Aim and objectives: The aim of this study was to evaluate the spectrum of breast diseases in the children and adolescents in a single hospital and to evaluate the presentation and outcome of these diseases. Methods: This prospective study of childhood and adolescents (2-19years) breast diseases conducted on 47 patients over a period from January 2009 to December 2011. A total of 47 (forty seven) children and adolescents (<19yrs) who complained of a breast lump or pain or nipple symptoms were included for the study. History, clinical examination along with breast ultrasound imaging and cytology and/ or biopsy in some cases was done for diagnosis. Patients were categorized according to their clinical properties and inflammatory, benign and malignant cases were done necessary operations. Other lesions was on medical treatment and under observation and follow-up. Results: The commonest complain was lump followed by pain. Most patients present in the age group 16-19 years (87%). The commonest breast lesion was fibroadenoma(36%). Most of them occur in 16-19 years age group (29.7%). Fibrocystic disease (8.4%) and breast abscess(8.4%) are next 2 common diseases. Diagnostic approach is mostly conservative with ultrasonogram. Few cases need FNAC and excision biopsy. Secondary malignancy may be a possible diagnosis. Background: Different pathology of breast is found among the women. Objective: The purpose of the present study was to see the profiles of breast disease among women seeking for treatment during Covid-19 pandemic at the OPD of medical university of Bangladesh. Methodology: This descriptive cross-sectional study was conducted in the Department of Surgery at Bangabandhu Shiekh Mujib Medical University, Dhaka, Bangladesh from 1 April to 31 October 2020 for a period of six months. All the women who were attended at the OPD of the Department of Surgery with the different complaints of breast at any age were selected as study population. The different disease profiles were recorded in the data sheet. Result: A total number of 1625 women were recruited for this study. The mean age with standard deviation of the study population was 36.4±12.54 years. Among 1492 women the most common breast complaints was mastalgia which was 492(29.9%) cases followed by fibrocystic disease, breast lump, fibroadenoma and carcinoma breast which was 296(18.0%) cases, 202(12.3%) cases, 141(8.6%) cases and 84(5.1%) cases respectively. Similarly duct ectasia, fibroadenoma, fibrocystic disease and lipoma were also most common among the less than or equal to 45 years age group of women which were 47(94.0%) cases, 137(97.2%) cases, 269(90.9%) cases and 75(51.4%) cases respectively. Conclusion: In conclusion breast lump and fibrocystic disease are the most common diseases among the women attending during covid19 era. Breast lumps have diverse causes including benign as well as malignant lesions. Fibroadenoma (FA) is a common cause of breast lump. Complex fibroadenomas, a particular subtype, pose diagnostic dilemmas due to confusing the radiological findings and hold higher incidence of transformation to malignancy. A 49 year old female presented with a painless lump in right breast for 2 years which appeared calcified on radiology. Excision biopsy was performed which grossly revealed an encapsulated grey-white firm to hard with cystic areas. Microscopically the sections showed a well encapsulated fibroepithelial tumor showing areas of sclerosing adenosis, hyalinization, cysts lined by cuboidal cells with eosinophilic secretions, areas of calcification and ossification. No evidence of lobular hyperplasia or in-situ carcinoma was seen. Patient was advised follow up. This case represents the unusual occurrence of complex fibroadenoma in a middle-aged female showing with ossification.

Background: Breast diseases, among the most common diseases in females occur in various pat­terns from benign to malignant. Their identification is crucial as some of the benign disorders pose threat to turn into malignancy. This study was aimed to determine the spectrum of breast diseases. Methods: This was a hospital based retrospective study among all the patients in General Surgery OPD of Karnali Academy of Health Sciences, Teaching Hospital, diagnosed with breast diseases during one year period from August 2019 to July 2020 AD. Data were extracted and entered in SPSS version 16 for further analysis. Results: Out of 110 cases, only 4 (3.96%) were male. Almost half 54 (49.09%) of the cases were in the age group of 21-30 years. Fibrocystic disease, Breast Abscess and fibroadenoma, and cracked nipple were 39 (35.45%), 38 (34.54%, 15 (13.63%) and 8 (7.28%) respectively. The benign diseases were seen mostly in the younger population while malignant breast diseases were seen in the older population. Mastalgia was mostly seen in the twenties (17.3%) and the thirties (10%) while Breast abscess was commonly encountered in the twenties (16.4%) and the teens (11.8%). The gynaecomastia were seen only in four males. Conclusions: Fibrocystic breast diseases, Mastitis with or without abscess, fibroadenoma and cracked nipples were the first four most common breast diseases in females. The younger popula­tion had predilection towards the benign diseases which occurred most frequently in the third decade while the malignant occurred in sixth and seventh decade.

