Caries was qualified by UN as non-communicable disease – NCD. It can be avoided by preventive measures. Especially, deep caries lesions in immature permanent teeth constitute a serious clinical problem for contemporary pediatric dentistry. It is caused by a different anatomical and histological structure compared to mature permanent teeth. The aim of the study was to present contemporary methods of deep caries treatment in histologically and morphologically immature teeth with currently used materials. The literature review was based on Pubmed and Elsevier databases (2005-2018) and two articles out-of-range. The search criteria were: “caries”, “remineralization”, “indirect capping” and “immature teeth”. Development in medicine and biomedical science leads to creating more modern procedural records when it comes to deep caries treatment. They are based on minimally invasive dentistry. Development of bioengineering enables creation of more biocompatible materials which do not have irritating influence on dental pulp. These materials allow deeper remineralization of dental tissues. Deep caries treatment still involves usage of restorations with calcium hydroxide, zinc oxide with eugenol and glass ionomers. Good cooperation with patient, prospects of early diagnosis, and regular examinations of treated teeth have great impact on success of treatment. Further research on methods of diagnosis and treatment needs to be carried out. They should allow regeneration of early qualitative changes to avoid quantitative damages in a tooth’s hard tissue.

Since January 2019, insurance coverage for caries treatment of permanent teeth using composite resin in children aged 12 and under has started in South Korea. The purpose of this study was to compare the pattern of permanent molar composite resin restoration aged 12 and under before and after the insurance coverage from January 2009 to March 2020 in the Department of Pediatric Dentistry at Kyung Hee University Dental Hospital at Gangdong.<br/>Since the insurance coverage was started in 2019, the frequency of permanent molar composite resin restoration aged 12 and under has increased more than twice. There was a significant change in frequency of resin restoration by shape of cavity comparing before and after the insurance coverage, whereas there was no significant difference in changes by patient age, gender and position of tooth.<br/>As accessibility to the permanent tooth composite resin restoration is increased by the insurance coverage, which shows a fair survival rate, it is expected that it could be able to secure oral health in the early permanent dentition aged 12 and under.

ABSTRACTDental caries on tooth surfaces is still a problem in many industrialized countries. For many years, dentistry was influenced by a mechanical approach characterized by the use of high-speed rotary cutting instruments, and dentists predominantly used surgical methods to address caries. This included radical removal of diseased portions of the tooth, along with material-driven geometric extensions to areas that were assumed to be caries-resistant. This concept of extension for prevention was introduced by G. V. Black and influenced dentists for more than 120 years. Recently, a new paradigm of operative conservatism, sometimes referred to as “minimally invasive dentistry,” has gained popularity. This paradigm is designed to promote maximum preservation of healthy dental structures over a lifetime. The aim of this review is to discuss the efficacy of current nonsurgical treatments for non-cavitated caries lesions in permanent teeth. Based on results obtained from clinical trials, this review evaluates treatments such as consumption of CPP-ACP added gums, resin infiltration and fissure sealing. Although in a few cases an invasive approach is needed to arrest caries progression, the non-surgical approach generally provides potential benefits that include conserving structure by delaying intervention or minimizing the operative procedure. All current non-invasive methods are effective in treating non-cavitated caries lesions. The adoption of non-invasive approaches in the management of these lesions can preserve dental tissues, thus increasing tooth longevity. (Eur J Dent 2011;5:229-236)

Abstract Background To assess the resolution of clinical symptoms and radiographic changes in root length and apical diameter in immature permanent teeth with irreversible pulpitis or apical periodontitis with vital inflamed pulp therapy (VIPT).Methods The faculty members at the Ninth People’s Hospital pediatric dentistry department were invited to submit consecutive VIPT cases treated by them, irrespective of outcome, between 2014 and 2016. Clinical success rate, radiographic changes in periapical radiolucency, and apical closure were analyzed, and radiographic changes in the apical diameter and root length were quantified.Results Thirteen of 14 submitted cases of irreversible pulpitis in immature teeth met the inclusion criteria. The follow-up period ranged from 17 to 37 (average, 26.5 ± 7) months. All 13 treated teeth (100%) survived and 12 (92.3%) met the clinical criteria for success throughout the follow-up period, with 92.3% of cases (12 of 13) showing a significant periapical radiolucency decrease and 84.6% (11 of 13) showing complete apical closure at the last visit. The change in apical diameter and root length were obvious. Conclusions Vital inflamed pulp therapy approaches might be of particular value in restoring root development and apical closure and can be an option in treating immature teeth with irreversible pulpitis, even apical periodontitis.

