Introduction : when a healthy pregnant woman with no obstetric or medical risk factors goes into spontaneous labour, she should anticipate a normal delivery. But still 580, 000 women in the world die from pregnancy and childbirth each year.1 Lack of attention to postpartum care in developing countries is neglected tragedy and requires immediate attention. This study evaluates the incidence of complications of early puerperium and also focuses the specific types of complications and to detect the risk factors associated with these complications. Aims and objectives : Main aim of the study is to evaluate the common complications in early puerperium with special attention to identify the risk factors. Materials and method : This study was carried out in Dhaka Medical College Hospital and Sir Sallimullah Medical College Hospital within the period of March ’2004 to August’ 2004. Two hundred patients were selected for this study who were delivered by caesarean section in these two institutions and complications occurring within 7 days excluding first 24 hours. Patients delivered outside the hospital and with known medical diseases were excluded from this study. Detailed history taking and appropriate clinical examination and specific investigations were done for diagnosis of complications. All relevant informations was noted in prescribed data sheet and results were tabulated and statistical analysis was done. Results : Among 200 cases only 44(22%) cases had early puerperal morbidity.Most of the patients who suffered complications underwent caesarean section for obstructed labour and fetal distress who had trial at home. Many of the patients suffered from wound infection and endometritis (40.9%). Among patients, age group -<20 years suffered from wound infection(53.8%) and endometritis(45.8%) were common in age group 21-30 years. VVF was common in patients in age <20 years.Endometritis (42.2%) was maximum in women with parity 1-3 and wound infection (50%) in women with parity 4-6. Some of the patients had more than one early puerperal morbidity. Maximum number of poor class suffered from wound infection (42.4%).Early puerperal morbidity was more prevalent in women with duration of labour <18 hours and Premature rupture of membrane had relationship with endometritis (15.9%), wound infection (13.6%) and UTI (11.4%). Conclusion : Incidence of early puerperal morbidities is still quite high (22%) and many patients experiences more than one problem at a time.Long term sequelae of early puerperal infectious morbidity should not be neglected as this may interfere quality of life of a woman. To find out the common indications of caesarean section and complications of this operation, both for mother and baby. This observational study was carried out on 100 cases of caesarean sections, who were operated in the Department of Obstetrics & Gynaecology, Tairunnessa Memorial Medical College & Hospital. Previous caesarean section and oligohydroamnios with fetal distress contribute the major causes of caesarean section. Contribute the major causes of caesarean section. Common complications are UTI, wound infection. Indications of caesarean section should be properly identified to decrease the unnecessary operation, which ultimately decrease indication of previous caesarean section. Maintenance of proper asepsis would avoid the complications of this operation.Medicine Today 2016 Vol.28(2): 66-68

Background: Most frequently performed gynaecological surgery is hysterectomy. The prevalence of hysterectomy varies within different regions. There are mainly two types of hysterectomies according to which part of uterus is removed. The main types of hysterectomy are total and subtotal or partial hysterectomy. The hysterectomy can be performed by three routes abdominal, laproscopic and vaginal hysterectomy. The main objective of our study is to know most common pathology and different age groups of the patients underwent hysterectomy. Materials and Methods: In this retrospective study was carried during period 6 months, at GCS medical college, hospital and research centre, Ahmedabad. Total 100 hysterectomy specimens were analyzed for histopathological lesions. We had taken the clinical and histopathological findings of these cases from the records of department of pathology, GCS medical college , hospital and research centre. Results: In our study of 100 cases, most common age group underwent hysterectomy was 40-49 years and least common age group was 20-29 years. Type of hysterectomy performed most commonly in this study was total abdominal hysterectomy with bilateral salpingoophorectomy. Most of lesions were seen in the myometrium 42 cases (42%), Endometrium 32 cases (32%), Cervix 16 cases (16%) and Ovary 10 cases (10%). Hysterectomy remains the widely used treatment modality.

AbstractLimited information is available regarding the functioning of DOTS (Directly Observed Treatment Short course) center for tuberculosis treatment under public private partnership with a medical college hospital. The present study was done to assess the various parameters related to the effectiveness of follow up treatment and its outcome in a DOTS center attached to a medical college hospital in coastal South India.Reports of patients with tuberculosis diagnosed and treated under Revised National Tuberculosis Control Programme at KS Hegde Medical College hospital DOTS center under Mangalore TB unit were reviewed retrospectively for the period from July 2004 to June 2011. Data was described in proportion and chi square test was applied to assess the association of cases with age group and sex. Among 60 patients serially registered under DOTS, 41 (68.33 %) cases were females and 19 (31.67%) cases were males. Maximum number of cases belonged to 15-29 years age group (20, 33.3%) and the difference observed between the categories of TB and age group was found to be significant (P<0.001). Overall 23 (38.33%), 12 (20%) and 25 (41.67%) cases received Cat-I, Cat-II and Cat-III treatment respectively. Success rate for new smear positive cases was 92.85% (13/14) and for Re-treatment smear positive cases was 88.89% (8/9). Sputum conversion rate among new smear positive cases (n= 14) at 2 months and 3 months was 85.71% and 92.86% respectively. Sputum conversion rate for Re-treatment cases (n=9) at 2 months and 3 months was 55.56% and 88.89% respectively. RNTCP is running successfully in this DOTS center with very low treatment failure rate and better outcome indicators, highlighting the importance of public private partnership of poorly functioning DOTS centers with medical colleges.