Eight cases of hypoparathynoidism in patients less than 17 years of age have been presented and discussed. One case was primary in type and seven were secondary to thyroidectomies performed for thyrotoxicosis or carcinoma of the thyroid gland. The similarity of symptomatology, time of onset, treatment and long-term management between cases of hypoparathyroidism in the younger age group and in adults has been emphasized. Prophylaxis against the development of hypoparathyroidism following thyroid surgeny is the responsibility of the surgeon. When present, the disorder must be recognized early and treated promptly with adequate dosages of appropriate drugs. Conscientious long-term clinical and laboratory follow-up examinations are necessary to avoid the serious complications of chronic hypocalcemia. Tests to quantitate more accurately degrees of parathyroid function are needed in secondary hypopanathyroidsm which is thought to be temporary in nature. Children and adolescents with hypopanathyroidism deserve special care in diagnosis, treatment and long-term management.

This prospective study was conducted in the department of pediatric surgery, BIRDEM General Hospital and other Private Hospital in Dhakato evaluate the morphological parameters after orchiopexy in Undescended testis (UDT). Fifty five children of cryptorchidism in the age group of 6 months to12 years were included in the study from September 2012 to March 2017. Patients' clinical characteristics, age at the time of orchiopexy, pre-operative ultrasonogram finding and intraoperative findings were recorded. On follow up postoperative complications and scrotal ultrasonogram findings were also recorded. Total 45 patients received regular ultrasound follow-up in next 6 months. Testicular length, width, position of the testes and any abnormal findings were documented. The testicular volume was then calculated with Hansen formula:Testicular volume= length (L) x width (W)2 x 0.52. The mean age at operation was 4.15 years. Sixty eightpercent of undescended testes were palpable, 97% of which could be initially placed in the scrotal position by surgery. Ninety threepercent remained in the scrotum at 6-month follow up. In 32% of cases, the testes were impalpable, 47% were intra-abdominal and 15.7% were absent. Eighty seven percent of all impalpable testes could be placed in the scrotum. At 6 months follow up, only 69% were in the scrotal position. Eighteen percent of impalpable testes and 2.4% of palpable testes underwent atrophy. The volume of scrotalized testis increased significantly after orchiopexy though the volume of undescended testis was smaller than that of normal descended testis in all age subgroups and revealed a slow growing trend.Medicine Today 2017 Vol.29(1): 35-38

Background: Breast lump is one of the most common prevalent complaint of patients attending breast clinics. Objective: To determine if there is any change in the pattern of common breast, diseases presenting as breast lumps between pregnant and non-pregnant women among patients attending Al-Elwiya Breast Clinic. Methods: This is a cross – sectional study, with convent's patient sampling setting in AL-Elwiya Breast Cancer Early Detection Clinic from 1st Feb. to 1st May 2018, we collected data from patients with breast lumps including the age groups, pregnancy status, parity status, previous breast diseases, hormonal drugs, menstrual cycle, breast feeding. Breast lump was examined, sonography and needle biopsy, and histopathology. Results:  This study involves 306 married patients, fibro adenoma was the commonest pathology in the age group 20-29 year-old (29.9%) followed by Aberration of Normal Development and Involution which was found mainly in 30-39-year-old age group (36.1%). Carcinoma found mainly in the >40-year-old age group (10.3%). Aberration of Normal Development and Involution was the main pathology in non-pregnant patients (36.1%), in pregnant patients fibro adenoma is the commonest (35.7%). Conclusion: The majority of breast lumps are benign. In pregnant women, fibro adenoma is the commonest, while in non-pregnant women, Aberration of Normal Development and Involution is the main cause of breast lumps