The pulp treatment of primary and immature teeth in the children and adolescents is affected by the environment of the patients, clinical careers and dentists performing the techniques, as well as clinical modalities and pulpal diagnosis. The purpose of this study was to investigate the current status of pulp treatment performed in real clinical practice, as well as the reasons behind the choice of treatment methods.The study was conducted on a total of 153 members of the Korean Academy of Pediatric Dentistry, and statistical data from the Health Insurance Review and Assessment Service (HIRA) from 2010 to 2018 were analyzed together. Both the survey results and the statistics from the HIRA showed that 1-visit pulpectomies of primary teeth were most common, and the frequency of the use of the Ni-Ti file was increased in children with primary or mixed-dentition. In the treatment of immature permanent teeth, regenerative endodontic treatment by individual dentists was found to be less frequent than in university hospitals.

Prevention of dental caries in children is one of the current problems of modern pediatric dentistry, which is due to the high prevalence and intensity of this disease. Analysis of the results of epidemiological studies conducted among children of different ages in the regions of Ukraine shows a significant increase in caries growth in recent years, both temporary and permanent teeth, which leads to in-depth study of the hardness of dental tissues to cariogenic factors in connection with the need in the constant improvement of methods of treatment and prevention of this pathology. The purpose of the study was to analyze the value of enamel caries resistance as a detector of resistance of dental hard tissues to carious process and its complications. The subject of the research is the work of domestic and foreign researchers on this topic. In the course of the research the bibliosemantic method and structural-logical analysis were used. Today, dental caries is the most common disease in different age groups, which is confirmed by the prevalence – 90-100% and a sharp trend of increasing its intensity. Given the high rates of this pathology, it should be remembered that its untimely treatment leads to the following complications: pulpitis, periodontitis, tooth extraction. An important concept that indicates the possibility of caries is caries resistance, which indicates the level of resistance of the enamel to caries and has an informative diagnostic value for the dentist. According to modern ideas, in the pathogenesis of dental caries the leading role is played by caries-resistant enamel. Its mineral base is formed by isomorphic crystals of apatite, which contain various chemical elements, and their amount in the body is closely related to the amount in the environment. It is established that more than 40 chemical elements take part in the process of tooth mineralization, the most important of which belongs to calcium, phosphorus, fluoride. Insufficient amount or excess of certain micronutrients in the environment, especially in soils and drinking water, can change their content in the body and, consequently, in the hard tissues of the teeth, affecting their resistance to caries. It is well known that the influence of a number of adverse environmental factors leads to a decrease in the resistance of the body as a whole and, as a consequence, to a decrease in the caries resistance of the enamel of the hard tissues of the teeth. Sensitivity to the effects of adverse factors increases significantly in the prepubertal period, a time not only of intensive growth of the child and the formation of his personality, but also a period of mineralization of more than half of permanent teeth. Therefore, exactly at this time dentists should pay special attention to the prevention of caries of permanent teeth in children by improving the conditions of secondary mineralization of tooth enamel, thereby increasing their level of caries resistance

Tissue engineering is a growing field. In the near future, it will probably be possible to generate a complete vital tooth from a single stem cell. Pulp revascularization is dependent on the ability of residual pulp and apical and periodontal stem cells to differentiate. These cells have the ability to generate a highly vascularized and a conjunctive rich living tissue. This one is able to colonize the available pulp space. Revascularization is a new treatment method for immature necrotic permanent teeth. Up to now, apexification procedures were applied for these teeth, using calcium dihydroxide or MTA to produce an artificial apical barrier. However, the pulp revascularization allows the stimulation of the apical development and the root maturation of immature teeth. Two pulp revascularization techniques are used in the literature, one using calcium dihydroxide and the second using a triple antibiotic paste. Based on these two different pulp revascularization protocols, which obtain the desired therapeutic success, the literature will be reviewed and analyzed according to the relevance of their choice of materials. Based on the literature, we propose a new relevant protocol and a new mixture of antibiotics.

Odontomas represent the most common type of odontogenic benign jaws tumors among patients younger than 20 years of age. These tumors are composed of enamel, dentine, cementum, and pulp tissue. According to the World Health Organization classification, two distinct types of odontomas are acknowledged: complex and compound odontoma. In complex odontomas, all dental tissues are formed, but appeared without an organized structure. In compound odontomas, all dental tissues are arranged in numerous tooth-like structures known as denticles. Compound odontomas are often associated with impacted adjacent permanent teeth and their surgical removal represents the best therapeutic option. A case of a 20-year-old male patient with a compound odontoma-associated of impacted maxillary canine is presented. A minimally invasive surgical technique is adopted to remove the least amount of bone tissue as far as possible.

