The author is a professionally trained mathematician, physicist and engineer. His view of health and medicine is similar to his past experience on designing a physical object, such as a building structure or a working machine, which he calls an “object”. The object’s strength or its expected lifespan is similar to the health conditions and longevity of a human being based on the following three key factors: (1) The original strength of the object’s material which is similar to the genetic factors of a human body. (2) The engineering design and site construction of this object are similar to the fundamental influential causes, including lifestyle details, life-long habits, and environmental factors, on the human health. Among those external causes, if the author has to pick up one category which has the most influence above of all that is the “food”. (3) The object endures different operational problems due to external forces or impacts which are similar to various diseases suffered by humans. After the object experiences external forces or impacts, we use some structural reinforcements to fix the building’s damaged cracks or replace the malfunctioned part of the machine. These engineering actions are similar to the “medical treatments” doctors provide patients. The medical treatments, including medication interventions (oral drugs or biochemical injections), necessary surgeries, or certain organ transplants are similar to the engineering repair of the damaged object. This article emphasizes the relationship between causes and results such as symptoms of diseases. It particularly addresses the inter-relationship of 5 selected diseases: cancers, cardiovascular diseases (CVD), chronic kidney diseases (CKD), diabetic retinopathy (DR), and type 2 diabetes (T2D), where the most important cause for most diseases is “food”. This food category in his study contains 25 defined elements include input data of food are collected via the developed iPhone APP. There are 5 elements for “Food Quantity”: breakfast amount, lunch amount, dinner amount, amount of between-meals snacks and fruits, and the average daily food consumption amount. In addition, there are 20 selected elements for “Food Quality” (see Figure 1). The combined Food score is the average value of both food quantity score and food quality score.

Generally speaking, the genetic (not “epigenetic”) portion only contributes 15% to 30%, approximately 20% or less, of having various chronic diseases. (2) The quality of engineering design and construction or manufacturing of this object are similar to the fundamental influential causes, including lifestyle details, life-long bad habits, and environmental damaging factors on human health. Among the external causes, one category that has the most impact on health is food, particularly with processed foods causing the most damage. Therefore, he tries to exclude all kinds of processed foods from his own food category. (3) The object suffers from different operational problems due to external forces or impacts which are similar to various diseases affecting humans. After the object suffers from external forces or impacts such as an earthquake or hurricane, we must use some structural reinforcements to fix the problems or replace the malfunctioned parts of the machine. These engineering after-actions are similar to the medical “treatments” post-injury/infection provided to patients by doctors. The medical treatments include medication interventions (either oral drugs or biochemical injections), necessary surgeries or organ transplants, which are similar to the engineering repair of the damaged object. Nevertheless, all type of medical treatments bring some degree of traumatic effects on the human body. In addition, up to now, there are no medications which can cure chronic diseases induced by metabolic disorders. The different medications given to patients only suppress the symptoms of different chronic diseases and do not deal with the root causes. Therefore, they are not able to reverse, repair, or cure chronic diseases.

Abstract Objective: The Food Quality Observatory was created in the province of Quebec (Canada) in 2016. In this study, the Observatory aimed to generate a methodology to (1) test the use of sales data combined with nutrient values to characterise the nutritional composition of ready-to-eat (RTE) breakfast cereals offered and purchased in the province of Quebec (Canada) and (2) verify the extent to which a front-of-pack label based on the percentage of daily value (DV) for total sugar, as a strategy to improve the food supply, would be distributed in this food category. Design: Nutritional information were obtained by purchasing each RTE breakfast cereal available in the Greater Montreal area. Cereals were then classified according to their processing type. Setting: The nutritional values of 331 RTE breakfast cereals available in Quebec were merged with sales data covering the period between May 2016 and May 2017. A total of 306 products were successfully cross-referenced. Results: Granola and sweetened cereals were the most available (36·6 % and 19·6 %, respectively) and purchased (19·8 % and 40·9 % of sales, respectively). When compared with other types of cereals, granola cereals had a higher energy, fat, saturated fat, protein content and a lower Na content. A larger proportion of chocolate (65 %) and sweetened cereals (49 %) were above 15 % of the DV for sugar. Conclusions: This study showed that the methodology developed generates important data to monitor nutritional quality of the food supply and ultimately contribute to improve the nutritional quality of processed foods.

