Although there are several high-capacity screw piles in use currently, there are few studies on their lateral performance. The aim of this study is to investigate the lateral behaviour of several models of screw pile group (1×2), (2×1), and (2×2) embedded in soft clay and extended to stiff clay under lateral static load. Three spacing between pile (1.5, 3, and 4.5) Dh (helix diameter) with a shaft diameter of 10 mm, single and double helix, and embedded length ratio L/d 40 were used. The results showed that increasing the number of the piles in the group had a larger effect, the lateral resistance of group (2×2) increase to about (2.5 and 3.2) times more than groups (2×1) and (1×2) respectively. While the increase of pile spacing in the groups from (1.5 Dh) to (3 and 4.5 Dh) increases the lateral resistance about 6-23% and 16-52% respectively. Also, the result showed that the screw pile with double helix gives an increase in lateral resistance about 3-8% from the single helix.

Despite the great development in the manufacture of the helical pile and the development of their use, especially in transmission towers and wind turbines, there is little research on their lateral behavior. In this laboratory study investigate the behavior of screw piles group (2×1) and (1×2) with the spacing to the diameter of helix ratio (S/Dh =1.5, 3, and 4.5) having a diameter (10 mm) and embedded length to diameter ratio (L/D = 40) by using single and double helix embedded in soft clay and extend to stiff clay under a cyclic lateral load of frequency (0.2 Hz). The results showed that increasing the distance between the piles had a great effect on increasing the lateral resistance. the increase of pile spacing in the groups from (1.5 Dh) to (3 and 4.5 Dh) increases the lateral resistance about 34-38% and 50% respectively. Also, from result showed that the group (2×1) gave a lateral resistance more than the group (1×2) about 11% for single helix and about 6% for a double helix, and for the same spacing and configuration the screw pile with double helix gives an increase in lateral resistance about 5-10 % from the single helix.

Screw piles are widely used in a variety engineering applications supplying stability against compression, overturning moment, uplift tension, and horizontal loads. Screw pile is a famous solution for support light structures, roads and rail signs which have relatively low-capacity foundation. In this study, the behavior of circular (10) mm solid screw pile models embedded in a bed of soft clay soil covering a layer of sandy soil has been studied. The 200 mm thick sand layer was compacted in a steel container with a diameter of 300 mm into four sublayers. The sandy soil layer was compacted at a relative density of 70%. The 300 mm thick soft clay soil bed with Cu (30) kPa was compacted in six sub-layers on the sandy bottom layer. Model tests are carried out with screw piles with a length of 300 mm, 350 mm and 400 mm and a helix diameter of 30 mm. Also, single and double helix and different S/Dh ratio were used for these piles and a comparative study between screw piles and ordinary piles (without helices) is accomplished. This study revealed that introducing screw pile of double helix increases its bearing capacity in soft clay soil by up to a (4-8) % as compared to a single helix screw pile. The results showed that the behavior of screw pile essentially depends on the geometric properties of the pile. According to the achievements, compressive load capacity of screw piles depends on embedded length, spacing ratio (S/Dh) and number of helical plates.

Iron chelation therapy with deferoxamine (DFO),the current standard for the treatment of iron overload in patients with betathalassemia,requires regular subcutaneous or intravenous infusions. This can lead to reduced quality of life and poor adherence,resulting in increased morbidity and mortality in iron-overloaded patients with beta-thalassemia. Deferasirox (DFX) is an orally administered iron chelator that has been approved for use in many countries. The requirement of an effective,well tolerated iron chelator with a less demanding mode of administration has led to the development of deferasirox. The present study was aimed to compare the satisfaction and compliance with deferoxamine versus deferasirox (Exjade®),a novel oral iron chelator in patients with transfusion - dependent beta- thalassemia. A cross-sectional,single-center investigation study was carried out in the Thalassemia Center of Ibn-Atheer Teaching Hospital in Nineveh province,Iraq. One hundred and eight thalassemic patients aged between 2- 20 years old having received multiple blood transfusions and a serum ferritin greater than 1500 ng/ml. Patients were randomised into two groups. Group 1 received deferoxamine at a dose of 20-50mg/kg/day and group 2 received deferasirox at the dose of 10-30 mg/kg/day. Another 56 apparently healthy volunteers were used as a control group. The assessment of chelation was done during the period between November 2013 and February 2014 by measurement of serum ferritin. Satisfaction and compliance was assessed by using a special questionnaire prepared by the researcher. Out of the 108 thalassemic patients enrolled there was no discontinuation in treatment with the two drugs under study. The serum ferritin did not change significantly in any of the chelation groups. In comparison with the patients who were treated with DFO,those receiving DFX reported a significantly higher rate of compliance and satisfaction (P < 0.05). However,no significant difference was observed between the two groups regarding their satisfaction (P > 0.05).Compliance with deferasirox (50 %) was more than that with deferoxamine (20 %). Satisfaction with deferoxamine was significantly lower than deferasirox (p= 0.00).

The primary aim of this study was to compare the prevalence of subclinical hypothyroidism (SCH) using two different cut-off levels for TSH values (≥2.5 mIU/L versus ≥4.1 mIU/L). The secondary objective was to analyze the clinical-biochemical characteristics in women with and without SCH. This was a retrospective cross-sectional study. In total, 1496 Mexican women with infertility were included: Group 1, women with TSH levels ranging between 0.3 and 2.49 mIU/L, n = 886; Group 2, women with TSH between 2.5 and 4.09 mIU/L, n = 390; and Group 3, women with TSH ≥4.1 mIU/L n = 220. SCH prevalence was 40.7% (CI 95%: 38.3–43.3%) with TSH cut-off ≥ 2.5 mIU/L, and 14.7% (CI 95%: 12.7–16.5%) with TSH cut-off ≥ 4.1 mIU/L, (p = 0.0001). The prevalence of overweight was higher in Group 2 than in Groups 1 and 3. Thyroid autoimmunity, obesity and insulin resistance were higher in Group 3 than in Group 1 (p < 0.05). No other differences were observed between groups. Conclusions: The prevalence of SCH in our selected patients increased almost three times using a TSH cut-off ≥ 2.5 mIU/L compared with a TSH cut-off ≥ 4.1 mIU/L. Women with TSH ≥4.1 mIU/L compared with TSH cut-off ≤ 2.5 mIU/L more often presented with obesity, thyroid autoimmunity and insulin resistance.

