The article presents the results of the effectiveness of the combined antimicrobial drug Guinex Forte, the effect of which is caused by metronidazole and miconazole, and the Orgil tablets at the stage of pregravid preparation in women of high-risk group, with regard to the development of placental insufficiency of infectious genesis and intrauterine infection. The objective: is to demonstrate the effectiveness of pregravid preparation for the normalization of vaginal biocenosis in pregnant women of high infectious risk. Materials and methods. 150 pregnant women were investigated, of which 100 with a high risk of infectious risk for placental dysfunction and intrauterine infection: Group I – 50 pregnant women who did not undergo pregravid preparation; Group II – 50 pregnant women who planned pregnancy and conducted pregravid preparation for prevention and treatment of bacterial vaginosis and vaginal candidiasis; Control group consisted of 50 pregnant women who gave birth again, without obstetrical and extragenital pathology in history. per vaginum. Results. In pregnant women in Group II, an intermediate type of dysbiosis was 1.2 times less likely than in pregnant women of group I, and vice versa, normocenosis was achieved 9.7 times more often in pregnant women who received pregravid preparation. After the therapy in the pregravid period, in pregnant women of group II in the first trimester of pregnancy quantitative and qualitative indices of biocenosis of the vagina were approaching, in most cases, to normal. In general, the spectrum of the microflora decreased from 21 to 14 species due to the reduction of pathogenic forms of staphylococci, streptococci, enterobacteria, E. coli, klebsiela, cornebacteria and clostridia. In patients of group II, the concentration of representatives of resident flora increased (lactobacillus Lg 5.06±0.7 CFU / ml and bifidobacterium-Lg 4.4±0.6 CFU / ml) and close to normal. Conclusion. Our proposed scheme of therapy and prevention of dysbiotic conditions in the pregravid period, in women of high infectious risk group led to a decrease in bacterial contamination of maternity paths of pregnant women in group II, which contributes to the restoration of vaginal microbiocenosis and positively affects the course of pregnancy, the condition of the fetus and the newborn. Background: Lower reproductive tract infections in women are important causes of morbidity but can also lead to complications and sequelae. This study aimed to establish the prevalence and risk factors of lower genital tract infections among women of reproductive age in Dakar (Senegal). Methods: This was a prospective study conducted in 6 maternity hospitals from July to November 2015. Participants ranged in age from 18 to 49 years and presented at health facilities with signs and symptoms of genital infection. Consenting individuals who met the inclusion criteria were recruited for the study. Results: During the reporting period, 276 patients were enrolled. According to the laboratory results, the prevalence of any genital infection was 69.6% (192 of 276). The most common vaginal infections were bacterial vaginosis (39.5%) and vaginal candidiasis (29%), with the third most common cause, trichomoniasis, trailing behind in terms of prevalence (2.5%). Among the microorganisms responsible for cervical infections, Ureaplasma urealyticum was the most frequent (27.5%), followed by Mycoplasma hominis (14.5%), Chlamydia trachomatis (4.7%), and Neisseria gonorrhoeae (1.1%). Multivariate analysis showed that young women and women with low levels of education were at increased risk for vaginal/cervical infections. Conclusions: This study revealed a high prevalence of bacterial vaginosis and vaginal candidiasis and suggests that health care providers should increase awareness and communication to improve vaginal hygiene practices. If infection with Trichomonas vaginalis, C trachomatis or N gonorrhoeae is suspected, we also recommend systematically performing laboratory diagnostic confirmation.

Key Points Vaginal discharge is caused by infection, inflammation, or changes in the vaginal flora.Vaginal discharge in the prepubertal child can be caused by infection, congenital abnormalities, trauma, or dermatologic conditions.Nonspecific vaginitis is the most common gynecologic problem in the prepubertal child.Prepubertal hypoestrogenic tissues are more susceptible to local irritants, foreign bodies, and infections.Most common causes of vaginitis in postpubertal women are bacterial vaginosis and vaginal candidiasis (Candida vulvovaginitis).Laboratory documentation of etiology of vaginal discharge is mandatory prior to treatment.Target treatment to etiology.

The aim of the work is to systematize the data of special literature for the clearest definition of the directions of studying the modern approach to the diagnosis, prognosis and treatment of pyoinflammatory processes in the maxillofacial region and neck. As a result of the analysis of the data of special literature, it becomes obvious that in recent decades the idea of the etiology and pathogenesis of pyoinflammatory processes in the maxillofacial region and neck has changed, which determines the need for further search for new informative methods of diagnosis, prognosis, as well as the most effective methods of treatment and prevention of complications for this pathology.

The review focuses on modern treatment aspects and prevention of ophthalmic herpes.The generalized experience of the clinical nucleoside administration, the basic antiviral drugs in the treatment of this pathology, is presented in detail. Information about indications, side effects and various schemes for their systemic and local application is provided. Questions on the duration of nucleoside administration in various clinical forms of eye herpes are considered. A meta-analysis of the comparative therapeutic nucleoside efficacy based on the results of more than 100 placebo-controlled randomized trials is presented. Special attention is paid to the rare but severe eye pathology, the acute retinal necrosis. Also data on the indications and contraindications of corticosteroids and on the current view of immunotherapy of ophthalmic herpes is presented. The question of the modern approach to the disease prevention recurrences is considered in detail, particularly, the results of the nucleoside administration have a high level of evidence, and, herpetic vaccines in the rehabilitation of patients with herpes infection are not widely used due to the lack of reliable methods for evaluating the efficacy. The information on the search for new effective drugs that have a targeted effect on certain patient cell receptors that can inhibit the penetration and spread of the herpes virus that can cause a controlled immune response is provided. Expanding the arsenal of effective and safe antiviral therapy will definitely optimize the treatment of ophthalmic herpes and prevent the occurrence of its recurrence and severe outcomes.

Background. The structure of lower urinary tract infections has clear sex-based differences. The vaginal microbiota is a key factor in the pathogenesis of lower urinary tract infections: aerobic vaginitis predisposes to infection and increases the incidence of cystitis by 2.9 times. Aim. To determine the effectiveness of Nifuratel as monotherapy in patients with chronic cystitis and concomitant bacterial vaginosis (BV). Materials and methods. Study design: an open-label, non-comparative, pilot, single-center, prospective study, which included 23 patients with recurrent cystitis and BV. All women received monotherapy with Nifuratel (Macmiror manufactured by Doppel Farmaceutici S.r.l., Italy) in tablets of 200 mg 3 times a day for 7 days. Control visits were performed in 7.90 and 180 days. Results. BV was diagnosed in all patients; 19 (82.6%) women also had vaginal candidiasis. At the end of therapy, 17 (73.9%) patients showed an excellent outcome, 4 (17.4%) a significant improvement, 2 (8.7%) no effect. After the end of three-month therapy, 18 (81.8%) out of 22 patients who left in the study had no complaints of urinary disorders or vaginal discharge. Their urinalyses were normal. Molecular genetic analysis of the vaginal biocenosis showed moderate dysbiosis; Candida spp. was revealed only in 3 (13.6%) cases. 18 patients came for the 4th visit. Within six months after the end of monotherapy with Nifuratel, the patients had no complaints. They all achieved the recovery of vaginal normocenosis with a sufficient amount of Lactobacillus spp. Conclusion. Monotherapy in patients with recurrent cystitis and BV is highly effective for both diseases: 73.9% of women achieved stable normalization of urine and vaginal microbiota analyzes, removal of bacteriuria. The bi-directional action of Nifuratel allows to avoid polypharmacy in this category of patients.
