Maxillofacial injuries sustained through Road traffic accidents (RTAs) is one of the leading causes of hospitalization in India. Numerous factors such as ever increasing amount of two wheelers, assault conditions, lack of safety measures such as helmets, seat belts and lack of strict implementation of such rules, prevailing socioeconomic, cultural and environmental factors results in a substantial number of facial fractures from RTAs. By 2020, RTA injuries will rise as the 6th major cause of death worldwide. One of the most commonly employed means to thwart such RTAs include the use of helmet, following speed limits, obeying stop signs and traffic signals. Among two wheelers, the use of Helmets plays a crucial role in protecting the cranial vault and potentially the facial skeleton if a full face helmet is used. 25% of the RTAs are caused due to two wheelers rather than four wheelers. Pillion rider, who ride passenger behind the rider on a two wheelers more suspectible to morbid injuries due to lack of safety measures like not wearing a helmet, improper sitting patterns especially by females in India. A study showed injuries such as contusion and skull fractures are common when riding pillion. Here we report a case of mandibular symphysis fracture along with bilateral subcondylar fracture with medial displacement of the fractured segment sustained via RTA while riding pillion.

Background: Road traffic injuries claim more than 1.25 million lives each year and have a huge impact on health and development. They are the leading cause of death among young people aged between 15 and 29 years globally. In the South East Asian region of the World Health Organization, India alone accounted for 73 percent of these Road traffic accidents (RTA) burden. The importance of road safety measures needs to be emphasized in the prevention of the road traffic accidents. The objectives of the study were to assess the knowledge and practice of road safety measures among undergraduate medical students.Methods: A cross-sectional study was conducted among 310 medical undergraduates of M.R. Medical College from 1st September to 1st October 2016. Data was collected using pre-tested, semi-structured, self-administered questionnaire. The data collected was analyzed using SPSS version 16. Statistical analysis was done using relevant statistical tests.Results: Out of the 310 students 54.19% were males and 45.81% were females. 90.9% of the participants had driving licence, 32.1% of the students had taken training for driving the car and 66% of the students did not wear helmet. Female students had high knowledge of the road safety measures when compared to male students.Conclusions: The overall knowledge of road safety measures was high among the study participants. Regarding practice behaviours they were not desirable like practice of wearing helmet and exceeding speed limit. Undertaking proper road safety measures are the best available interventions to curb the epidemic of RTA.

The aim of the study is to investigate the organizational aspects of providing emergency medical care to victims of road traffic accidents with injuries of the facial skeleton. Materials and research methods. The study included 75 victims of road accidents that occurred in the city of Saratov in 2010–2019. In the total number of injured men – 42 (56.0%), women – 33 (44.0%). The age of the victims is 18–70 years, the average age is (37.5 ± 6.0) years. The study did not include: victims under the age of 18; victims with neck trauma, damage to the cerebral section of the skull and to other parts of the body. All the victims got medical assistance from the personnel of the ambulance brigades. Covering sheets, outpatient cards and medical records were used as primary documentation. Study criteria: time during which the ambulance was provided; who provided emergency medical care; correctness of its rendering. Research results and their analysis. Analysis of injuries to the facial skeleton showed: injuries of the facial skeleton are an actual pathology in victims of road traffic accidents in Saratov; victims of road accidents got open and closed injuries of the facial skeleton. With closed injuries, moderate and severe injuries were found in 23 (30.7%) victims, with open injuries — in 19 (23.3%) victims; majority (90.7%) of victims of road accidents got high-quality emergency medical aid in a timely manner. In 9.3% of cases, the quality of care was insufficient due to an incorrect assessment of severity of the victims’ condition and, as a consequence, due to non-fulfillment of anti-shock measures; scope of the provision of emergency medical care to victims of road accidents included temporary hemostasis, treatment of wounds and anti-shock measures; from the accident site were evacuated: to level III trauma centers — 24.0% of victims; to level II — 48.0; to level I trauma centers — 28.0% of victims; complications in the form of purulent-septic processes were observed in 16.0% of victims; competent and timely implementation of anti-shock measures determined an insignificant mortality rate — 4.0%.

Descriptive study with the aim to assess knowledge regarding prevention of road traffic accidents was done by administrating self-structured questionnaire among 100 adolescents (between 10-19 years) at Dharan Municipality. Questionnaire includes the information regarding demographic and knowledge about meaning, causes and prevention of road traffic accidents. Among the respondents 55% were male and 45% were female. Driver responsibility and not obeying traffic signals for occurrences of RTA, use of safety seat belts for minimizing Road Traffic Accident, were the most responses provided 85%, 91% and 81% respectively.  Overall, only 33.34% had good knowledge regarding prevention of road traffic accidents. The study concludes the need for knowledge about accident prevention.

Background: Trauma units of tertiary care centers of the north Indian state of Punjab are occupied with young individuals with a passion for driving on full acceleration. There is therefore a high rate of road traffic accidents with and fractures of the facial skeleton are frequently noted .This is noted more so in the male gender.Methods: A retrospective study of the records of 61 subjects admitted under Otolaryngology and Maxillofacial trauma units, during a period of 2 years (August 2013-August 2015) at Dayanand Medical College and hospital were analysed.Results: Males outnumbered the females in the ratio of 5:1. Maximum, 54% were seen in the age group 21-30 years and minimum at the extremes of age. The commonest cause of fracture was road-side accidents which was observed in 72% of patients. In 15% these were due to assaults, in 8% due to falls and only in 3.2% due to sports injury.Conclusions: Facial fractures are recorded more in middle aged males with vehicular trauma being the main aetiology.

Road traffic accidents leave medical and social repercussions on families and individuals involved. Moreover, it is saddening that the most vulnerable are from the youth and middle age groups. This deprives the society of much needed and precious human resources. Hence, the mean which was originally designed to serve mankind has become a means of destruction, cause of disability and sometimes death. The medical profession has to assume a key role not only in the management of the injured, but also in the prevention of accidents and road safety measures. In order to reduce the incidence of road accidents it is important to establish an efficient organizational set-up, highlight the role of mass-transit, periodic checkup of the vehicle for its road worthiness and education of masses with the hope that they will adhere to the rules while on road.

The number of road traffic casualties is still very high, even though the number of fatalities in European countries is falling. In road traffic accidents involving personal injury, economic resources are destroyed and the productivity of economy is correspondingly impaired. Costs resulting from traffic accidents represent the largest single part of the overall cost of traffic in economus. Knowledge about the harm it does to economy is essential if measures to reduce road traffic accidents are identified and introduced. Economic evaluation of road safety measures using cost‐benefit analysis is based on the costs incurred as a result of road accidents. The avoidance of such costs represents the economic benefit of road safety measures. Road safety audit is a formal procedure for independent assessment of the accident potential and likely safety performance of a specific design for a road or traffic scheme - either new construction or alteration of an existing road.

AbstractNaso-orbito-ethmoid (NOE) injuries constitute about 5% of facial fractures and most of them are as a result of road traffic accidents. Deformities in the region tend to be more cosmetically apparent. Considering the proximity of the area to critical structures like brain and eye, such injuries need to be thoroughly evaluated both clinically and radiologically to understand the extent of the injury as well as initiate emergency management if required prior to constituting a definitive treatment plan. CT scans are the most useful investigation modality. Identification and maintaining the attachment of Medial canthal ligament in appropriate position along with reducing / stabilizing the bony fracture is the key to getting good results in these fractures. Primary management of the deformity produces better results compared to secondary correction.
