We report a fatal case of overwhelming pneumococcal infection in an asplenic young adult not vaccinated against Streptococcus pneumoniae (S. pneumoniae). Post-mortem microbiological investigations revealed the presence of S. pneumoniae in blood samples and lungs. Serotyping by molecular methods identified the presence of a 6C serotype not comprised in the current 23-valent pneumococcal vaccine, highlighting that a risk of fatal infections may persist even in vaccinated splenectomised individuals.

Necrotizing fasciitis, caused byStreptococcus pneumoniae, is an extremely rare and life-threatening bacterial soft tissue infection. We report a case of early necrotizing fasciitis associated withStreptococcus pneumoniaeinfection in a 26-year-old man who was immunocompromised with mixed connective tissue disease. The patient presented with acute, painful, erythematous, and edematous skin lesions of his right lower back, which rapidly progressed to the right knee. The patient underwent surgical exploration, and a diagnosis of necrotizing fasciitis was confirmed by pathological evidence of necrosis of the fascia and neutrophil infiltration in tissue biopsies. Cultures of fascial tissue biopsies and blood samples were positive forStreptococcus pneumoniae. To our knowledge, this is the first report of necrotizing fasciitis resulting fromStreptococcus pneumoniaediagnosed at early phase; the patient recovered well without surgical debridement.

Pneumococcal infections are the most frequent vaccine preventable cause of death in Germany next to influenza. Asplenic patients in particular should be vaccinated to avoid serious illness and even death, as the following case reported in Germany’s surveillance bulletin shows (1). A 37 year old woman who had had a splenectomy after a road traffic accident in 1990, and had recently been treated with mirtazapine as a hospital inpatient for postpartum depression, developed a feverish infection with shivers, dizziness, and vomiting three weeks after being discharged. The patient herself suspected food poisoning but no food samples were available for testing. She deteriorated and developed petechiae, and the next day the emergency doctor admitted her to intensive care. She developed Waterhouse-Friderichsen syndrome with disseminated intravascular coagulation (DIC) and died in the evening of the same day from septic toxic shock. No meningococci or botulinum toxin were found but Streptococcus pneumoniae was isolated from a blood specimen.

A young adult labrador was presented for evaluation of hind leg weakness, depression, vomiting and severe icterus of skin and mucosa. Physical examination and laboratory findings suggested cerebral babesiosis. Unresponsiveness to treatment and unknown vaccinal status aroused suspicion of rabies which was confirmed post mortem.

Empyema necessitans (also called empyema necessitatis) is a sequelae of empyema/parapneumonic effusion where infection progresses beyond the pleural space into the soft tissues of the chest wall. Typical pathogens includeMycobacterium tuberculosis,Streptococcus pneumoniae, andStaphylococcus aureus. Fungal empyema necessitans is uncommon, with the majority of cases secondary to Candida species. We report a rare and fatal case of empyema necessitans presenting in a patient with cystic fibrosis more than 10 years following double-lung transplant.

ABSTRACT PspA is an important pneumococcal vaccine candidate that is capable of inducing protection in different animal models. Because of its structural diversity, a PspA-based vaccine should contain at least one fragment from each of the two major families (1 and 2) in order to elicit broader protection. In the present work, we have tested the potential of PspA hybrids containing fused portions of family 1 and 2 (PspA1ABC-4B and PspA1ABC-3AB) PspA fragments to induce protection against pneumococci bearing distinct PspA fragments. Sera from mice immunized with these hybrid PspA fragments were able to increase C3 deposition on pneumococci bearing PspA fragments from both families, in contrast with sera made against the PspA family 1 (PspA1ABC) and PspA family 2 (PspA3ABC) fragments, which were effective only within the same family. Although PspA hybrids were able to extend protection against pneumococcal infection with strains bearing diverse PspA fragments, the immunity elicited by family 2 was clade dependent, suggesting that PspA fragments from family 2 clades 3 and 4 should both be included in a comprehensive PspA vaccine. These results indicate that PspA fusion proteins constitute an efficient immunization strategy for future PspA-based antipneumococcal vaccines since they are able to extend protection provided by a protein derived from a single transcript.

A fatal case of ferrous sulfate poisoning in a 29 month old boy, with autopsy findings, is recorded. He died approximately 4½ hours after ingestion of about seventy-five 0.3 gm. ferrous sulfate tablets (1.87 gm./kg. body weight). Signs of severe gastrointestinal irritation were followed by cyanosis and peripheral vascular collapse. Hemorrhagic gastroenteritis with mucosal slough and submucosal venous thromboses were demonstrated post mortem. Prevention and measures for correction of shock were emphasized in discussion of treatment. This case report should re-emphasize the potential danger of iron poisoning.

Based on the results of selective molecular genetic studies, the species structure of the community-acquired pneumonia pathogens in children of Perm was studied in association with the decreed vaccination against pneumococcal infection in 2014–2019. It has been established that in recent years, against a background of an increase in the volume of decreed immunisations with the conjugated pneumococcal vaccine Prevenar 13, there is a decrease in the frequency of extraction in patients with Streptococcus pneumoniae and, on the contrary, an increase in the frequency of Mycoplasma pneumoniae extraction.
