Significant number of Encephalitis patient was admitted in Faridpur Medical College Hospital (FMCH) in last years. There was an outbreak of Nipah Virus encephalitis in Faridpur in 2004. Among the 34 cases 26 patients were expired. After that, occurrence of disease is continuing. In this study a total number of 100 cases of clinically suspected encephalitis patients were studied for different clinical parameters and observed their clinical outcome. This is a retrospective study using data from hospital records. Among them 62% were male and 38% were female. Largest numbers of patients were under age group of 21-35 years, 46% of patients were from Faridpur district then 24% from Rajbari. Majority of patients were presented with neurological features (74%), 10% with pulmonary, 4% gastrointestinal, rest 12 % with combined neurological and pulmonary features. Maximum numbers (30%) of patients were attained treatment within 25 to 48 hours of onset of symptoms. Among them 78% of the patients recovered, 17% expired and 5% were referred. In this context, further prospective study is required to find out the epidemiological characteristics of Encephalitis in Faridpur region.Faridpur Med. Coll. J. Jan 2016;11(1): 20-22

Encephalitis is a relatively common clinical diagnosis of admitted patients in Faridpur Medical College Hospital. This is significant because there was an outbreak of Nipah Virus (Ni-V) encephalitis in Faridpur district in 2004 with 34 cases including 26 deaths. Recent death of an intern doctor of FMCH from Ni-V encephalitis further emphasizes the gravity of situation. In this study a total number of 100 cases of clinically suspected encephalitis patients were studied for different clinical parameters. This is a retrospective study using data from hospital records. Majority of patients presented with neurological features (85%), 6% with pulmonary, 7% with combined neurological and pulmonary, and 2% with other features. 5-18 years age group comprises the highest number 44%, followed by >18 years age group 34%. Male-female ratio was 33:17. July to November was the period of maximum number of admission, the highest being in August. Patient from all upazillas of Faridpur were admitted, the highest being from Sadar Upazilla. 48% of the patients recovered, 19% expired and 7% were referred. In this context, further prospective study is urgently required to find out the epidemiological characteristics of Nipah virus encephalitis in Faridpur region. Background: Most frequently performed gynaecological surgery is hysterectomy. The prevalence of hysterectomy varies within different regions. There are mainly two types of hysterectomies according to which part of uterus is removed. The main types of hysterectomy are total and subtotal or partial hysterectomy. The hysterectomy can be performed by three routes abdominal, laproscopic and vaginal hysterectomy. The main objective of our study is to know most common pathology and different age groups of the patients underwent hysterectomy. Materials and Methods: In this retrospective study was carried during period 6 months, at GCS medical college, hospital and research centre, Ahmedabad. Total 100 hysterectomy specimens were analyzed for histopathological lesions. We had taken the clinical and histopathological findings of these cases from the records of department of pathology, GCS medical college , hospital and research centre. Results: In our study of 100 cases, most common age group underwent hysterectomy was 40-49 years and least common age group was 20-29 years. Type of hysterectomy performed most commonly in this study was total abdominal hysterectomy with bilateral salpingoophorectomy. Most of lesions were seen in the myometrium 42 cases (42%), Endometrium 32 cases (32%), Cervix 16 cases (16%) and Ovary 10 cases (10%). Hysterectomy remains the widely used treatment modality.

AbstractLimited information is available regarding the functioning of DOTS (Directly Observed Treatment Short course) center for tuberculosis treatment under public private partnership with a medical college hospital. The present study was done to assess the various parameters related to the effectiveness of follow up treatment and its outcome in a DOTS center attached to a medical college hospital in coastal South India.Reports of patients with tuberculosis diagnosed and treated under Revised National Tuberculosis Control Programme at KS Hegde Medical College hospital DOTS center under Mangalore TB unit were reviewed retrospectively for the period from July 2004 to June 2011. Data was described in proportion and chi square test was applied to assess the association of cases with age group and sex. Among 60 patients serially registered under DOTS, 41 (68.33 %) cases were females and 19 (31.67%) cases were males. Maximum number of cases belonged to 15-29 years age group (20, 33.3%) and the difference observed between the categories of TB and age group was found to be significant (P<0.001). Overall 23 (38.33%), 12 (20%) and 25 (41.67%) cases received Cat-I, Cat-II and Cat-III treatment respectively. Success rate for new smear positive cases was 92.85% (13/14) and for Re-treatment smear positive cases was 88.89% (8/9). Sputum conversion rate among new smear positive cases (n= 14) at 2 months and 3 months was 85.71% and 92.86% respectively. Sputum conversion rate for Re-treatment cases (n=9) at 2 months and 3 months was 55.56% and 88.89% respectively. RNTCP is running successfully in this DOTS center with very low treatment failure rate and better outcome indicators, highlighting the importance of public private partnership of poorly functioning DOTS centers with medical colleges.

