Public-Private Partnership as a Tool of Health Infrastructure Development in Digital Economy

The article is devoted to analyze the digitalization process in the development of public-private partnership mechanism in informational healthcare infrastructure. The importance of information asymmetry problem is shown in the health insurance market, and the mechanisms that can minimize it are analyzed: public-private partnership and concession. Based on the analysis of report “The Development of Digital Economy in Russia”, the importance of the digitalization process was identified for the social sphere, and the main directions were outlined for the development of the digital economy in Russia. Aim of the article is to analyze the features of public private partnership mechanism in healthcare related to the process of digitalization of the economy. The object of study is the public-private partnership mechanism. Subject of the study include economical, organizational and social relations arising in the process of applying the PPP mechanism for the development of digitalization. The analysis method made it possible to comprehend the main trends of the digitalization process in relation to the healthcare sector. The method of synthesis helped to find the relationship between these trends. Comparative analysis allowed comparing two key models of health insurance. The article allows understanding the key trends of the digitalization process in health care. Аs a result, it will be useful for experts involved in the development of healthcare infrastructure based on the PPP mechanism.

The article shows the importance of using the public-private partnership mechanism for the development of the social sphere of the village. Review of PPP implementation by countries G20 showed different legal approaches in PPP implementation, parallel use and concessions, and agreements, application of guaranteed minimum income and risk sharing among participants, use of different PPP support measures. The presented results of the questionnaire indicate the largest use of PPPs in housing and communal services — 272 projects (81.68 %), which is due to the high degree of wear and tear in water and heat supply, in education — 22 projects (6.61 %), mainly the construction of preschool institutions, school repairs, in the field of sports and tourism 14 projects (4.2 %) — the construction of sports grounds and stadiums, sports centers, 10 projects (3 %). Public-private partnership projects in the social sphere of rural areas are less marginal, their financing is associated with significant difficulties due to the heterogeneity of demand, the difficulty of forecasting consumer demand and assessing social effect. The development of public-private partnerships is constrained by the lack of necessary budget funds to ensure the minimum guaranteed income of a private partner, the duration and complexity of maintaining procedures.

The article explores the issues of implementing public-private partnership projects in the social security system of the Kursk region. The relevance of this topic is due to the fact that public-private partnership is becoming an increasingly popular way of solving the problems of the development of a socially significant sphere in the conditions of a shortage of budget resources, where it is impossible to fulfill the set goals without attracting investments and the potential of the institution of entrepreneurship. In Russia, the relevance of public-private partnership in the social sphere is due to the low level of its development in terms of applying advanced management methods. This sector, which for many decades was the object of direct government, is experiencing today the greatest need for various resources for the implementation of the set tasks of socio-economic development. The article presents theoretical, methodological and practical approaches to reflecting the development of public-private partnership in the social security system of the region. The system of indicators that characterizes the quantitative and qualitative aspects of the social sphere of the region is analyzed. In the course of the study, specifics of the implementation of public-private partnership projects at the regional level were revealed. The analysis of the dynamics of key indicators of the social security development in the region was carried out. With the financial capacity of the public sector reduced and the private sector has the potential to effectively implement innovative technologies to solve socially important tasks to modernize the social services infrastructure aimed at improving the quality and accessibility of socially important services, it requires a combination of opportunities and efforts of the public and private sectors. The article provides a comprehensive analysis of the main indicators of investment projects of public-private partnerships, suggests approaches to increase the efficiency of implementation of these projects in the Kursk region.

