The patients with essential hypertension are increasing in our country and all over the world. Insulin secretion by the pancreatic beta cells may be increased in several metabolic disorders including glucose intolerance, dyslipidemia and essential hypertension. Many studies suggested an association between essential hypertension and defective insulin secretion. So, insulin secretory capacity may act as important clinical and biochemical determinant which will provide further information to minimize hypertension related other complications. This study was aimed to assess insulin secretory capacity in adult male with essential hypertension. This cross sectional study was conducted in the Department of Physiology, Dhaka Medical College, Dhaka from July 2012 to June 2013. A total number of one hundred fifty male subjects were selected with age ranging from 25 to 45 years. Among them, seventy five male essential hypertensive subjects were included in the study group. The study subjects were selected from Out- patient Department of Medicine, Dhaka Medical College Hospital. Age matched 75 apparently healthy males were studied as control group (A) for comparison. or the calculation of insulin secretory capacity, fasting serum insulin level was measured by ELISA method and fasting serum glucose by glucose oxidase method in the laboratory of Department of Physiology and Molecular Biology, BIRDEM Academy, Dhaka.In this study, 134 insulin secretory capacity (HOMA%B) was significantly higher in essential hypertensive male patients than those of the control subjects. Hyperproinsulinemia is commonly present in subjects with impaired glucose tolerance. The present study was undertaken to investigate the proinsulin level in Bangladeshi IGT subjects and to explore its association with insulin resistance. This observational study was conducted under a case-control design with IGT subjects (n=50) and controls (n=44). IGT was diagnosed following the WHO Study Group Criteria. Serum glucose was measured by glucose-oxidase method, serum lipid profile by enzymatic method and serum insulin and serum proinsulin were measured by ELISA method. Insulin secretory capacity (HOMA%B) and insulin sensitivity (HOMA%S) were calculated from fasting serum glucose and fasting serum insulin by homeostasis model assessment. The study subjects were age- and BMI- matched. Mean (±SD) age (yrs) of the control and IGT subjects were 40±6 and 40±5 respectively (p=0.853). Mean (±SD) BMI of the control and IGT subjects were 23±3 and 22±2 respectively (p=0.123). Fasting glucose was not significantly higher in IGT subjects, but serum glucose 2 hours after 75 gm glucose load was significantly higher in IGT subjects. Median (Range) value of fasting serum glucose (mmol/l) of control and IGT subjects were 5.3 (3.8-6) and 5.2 (4-12) respectively; (p=0.297). Median (Range) value of serum glucose (mmol/l) 2 hours after 75 gm glucose load of control and IGT subjects were 6.1 (3-7.8) and 7.9 (5- 21) respectively; (p=0.001). Fasting TG was significantly higher in IGT subjects and LDL-c was significantly lower in IGT subjects. Serum Total cholesterol and HDL-c were not significantly different between the IGT and control subjects. Median (Range) value of fasting serum TG (mg/dl) of control and IGT subjects were 119 (51-474) and 178 (82-540) respectively; (p=0.001). Median (Range) value of fasting serum T chol (mg/dl) of control and IGT subjects were 180 (65-272) and 186 (140-400) respectively; (p=0.191). Median (Range) value of fasting serum HDL-C (mg/dl) of control and IGT subjects were 29 (19-45) and 31 (15-78) respectively; (p=0.914). Median (Range) value of fasting serum LDL-C (mg/dl) of control and IGT subjects were 117(29-201) and 111(41- 320) respectively; (p=0.001). Fasting serum proinsulin was significantly higher in IGT subjects. Median (Range) value of fasting serum proinsulin (pmol/l) of control and IGT subjects were 9.2(1.8-156) and 17(3-51) respectively; (p=0.001). Insulin secretory capacity (HOMA%B) was higher but insulin sensitivity (HOMA%S) was significantly lower in case of IGT subjects. Median (Range) value of HOMA%B of control and IGT subjects were 97(46-498) and 164(17-300) respectively; (p=0.001). Median (Range) value of HOMA%S of control and IGT subjects were 68(19-270) and 39(15-110) respectively (p=0.001). In multiple regression analysis a significant negative association was found between fasting proinsulin and insulin sensitivity (p=0.037). The data led to the following conclusions: a) Insulin resistance is the predominant defect in Bangladeshi IGT subjects. b) Basal proinsulin level is significantly increased in IGT subjects. c) Insulin resistance is negatively associated with serum proinsulin in IGT subjects. Background: An association between essential hypertension and defective insulin secretion has been identified. Objective: To estimate fasting serum insulin level in adult male with essential hypertension to observe its relationship to hypertension. Methods: This cross sectional study was conducted in the Department of Physiology, Dhaka Medical College, Dhaka from July 2012 to June 2013. A total number of one hundred fifty male subjects were selected with age ranging from 25 to 45 years. Seventy five male essential hypertensive were enrolled from Out- patient Department of Medicine, Dhaka Medical College Hospital, Dhaka. Age matched seventy five apparently healthy males were studied as control. Fasting serum insulin level was measured by ELISA method and fasting blood glucose by glucose oxidase method. For statistical analyses, unpaired