Objective: To reduce instability rate after hip replacement (HR) in postmenopausal osteoporosis in multiparous women. Methods: Analyzed data of HR performed by 126 multiparous women with postmenopausal osteoporosis and hip fractures and their consequences. In the main group (n=65; 51.6%) of patients prevention of instability of endoprosthesis components was carried out using the proposed measures, in the control group (n=61; 48.4%) of patients – with traditional approaches. Clinical and X-ray methods, densitometry, a comprehensive method of assessing hip function by Harris, statistical methods are used in the implementation of the work. Results: In the main group influenced by proposed approaches in the dynamics have seen a decrease in the frequency of osteoporosis and due to this – an increasing osteopenia. In addition, there was a decrease in the frequency of instability of endoprosthesis components compared to the control group from 14.8% to 4.6% (p<0.005). Conclusions: Rational operation planning, and its implementation taking into account the peculiarities of the current of osteoporosis, continuous medication and rational post-operative rehabilitation are effective ways to reduce the frequency of instability in hip replacement in considered pathology. The article presents the results of rate hygienic assessment for sambo-practicing athletes, and shows the expediency of amateur combative sport involving students of higher educational institutions as an alternative or an elective course to traditional approaches is showny. During the research, 2 experienced groups were formed from among the students of Barnaul at the age of 19-21 years, engaged in sambo on an amateur level for 2-3 years, engaged mainly in anaerobic load, aimed at increasing muscle mass and as a control group, students, not engaged in any kind of sport, while related to the main group of health. As a result, it was found that physical performance and physical fitness indicators in most cases are statistically significantly higher in the experimental groups, compared to students who do not engage in any kind of sport, while belonging to the main group of health in relation to physical training, somatometric and somatoscopic the indices determined in the two control and experimental groups showed that students of sambo-wrestlers do not have pathology of the arch of the foot. Physiometric parameters reflecting the functioning of the cardiovascular and respiratory systems of the organism showed great training of the systems in the representatives of the first control group, which was reflected in the optimal values of arterial pressure and statistically significantly greater vital capacity of the lungs, which is due to prolonged aerobic and intermittent physical exertion. Thus, it seems promising to introduce the training process of fighting sambo in the physical activity of student youth.

Background. This systematic review and meta-analysis assessed the role of teriparatide in improving hip fracture healing and function to provide a clinical guide. Methods. The systematic literature review identified randomized controlled trials (RCTs) and controlled studies evaluating teriparatide for elderly hip fractures. A meta-analysis was performed using RevMan version 5.3. Results. This study included two RCTs and four retrospective studies comprising 607 patients, with 269 and 338 patients in the teriparatide and control groups, respectively. The quality of these six studies was moderate. Compared to the control group, teriparatide reduced the time to union (weighted mean difference WMD=−1.95; 95% confidence interval (CI): -3.23–-0.68; P=0.003) but did not improve the rate of fracture union at 3 months (odds ratio OR=1.46; 95% CI: 0.50–4.24; P=0.49) or 6 months (OR=0.89; 95% CI: 0.44–1.81; P=0.75). In addition, teriparatide did not decrease the complications, need for reoperation, mortality, rate of deformity after fracture healing, and subsequent fracture or improve hip function. Conclusions. The current limited evidence did not support that teriparatide improves fracture healing in hip fractures, due to study heterogeneity and various sources of biases. Further high-quality, large-sample trials are needed. This trial is registered with PROSPERO with registration number CRD42020152205.

The article deals with the problem of physical therapy after total hip arthroplasty. The urgent issue of early mobilization and activation of patients after total hip arthroplasty by means of physical therapy is raised. The purpose of the article is to evaluate the effectiveness of the proposed complex of means of physical therapy of patients after total hip arthroplasty. The study involved 16 patients at the Clinic of Orthopedics, Arthrology and Sports Injury, which were divided into two groups: control and primary, 8 people each.Empirical research methods were the WOMAC Western Ontario and Mc Master Universities osteoarthritis Index and the Leken algal function index. The main group was engaged in the method of N. Kehlet, active mobilization of patients began in the first day after surgery, the control group received traditional physical therapy with verticalization on the second day after surgery. The average hospital stay was 7 days. This is due to the requirements of the surgeons and does not depend on the functionality of the patient. Physical therapy after surgery was aimed at: reducing swelling; prevention of blood clots; pain control; prevention of constipation; improving sleep; improving postural control; increasing exercise tolerance. At the present stage, the method of total hip replacement is one of the progressive and widespread types of surgical treatment of coxarthrosis. Today, due to the introduction into clinical practice of high-efficiency technologies of prosthetics, the use of modern joint designs, orthopedic surgeons in most cases manage to achieve effective results and restore lost function. The patients of the main group had reached the functional criteria of discharge several days earlier. The study are presented, according to the results of which we believe that the work by the method of N. Kehlet is quite effective and that it should be used in working with middle-aged persons after total hip replacement due to deformable coxarthrosis.