Objetivo: descrever as internações, óbitos e custos hospitalares pelas intercorrências dialíticas em pacientes renais crônicos no Nordeste. Método: trata-se de um estudo quantitativo, descritivo e ecológico, voltado para a análise de dados do Sistema de Informações Hospitalares, organizados em frequências absolutas e relativas, a partir de tabelas construídas no software Excel. Resultados: notificaram-se 14.052 internações e 987 óbitos no Nordeste. Gerou-se, como consequência, um custo superior a R$ 19,6 milhões aos cofres públicos, com um valor médio de internação de R$ 1.543,09 e uma média de permanência de 9,1 dias. Destacaram-se os Estados de Alagoas, com a prevalência das internações (38,2%), e Bahia, em relação aos óbitos (40%), custos hospitalares (61,4%), média de permanência (14,4 dias) e valor médio de internação (R$ 2.794,42). Conclusão: aponta-se que as internações e óbitos pelas intercorrências dialíticas constituem um importante problema na Nefrologia, causando prejuízos diretos aos recursos financeiros públicos, especialmente, nos Estados da Bahia e Alagoas, por evidenciarem a maioria dos casos. Method: this is a quantitative, descriptive and ecological study, focused on the analysis of data from the Hospital Information System, organized in absolute and relative frequencies, using tables built using Excel software. Results: 14,052 hospitalizations and 987 deaths were reported in the Northeast. As a result, the public coffers cost over R $ 19.6 million, with an average hospitalization value of R $ 1,543.09 and an average length of stay of 9.1 days. The states of Alagoas stood out, with the prevalence of hospitalizations (38.2%), and Bahia, in relation to deaths (40%), hospital costs (61.4%), average length of stay (14.4 days) and average value of hospitalization (R $ 2,794.42). Conclusion: it is pointed out that hospitalizations and deaths due to dialysis complications are an important problem in Nephrology, causing direct damage to public financial resources, especially in the states of Bahia and Alagoas, as they show the majority of cases. Conclusión: se observa que las hospitalizaciones y muertes debidas a las intercurrencias dialíticas se constituyen un problema importante en la nefrología, lo que implica directamente en los cofres públicos, especialmente en los Estados de Bahía y Alagoas, como evidencia de una mayor prevalencia de casos. Objetivo: descrever a morbimortalidade hospitalar e custos públicos relacionados à insuficiência renal aguda no Nordeste brasileiro. Método: trata-se de estudo quantitativo, ecológico e descritivo realizado com dados do Sistema de Informações Hospitalares. Coletaram-se as variáveis: internações; óbitos; mortalidade; valores dos serviços hospitalares; valores médios gastos com as internações e média de permanência hospitalar. Analisaram-se os dados com estatística descritiva, apresentando-os em tabelas elaboradas pelo software Excel. Resultados: registraram-se 21.100 internações, 4.460 óbitos e mortalidade de 21,1%. Destacaram-se a Bahia nas internações (28,6%) e óbitos (28,7%) e Sergipe na letalidade (35,6%). Gerou-se custo público superior a R$ 37,6 milhões e a Bahia responsabilizou-se por 32,1%. Revelou-se valor médio de R$ 1.969,97 para cada internação e a média de permanência hospitalar foi de 10,2 dias. Conclusão: conclui-se que as variáveis epidemiológicas apresentaram maior prevalência nos Estados da Bahia e Sergipe. Evidenciou-se, além disso, comportamento crescente das internações, implicando o incremento dos custos públicos. Conclusion: it is concluded that the epidemiological variables were more prevalent in the states of Bahia and Sergipe. In addition, the increasing behavior of hospitalizations was evidenced, implying an increase in public costs. The author was a mathematician and an engineer. His view of longevity is similar to his past experience on designing and building a structure or a machine which he calls an “object”. The expected lifespan of an object is similar to the longevity of a human being based on the following three factors: (1) The availability of good and strong building materials that are similar to the genetic factors of a human body health conditions. (2) The engineering design and site construction of this object are similar to the lifestyle, life-long habits, and environmental factors which are related to the health of the human body. (3) The building’s damaged cracks or the machine’s malfunctioned parts under external forces are similar to the medical conditions and symptoms of disease in the human body. If the object suffers from operational problems due to external forces that is similar to the human body being affected by various diseases, then we can repair the object such as adopting the reinforcement of the building structural part or replacing the operational part of the machine. They are similar to the medical treatments including medication interventions, different surgeries, or organ transplants. Once the author understood the analogy and similarity between an engineering object and a human body, he can then distinguish the differences among genetic reason, lifestyle maintenance, disease control, and medical treatments. It is extremely difficult, almost impossible at present time, to change our genes because we cannot select our biological parents; however, we can focus on our daily lifestyle management to either prevent from having diseases or controlling the progression of existing diseases.

