-six cases was the control group which was equal to the study group in age, gender, and refraction. A complete ophthalmic examination was performed in both groups. All cases were evaluated by Pentacam. The axial length (AL) of eyes was measured by ultrasound. Quantitative data obtained from these measurements were compared between two groups.Results. There was no statistically significant difference for age, gender, axial length, and spherical equivalent measurements between two groups (p=0.625,p=0.830,p=0.234, andp=0.850). There was a statistically significant difference for central corneal thickness (CCT), corneal volume (CV), anterior chamber angle (ACA), and pupil size measurements between two groups (p=0.001,p=0.0001,p=0.003, andp=0.001). Also, there was no statistically significant difference for anterior chamber depth (ACD), anterior chamber volume (ACV), and lens thickness (LT) measurements between two groups (p=0.130,p=0.910, andp=0.057).Conclusion. We determined that CCT was thinner, CV was less, and ACA was narrower in patients with TDS. There are some changes in the anterior segment of the eyes with tilted disc.

Purpose: To investigate the distribution of white-to-white (WTW) corneal diameter and anterior chamber depth (ACD) in Chinese myopia patients.Methods: This was a cross-sectional observational study conducted at five ophthalmic centers. Anterior segment biometry was performed in 7,893 eyes of the 7,893 myopic patients using Pentacam, and the WTW and ACD were recorded. The distribution patterns of WTW and ACD were evaluated and the correlation between WTW and ACD was analyzed statistically.Results: There were 4416 (55.95%) males and 3477 (44.05%) females. The age of the study population was 25.14 ± 5.41 years. Distribution of WTW was slightly positively skewed (Skewness = 0.0076, Kurtosis = 0.3944, KS P = 0.020) with a mean of 11.65 ± 0.38 mm and a 95% normal range of 10.91–12.39 mm. A significant difference in WTW was found among different myopia groups (P &lt; 0.001). The ACD was normally distributed (Skewness = 0.899, Kurtosis = 0.027, KS P = 0.086). The mean ACD was 3.25 ± 0.26 mm and the 95% normal range of was 2.74–3.75 mm. A significant difference in ACD was also found among different myopia groups (P = 0.030). There was a significant correlation between WTW and ACD (r = 0.460, P &lt; 0.001).Conclusions: In our study, 95% of the Chinese myopic patients had a WTW within 10.91–12.39 mm and an ACD within 2.74–3.75 mm. ACD and WTW were significantly different among different myopia, gender and age groups. WTW was positively correlated with ACD.

Aim: To evaluate ocular anterior chamber depth (ACD) and body mass index (BMI) in a normal population in Port Harcourt City Local Government Area (LGA), with a view to determine formulae in estimating intraocular lens power for cataract surgeries and possible association with angle closure glaucoma and other ocular pathological conditions.            Methods: This is a multi-stage study with inclusion criteria of Visual Acuity > 6/18, age greater than 18 years with no history of past ocular surgeries or trauma. Data obtained through a structured proforma included age, sex, tribe, occupation and level of education. Body Mass Index (BMI) was measured using a standard height and weight automated scale (SECA 769,220). Comprehensive ocular examination done and Anterior Chamber Depth (ACD) measured using Amplitude (A) scan ultrasonography (SONOMED PACSCAN 300AP). Data was analyzed using SPSS (Version 17), and p value was set at ≤ 0.05. Results: Four hundred and sixty six (466) subjects participated in the study made up of two hundred and twelve (212) males (45.5%) and two hundred and fifty four (254) females (54.5%) with M: F ratio of 1:1.2. The age range was 18-92 years and mean age of the subjects studied 43.0±14.2 years. Findings revealed mean ACD and Body Mass Index to be 3.1±0.5 mm and 26.9±6.2 kg/m2 respectively. The mean ACD was greater in males than females. There was a statistically significant relationship between age and ACD. Obesity was found to be higher in females (n=97; 78.2%) compared to the males among those with BMI >30Kg/m2 and this was found to be statistically significant (p=0.0001). A larger proportion of subjects with normal BMI and overweight BMI 25-29.5 Kg/m2 were males. There was a statistically significant difference in the ACD values between genders among those overweight (BMI 25- 29.5 Kg/m2) and the obese (BMI >30 Kg/m2). Conclusion: There was a statistically significant difference in the ACD values between genders among those overweight.

