Mesiodens is a supernumerary tooth located in the anterior maxilla, placed palatally, labially or in between the maxillary central incisors. It may be single or paired, erupted or impacted and at times even inverted. This case report describes a palatally positioned two mesiodens in an 8-year-old male. The clinical and radiographic appearance and therapeutic considerations are presented. The purpose of this article is to alert the clinician to the importance of radiographic localization prior to surgical intervention, because in some instances a labial surgical approach is preferred over the traditional palatal technique. In this case a labial surgical approach was utilized which allowed for greater conservation of alveolar bone and improved access during removal of the mesiodens teeth.

Introduction: Hyperdontia is the condition of having supernumerary tooth, or teeth, which appear in addition to the regular number of teeth. It is a developmental anomaly and has been argued to arise from multiple aetiologies. It may remain embedded in the alveolar bone or can erupt into the oral cavity. Mesiodens is a supernumerary tooth located in the anterior maxilla, placed palatally or in between the maxillary central incisors. It may be single or paired, erupted or impacted and at times even inverted. Aesthetic or functional problems are two main issues associated with mesiodens. This case report demonstrates the orthodontic management of a case of a nonsyndromic male patient with conical type supernumerary tooth at the maxillary anterior region with a Class I malocclusion complicated by palatally erupted upper right lateral tooth with Bolton discrepancy. Good aesthetic and functional result achieved at the end of treatment.

Introduction The occurrence of isolated spheno-choanal polyps are not very common and can be very well confused with antro-choanal polyps or adenoids in adolescents and children. Appropriate diagnosis and prompt surgical intervention are essential for its removal.   Case Report We report one such case of a 16 year old female who presented with complaints nasal obstruction and reduced hearing. Endoscopic polypectomy was done with complete removal of the polyp.   Discussion Sphenochoanal Polyp is often misdiagnosed and proper investigations are thus essential for its diagnosis. Surgical Approach is the mainstay form of treatment with complete excision of the polyp.

Background. Macroprolactinemia is defined as predominance of high molecular weight prolactin forms in the circulation. Although macroprolactin is considered as a biologically inactive molecule, some authorities suggest treatment in symptomatic cases. Gigantomastia is defined as excess breast tissue and most cases in the literature were treated by surgical intervention.Case. A 44-year-old woman was admitted to our clinic with gigantomastia and galactorrhea. The patient had a demand for surgical therapy. In laboratory examination, she had hyperprolactinemia and macroprolactinemia. Pituitary imaging revealed 6 mm microadenoma in right side of the hypophysis. Since she was symptomatic, cabergolin treatment was started. Macroprolactin became negative, breast circumference decreased significantly, and galactorrhea resolved after treatment.Conclusion. Gigantomastia might be the presenting symptom in patients with macroprolactinemia. In these patients medical treatment with cabergoline may be used initially as an alternative to surgical approach.

Introduction Dentigerous cysts are caused by a developmental abnormality derived from the reduced enamel epithelium of the tooth-forming organ. Dentigerous cyst rarely involve impacted supernumerary tooth in anterior maxilla while commonly involve third molar tooth.   Case Report A case of dentigerous cyst in association with supernumerary tooth in a 15-year-old male patient is reported causing right nasal cavity obstruction. The present case report describes the management of a dentigerous cyst by surgical enucleation.

Abstract Mesiodens is a supernumerary tooth occurring in the anterior maxilla, located between the maxillary central incisors. Extraction of a mesiodens at a time appropriate for promoting self-eruption in the early mixed dentition may result in better alignment of the teeth and may minimize the need for orthodontic treatment. This case report features management of erupted mesiodens which caused the permanent maxillary right central incisor to be displaced labially in a ten year old female who was treated successfully by timely intervention in her mixed dentition. Many of medicaments used historically in root canal treatment have been shown to be cytotoxic. Paraformaldehid agents (such as Toxavit and Depulpin) are used to devitalize inflamed pulp when local anesthesia is ineffective. The misuse of pulp devitalizing agents may cause damage to gingiva and alveolar bone. This case report demonstrates complications arising after application of paraformaldehyde containing paste, necrosis of the gingiva and alveolar cortical bone, which resulted in great loss of supporting bone. Surgical intervention was required wherein necrotic bone was removed and bone defect was filled with xenograft of bovine origin. After three months endodontic treatment was performed. After the treatment, the patient?s complaints were resolved. The use of paraformaldehyde-based agents during endodontic therapy requires special caution.

Abstract: A rare, benign odontogenic tumour - Odontogenic myxoma has a locally aggressive behavior, and is relatively rare in the maxilla. A variety of surgical approaches for this type of tumour have been tried but no clear-cut management protocol has been defined. This case addresses the surgical management of odontogenic myxoma and its behavior in the clinical setting.

Abstract The rationale behind endodontics is to thoroughly disinfect the root canal system, and to provide hermetic seal which in turn ceases the progress of the periapical infection. Most of these periapical infections attributed directly or indirectly to oral microbes. Surgical interventions in larger periapical infection are sometimes accompanied by certain complications. Hence, non-surgical approach can be considered. This article presents non-surgical healing of a periapical lesion of endodontic origin associated with maxillary left central and lateral incisors, observed over a period of 6 months, using calcium hydroxide as an intracanal medicament. ABSTRACT This is a case report that describes a labially positioned rudimentary mesiodens in between the fully erupted maxillary central incisors in a 10-year-old girl. It was partially erupted in the oral cavity, without causing malocclusion or displacement of adjacent teeth. The clinical and radiographic appearance and therapeutic considerations are presented. The diverse modes of presentation of this common dental anomaly are described in this case report to alert the clinician.