This study aims to formulate recommendations and opportunities for potential infrastructure development in the social sector by using KPBU scheme. The direct beneficiaries of this activity are the Deputy of Investment Planning as a unit that is directly responsible for formulating the proposed establishment of the Act related KPBU in the field of infrastructure investment. The output of this activity is also expected to be useful for investors in infrastructure and related stakeholders. Model cooperation Public Private Partnership in social infrastructure should take into account the uniqueness of each sector, particularly the constraints, risks and macro conditions, including fiscal policy and commitment to the objective of each sector. So that needs to be made cooperation mechanism Public Private Partnership in the field of social infrastructure by making adjustments to the conditions and limitations in the field of social infrastructure. The article outlines the issues of social infrastructure development in Ukraine. The need of addressing these issues through implementing public-private partnership projects is justified. Some priority areas for introducing public-private partnership in Ukraine are identified. The best foreign practices of attracting private sector to solving problems of social infrastructure development are considered. It is revealed that priority areas where public-private partnerships operate are dependent on the level of socio-economic development of the country. Some practices of public-private partnership projects in education in the UK, Australia, Germany, and Egypt are analyzed in detail and their common characteristics are systematized. Based on the analysis of best foreign practices applied in implementing public-private partnership projects in healthcare, the following key measures are formulated: direct provision of medical services, management of medical assets, development and production of pharmaceuticals, improvement of access to medical services and products. The performance of public-private partnership projects in the healthcare sector in the UK, France, Australia and Sweden is analyzed in detail. The paper describes both more and less successful examples of publicprivate partnership in this sector. The article defines the following reasons for low-level implementation of public-private partnership projects in healthcare: schedule delays in construction, operation-cost overruns, poor hospital and ward layout, use of low-cost medical equipment which requires regular renewal. The research also considers the best foreign practices of introducing public-private partnership projects in the field of culture. Some characteristic features of public-private partnership projects in social services in foreign countries are identified. The national practices of introducing public-private partnership projects are highlighted and priority areas of their operation are singled out. It is found out that the implementation of public-private partnership projects in social services is not popular among domestic investors. Some individual projects of public-private partnership in social services are considered. A particular attention is paid to both more and less successful ones. The main problems of the implementation of public-private partnership projects in the national education, healthcare, culture are outlined and ways for their solution are proposed.

Using the mechanism of public-private partnership (PPP) is an effective way to attract investment for the modernization of urban infrastructure in the face of budget shortfalls, which is especially important for the sustainable development of the social infrastructure of the territories. The aim of the study is to analyze the models of public-private partnership (PPP) and substantiate the choice of the model to ensure sustainable development of the social sphere. The life-cycle cost (LLC) model is represented as the most effective one for realizing municipal facilities development projects.

Public-private partnership acts as a form of effective interaction between the public partner and private investors in order to solve various social problems. The social sphere, being mainly the object of direct public administration and financing, is experiencing significant needs in resources, the introduction of modern management methods and advanced technologies. Budget funds do not allow to fully and in a short period to solve these problems, so it becomes necessary to attract directly the competencies and investments of the private sector. Education and health are at the center of the social sectors. The relevance of the use of public-private partnership in the social sphere is due to the fact that in a relatively short period of time it is possible to eliminate gaps in the development of social infrastructure by combining efforts, funds, risks of two economic entities – the state and business. The study of foreign experience in the implementation of socially significant projects using public-private partnership tools is necessary to assess the possibility of using similar models in Russia. Interaction between the state authorities and the private sector in terms of pooling resources and potentials within this task will actively attract investment, efficiently and timely implement social projects, improve the quality of social services to the population, increase the level of management of social infrastructure, promote the development of public institutions of the social sphere. At the same time, the public-private partnership will help to use effectively financial resources, experience and professionalism of the private sector while maintaining state control over social assets. Based on the analysis of the uncovered need for resources for the accelerated development of the social sphere, the author of the article proposes: the launch of new mechanisms of financial support from the Federal budget and the increase in budget funding for the purpose of providing “budget leverage” in public-private partnership projects; improvement of legislation in the field of public-private partnership to eliminate “barriers” and simplify the procedure for launching public-private partnership projects in education and health care; updating planning documents (changing existing documents of state strategic planning), including with a view to change priorities in infrastructure development and the mechanisms used for this.