Student’s ‘t’ and Pearson’s correlation coefficient (r) tests were performed. Results: Fasting serum insulin level was significantly (P<0.001) higher in essential hypertensive male patients than normotensive subjects. Fasting serum insulin level shows significant positive correlation with systolic blood pressure in hypertensive patients. Conclusion: This study reveals that essential hypertension has positive and significant relationship with fasting serum insulin level. Abstract Background: One of the largest plant genera of the Umbelliferae family is Eryngium including 274 species, of which only 9 species are native to Iran and one of the most dominant species is E. billardieri. Numerous pharmacological effects of E. billardieri such as anti-inflammatory, antimicrobial, antioxidant have been reported in several studies. Objective: The present study aimed to investigate the anti-diabetic effects of E. billardieri on streptozotocin-induced diabetes in male mice.Methods: The extraction was performed by maceration with 70% ethanol solvent. Forty eight mice, weighing 32 g, were randomly divided into 8 groups (6 mice per group): healthy control, diabetic control (151 mg/kg STZ; IP), healthy extract control group (receiving the high dose of the extract orally), diabetic groups (receiving low dose and high dose of extract in the form of IP and Oral), the treatment group which received STZ (IP) and high dose of the extract group (Oral) for five days. To induce diabetes, a single dose of STZ (151 mg/kg) was injected intra-peritoneally. After diabetes, 100 mg/kg and 300 mg/kg of extract were injected into diabetic animals for 18 days as IP and Oral were given, and finally, serum samples were isolated and stored at -71 °C. Three days before surgery, OGTT test was performed.Results: IP injection of 300 mg/kg of hydroalcoholic extract of E. billardieri reduced the weight of diabetic rats and significantly reduced glucose tolerance. Furthermore, gavage at a dose of 300 mg/kg of extract caused significantly the increase in serum insulin while the decrease in blood sugar, glucose tolerance and malondialdehyde. Conclusion: E. billardieri extract, due to its components, probably increases serum insulin and decreases serum malondialdehyde by two mechanisms including protects pancreatic beta cells from further damage by streptozotocin and stimulation of insulin secretion from the remaining healthy pancreatic beta cells. In other words, part of the beneficial effects of the extract is associated with inhibition of oxidative stress and increased insulin secretion.

The effect of cachexia on insulin secretion was examined in adult male rats. Isolated islets of Langerhans from Walker 256 tumor-bearing rats secreted less insulin by glucose stimuli as compared with the control group; this was accompanied by significant change in 45Ca2+ outflow rate. Reduced insulin secretion to glucose stimuli in tumor-bearing rats probably led to low insulinemia (one-third). These findings indicate that reduced insulin secretion is probably an important factor for the development of cachexia in Walker 256 tumor-bearing rats.

Diabetes mellitus is one of the most widespread endocrine disorder in female and its complications are increasing all over the world, leading to life threatening medical problems like cardiovascular diseases, stroke and end stage renal diseases. A relation between hyperlipidemia and diabetes mellitus has been identified. The study was carried out to observe the serum triglycerides and total cholesterol levels its relation with type 2 diabetes mellitus in adult female subjects. This cross sectional study was conducted in the Department of Physiology, Dhaka Medical College, Dhaka, during the period of January 2011 to December 2011. A total number of sixty female subjects were selected with age ranging from30 to50 years. Among them 30 female subjects with diabetes were included from out-patient department of Endocrinology, Dhaka Medical College Hospital, Dhaka as study group(B) and 30 apparently healthy female were taken as control group(A) for comparison. Estimation of fasting serum triglycerides (TG) and total cholesterol (TC) levels were done by enzymatic method in the Department of Physiology, Dhaka Medical College Dhaka in both groups. Fasting serum insulin level was measured by ELISA method in the Department of Laboratory of National Institute of ENT, Dhaka and fasting blood glucose was estimated by glucose oxidase method in Department of Dhaka Medical College in both groups. Data were analyzed by Unpaired Student’s- test and Pearson’s correlation co-efficient (r) test as applicable. The value of fasting serum TG and TC level were significantly higher in study subjects than those of control. In study subjects fasting serum TG and fasting serum TC levels showed positive correlation with fasting blood glucose and serum insulin level. Conclusion: Present study revealed that fasting serum triglyceride (TG) and fasting serum cholesterol (TC) levels have positive relationship with fasting serum insulin and fasting blood glucose levels. Background: The relative contribution of insulin secretion and sensitivity in the development of Type 2 diabetes mellitus (T2DM) vary from population to population due to the heterogeneous nature of the disease. The study was undertaken to evaluate insulin secretory capacity and sensitivity in a Bangladeshi Type 2 diabetic population and to explore the association of some of the anthropometric and biochemical factors known to modulate B-cell function and insulin action. Methods: Ninety