Essential hypertension and its common complication - chronic heart failure, is one of the most significant medical, economic and social problems in the XXI century. At the same time, this pathology is 30-40% genetically predisposed. One of significant pathogenic factors is the inheritance of definite variants of genes, coding the receptors to angiotensin II type 1. For this reason, the effective and inexpensive researches for screening of the mentioned genetic phenomenon are being carried out. Objective: to improve the screening diagnostic methods for carriers of polymorphic genes of angiotensin II type 1 receptors with essential hypertension and essential hypertension complicated by chronic heart failure. There were studied dermatoglyphic prints of men 40-60 years, who are carriers of polymorphic gene variants of angiotensin II type 1 receptor with no cardiovascular diseases (n=79), male patients with essential hypertension and hypertrophy of the myocardium (stage II), II-III stages (n=62) and essential hypertension (n=50) complicated by chronic heart failure, residents of Podillya region of Ukraine. Genotyping of the gene of angiotensin II type 1 receptor was performed using polymerase chain reaction. All patients included in the control group and those with essential hypertension underwent dermatoglyphic examination of fingers on both hands using modern portable rolling scanner Futronic FS50 (Korea). Interpretation and decoding of dermatoglyphic prints was conducted by T. D. Gladkova’s method. Statistical analysis was done on personal computer using standard statistical package “STATISTICA 10.0”. The found ulnar loop was dominant dermatoglyphic print regardless of the presence or absence of essential hypertension and chronic heart failure in 40-60 year old men. Besides, positive correlation relationship was revealed between the inheritance gene of angiotensin II type 1 receptors and fingerprint patterns: the third finger on the left hand in males with no cardiovascular pathology (weak strength) and the second finger on the right hand in patients with essential hypertension (medium strength). In individuals with no cardiovascular diseases, carriers of genotype A1166A, prevalence of ulnar loop on the third finger of left hand is higher than in carriers of C allele gene of angiotensin II type 1 receptors. In males with essential hypertension, carriers of C allele, degree of incidence of the whorl on the second finger of right hand is significantly higher than in carriers of genotype А1166А, offering the possibility to determine the carriership of a particular gene of angiotensin II type 1 receptors. Carrying out dermatoglyphic examination in patients with an increase in blood pressure and determining the variant of carrier of the gene of the receptor to angiotensin II type 1 will make it possible to facilitate the selection of those individuals who in the future need to spend all the necessary amount of research to confirm the diagnosis of essential hypertension and the possible development of complications of the course of this cardiovascular disease.

One of the main etiological causes of development of heart failure is essential hypertension. The diagnosis of heart failure is usually made on the basis of comprehensive analysis of medical history, sonographic and biochemical examination. Normal ejection fraction does not exclude dyspnea of cardiac origin. Objective: to determine the role of galectin-3 as a marker of structural and functional changes of the myocardium in males with essential hypertension and CHF, carriers of polymorphic AT1R genes, residents of Podillya region of Ukraine. In this contingent, the surveyed were studied сoncentrations of galectin-3 and brain natriuretic peptide (BNP), parameters of central and systemic hemodynamics in carriers of polymorphic variants of angiotensin II type 1 receptor gene (АТ1R) - individuals with no cardiovascular pathology (n=79), male patients with II-III degree essential hypertension (EH) and hypertrophy of the myocardium (n=62), and essential hypertension (n=50) complicated by chronic heart failure (CHF), residents of Podillya region of Ukraine, were studied. Genotyping of АТ1R gene was performed using polymerase chain reaction. Galectin-3 and brain natriuretic peptide levels were determined by enzyme immunoassay. Structural and functional parameters of myocardium were assessed by ultrasound using the apparatus “RADMIR ULTIMARA”. Statistical analysis of the results obtained was done on personal computer using standard statistical package Statistica 10.0. The data are represented as mean values (M) and standard deviations (±m). Carriers of C allele of angiotensin II type 1 receptor gene were found to be dominating among the males with essential hypertension and resultant myocardial hypertrophy. Concentrations of galectin-3 and brain natriuretic peptide were significantly higher in men with essential hypertension and essential hypertension associated with chronic heart failure, as compared to those with no cardiovascular diseases, as well as the carriers of C allele of angiotensin II type 1 receptor gene. It was found that concentrations of study biomarkers were higher in individuals with severe and eccentric left ventricular hypertrophy, as well as in those with decreased ejection fraction of the left ventricle. Therefore, those biomarkers can be used in complex diagnosis of left ventricular hypertrophy in essential hypertension and the development of chronic heart failure in such patients.

The open randomized placebo-controlled prospective study was conducted; we documented improvement of global contractility and diastolic function of left ventricle, clinical and psychological status in 60 patients with chronic heart failure receiving a very low dose of antibodies to C-end fragment to AT1 receptors of angiotensin II for 6 months (Cardosten). The drug was well-tolerated.

The review presents data on the causes of dysregulation of calcium metabolism and its association with the main elements of the cardiovascular continuum. Particular attention was paid to its role in the regulation of endothelial function, systemic inflammation, myocardial contractility, lipid and carbohydrate metabolism, the disruption of which determines the initiation and progression of diseases such as essential hypertension and chronic heart failure. We also systematized data on the role and methods of calcium dysmetabolism correcting in the prevention and treatment of these pathologies.

Qili qiangxin capsule (QL), a traditional Chinese herbal compound, has been proved to be effective and safe for the treatment of chronic heart failure (CHF). Upregulation of aquaporin-2 (AQP2) accounts for the water retention in CHF. The aim of the present study was to evaluate the effects of QL on the expression of AQP2 in rats with CHF induced by acute myocardial infarction and to investigate the underlying mechanisms. The urine output of all rats was quantified and collected every day at the first week and the 4th week after administration of QL or Valsartan. The expression of AQP2, vasopressin type 2 receptor (V2R), and angiotensin II type 1 receptor (AT1R) were examined after treatment for 4 weeks. Urinary output increased significantly after administration of QL. Importantly, the protein expression of AQP2 and AQP2 phosphorylated at serine 256 (pS256-AQP2) was downregulated after administration of QL and Valsartan to CHF rats. Furthermore, QL reduced plasma arginine vasopressin (AVP) and angiotensin II (AngII) level and downregulated V2R and AT1R protein expression. Thus, QL exerts its diuretic effect and improves cardiac function in CHF rats by reversing the increases in both AQP2 and pS256-AQP2 expression. The possible mechanisms may involve inhibition of V2R and AT1R.