Background: Intestinal obstruction is a common surgical problem that surgeons face in clinical practice. Still intestinal obstruction is a major problem in medical college hospitals in Bangladesh. Causes of intestinal obstruction remain same as earlier. There is lack of statistical data in our country. So, this study will be helpful in various aspect of management of intestinal obstruction patients by providing data. Objective: To find out similarities between preoperative diagnosis with actual peroperative findings of intestinal obstruction. Materials and Methods: This cross-sectional study was conducted from April 2013 to April 2014 at Enam Medical College Hospital and Dhaka Medical College Hospital. Two hundred patients were selected by purposive sampling of diagnosed cases of intestinal obstruction. Results: Among the subjects male were 124 (62%) and female were 76 (38%). Mean age was 34.56 ± 9.13 years. The maximum respondents were from 21–30 years age group (54%) followed by 31–40 years (18%) age group. Chronic intestinal obstruction was present in 53% cases and acute intestinal obstruction in 47% cases. The clinical profile showed that all patients had abdominal pain followed by vomiting (78%). The preoperative diagnosis of intestinal obstruction was neoplastic obstruction (24%) followed by obstructed hernia (22.5%). The highest similarities (100%) were observed in case of volvulus and ileosigmoid knotting. Obstructed hernia had the highest accuracy (97.7%) followed by neoplastic obstruction (91.66%). Maximum patients (13.5%) suffered from wound infection followed by burst abdomen (3%). Conclusion: The preoperative diagnoses are very much similar to peroperative findings. The clinical parameters and other mandatory diagnostic tools commonly used in tertiary level hospitals in our country showed diagnostic accuracy in diagnosis of intestinal obstruction. Introduction : when a healthy pregnant woman with no obstetric or medical risk factors goes into spontaneous labour, she should anticipate a normal delivery. But still 580, 000 women in the world die from pregnancy and childbirth each year.1 Lack of attention to postpartum care in developing countries is neglected tragedy and requires immediate attention. This study evaluates the incidence of complications of early puerperium and also focuses the specific types of complications and to detect the risk factors associated with these complications. Aims and objectives : Main aim of the study is to evaluate the common complications in early puerperium with special attention to identify the risk factors. Materials and method : This study was carried out in Dhaka Medical College Hospital and Sir Sallimullah Medical College Hospital within the period of March ’2004 to August’ 2004. Two hundred patients were selected for this study who were delivered by caesarean section in these two institutions and complications occurring within 7 days excluding first 24 hours. Patients delivered outside the hospital and with known medical diseases were excluded from this study. Detailed history taking and appropriate clinical examination and specific investigations were done for diagnosis of complications. All relevant informations was noted in prescribed data sheet and results were tabulated and statistical analysis was done. Results : Among 200 cases only 44(22%) cases had early puerperal morbidity.Most of the patients who suffered complications underwent caesarean section for obstructed labour and fetal distress who had trial at home. Many of the patients suffered from wound infection and endometritis (40.9%). Among patients, age group -<20 years suffered from wound infection(53.8%) and endometritis(45.8%) were common in age group 21-30 years. VVF was common in patients in age <20 years.Endometritis (42.2%) was maximum in women with parity 1-3 and wound infection (50%) in women with parity 4-6. Some of the patients had more than one early puerperal morbidity. Maximum number of poor class suffered from wound infection (42.4%).Early puerperal morbidity was more prevalent in women with duration of labour <18 hours and Premature rupture of membrane had relationship with endometritis (15.9%), wound infection (13.6%) and UTI (11.4%). Conclusion : Incidence of early puerperal morbidities is still quite high (22%) and many patients experiences more than one problem at a time.Long term sequelae of early puerperal infectious morbidity should not be neglected as this may interfere quality of life of a woman. Transfusion transmitted infections (TTI) are a great concern of safety for patients. Blood transfusion is a lifesaving procedure but always caries a potential risk of infections.The present study was designed to evaluate the prevalence of infectious disease markers and frequency in males and females of various age groups among healthy blood donors. A retrospective review of donor record over a period of 2 years between 2015 to2016 was done at the department of transfusion medicine Khulna medical college Hospital, Khulna, Bangladesh.Among the total 37,266 donors,358(0.96%) donors were infected and therefore not suitable for blood donations.HBV was found to be the most frequent infection with a total frequency of 0.72% followed by TP (0.20%), HCV (0.03%) and HIV (0.01%). None of the blood samples showed positivity for malarial parasite.Majority of the seropositive donors, (47.96%) were in the age group of 26 to 35 years followed by 29.47% in 18 to 25 years’ age group. The first case of 2009 pandemic influenza A (H1N1) virus infection in Bangladesh was documented on June 18. Subsequently, persons with suspected cases of infection and contacts of those with suspected infection were tested. Individuals in whom infection was confirmed were hospitalized and quarantined, and some of them were closely observed for the purpose of investigating the nature and duration of the disease. In Dhaka Medical College Hospital, there were 28 admitted patients in flu ward and 2335 registered suspected H1N1 influenza cases attended in flue corner but 833 highly suspected cases were recorded in case record form. We describe the epidemiological characteristics of 28 indoor (flu ward) and 833 outdoor (flu corner) patients. Most of them were in between 20 to 30 years age group. Among 833 outdoor patients 611(73.34%) were male and 222(26.65%) were female. 