Aim. To improve diagnostics of fat embolism syndrome in patients with multitrauma.Material and methods. The study included 250 patients with multitrauma at the age of 18 to 60 years that received treatment during 2014-2010. There were 174 (69.6%) men and 76 (30.4%) women. The patients were divided into two groups. The main group included 128 (51, 2%) patients with multitrauma, in whom the diagnosis, prevention, and treatment of fat embolism syndrome were carried out using improved approaches. The control group of 122 (48, 8%) patients with multitrauma received a diagnosis, prevention, and treatment of fat embolism syndrome using traditional approaches.Results and methods. Comprehensive analysis of data with the formalization and standardization of data by comparing the frequency of symptoms in the group of patients with and without fat embolism syndrome helped to reveal integral criteria for the diagnosis of various clinical manifestations of this symptom-complex were identified. Based on the data obtained, a highly effective method for the diagnosis of fat embolism syndrome has been developed. In the main group, differentiated treatment of patients using the developed approaches contributed to an increase in the proportion of positive results (43.7%) and a decrease in unsatisfactory results (8.6%) and mortality (8.6%). In the control group, satisfactory (36.0%) and unsatisfactory results (15.6%), as well as lethal outcomes (16.4%), prevailed.Conclusion. The use of improved approaches in the diagnosis and treatment of fat embolism syndrome and the construction of treatment tactics on this basis are among the effective ways to improve the results of multitrauma.

SummaryThe effect of subcutaneous low-dose heparin on postoperative deep-vein thrombosis (D. V. T.) (diagnosed by the 125I-labelled fibrinogen test) has been investigated in a trial of 143 patients undergoing the operation of total hip replacement. Two randomized studies were carried out: in one the scanning for D.V.T. was carried out daily for 7 days post operatively and in the other for 15 days. In both, the incidence of D.V.T. was significantly lower in the heparin-treated patients (P<0.005). Bilateral D.V.T. was also prevented (P<0.05), through the extension of D.V.T. to the distal veins of the thigh was not significantly reduced. Heparin treatment was, however, followed by a higher incidence of severe postoperative bleeding (P< 0.02) and wound haematoma formation (P< 0.005), and the postoperative haemoglobin was significantly lower than in the control group (P<0.005). A higher number of transfused blood units was also needed by the heparin treated patients (P<0.001).

Introduction. Glaucoma is one of the most significant eye diseases. It is often diagnosed, not always amenable to therapy, and can lead to a complete loss of visual functions. In recent years, the method of osteopathic correction has become widespread as one of the effective methods of treatment and rehabilitation of patients with pathologies of various body systems. In the pathogenesis of glaucoma, it is customary to distinguish a dystrophic concept, which considers primary open-angle glaucoma as a result of dystrophic changes in the connective tissue, as well as in the endothelial lining of the trabeculae and Schlemm′s canal, especially destructive changes in mitochondria and the alteration of their functional activity. A vascular concept is also distinguished. According to this concept, the central link in the pathogenesis of glaucoma is circulatory disorder in the ciliary vessels, ocular artery, and major vessels of the head and neck, it can be assumed that osteopathic correction in the treatment of patients with open-angle glaucoma will be pathogenetically substantiated and will have a positive effect on intraocular pressure and trophicity of the optic nerve. The goal of research — to study the influence of in osteopathic correction on the nature of unoperated glaucoma (stage IIA) and to substantiate the possibility of using osteopathic correction in the complex treatment of patients with this pathology.Materials and methods. A prospective controlled randomized study was conducted at 52 city polyclinics, branch 3, Moscow, from January 2018 to January 2019. 40 patients (70 eyes) aged 50 to 75 years with primary open-angle glaucoma IIA stage were examined. At this stage of the disease, patients most often seek medical care and the issue of conservative management is primarily considered. All patients were divided into two groups of 20 people: the main group and the control group. The treatment in the main group included hypotensive drug therapy and osteopathic correction. Patients of the control group received only drug therapy. All patients underwent ophthalmic (visometry, tonometry, perimetry) and osteopathic examination twice: before the treatment and after 3 months.Results. For patients with primary open-angle IIA non-operated glaucoma, regional (most often regions of the head, neck, dura mater) and local (abdominal diaphragm, iliac bones, hip and knee joints) somatic dysfunctions were the most typical. In the main group a statistically significant decrease in the frequency and severity of dysfunctions at all levels was stated. Also, in patients receiving osteopathic correction, a significant decrease in the level of intraocular pressure and perimetric indices was noted. In patients of the control group, no reliable changes in these indicators were obtained.Conclusion. The results obtained indicate that osteopathic correction is clinically effective in the complex treatment of patients with primary open-angle II A glaucoma.

