Balancing diet in patients with congenital haemorrhagic diatheses is very important because of the need to eliminate excess weight and at the same time strengthen muscle mass. This has an important role in the treatment and prevention of strokes. The food provided to these children should secure the right amount of energy nutrients and fluids in the right proportions. After investigating the cause and complications associated with the course of the disease, we can reduce symptoms. An individual approach to each patient and the choice of a right diet will reduce the duration of treatment, ensure better development and growth, as well as improving quality of life. Additionally, it is necessary to assess the level of physical activity of each patient and, when it is too low, recommend them to increase it. Proper rehabilitation, which mainly concerns patients with congenital plasma diatoma, is especially important. Another symptom, occurring mainly in girls, is profuse menstruation and associated iron deficiency anemia. In this article we present the most important nutritional problems which a physician dealing with patients (both boys and girls) with congenital coagulation disorders should manage.

Abstract Objectives: The mini right axillary thoracotomy is an alternative surgical approach to repair certain congenital heart defects. Quality-of-life metrics and clinical outcomes in children undergoing either the right axillary approach or median sternotomy were compared. Methods: Patients undergoing either approach for the same defects between 2018 and 2020 were included. Demographic details, operative data, and outcomes were compared between both groups. An abbreviated quality of life questionnaire based on the Infant/Toddler/Child Health Questionnaires focused on the patient’s global health, physical activity, and pain/discomfort was administered to all parents/guardians within two post-operative years. Results: Eighty-seven infants and children underwent surgical repair (right axillary thoracotomy, n = 54; sternotomy, n = 33) during the study period. There were no mortalities in either group. The right axillary thoracotomy group experienced significantly decreased red blood cell transfusion, intubation, intensive care, and hospital durations, and earlier chest tube removal. Up to 1 month, parents’ perception of their child’s degree and frequency of post-operative pain was significantly less after the right axillary thoracotomy approach. No difference was found in the patient’s global health or physical activity limitations beyond a month between the two groups. Conclusions: With the mini right axillary approach, surrogates of faster clinical recovery and hospital discharge were noted, with a significantly less perceived degree and frequency of post-operative pain initially, but without the quality of life differences at last follow-up. While providing obvious cosmetic advantages, the minimally invasive right axillary thoracotomy approach for the surgical repair of certain congenital heart lesions is a safe alternative to median sternotomy.

AbstractPhysical activity provides many benefits in patients with congenital bleeding disorders. Patients with hemophilia are encouraged to participate in exercise and sports, especially those patients receiving prophylaxis. Several publications and guidelines have explored this issue in hemophilia patients, evaluating in particular the impact of physical activity on patients' well-being and quality of life. The other rare congenital bleeding disorders are less studied; they are heterogeneous in terms of clinical bleeding phenotype, incidence of hemarthrosis, and arthropathy. Furthermore, prophylaxis in these patients is less common than in hemophilia patients, which must be considered when choosing the type of physical and sporting activity. In this review, the authors have analyzed the literature focusing their attention on those rare coagulation disorders that may be complicated by arthropathy and the role of exercise and sports in this context.

Iron deficiency anemia (IDA) is one of the causes of chronic fatigue with a decrease in the quality of life, impaired cognitive function and poor labor productivity. Obesity as a condition of chronic low-intensity inflammation is one of the factors that increases the risk of anemia and impairs the quality of life in the general population. We drew attention to the high global prevalence of anemia among pregnant women of reproductive age in the world and the prevalence of high body mass index among women compared to men in most countries. The objective of the work was to determine the impact of obesity on clinical features and quality of life of women with ІDA. We analyzed the main markers of ІDA and hematologic indices in women with obesity (n = 30) compared with the control group (n = 10), where there were women with ІDA but without obesity. It was found that there was no statistical difference between these parameters, but nevertheless, the serum ferritin level in obese women was (4.70 ± 2.68) ng/ml compared to (3.50 ± 2.93) ng/ml and had a significant positive correlation with body mass index. The analysis of the questionnaires revealed that obesity is still a significant factor in women with ІDA and obesity, which determines the decrease in quality of life mainly in the physical aspect in the absence of significant influence on the psychosocial components of health.

Anemia is believed to be a medico-social problem affecting a great part of population. Anemia of chronic diseases represents a considerable number of these disorders, which is particularly challenging for long-standing inflammatory processes, systemic diseases and neoplasms. Pathogenesis of this type of anemia is characterized by complex and miltifactorial nature. Pathogenesis is based on disturbance of erythropoietin synthesis and erythropoiesis precursor cells sensitivity, hyperproduction of factors inhibiting erythropoiesis (tumor necrosis factor, interleukins), disturbance of iron metabolism and other hemopoiesis co-factors. Anemia aggravates associated underlying disease course. Quality of life and overall survival deteriorate. Treatment efficacy for anemia influences the time of patient recovery, as well as success of treatment of other diseases. Iron deficiency and erythropoiesis disturbance, similar to hypoferric anemia, are of great importance. However, iron therapy may aggravate patients condition, hence differential diagnosis of this anemia type and true iron deficiency anemia has a great practical value. In the treatment of anemia in the presence of chronic diseases and anemia in the presence of hematologic diseases, the goal of the treatment is generally limited by the improvement of patients quality of life, target value of hemoglobin for transfusion or erythropoiesis stimulating therapy remaining the subject for discussion. Hemoglobin target determination in individuals with cardiac and pulmonary insufficiency who are on chemotherapy, is critically important, because it is this category of patients that does not demonstrate generally accepted target values of hemoglobin, the level of which is not always indicative of hypoxia absence. The review presents current data on diagnosis, treatment of chronic disease anemia (1 figure, 2 tables, bibliography: 12 refs).

Women are the pioneers and key to sustainable development and quality of life in the family. Hindu culture respects women as Goddess due to this power of creativity of life inside her womb. This energy of procreation starts with menarche and ends with menopause. Ayurveda focusses on preventive and curative measures of female health. Acharya Charak mentioned “Yonishu Shuddhasu Garbham Vindati Yoshitaha” i.e., normal healthy status of Yoni (vagina) is the key factor for achieving the conception. Various gynaecological disorders like Yoni Rogas, Artava Doshas and Raktapradar hampers the achievement of conception. Raktapradar i.e., excessive excretion of men- strual blood, either in quantity or in duration is explained in Ayurvedic Classics. It is the common cause of Iron deficiency anemia & general debility. Abnormal uterine bleeding (AUB) is one of the most common frequent reasons for contacting a gynecologist and conducting intrauterine interventions. AUB causes iron deficiency anemia and a decrease quality of life of women, which in turn is the reason of their social and clinical significance. Based on the analysis of literature and international data recommendations, the review outlines the basic principles of AUB diagnosis, non-hormonal and hormone therapy Advantages of continuous regimen for the administration of progestogens, in particular the LNG-IUS, and COCs containing estradiol valerate, to reduce menstrual flow blood loss are presented in the article. The need for a differentiated approach of choosing the therapy based on age, causes of bleeding, and balance benefits and risks of medicines.