ABSTRACT Soft tissue laser, such as diode and Nd:YAG lasers were initially used in soft tissue lesions because of its increased success rate. It was because of the fact that these lasers were well-absorbed by chromophores, such as hemoglobin and melanin which are found abundant in the oral mucosa. The introduction of erbium family in 1990 comprising the Er:YAG and Er,Cr:YSGG lasers made the hard tissue laser a boon for dentistry. Erbium, chromium-doped yttrium, scandium, gallium and garnet (Er,Cr:YSGG) was introduced in 1997 for the surgical needs of clinical dentistry in general practice. The erbium belongs to the rare earth which is embedded in a host crystal. The actual lasing process takes place in the Er ion Er3+. Two host crystals consisting of yttrium, aluminum, and garnet (Y3A5O12) and yttrium, scandium, gallium, and garnet (Y3Sc2Ga3O12) are added to the erbium. The interest to use these hard tissue laser in the treatment of soft tissue lesion was because of the properties of these lasers which are well-absorbed by chromophore water apart from hydroxy appetite crystals. Erbium laser energy is absorbed by collagen, hydroxyapatite, and water components. It allows the laser to cut soft tissue, tooth structure, and bone. In the noncontact mode, the incision is scalpel-like, with very little hemostasis. In contact mode, it performs soft tissue sculpting with adequate hemostasis. The Er,Cr:YSGG is the world's most advanced dental laser, which is ideal all-tissue laser because all dental tissues contain water, for the multidisciplinary dentist who performs a broad spectrum of procedures. It delivers the highest level of clinician control, operating efficiency, flexibility in tip, and accessory selection. For optimal clinical results and patient comfort in hard and soft tissue procedures, the erbium lasers have set a new standard of clinical performance. The present case series aims to highlight the various soft tissue applications of Er,Cr:YSGG (Waterlase Biolase®, Biolase, Inc, San Clemente, California, USA) in pediatric patients. Goal of contemporary dentistry is to decrease the patient?s discomfort during treatment. Dentists aim to achieve maximum with the newly developed dental materials as well as with new cavity preparation techniques in the shortest time span. Since the development of the first constructed borer (drilling machine) for caries removal, the preparation techniques have considerably changed. The progress of dental materials as well as the cavity preparation techniques has led us to contemporary carbide tungsten and diamond borers that are used with obligatory water cooling. The innovation within this field represents newly developed polymer borers that can detect the difference between carious lesions and healthy tooth structure. In this way the cavity preparation may be performed without damaging dental healthy tissue. This is possible owing to their hardness which is lower than the hardness of intact dentin. Polymer borer preparation is painless with less vibration, while the increase in temperature is negligible. Lasers have been used in clinical dentistry since 1980s so it can be said that they represent a new technology. The function of lasers is based on ablation which requires water. Erbium lasers have shown the highest potential with their ability to produce effective ablation of hard dental tissues. Laser application in dentistry requires special training as well as some protective measures. Laser advantages, compared to traditional preparation techniques, involve the absence of vibration, painless preparation, possibility of preparation without anesthetic and easier patient?s adjustment to dental intervention which is of importance, especially in pediatric dentistry.

The aim: To determine the structure of acute injuries of temporary and permanent frontal teeth in children, to analyze the applied diagnostic and treatment measures for acute tooth trauma and to investigate their effectiveness. Materials and methods: The subject of the study were 31 children aged 2-12 years with acute temporary and permanent tooth trauma. Methods: clinical (radiological, thermodiagnosis), medical and statistical. Results: The number of injured permanent teeth was greater than the number of injured temporary teeth. The pattern of temporary and permanent tooth injuries differed, so in temporary teeth dislocations were observed, and in permanent teeth fractures prevailed over dislocations. In our opinion, this is due to the peculiarities of the anatomy of the temporary teeth. Usually, the therapeutic tactics of acute temporary teeth injuries is to remove them, despite their important role in the growth and development of jaws, physiological formation and eruption of permanent teeth. The therapeutic tactics of dynamic observation in the case of intrusive dislocation of the temporary tooth were selected in the clinic of the Department of Pediatric Dentistry. In some cases, with a slight change in the position of the temporary tooth, self-regulation of its position was observed, in other cases the tooth remained dystopic, but its viability remained in 50% of cases. Therapeutic tactics of acute injuries of permanent teeth were selected according to the type of trauma. In the case of permanent tooth dislocation, with a slight change in its position, tooth immobilization was carried out by splinting with fiberglass tape and photopolymer composite material. Conclusions: Our observations have shown that the complex of modern specialized medical care for the affected children with acute traumatic injuries of the teeth should be guided by their preservation, which ensures the subsequent normal formation of the dental-jaw apparatus. To ensure these conditions, a long, reliable immobilization of the damaged tooth is required as soon as possible after injury.