Abstract Background To assess the changes of anterior chamber angle in patients with shallow anterior chamber and normal anterior chamber after phacoemulsification and intraocular lens implantation (IOL) using anterior segment swept-source optical coherence tomography (AS-SS-OCT). Methods This was a prospective case control study; 60 eyes of 60 patients who underwent cataract surgery. Based on anterior chamber depth (ACD) and gonioscopy findings, the eyes were classified into two groups, shallow anterior chamber group (30 eyes) and normal anterior chamber group (30 eyes). AS-SS-OCT was used to measure ACD and angle parameters angle opening distance (AOD), angle recess area (ARA), trabecular iris space area(TISA), and trabecular iris angle (TIA). Serial changes in each group were measured before and 3 months after phacoemulsification and IOL, and the differences between the two groups were compared. Results All angle parameters including ACD, AOD, ARA, TISA, and TIA in both groups at 3 months after cataract surgery were significantly different from the preoperative values (p<0.01). In addition, we found significant differences of all angle parameters between groups of shallow anterior chamber (SAC) and normal anterior chamber (NAC) (All P< 0.05). Besides, the TISA750 in superior quadrant and anterior chamber volume (ACV) were significantly smaller in the group of SAC than those in the group of NAC (P = 0.041 and 0.002, respectively). Conclusions The parameters of anterior chamber increased significantly in both two groups but not increase to the same extent 3 months after phacoemulsification. Iris thickening may also be one of the narrow angle factors,not just age-related lens thickening.

Purpose: To compare the parameters of capsulorrhexis and intraocular lens decentration after femtosecond laser capsulotomy and manual continuous curvilinear capsulorrhexis in high myopic patients with cataracts.Methods: This is a prospective consecutive non-randomized comparative cohort study. Selected patients with axial length &gt; 26.0 mm were divided into femtosecond laser capsulotomy (FS) group and manual continuous curvilinear capsulorrhexis (CCC) group. Five experienced phacoemulsification surgeons conducted all surgeries. Intraoperative complications and post-operative anterior segment photography were recorded. Intraocular lens decentration, area of capsulorrhexis, circularity, and capsule overlap were measured at 1 week, 1 month, and 2 years after surgery. Between group differences of parameters were determined with independent-sample t-test or the Mann–Whitney U-test, analysis of variance test, Pearson chi-square test, and Spearman rank correlation test.Results: The study included 142 eyes (108 patients), 68 eyes in the FS group, and 74 eyes in the CCC group. At 1 week, 1 month, and 2 years after surgery, the area of capsulorrhexis in the CCC group was significantly larger than in the FS group (P &lt; 0.05), while no significant difference was noted in circularity values. The complete overlap ratio in the FS group was significantly higher than that in the CCC group (P &lt; 0.05) at each measured timepoint. Significant correlations were noted between the anterior chamber depth and the area of capsulorrhexis in the CCC group (R = 0.25, P = 0.04), but did not correlate in the FS group (P &gt; 0.05). In patients with an anterior chamber depth &gt;3 mm, the capsule-intraocular lens (IOL) overlap of the CCC group was less than that of the FS group at all measured timepoints after surgery (P &lt; 0.05). Meanwhile, the IOL decentration in the CCC group was significantly greater than that of the FS group in those patients at 2 years after surgery (P &lt; 0.05).Conclusion: In high myopic patients with cataracts, with anterior chamber depth more than 3 mm, femtosecond laser capsulotomy can achieve better capsulorrhexis sizing and centering. Due to more precise capsulotomy and a better capsule-IOL overlap in the FS group, femtosecond laser capsulotomy resulted in better long-term centration of the IOL.