Background: Wound infections following caesarean section (or) gynaecologic surgery is a common complication that accounts for significant extension of hospital stays.Methods: This is a prospective analytical study, conducted in the department of obstetrics and gynaecology, kilpauk medical college, Chennai from Januray 2013 to November 2013. All cases who underwent ceaserean section were included in the study. Analysis was done in the form of percentages and proportions and represented as tables where necessary.Results: A total of 700 cases were analysed. The incidence of wound infections after caesarean section in this study was 8.14%. Study confirms that risk factors like anemia, chorioamnionitis; PROM, obstructed labour, preeclampsia, increased surgical time, increase BMI poses risk for wound complications. Whereas chorioamnionitis (66%) and obstructed labour (66%) showed increased risk for wound complications.Conclusions: Correcting malnutrion, anemia, stabilizing diabetes and eradicating all infection such as urinary tract infection, proper preparation of skin, proper surgeons scrubbing and using proper surgical technique can decrease the risk of postoperative wound infection.

Background: Intestinal obstruction is a common surgical problem that surgeons face in clinical practice. Still intestinal obstruction is a major problem in medical college hospitals in Bangladesh. Causes of intestinal obstruction remain same as earlier. There is lack of statistical data in our country. So, this study will be helpful in various aspect of management of intestinal obstruction patients by providing data. Objective: To find out similarities between preoperative diagnosis with actual peroperative findings of intestinal obstruction. Materials and Methods: This cross-sectional study was conducted from April 2013 to April 2014 at Enam Medical College Hospital and Dhaka Medical College Hospital. Two hundred patients were selected by purposive sampling of diagnosed cases of intestinal obstruction. Results: Among the subjects male were 124 (62%) and female were 76 (38%). Mean age was 34.56 ± 9.13 years. The maximum respondents were from 21–30 years age group (54%) followed by 31–40 years (18%) age group. Chronic intestinal obstruction was present in 53% cases and acute intestinal obstruction in 47% cases. The clinical profile showed that all patients had abdominal pain followed by vomiting (78%). The preoperative diagnosis of intestinal obstruction was neoplastic obstruction (24%) followed by obstructed hernia (22.5%). The highest similarities (100%) were observed in case of volvulus and ileosigmoid knotting. Obstructed hernia had the highest accuracy (97.7%) followed by neoplastic obstruction (91.66%). Maximum patients (13.5%) suffered from wound infection followed by burst abdomen (3%). Conclusion: The preoperative diagnoses are very much similar to peroperative findings. The clinical parameters and other mandatory diagnostic tools commonly used in tertiary level hospitals in our country showed diagnostic accuracy in diagnosis of intestinal obstruction. Transfusion transmitted infections (TTI) are a great concern of safety for patients. Blood transfusion is a lifesaving procedure but always caries a potential risk of infections.The present study was designed to evaluate the prevalence of infectious disease markers and frequency in males and females of various age groups among healthy blood donors. A retrospective review of donor record over a period of 2 years between 2015 to2016 was done at the department of transfusion medicine Khulna medical college Hospital, Khulna, Bangladesh.Among the total 37,266 donors,358(0.96%) donors were infected and therefore not suitable for blood donations.HBV was found to be the most frequent infection with a total frequency of 0.72% followed by TP (0.20%), HCV (0.03%) and HIV (0.01%). None of the blood samples showed positivity for malarial parasite.Majority of the seropositive donors, (47.96%) were in the age group of 26 to 35 years followed by 29.47% in 18 to 25 years’ age group. Background: Postdated pregnancy complicates about 10% of all gestation and is associated with an increase in perinatal mortality and morbidity. This study was designed to observe fetal as well as maternal outcome in the management of postdated pregnancies admitted into Rajshahi Medical College Hospital. Methods: The study was carried out from July 2008 to June 2010. 100 cases of postdated pregnancies were included. Patients’ age, occupation, parity, mode of delivery, the indication of LUCS, fetal & maternal outcomes were recorded in a predesigned questionnaire. Results were expressed as Mean± SD, actual number and percentage of total where applicable. Results: Mean age (±SD) was 21.3 (±5.32) years. 90% were housewives, 5% were in service and 5% were in other occupations. 53% were nullipara, 21% para-2, 18% were para-3, 8% were para-4.57% of the study patients underwent caesarean section and 43% underwent vaginal delivery. Fetal distress (n=34), non-progression of labour (n=19), cord prolapse (n=1) & CPD (n=3) were the causes of LUCS. Regarding fetal outcome, healthy babies were 76%, meconium aspiration 13%, birth asphyxia 11%. Among birth asphyxia, neonatal death was 1% & still born was 2%. Maternal complications at delivery were tear in the genital tract 8%, PPH 5% & wound infection in 4% cases. Conclusion: Here, still-birth rate was 2% (normally about 1 in 3000 deliveries) and perinatal mortality rate is 3% (normally about 2-3 in 1000 deliveries). So, patients should be encouraged to attend regular antenatal check up to prevent postdated pregnancy and its complications. TAJ 2018; 31(2): 50-53