354 (42.49%) patients were student, 195 (23.40%) were service holder. Aims: To study the demographic, clinical features, treatment as well as outcomes of tetanus patients in the Bangladeshi population from 1994 to 2003. Design: A retrospective descriptive study. Setting: A large public Medical college hospital with a regional as well as referral service. Materials and Methods: All cases of tetanus in adult patients from January 1994 to December 2003 were identified from the medical record at the hospital and these were then retrospectively reviewed. Results: A total of 80 cases of tetanus were seen at the Faridpur Medical college hospital in the 10 years period with a mean of 8 cases per year. There were 55 male (68.75%) and 25 female (31.25%) patients. The mean age of the study population was 51.7±10.08 years. Most cases occurred in the age group of 60-69 years. Risk factor analysis revealed identifiable acute injury (puncture, prick/laceration) in 85%, CSOM (2.5%), surgery (2.5%), abortion (2.5%), skin ulcer (1.25%), burn (1.25%), child birth (1.25%), boil (1.25%). Thirty-two (40%) patients had medical wound care before hospital admission but none received tetanus immunoglobulin despite the absence of tetanus immunity. All the patients had the generalized type of disease. Body stiffness, trismus and dysphagia were the three commonest presenting complaints. All the patients with injury and wound (74/80) had their wound debrided. All the tetanus patients (80/80) received intravenous diazepam infusion as part of their management. Seventy two (90%) patients received intravenous crystalline penicillin. Sixty-five (81.25%) patients received intramuscular human antitetanus immunoglobulin and fourteen (17.5%) had tracheostomy performed. Inhospital complications were observed as respiratory (80%), cardiovascular (65%), gastrointestinal (57-5%), renal (33.37%), neurological (17.%) and others (22.5%). There were eighteen deaths in hospital, accounting for overall mortality of 22.5% (18/80). Higher mortality were observed in age group above 50 years than the age group below 50 years (29.16% vs 12.5%, P<0.05), in female than the male (40% vs 14.54%, P<0.05), in farmer than nonfarmer (30.95% vs 13.15%, P<0.05), in patients who had short incubation period than those who had incubation period more than one week (53.33% vs 23.25%, P<0.05). Conclusion: In general, tetanus remains in Bangladesh an important disease with substantial mortality that primarily affects unvaccinated or inadequately vaccinated individuals. Prevention during wound management of tetanus prone wounds was inappropriate in many patients. The elderly population may have the highest risk for tetanus since they may not have had tetanus toxoid immunization or regular booster injections. It is, however, highly preventable through both routine vaccination and appropriate wound management. Our case series show comparable pattern and outcome with other case series in the developing countries reported in the literatures. (J Bangladesh Coll Phys Surg 2007; 25 : 62-69)

This study was carried out among the blood donors attending the Blood TransfusionDepartment of Khulna Medical College Hospital. Both ABO and Rhesus (Rh) blood grouping wasdone to see the pattern of ABO and Rh blood group among them. Maximum blood donors werebetween age group 18-37 years. Among the donors maximum were male 12105 (94.73%). Male:female ratio was 18:1. Among them B+ve was found 4286 (33.54%), B-ve was 127 (5.0%), O+vewas 4091 (32.02%), O-ve was 131 (1.03%), A+ve was 3014 (23.59%), A-ve was 74 (0.58%), AB+vewas 1019 (7.98%), AB-ve was 37 (0.29%). Background: The objective was to study about the drugs used in management of diabetic foot ulcer (DFU) and to grade the wounds using Wagner’s scale.Methods: An observational study was conducted at Department of Pharmacy, Rajah Muthiah Medical College Hospital over a period of six months from November 2018 to April 2019. The data was collected from 85 patients using data collection form. The patients were selected based on inclusion and exclusion criteria.Results: Overall 85 patients were enrolled in this study. The prevalence of DFU found to be 36% more in males 58 (68%) than in females 27 (32%). Considering the age group, higher prevalence was reported in patients among the age group of 60-70. This study shows, patients were found commonly to have habits of alcohol 18 (21%), smoking 11 (13%) and both 22 (26%). Overall, 23 classes of drugs were used in this study. Among these, mostly were antibiotics 12 (52%) then hypoglycemic drugs 5 (21%) and others are analgesics (9%), anti-ulcerant and vitamins. This study shows that the common antibiotics prescribed are metronidazole 39 (24%), cefotaxime 36 (22.7%), ciprofloxacin 25 (15.8%) and piperacillin 21 (13.2%). The common hypoglycemic drug used were of insulin 63 (74%), metformin 43 (77%), glimepiride 12 (14%) and metformin and glimepiride 12 (14%). By using Wagner’s scale, out of 85 patients, most of the ulcer were predominantly between grade IV and V.Conclusions: Targeting range of glycaemic levels and proper antibiotics is the best way of treating DFU. This study has provided the base line data regarding management of DFU which helps to improve therapeutic outcome.