Abstract Purpose: To determine patient preoperative anatomical features and the parameters of implantable collamer lenses (ICLs) relevant in explaining vault variability.Setting: Ophthalmology Xi’an Aier Gucheng Eye hospital , Xi’an China.Design: Retrospective case series.Methods:This study comprised 88 eyes of 45 patients implanted with myopic or toric ICLs between May 2021 and August 2021.Pentacam imaging was used for assessing white-to-white (WTW) diameter, central keratometry, anterior chamber depth(ACD), central corneal thickness and vault. Anterior-segment optical coherence tomography(AS-OCT) was used to measure the horizontal anterior chamber angle distance (ATA). Ultrasound biomicroscopy (UBM) was used to measure horizontal and vertical sulcus to sulcus(vSTS). According to different ACD,we divide ACD into shallow group(2.8 to 3.2 mm),medium group(3.2 to 3.5 mm),and deep group(>3.5mm).Results: Linear regression analysis found significant correlation between WTW diameters and ATA diameters( y= 0.9605x+0.1491,R² = 0.9148),with a coefficient of determinant of 0.9148 (P<0.01). Linear regression analysis found significant correlation between hSTS and vSTS(y=0.9855x-0.0178,R²=0.1979),with a coefficient of determinant of 0.1979 (P<0.01). WTW diameters showed statistically significant difference in shallow and medium ACD (t=-3.28,P<0.01 ), significant difference in shallow and deep ACD (t=-4.69,P<0.01 ),there was no correlation in medium and deep ACD(t=-1.41,P>0.05 ). There was a statistically significant correlation between WTW diameters and hSTS diameters when the ACD was less than 3.5 mm (t=-0.451, P=0.000) (t=1.406, P=0.026),but weak correlation when the ACD was bigger than 3.5 mm(t=1.594, P=0.051). ATA and WTW with a mean difference close to zero(-0.66 mm). Despite the relatively high correlation (intraclass correlation co-efficient =0.689), the range of agreement is quite broad (1.33 mm).Conclusions:ATA measured with AS-OCT can not be used interchangeably with WTW obtained with Pentacam. WTW diameters and hSTS diameters have a statistically significant correlation when the ACD was less than 3.5 mm. WTW, ATA, hSTS and vSTS all should be considered during design the size of lens and regulating the vault after surgery.

Purpose: tо evaluate the diagnostic capabilities of optical coherence tomography (OCT), Scheimpflug camera and ultrasound biomicroscopy (UBM) in examining topography of the ocular anterior segment and lens in mature intumescent cataract. Patients and methods. 23 eyes with mature intumescent cataract were examined using OCT (RTVue-100, Optovue, USA), Scheimpflug camera (Pentacam HR, Oculus, Germany) and UBM (UD 8000, Tomey, Japan). We examined the anterior chamber depth, profile and magnitude of the anterior chamber angle, distance “trabecula-iris at 500 microns”, thickness of the liquid lenticular mass layer, lens thickness, Zinn ligament length within four segments, equatorial angle. Results. We managed to measure the anterior chamber depth using UBM and Scheimpflug camera. UBM measurement was 1.96 ± 0.14 mm, Scheimpflug camera — 1.91 ± 0.11 mm. We revealed narrowing of the anterior chamber angle up to 11.54 ± 2.19° in UMB examination, to 11.49 ± 2.17° in OCT measuring and to 11.63 ± 2.21° in examining by Scheimpflug camera. Distance “trabecula-iris 500” was 0.212 ± 0.037 mm in UMB examination, 0.218 ± 0.042 mm — by means of OCT, Scheimpflug camera measurement was unsuccessful. The rest parameters were examined only by UBM. The lens thickness was 5.26 ± 0.13 mm. The anterior layer of liquid lenticular mass was 0.85 ± 0.06 mm. Zinn ligament length in the outer segment was 0.708 ± 0.072 mm, in the internal segment — 0.731 ± 0.089 mm, in the superior segment — 0.704 ± 0.084 mm, in the inferior segment — 0.876 ± 0.089 mm. The equatorial angle in two opposite segments was 32.52 ± 0.92°. Conclusion. Ultrasound biomicroscopy has the biggest value in examining topography of the ocular anterior segment and lens in mature intumescent cataract, since only this method allows achieving the whole complex of data necessary to evaluate the swelling lens parameters. This can serve as a basis for developing the appropriate tactics of surgical intervention.