one T2DM subjects and 32 age-matched controls were studied for their fasting plasma glucose (FPG), lipids, HbA1c (by HPLC), leptin and C-peptide (ELISA). Insulin secretion (HOMA B) and insulin sensitivity (HOMA S) were calculated by homeostasis model assessment (HOMA). Results: Both insulin secretion and sensitivity were significantly reduced in diabetic as compared to control (HOMA B%, geometric mean±SD, 35.65±1.75 vs. 96.29±1.50, p < 0.001; HOMA S%, 68.66±1.71 vs. 104.951.63, p < 0.001). However, B-cell dysfunction was predominant than insulin resistance in predicting T2DM as the discriminate function coefficient for HOMA B (1.098) was greater than that for HOMA S (0.821). In T2DM, HOMA B had positive correlation with BMI (r=0.368, p < 0.001) and HOMA S was inversely correlated to BMI (r=-0.261, p < 0.01), WHR (r=-0.258, p < 0.01) and plasma TG (r=-0.233, p < 0.001). On multiple regression analysis HOMA B and HOMA S were found to be inversely associated to FPG (p < 0.001) and leptin (p < 0.05) in T2DM. Conclusions: Both insulin secretory dysfunction and insulin resistance are present in Bangladeshi T2DM subjects, but B-cell failure seems to be the predominant abnormality. BMI, plasma glucose, insulin and leptin are the major determinants of insulin secretory capacity and generalized as well as central obesity, plasma glucose, triglycerides, insulin and leptin are among the major determinants of insulin sensitivity in this population. An association between hyperlipidemia and hypertension has been identified. Among them 30 female subjects with hypertension were included from the out-patient department of Medicine, Dhaka Medical College Hospital, Dhaka as study group (B) and 30 apparently healthy females were taken as control group (A) for comparison. Estimation of fasting serum triglycerides (TG) and total cholesterol (TC) levels were done by enzymatic method in the Department of Physiology, Dhaka Medical College Dhaka in both groups. Systolic blood pressure and diastolic blood pressure of all participants were recorded by standard method. Data were analyzed by Unpaired Student’s- test and Pearson's correlation co-efficient (r) test as applicable. The values of fasting serum TG level were significantly higher in subjects with hypertension than those of control. Fasting serum TC level was significantly higher in subjects with hypertension in comparison to controls also. In subjects with hypertension fasting serum TG and fasting serum TC levels were higher and showed positive correlation with systolic and diastolic blood pressure. Present study revealed that systolic and diastolic blood pressure has positive relationship with fasting serum triglyceride (TG) and fasting serum cholesterol (TC) levels. Objectives: This study investigated the antidiabetic and antihyperlipidemic effects of the powdered seeds of Ajwa Al-Madina in streptozotocin (STZ)-induced diabetes in rats. Besides investigating the possible underlying mechanisms. Methods: Rats were assigned to one of six groups (n = 5) as follows: normal control, vehicle control, Ajwa seeds control, diabetic control, Diabetic + Metformin, and Diabetic + Ajwa seeds. Metformin and Ajwa seeds were injected into rats orally via oral gavage 6 days/week along 4 weeks period Results: Ajwa seeds decreased fasting serum glucose, increased fasting serum insulin and decreased fasting serum triglycerides cholesterol, low-density lipoprotein, and increased fasting serum high-density lipoprotein. Besides, it upregulated insulin protein immunoexpression in the beta cells of Langerhans islets. Ajwa seed also preserved the normal histological structure of the pancreatic beta cells tissue. Conclusion: Ajwa seeds produced significant hypoglycemic and hypolipidemic effects in diabetic rats mainly through enhancement of insulin secretion. The plant is a promising adjunctive therapy in diabetes mellitus treatment.

Insulin secretion in response to glucose stimulation is reduced in endurance-trained humans. In this study, a modified hyperglycemic clamp, with a superimposed arginine infusion and fat meal, was performed on eight endurance-trained and nine untrained men to determine whether insulin secretory capacity is reduced by exercise training. Raising the plasma glucose concentration to approximately 450 mg/dl resulted in a plasma insulin response in the trained men that was approximately 64% lower than that of the untrained (peak values: 54 +/- 8 vs. 149 +/- 35 microU/ml; P less than 0.001). When a primed continuous infusion of arginine was superimposed on the hyperglycemia, the plasma insulin response was also markedly lower (66%) in the trained subjects, reaching peak values of 333 +/- 68 and 974 +/- 188 microU/ml for trained and untrained subjects, respectively (P less than 0.005). When insulin secretion was further stimulated during the arginine-infused hyperglycemia by the ingestion of a high-fat meal, peak insulin concentrations averaged 989 +/- 205 microU/ml in the trained compared with 2,232 +/- 455 microU/ml in the untrained subjects (P less than 0.01). The response of gastric inhibitory polypeptide (GIP) to the fat meal was delayed and blunted, suggesting that some enteric factor(s) other than GIP mediated the insulinotropic effect of the fat meal. The reduced plasma insulin response in trained people to the stimuli investigated suggests that regular exercise produces either several adaptations within the beta-cell or a single alteration of the beta-cell that results in an attenuation of the insulin secretory response to glucose, arginine, and fat ingestion.
