Enteroviral infection in Ural Federal District and West Siberia: results of epidemiologic observation involving molecular-genetic methods

We conducted a comparative analysis of the parenteral viral hepatitis B and C (HBV and HCV) prevalence and their molecular genetic characteristics among prisoners of persons diagnosed with HIV infection (41 samples), HIV-positive free citizens (187 samples) and «conditionally healthy population» with the lack of information about the presence of a diagnosis of chronic viral hepatitis of any etiology and HIV infection (231 samples). Immunological and molecular biological research methods were used. Obtained data analysis showed that the prevalence of infection markers with viruses of parenteral hepatitis was significantly higher in the groups of HIV-positive individuals (imprisoned and freemen). The HBsAg-negative form of the disease was determined among the HIV-positive free population and in the «conditionally healthy population». Over the past 10 years (2009–2018), the proportion of HIV-positive prisoners in custody of people with HCV monoinfection doubled, HBV was increased in 8.7 times. Significant decrease in the combined infection of HBV and HCV of this contingent was found. Given this decrease in the penitentiary system in HIV-positive individuals, the overall burden of HBV infection (both in mono form and coinfection with HCV) significantly (5.3 times) decreased , which can be attributed to successful widespread vaccination against hepatitis B in Russia. The most common HCV genotypes among HIV-positive individuals were 1b and 3a, genotypic structure of HBV prevailed genotype D.

The Vologda Region is characterized by a relatively calm epidemic situation for tuberculosis in Russia: the incidence rate in 2010—2018 is decreased from 45.2 to 15.8 per 100 thousand of the population (44.4 in Russia). However, the proportion of patients with multiple drug resistance (MDR) of the pathogen increased from 12.1% in 2016 to 23.7% in 2018. The aim of the study was to characterize the genetic structure of the M. tuberculosis population and identify the main genotypes associated with the primary multidrug resistance of the pathogen in the Vologda Region. A total of 82 strains of M. tuberculosis isolated in 2018 from newly diagnosed tuberculosis patients were studied. Drug susceptibility testing was performed using the standard method of absolute concentration and BACTEC MGIT 960 kit. M. tuberculosis strains were assigned to the Beijing genotype and its main subtypes based on the analysis of specific markers. The Beijing strains were subtyped by the MIRU-VNTR method (24 standard loci), calculating the Hunter-Gaston Discriminatory Index (HGDI). Other strains of the non-Beijing group were spoligotyped. The majority of the strains were of the Beijing genotype (62.2%; 51 of 82). The most numerous cluster was Central Asian/Russian (41.5%; 34 of 82 strains). The shares of the Central Asia Outbreak (CAO) subtype and cluster B0/W148 amounted to 8.5% and 7.3%, respectively. The non-Beijing strains belonged to the genetic families T (11%; 9 of 82), LAM (11%), Haarlem (6.1%), and Ural (4.9%). Among 82 M. tuberculosis isolates, 33 (40.2%) MDR strains were identified, counting 27 of the Beijing genotype, including those of the Central Asian/Russian — 18 (66.7%), B0/W148 and CAO — 4 each (14.8%) clusters. MIRU-VNTR typing of 51 Beijing strains revealed 22 profiles (HGDI = 0.852); the largest clusters were 94-32 (35.3%) and 95-32 (15.7%), which included strains Central Asian/Russian and CAO. Four strains of genotype B0/W148 belonged to cluster 100-32. The loci QUB26 (HGDI = 0.493) and MIRU26 (HGDI = 0.388) had the highest polymorphism. For the first time, a molecular genetic study carried out in the Vologda region revealed the heterogeneity of the M. tuberculosis population with strains of the Beijing genotype dominated. At the same time, the share of the associated with MDR, epidemiologically and clinically significant cluster Beijing B0/W148, well defined in Russia and abroad, was only 7.3%, which is significantly less than in other regions of the Northwestern Federal District of the Russian Federation (~19%). Concurrent, representatives of the Central Asian/Russian cluster of the Beijing genotype prevailed in the structure of genotypes and among MDR M. tuberculosis strains.

The results of molecular genetic studies of clinical samples taken from three foci of outbreaks of enterovirus infection, registered on the territory of Khabarovsk Krai and Amur Oblast from 2012 through 2013 were reported. Several strains of enteroviruses were obtained: strains of Coxsackie A-4 and Coxsackie A-16 from De-Kastri rural locality, strains of ECHO-6 from Ust’-Urikma (Amur Oblast) and strains of Coxsackie A-6 from Raichihisnsk (Amur Oblast). Use of molecular genetic methods for investigation of collective morbidity and outbreaks of enterovirus infection help to confirm the identity of enteroviruses, isolated from patients and to identify the imported cases of infection.