In order to increase the pregnancy rate in the assisted reproductive technology, the selection of one embryo with the highest implantation potential it is very important. Time-lapse microscopy (TLM) is a tool for selecting quality embryos for transfer. This study aimed to assess the benefits of single-embryo transfer of autologous oocytes performed on day 5 of embryo incubation in a TLM-equipped system in IVF and ICSI programs. Single-embryo transfer following incubation in a TLM-equipped incubator was performed in 282 patients, who formed the main group; the control group consisted of 461 patients undergoing single-embryo transfer following a traditional culture and embryo selection procedure. We assessed the quality of transferred embryos, the rates of clinical pregnancy and delivery. The groups did not differ in the ratio of IVF and ICSI cycles, average age, and infertility factor. The proportion of excellent quality embryos for transfer was 77.0% in the main group and 65.1% in the control group (p = 0.001). In the subgroup with receiving eight and less oocytes we noted the tendency of receiving more quality embryos in the main group (р = 0.052). In the subgroup of nine and more oocytes the quality of the transferred embryos did not differ between two groups. The clinical pregnancy rate was 60.2% in the main group and 52.9% in the control group (p = 0.057). The delivery rate was 45.0% in the main group and 39.9% in the control group (p > 0.050).

The objective: the study of immunomodulatory effects of a probiotic, which contains lyophilized Lactobacillus (Lactobacillus rhamnosus) – 13 mg (2,0ґ109 CFU) and lyophilized bifidobacteria (Bifidobacterium lactis) – 4 mg (2,0ґ109 CFU) the level of serum immunoglobulin IgA as a marker of local immunity in the plasma of women of reproductive age with the violation of the biocenosis of the vagina. Patients and methods. The study involved 86 patients of reproductive age with the violation of the vaginal biocenosis, which were divided into two groups according to received treatment. A survey was conducted for all patients in both groups: determine the level of serum IgA, measuring pH of vaginal environment and the quantification of lactobacilli and pathogenic flora with the help of test-system «Florotsenoz» before treatment and in 6 weeks after treatment. The state of vaginal microbiocenosis in both groups before treatment was homogeneous. Patients in both groups as therapy at the first stage of treatment received, if necessary antimicrobial therapy depending on the selected flora. In the second stage (restoration of microflora) patient of the main group received systemic probiotic combined with a complex prebiotic local action, patients in the control group, the probiotic localy in the form of the vaginal candles or tablets. Results. The research stated the increasing level of serum IgA in blood plasma of patients of the main group compared to control group at 20%, normalizing the pH of the vaginal environment in the main group in 94% of cases, which indicates an increase of immunity in mucosal. Conclusion. The inclusion of the systemic probiotic in the scheme of treatment of disorders of biocenosis of the vagina system enhances the increasing of immunity of the mucous membranes, and the vaginal tablets prebiotic of local action restores the own normal microflora of the vagina. The objective: To evaluate the clinical efficacy and safety of Enoxaparin-Pharmex for the prevention of thrombotic complications (pulmonary embolism) in the postoperative period in patients with moderate risk of these complications. Patients and methods. The study included 50 women after a caesarean section had an average degree of risk of pulmonary embolism. Patients were divided into the main group (n=25) and control group (n=25) in accordance with the treatment: patients of the main group received postoperative Еnoxaparin- Pharmex, group comparisons enoxaparin sodium (brand foreign manufacturer’s). Patients in both groups received the drug at a dose of 20 mg for 5 days, 1 time per day subcutaneously. Results. The research data analysis showed identity results of hemostasiogram of patients in the main group and the comparison group, no side effects after treatment in both groups. Conclusion. The clinical studies suggest the drug Enoxaparin-Pharmex is effective, safe LMWH, which can be used to prevent troboembolic complications, including post-operative treatment in obstetric practice. Spectrum of Enoxaparin-Pharmex can be extended to the prevention and treatment of thromboembolic conditions of varying severity with appropriate doses of the drug.