Abstract Background Pruritus is a common and often distressing symptom in patients with chronic kidney disease. Though the pathogenesis of uremic pruritus remains poorly understood, systemic inflammation has presented itself as one of the possible explanations. High blood lead levels (BLLs) have been noted to be associated with inflammation and poor nutritional status in hemodialysis patients. Our aim is to study the relation between blood lead levels and uremic pruritus. This is a cross-sectional study that enrolled 50 patients; all were on regular hemodialysis 3 times per week for at least 6 months. Patients were divided into 2 groups, group 1 (n =10) with no pruritus and group 2 (n=40) with varying degrees of pruritus. Group 2 was further divided according to intensity of pruritus by visual analog score (VAS) into mild (n=10), moderate (n=20), and severe pruritus (n=10). Results There was a significant difference in serum lead levels and ferritin levels between groups 1 and 2 (p value < 0.01 and < 0.05, respectively). There was a statistically significant difference in serum lead levels in the groups with varying intensity of pruritus, having higher serum lead levels in patients who exhibited severe pruritus (p value < 0.005) Moreover, a statistically significant relation between elevated blood lead levels and the duration of dialysis was observed in this study. Conclusion Uremic pruritus is a multi-factorial phenomenon, and our study showed that blood lead levels in hemodialysis patients might be associated with increased intensity of pruritus.

Abstract Objective Japanese encephalitis (JE) is an arthropod-borne flavivirus infection having high mortality and morbidity. This study was performed to evaluate the conventional magnetic resonance imaging (MRI) findings in JE and to find out any difference between pediatric and adult JE. Materials and Methods This retrospective study was performed on serologically positive 54 JE patients presented to a tertiary care hospital with acute encephalitic symptoms between April 2016 and October 2019. Relevant neurological examination, cerebrospinal fluid analysis, and MRI scan of the brain were performed. Results Fifty-four JE patients (n = 31 males and n = 23 females) having 32 pediatric and 22 adult JE were included in the study sample. Group 1 JE (n = 16) patients had encephalitic symptoms with duration less than 15 days up to the day of MRI scan and group 2 JE (n = 38) had symptoms more than 15 days. Group 1 JE had mean apparent diffusion coefficient (ADC) value of 0.563 ± 0.109 (standard deviation [SD]) × 10–3 mm2/sec and group 2 JE had 1.095 ± 0.206 (SD) × 10–3 mm2/sec. The mean ADC value of pediatric JE was 0.907 ± 0.336 (SD) × 10–3 mm2/sec and adult JE was 0.982 ± 0.253 (SD) × 10–3 mm2/sec. Conclusion The majority of the JE patient shows abnormal signal alterations in bilateral thalami and substantia nigra. Diffusion-weighted imaging with ADC mapping helps in evaluating the stage of the JE. No statistical significance of the various conventional MRI findings was found between the pediatric JE and adult JE.

Abstract Background The antistreptolysins O “ASLO” are anti streptococcal antibodies directed against streptolysin O. The assay of ASLO can provide proof of infection of streptococcal origin. However, it cannot confirm the presence of RAA or the degree of severity of the disease. A level of ASLO less than or equal to a critical threshold of 200 IU/ml is considered normal in children. Several factors influence the ASLO title and therefore their standard rate such as age, geographic location and seasonal variation. The objective of this study is to determine the upper normal limit (ULN) of ASLO in healthy children with angina or RAA at the CHU Hassan II of Fez in Morocco. Methods This is a cross-sectional study carried out from January 2016 to July 2019 in the cardiopediatric unit of CHU Hassan II and the Anarjiss health center in Fez. Patients below 18 years of age were included in this study. The children are classified into 3 groups: group 1: 152 children with RAA and who are diagnosed according to the modified Jones criteria, group 2: 177 children with angina, group 3: 157 healthy children who did not have an RAA, a history of recurrent or recent pharyngitis. Patient data is collected on operating sheets. Statistical analysis was performed using SPSS v 21 software. To establish a normal upper limit for ASLOs, the 90 percentile was used as it is a value below which at least 90% of cases studied in each group. Results The most common age group in the 3 groups is the age group between 5 and 15 years old. The frequency of children of urban origin is greater than that of rural origin in all the groups studied. The ULN is 421.4U/ml in the group of normal children, 641.95 U/ml in the RAA group and 561.8 U/ml in the group of children with pharyngitis. The study of changes in ASLO rates over time shows that they increase significantly in angina and tend to decrease in RAA. The LSN of ASLOs by gender shows that it is higher for boys than for girls. The ULN according to the residence shows that it is higher in urban areas. As well as the LSN of ASLO according to the season shows that it is higher in the cold period, especially in winter and spring (P &lt; 0.001). With regard to age, the ULN of ASLOs is higher in the 5–15 age group. Conclusion In this study, it can be concluded that an ASLO level less than or equal to 400 U/ml is the critical threshold in a normal child, while an ASLO level &gt;400 U/ml is considered pathological in children. children in Morocco.
