The antral hormone gastrin potently regulates gastric acid secretion and fundic mucosal growth. Consequently, appropriate gastrin secretion and plasma concentrations are important for the early phases of digestion. This review describes as the first premise the normal biogenesis of gastrin in the antral mucosa, but also mentions the extraantral expression. Subsequently, the molecular nature and concentration levels of gastrin in serum or plasma are overviewed. Third, assays for accurate measurements of plasma or serum concentrations are commented. Finally, the problem of moderate hypergastrinemia due to Helicobacter pylori infections and/or treatment with proton-pump inhibitors (PPI) is discussed. The review concludes that accurate measurement of the true concentrations of bioactive gastrins in plasma is important. Moreover, it suggests that moderate hypergastrinemias are also essential health issues that require serious attention.

Compared with other inhaled anaesthetics, sevoflurane has a faster onset and offset, causes less irritation to the mucous membranes, and has a better safety profile. These characteristics warrant investigating the effect of sevoflurane in depression. In this Case Report, we describe that sevoflurane treatment was feasible and well tolerated by a patient with treatment-resistant depression (TRD). Sevoflurane had rapid and durable antidepressant effects, with few adverse effects. Moreover, the patient showed significant improvements in neurocognitive measurements. Our preliminary results suggest that further clinical trials are needed to determine the independent efficacy and safety of sevoflurane in patients with TRD.

The article presents a clinical case of Chauffard-Still syndrome. It is shown that for the correct etiological diagnosis of iridocyclitis in Still's syndrome, careful collection of anamnesis, the connection of the articular syndrome with uveitis and characteristic ophthalmological changes in the form of ribbon-like dystrophy of the cornea are important. Key words: Chaufard-Still syndrome, uveitis, corneal ribbon dystrophy.

Chronic diarrhea is a frequent reason for the referral to the specialist and its prevalence is estimated at around from 4 to 5%. Reaching an etiological diagnosis is a real clinical challenge. The pathophysiological mechanisms involved in its genesis are multiple and sometimes coexist in the same individual. A correctly oriented anamnesis and physical examination will allow to identify key data, mainly the presence of symptoms or alarm signs with the intention of discriminating between the functional and the organic causes. The doctor must know how to use his clinical judgment, evaluate the probabilities and use the diagnostic methods available with the precise orientation to adapt them to each particular case. The categorization of diarrhea as watery, malabsorptive or inflammatory attempts to simplify the diagnostic process, although it is only indicative. The objective of the following review is to present the different pathophysiological mechanisms involved in chronic diarrhea, as well as the available diagnostic and therapeutic tools that allow us to carry out an adequate approach for its study.

The article presents a clinical case of a 44-year-old female patient F. with a diagnosis of Functional dyspepsia. Dyspepsia associated with Helicobacter pylori. Chronic gastritis associated with H. pylori. The patient has been complaining of dyspepsia, shortness of breath, vomiting for 12 years. During this time, the respiratory, cardiovascular and digestive systems were examined, but no organic pathology was revealed. Three lines of H. pylori eradication were performed. Complete clinical remission after each course of H. pylori eradication was not achieved.

This chapter provides a summary of a landmark study on the pharmacologic management of treatment-resistant schizophrenia. This clinical trial, conducted by Kane and colleagues, asks whether clozapine is an effective option in the care of patients with treatment-resistant schizophrenia who have not responded to past trials of first-generation antipsychotics. To answer this question, the chapter reviews the basics of the study, including funding sources, study location, inclusion and exclusion criteria, number of participants, research design, interventions, follow-up, endpoints, results, criticisms, and limitations. The chapter briefly reviews other relevant studies and information, discusses implications, and concludes with a relevant clinical case.
