Background: Hypertension cases to overcome the patient's hypertension condition would be well or prosperous condition and the patient could prevent complications and control blood pressure. Activities in Chronic Disease Management Program (Prolanis) include medical or educational consultations, home visits, reminders, club activities.Purpose: This study aimed to analyze the application of the prolanis program based on the caring theory by reducing blood pressure in hypertensive patients.Methods: The design of this research is pre-experimental with cross sectional study approach. The population in this study were all patients with hypertension who took part in management program activities in the public health center (PHC), a number of 40 people, with the sampling technique used was accidental sampling and the total sample was 28 people. The data collection method used a questionnaire. The data analysis of this research is to use the Paired t test.Results: The result of this study is that there is a relationship between the application of a chronis disease management programs based on caring theory with blood pressure reduction with an average systolic blood pressure before the intervention of 142?26.15 and after the intervention of 138?1.21 with p= 0.000 which means that the module intervention can decrease systolic and diastolic blood pressure. While for diastolic blood pressure, the results of the study showed that the mean blood pressure before the intervention was 85?10.36 and after the intervention was 85?7.61 with p= 0.000.Conclusion: It can be concluded that there is a possitive effect between giving prolanis module based on caring theory with systolic and diastolic blood pressure

Since 2014 BPJS Health has implemented the Chronic Disease Management Program (PROLANIS) which has a program of medical/educational consultation activities, home visits, reminders, club activities such as gymnastics and health status monitoring with the target of all BPJS health participants with chronic diseases (Diabetes Mellitus) Type 2 and Hypertension). The Batam Sehat Clinic conducted a chronic disease management program in 2015 with 154 registered patients, while BPJS patients who took part in the PROLANIS exercise in July-September 2018 totaled 155 people. This type of research is an observational analytic survey with a cross-sectional approach conducted at Batam Sehat Clinic. Data obtained by taking medical records in the form of attendance PROLANIS gymnastics with a total sampling technique that the overall number of BPJS patients who take PROLANIS exercises from July-September is 155 BPJS patients. From 155 BPJS patients, the frequency distribution of BPJS patients participating in PROLANIS and blood pressure was controlled 71 (85.5%) and uncontrolled blood pressure was 12 (14.5%). While those who did not participate in PROLANIS exercise were controlled blood pressure 32 (44.4%) and 40 (55.6%) uncontrolled blood pressure. Based on the results of statistical tests it is known that there is a correlation between the participation of PROLANIS gymnastics and blood pressure control in BPJS patients in Batam Sehat Clinic with p-value = 0,000 where p <0.05. Based on this research, it can be concluded that there is a correlation between the participation of PROLANIS gymnastics and the control of blood pressure in BPJS patients.

Objective This study assessed pharmacological treatment adherence using the Morisky-Green Test and identified related variables. Method A longitudinal and retrospective study examined 283 patients with hypertension (62.5% women, 73.4 [10.9] years old) who were being monitored by a chronic disease management program for 17 months between 2011 and 2012. Nurses performed all the actions of the program, which consisted of advice via telephone and periodic home visits based on the risk stratification of the patients. Results A significant increase in treatment adherence (25.1% vs. 85.5%) and a decrease in blood pressure were observed (p<0.05). Patients with hypertension and chronic renal failure as well as those treated using angiotensin-converting enzyme inhibitors were the most adherent (p<0.05). Patients with hypertension who received angiotensin receptor blockers were less adherent (p<0.05). Conclusions Strategies such as nurse-performed chronic disease management can increase adherence to anti-hypertensive treatment and therefore contribute to the control of blood pressure, minimizing the morbidity profiles of patients with hypertension.

Over the past two decades, there has been a significant epidemiological transition, non-infectious diseases have become a major burden, even though the burden of non-infectious diseases is still heavy. The number of Prolanis Clubs in Central Lampung is 78, which consists of 39 clubs which are in 39 Community Health Centers and 39 Clubs from Private First Level Health Clinics and Private Practitioners. The number of Prolanis participants in Central Lampung Regency was 3397 participants, with the most in the ArafaH Clinic as many as 396 participants. The purpose of this study is to determine the factors associated with the visit of Chronic Disease Management Program participants to the Prolanis Club in Central Lampung Regency in 2020. This study uses a cross sectional study design. The study population was all Prolanis participants. A sample of 215 people. Data collection using a questionnaire filled directly by respondents, data analysis using logistic regression tests. The results showed there was a relationship of knowledge (p value 0.002. OR 2.5), attitude (p value 0,000. OR 5.29), perception (p value 0,000. OR 3.8), family support (p value 0.003. OR 2 , 4), accessibility (p value 0,000. OR 3,2) with Prolanis participants 'visit to Prolanis Club in Central Lampung Regency in 2020. Attitude is the most dominant factor related to Prolanis participants' visit to Prolanis Club, which has the highest OR value, namely 11,266. Suggestions for the research are expected that health workers carrying out prolanist activities increase their role as educators by providing education about Type 2 Diabetes Mellitus and Hypertension and emphasizing the importance of participating in prolanist activities so as to increase the positive attitude of prolanist participants towards Prolanist activities.

