Reduction of bacterial after irrigation with 6% NaOCl, and continued irrigation with 17%EDTA and 2%CHX during Endodontic Treatment of Chronic Apical Periodontitis

The aim of this study was to assess the prevalence and risk factors of apical periodontitis in endodontically treated teeth in a selected population of Brazilian adults. A total of 1,372 periapical radiographs of endodontically treated teeth were analyzed based on the quality of root filling, status of coronal restoration and presence of posts associated with apical periodontitis (AP). Data were analyzed statistically using odds ratio, confidence intervals and chi-square test. The prevalence of AP with adequate endodontic treatment was low (16.5%). This percentage dropped to 12.1% in cases with adequate root filling and adequate coronal restoration. Teeth with adequate endodontic treatment and poor coronal restoration had an AP prevalence of 27.9%. AP increased to 71.7% in teeth with poor endodontic treatment associated with poor coronal restoration. When poor endodontic treatment was combined with adequate coronal restoration, AP prevalence was 61.8%. The prevalence of AP was low when associated with high technical quality of root canal treatment. Poor coronal restoration increased the risk of AP even when endodontic treatment was adequate (OR=2.80; 95%CI=1.87-4.22). The presence of intracanal posts had no influence on AP prevalence.

The objective of this clinical study was to evaluate by polymerase chain reaction (PCR) the antimicrobial efficacy and the outcome of an endodontic protocol treatment (EPT) performed by under-graduated dental students on infected root canals associated with periapical lesions. Fifty-six patients attending for treatment of pulp necrosis and apical periodontitis were included. A specific EPT approach was performed and DNA extracts were taken at the baseline (S1), after a disinfection-neutralization procedure (S2), post-chemomechanical preparation (S3) and after Intracanal medication (S4) for the presence of specific bacteria. The outcome of the EPT was assessed radiographically using the Periapical Index-system (PAI) after 18-month follow-up. An intergroup evaluation shows that when comparing S1 to S3, S1 to S4, S2 to S3, and S2 to S4, the presence of bacteria were significantly reduced (P < 0.05). Finally, S4 was not significantly reduced when compared with S3 (P > 0.05). The most prevalent species in their respect order were Actinomyces Israelii > Enterococcus Faecalis > Fusobacterium Nucleatum/Prevotella Nigrescens > Phorphyromonas Endodontalis. After the 18-month follow-up, the overall success rate of root canal – treated teeth was of 88% (PAI 1 or 2). This In Vivo study demonstrated that EPT significantly reduced the number of cases with positive results for the studied bacteria, showing a profound positive impact in the outcome of endodontic treatment of teeth diagnosed with Pulp Necrosis and Apical Periodontitis.

Objective: The aim of this study was evaluate radiographically prevalence of apical periodontitis (AP) in endodontically treated teeth (ETT) and relate with demographic factors, quality of endodontic treatment and coronal restoration. Material and methods: Data were collected from dental records wich contained full-mouth radiographic series. In ETT, arch, dental group and presence of AP were evaluated. Quality of root canal filling, presence and quality of coronal restoration and type of restorative material were also collected. Data were evaluated using descriptive statistics and Chi-square Test. Results: Of 70 patients included in the final sample, the majority were female (54.3%) and age ranged from 13 to 77 years (47.14 ± 13.18). Of 1,333 teeth evaluated, 73 (5.4%) had endodontic treatment, being the majority maxillary (74%) and anterior teeth (52.1%). Of teeth without endodontic treatment, 320 (25.4%) had AP. Failure rate was high (52.8%) and quality of root canal filling and coronal restoration were considered inadequate in majority cases (58.3% and 47.7%, respectively). Dental group had a statistically significant relationship with presence of AP in ETT, being more frequent in anterior teeth (p = 0.019). Conclusion: It was concluded that prevalence of AP in ETT was high and dental group was the factor that most influenced prevalence of AP in ETT.KeywordsApical periodontitis; Radiography; Root canal filling.

BACKGROUND Various intracanal medicaments have been used in cases of chronic apical periodontitis for appropriate disinfection of the root canal system to eliminate microbes specially from the inaccessible areas. Caclcium Hydroxide (CH) is the most common intracanal medicament available in various forms, but its effectiveness with or without iodoform using microbial culture is unknown. OBJECTIVE To compare the anti-microbial efficacy of Calcipex and Metapex in endodontic treatment of teeth presenting with chronic apical periodontitis METHODS 60 single rooted teeth of patients with diagnosis of chronic apical periodontitis were selected and the canals were debrided chemo-mechanically. The patients were randomized into two groups: Calcipex and Metapex. The first sample (S1) for bacterial culture was taken before placement of intracanal medicament and the second sample (S2) was taken after 7 days, before final obturation from the canal and sent to lab for culture. Colony Forming Unit (CFU) will be evaluated. Paired t-test will be used to assess difference bet¬ween antimicrobial efficacies within the group of medicaments. Independent sample t-test will be used to assess antimicrobial efficacies between groups. Level of significance will be kept at 0.05. RESULTS The results wil be shared upon completion of study. CONCLUSIONS There is no risk involved in this study as the study is based on routine procedure of endodontic treatment. It will be a benefit for future patients which will help us to decide which intracanal medicament is more effective to use in infected canals. CLINICALTRIAL NCT04336709

This paper, part 3 of the series, discusses the variation in maxillary sinus mucosal thickening when seen on radiographic images and the relation to disease. The role of apical periodontitis in disease of the maxillary sinus and its lining, the stages of endodontic treatment at which problems can arise and how these can be prevented, are considered. Complications involving extruded endodontic materials will also be discussed. CPD/Clinical Relevance: Apical periodontitis may very occasionally contribute to maxillary sinus infection, when the roots of teeth lie in close relation to the sinus. Apical displacement of infection, irrigants or materials during endodontic treatment may contribute to inflammation and infection and should be avoided.

Abstract The aim of this study was to evaluate the radiographic periapical repair and the synthesis of inflammatory mediators after endodontic treatment in a single session, using different irrigation protocols, in teeth with apical periodontitis. Experimental apical periodontitis were induced in dog’s teeth randomly assigned into 4 groups: G1 - Irrigation by Negative Apical Pressure (n= 20); G2 - Passive Ultrasonic Irrigation (n= 20), G3 - Positive Pressure Irrigation (n= 20); G4 - apical periodontitis without treatment (n= 20). After 180 days, the animals were euthanized, the tissues removed and submitted to histotechnical processing for immunohistochemical analysis of osteopontin (OPN), tumor necrosis factor-a (TNF-a) and interleukin 1-a (IL-1a). Radiographic analysis was performed using the Periapical Index (PAI), obtained prior to and 180 days following endodontic treatment. Data were analyzed using Wilcoxon signed-rank test, Fisher’s Exact test or Kruskal-Wallis test and Dunn’s post-test (a = 5%). Radiographically, after endodontic treatment, apical periodontitis persisted in 35% of G1 specimens, 40% of G2 and 40% of G3 (p>0.05), although a PAI reduction was observed (p<0.05). By immunohistochemical evaluation, endodontic treatment resulted in lower synthesis of TNF-a and OPN in periapical region, compared to apical periodontitis without treatment (p<0.05). Production of IL-1 was not modulated by endodontic treatment (p>0.05). Periapical healing was observed in approximately 60% of the cases after endodontic treatment performed in a single session with lower synthesis of TNF-a and OPN in the periapical region, regardless of the irrigation protocol used.