The population dynamics of the larch budmoth (LBM), Zeiraphera diniana, in the Swiss Alps are perhaps the best example of periodic oscillations in ecology (figure 7.1). These oscillations are characterized by a remarkably regular periodicity, and by an enormous range of densities experienced during a typical cycle (about 100,000-fold difference between peak and trough numbers). Furthermore, nonlinear time series analysis of LBM data (e.g., Turchin 1990, Turchin and Taylor 1992) indicates that LBM oscillations are definitely generated by a second-order dynamical process (in other words, there is a strong delayed density dependence—see also chapter 1). Analysis of time series data on LBM dynamics from five valleys in the Alps suggests that around 90% of variance in Rt is explained by the phenomenological time series model employing lagged LBM densities, R, =f(Ni-1,Ni-2,) (Turchin 2002). As discussed in the influential review by Baltensweiler and Fischlin (1988) about a decade ago, ecological theory suggests a number of candidate mechanisms that can produce the type of dynamics observed in the LBM (see also chapter 1). Baltensweiler and Fischlin concluded that changes in food quality induced by previous budmoth feeding was the most plausible explanation for the population cycles. During the last decade, the issue of larch budmoth oscillations was periodically revisited by various population ecologists looking for general insights about insect population cycles (e.g., Royama 1977, Bowers et al. 1993, Ginzburg and Taneyhill 1994, Den Boer and Reddingius 1996, Hunter and Dwyer 1998, Berryman 1999). These authors generally concurred with the view that budmoth cycles are driven by the interaction with food quality. A recent reanalysis of the rich data set on budmoth population ecology collected by Swiss researchers over a period of several decades, however, suggested that the role of parasitism is underappreciated (Turchin et al. 2002). Before focusing on the roles of food quality and parasitism in LBM dynamics, we briefly review the status of other hypotheses that were discussed in the literature on LBM cycles. First, the natural history of the LBM-larch system is such that food quantity is an unlikely factor to explain LBM oscillations.

Benthic species are partitioned into functional feeding groups based on food-acquiring mechanisms. Effects of food quality on voltinism, growth rate, and size at maturity are demonstrated for representatives of gougers and shredders, collectors, and scrapers. Food quality for predators is uniformly high, but food quantity (prey density) obviously influences their life histories. A food switch from herbivory to predation, or some ingestion of animal tissues, in the later stages is a feature of the life cycle of many aquatic insects. Temperature interacts with both food quality and quantity in effects on growth as well as having a direct effect on control of metabolism. Thus further elaboration of the role of food in life history phenomena will require controlled field or laboratory studies to partition the effects of temperature and food. Abstract In natural heterogeneous environments, the fitness of animals is strongly influenced by the availability and composition of food. Food quantity and biochemical quality constraints may affect individual traits of consumers differently, mediating fitness response variation within and among species. Using a multifactorial experimental approach, we assessed population growth rate, fecundity, and survival of six strains of the two closely related freshwater rotifer species Brachionus calyciflorus sensu stricto and Brachionus fernandoi. Therefore, rotifers fed low and high concentrations of three algal species differing in their biochemical food quality. Additionally, we explored the potential of a single limiting biochemical nutrient to mediate variations in population growth response. Therefore, rotifers fed a sterol-free alga, which we supplemented with cholesterol-containing liposomes. Co-limitation by food quantity and biochemical food quality resulted in differences in population growth rates among strains, but not between species, although effects on fecundity and survival differed between species. The effect of cholesterol supplementation on population growth was strain-specific but not species-specific. We show that fitness response variations within and among species can be mediated by biochemical food quality. Dietary constraints thus may act as evolutionary drivers on physiological traits of consumers, which may have strong implications for various ecological interactions.