Background: The pathogenesis involves disturbance in the breast physiology extending from an extreme normality to well defined disease processes. The clinician must provide a degree of diagnostic accuracy while at same time ensuring that an excessive biopsy rate is prevented. The aim of this study was to evaluate the benign breast lumps in different age groups and to determine the prevalence of benign breast lumps among the breast symptomatic.Methods: This clinico-pathological cross sectional study was conducted with a sample size of 100. The study was conducted during the period from July 2018 to June 2019 which included all patients presenting with benign breast lump during the study period.Results: Out of 100 studied cases of benign breast diseases, the most common benign breast tumor found as fibro adenoma and majority of the fibro adenoma found in the 2nd and 3rd decade of life. Right breast and upper outer quadrant were most commonly affected. Majority of benign breast lesions presented with painless lump in the breast and most of them were from rural background. Majority of benign Breast lumps (45%) cases were found in the age group of 21-30 years. Majority of the patients (60%) attain menarche between ages 13-15 years. Majority of cases (55%) were belonged to lower socio economic status.Conclusions: Our study indicates that FNAC is diagnostically more accurate, safe, cost effective and OPD procedure, however when FNAC was inconclusive, biopsy is the best choice for breast tumors.

Background/aims Treatments for Graves' disease (GD) in children and adolescents include oral antithyroid drugs (ATDs), near total thyroidectomy, and radioactive iodine (RAI). ATDs remain the preferred choice in this age group, but because persistent remission occurs in 30% of cases, RAI is becoming a common option for definitive therapy. Methods We performed a review of 65 medical records of GD patients under age 19 years who were followed between 1985 and 2005. Results The prevalence of GD was higher in females (3:1) and during puberty (for both genders). If no remission was detected during ATD treatment, RAI was indicated when the following criteria were present: non-compliance, relapse, or side effects that were related to ATDs, large goiter, and long-term use of ATDs. The majority of patients developed hypothyroidism within 6 months after RAI. A progressive higher dose regimen was implemented in the last 10 years of the study period. A second RAI dose was necessary in eight cases. During the follow-up period, three pregnancies occurred. One patient with a thyroid nodule and benign cytology was detected. Conclusions RAI therapy is effective and safe in the treatment of GD in children and adolescents.

Objective: To report a case of a 12-year-old boy with Necrotizing Lymphadenitis (Kikuchi-Fujimoto disease) presenting as lymphadenitis secondary to multiple diagnosis.   Methods: Design : Case Report Setting: Tertiary Private Hospital Patient: One   Results: A 12-year-old boy consulted for two-month history of cervical lymphadenopathy with the underlying cause remaining unclear despite multiple consults, diagnosis and medical treatment. Lymphoma was considered and excision biopsy with further investigations confirmed a diagnosis of Kikuchi-Fujimoto disease. Supportive management was given with no recurrence of symptoms noted on 18 months of follow up.    Conclusion:  Kikuchi-Fujimoto disease in this case was a diagnosis of exclusion. Even with a proper history and physical examination, experts in otolaryngology can be misled to manage this case as malignant. Awareness of the disease and appropriate examinations including immunohistochemistry are important for a timely diagnosis and proper intervention.   Keywords: lympadenitis; Kikuchi-Fujimoto disease; lymphoma; cervical lymph nodes

Introduction – FNAC has become increasingly popular in diagnosis of palpable breast masses. As it is sensitive, specific, economical and safe for screening of breast lesions Aims and objectives – to determine the histopathologic correlation of FNAC of breast lumps seen in department of pathology, Govt medical college, siddipet. Methods – 2 yrs. retrospective study if breast lumps , with all patients who had FNAC, with subsequent histopathological confirmation over the period July 2018 –July 2020. A total 232 cases included in which 187 cases are subjected to excision biopsy or mastectomy Results –2 yrs. under review (2018-2020) a total of 232 FNAC of breast lump were done. 197 (84.9%) benign cases, 13(11.6%) malignant cases, 8 (3.5%) inflammatory cases were diagnosed cytologically in which 98(42.24%) cases are fibro adenoma, 27(11.6%) are malignant cases and suspicious of malignancy, 26(11.2%) of gynecomastia, 23(9.91%) of fibrocystic disease with female predominance of 8:1 ratio (206:26). Age ranging from 9 yrs. – 85 yrs. with mean age of 47yrs ± 14.2 yrs. Conclusion – we therefore concluded that FNAC is a reliable diagnostic tool of breast lump in our hospital. FNAC guides the surgeons for further plan of management.