Introduction. Molecular-genetic monitoring of human immunodeficiency virus (HIV) drug-resistant strains circulation as well as analysis of territorial peculiarities of different genetic variants prevalence of the virus is a crucial part of epidemiological surveillance over HIV-infection spread. Objective of the research - to analyze the prevalence of HIV-1 genetic variants among HIV-positive people living in different constituent entities of the Far Eastern Federal District. Material and methods. Molecular-genetic analysis of the Pol-gene that is coding protease and a part of reverse transcriptase of HIV-1 was performed for a total number of 206 blood plasma samples. The biological material was collected from HIV-positive people living in Sakhalin, Magadan and Amur regions, Jewish Autonomous District, Khabarovsk region, Republic of Sakha (Yakutia) and Chukotka Autonomous Region (constituent entities of the Russian Federation in the Russian Far East). Results. The research demonstrated that HIV-1 sub-subtype A6 continues to be dominant and comprises 72.1±3.1% of all isolated subtypes in the regions of the Russian Far East. HIV-1 subtype B was identified in 14 cases (6.8±1.8%) and subtype C in 4 cases (1.9±1.0%). Spectrum of HIV-1 recombinant forms was determined for some regions. We identified five types of HIV-1 circulating recombinant forms (CRF): CRF03_AB - 2 (0.9±0.7%), CRF02_AG - 8 (3.9±1.3%), CRF63_02A1 - 27 (13.1±2.4%), CRF11_cpx - 1 (0.5±0.5%), CRF01_AE - 3 (1.5±0.8%). High heterogeneity of the virus was registered in the Jewish Autonomous District. Discussion. The conducted research revealed genetic differences in the landscape of HIV-1 subtypes circulation as well as differences in HIV-1 recombinant forms distribution frequency in the Russian Far East. Current findings are due to expansion of migration flows that promote cross-border importation and further spread of different HIV-1 genetic variants among population of the Far Eastern Federal District. Conclusion. The conducted research revealed that the molecular-genetic monitoring of HIV-1 genovariants should involve not only regions with high HIV-abundance but also with low HIV-prevalence and incidence.

Background. Enterovirus infection is widely spread around the world. It has different epidemiologic and clinical manifestations. However, to date, the amount of accumulated information on the epidemic process of enterovirus infection in different constituent entities of the Siberian Federal district of the Russian Federation is insufficient. Objectives. Our objectives were to investigate characteristics of the enterovirus infection epidemic process in Siberian constituent entities subjected to surveillance using the innovative molecular genetic analysis. Materials and methods. The analysis of long-term enterovirus infection incidence in the investigated constituent entities of Siberia was performed. The total number of 237 enterovirus strains circulating during 2017-2018 epidemic seasons in the analyzed regions were isolated using sequencing technique. A phylogenetic analysis of the obtained nucleotide sequences was performed. Results. The epidemiologic situation related to enterovirus infection was unfavorable in the majority of the Siberian constituent entities in the past two years. A significant increase in incidence rates was registered in the Irkutsk Region, Zabaykalsky Krai and Altai Republic. We distinguished major manifestations of the enterovirus infection epidemic process including a years-long excess of the national average and a rising trend in the annual incidence rate, prevalence of herpangina, exanthema and enterovirus meningitis in the clinical course of the disease among children aged 3–6. Prevalence of Coxsackievirus A-6 and Coxsackievirus A-9 in the landscape of circulation was revealed in a number of Siberian regions. Group incidence and imported cases were also registered. Discussion. Our results demonstrated the importance of molecular genetic monitoring of circulating enteroviruses as part of the epidemiologic surveillance. This state-of-the-art approach can reveal potential epidemiologic links between cases of the disease.

Emergence of various genovariants of the SARS-CoV-2 virus, which are characterized by a higher ability to spread and a more severe clinical manifestations compared to the initial variants, require molecular-genetic monitoring of strains circulating in the Russian Federation.The aim of the work was to identify the VOC SARS-CoV-2 genovariants in the territory of the Republics of Bashkortostan, Tatarstan, Udmurtia, and Samara, Penza, Saratov, Ulyanovsk, and Orenburg Regions.Materials and methods. The identification of genovariants and the determination of the type of mutations was carried out by the Sanger fragment sequencing method.Results and discussion. The study examined 298 samples of clinical material obtained from the Centers for Hygiene and Epidemiology in the Republics of Bashkortostan, Tatarstan, Udmurtia, Samara, Penza, Saratov, Ulyanovsk, and Orenburg Regions. In 17 % of cases, the variability of the SARS-CoV-2 virus was observed for one or more markers: in three samples, a new coronavirus of the B. 1.1.7 line (“British”) was detected; in a number of cases, only one mutation was detected in the virus found in samples – deletion Y144 or substitution D138Y, E484K, N501Y, and very rarely two mutations – deletion Y144 and substitution E484K. The presence of the L141-G142-V143 deletion localized in the recurrent deletion region RDR2 of the S-gene was shown in 10 % of the cases. The data obtained indicate the heterogeneity in macroorganism of the population of the new coronavirus with the deletion L141-G142-V143, which leads to a change in the antigenic structure of the virus, which probably allows the virus to evade the immune response.