Background and purpose:The prevalence of hypertension is increasing globally. A comprehensive approach is necessary including a community based chronic disease management program which has been implemented in public health centres in Indonesia. Only few studies have been published in Indonesia that evaluate the program, especially regarding the management of hypertension in primary care settings. This study aims to examine the association between participation in a chronic disease management program, medication adherence and decrease of blood pressure.Methods: A cross-sectional study was conducted in public health centres in Tabanan District. A total of 136 patients with hypertension were recruited at five groups that participate in the community based chronic disease management program called Program Pengelolaan Penyakit Kronis (Prolanis). The Prolanis groups were purposively selected based on the size of participants in the program. One group was selected at urban area and four groups at rural areas. Data were collected from May to June 2017 by conducting interviews, blood pressure measurements, and secondary data extraction from the medical record of the Prolanis database. The logistic regression analysis was used to determine the association between participation in a chronic disease management program, medication adherence and decrease of blood pressure.Results: As many as 75.7% of respondents actively participated in the Prolanis Program, and 81.6% were found with decreased blood pressure. Our study found a significant association between blood pressure decrease and active participation in the Prolanis Program with an adjusted odds ratio (AOR) of 6.38 (95%CI: 1.96–20.79), the good adherence towards medication with AOR=11.94 (95% CI: 3.60–39.56), and routine physical activities with AOR=3.84 (95%CI: 1.16-12.73). Conclusion: Active participation in the Prolanis Program, good adherence to medication and routine physical activities are independent factors of decreased blood pressure. These findings suggest the need for scaling up the Prolanis Program and increasing its coverage.

The government through BPJS Health has launched a chronic disease management program (PROLANIS) aimed at patients with hypertension cases to overcome the patient's hypertension condition. Activities in PROLANIS include medical or educational consultation activities, home visits, reminders, club activities, in fact there are still many people especially families who do not know about PROLANIS, so that many community members or families who have hypertension do not routinely control health facilities. The purpose of this activity is to run a chronic disease management program (prolanis) to reduce blood pressure and diabetes mellitus. The method used is lectures, discussions, home visits. Material or types of activities provided include health education about PROLANIS, hypertension and diabetes mellitus, attending elderly gymnastics, health consultations, checking blood pressure and blood sugar levels, and visiting home activities of PROLANIS members. Media used in the form of power points, blood pressure meters, blood sugar check devices, stethoscopes. The results of the assessment of this activity indicate the effectiveness of prolanis activities can affect the state of the patient, both blood pressure or physical activity. This is because there are activities that can improve patient understanding of the importance of managing hypertension and diabetes mellitus. Management of hypertension and diabetes mellitus is obtained through educational activities towards patients and reminders.

ABSTRACT Background: Diabetes Mellitus (DM) is a chronic and dangerous infectious disease. Various efforts to reduce morbidity and mortality due to diabetes have been carried out, one of which is the Chronic Disease Management Program (PROLANIS). Meanwhile, in its implementation, many are irregular. This study aimed to examine the relationship between age, gender, and level of knowledge with the complications of DM with obedience in following the PROLANIS. Subjects and Method: This was a cross sectional study conducted in the working area of the Gondang Community Health Center, Mojokerto Regency, East Java. Total of 42 DM patients were enrolled in this study using total sampling technique. The independent variables were age, gender, and level of knowledge. The dependent variable was obedience in following the PROLANIS. Data were collected using questionnaire and analyzed using Chi square test. Results: Factors associated with obedience in following PROLANIS were age (OR = 22.75; 95% CI = 2.42 to 213.73; p= 0.002), gender (OR = 12.00; 95% CI = 2.46 to 58.47; p = 0.001) and the level of knowledge about DM complications (OR = 7.00; 95% CI = 1.72 to 28.55; p = 0.006). Conclusion: Age, gender, and level of knowledge about complications due to DM are related to the DM patient’s obedience in following the Chronic Disease Management Program (PROLANIS). Abstract Background To exam the associations between the use of sodium glucose co-transporter 2 inhibitor (SGLT2i) and the risk of lower limb complications, and to analyze the associated factors. Methods Pubmed, Medline, Embase, the Cochrane Center Register of Controlled Trials for Studies and Clinicaltrial.gov were searched from the inception to November 2020. Randomized controlled trials of SGLT2i conducted in population containing diabetic patients with reports of amputation, peripheral arterial disease (PAD) and diabetic foot (DF) events were included. Random-effect model, fixed-effect model and meta-regression analysis were accordingly used. Result The numbers of SGLT2i users versus non-SGLT2i users in the analyses of amputation, PAD and DF were 40,925/33,414, 36,446/28,685 and 31,907/25,570 respectively. Compared with non-SGLT2i users, the risks of amputation and PAD were slightly increased in patients with canagliflozin treatment (amputation: OR = 1.60, 95% CI 1.04 to 2.46; PAD: OR = 1.53, 95 % CI 1.14 to 2.05). Meta-regression analyses indicated that greater weight reduction in SGLT2i users was significantly associated with the increased risks of amputation (β = − 0.461, 95% CI − 0.726 to − 0.197), PAD (β = − 0.359, 95% CI − 0.545 to − 0.172) and DF (β = − 0.476, 95% CI − 0.836 to − 0.116). Lower baseline diastolic blood pressure (β = − 0.528, 95% CI − 0.852 to − 0.205), more systolic blood pressure reduction (β = − 0.207, 95% CI − 0.390 to − 0.023) and more diastolic blood pressure reduction (β = − 0.312, 95% CI − 0.610 to − 0.015) were significantly associated with the increased risks of amputation, PAD and DF respectively in patients with SGLT2i treatment. Conclusions The risks of amputation and PAD were slightly increased in patients with canagliflozin treatment. Reductions in body weight and blood pressure were associated with lower limb complications in patients with SGLT2i treatment.
