Background: Advanced treatment options for non-small cell lung cancer (NSCLC) consist of immunotherapy, chemotherapy, or a combination of both. Decisions surrounding NSCLC can be considered as preference-sensitive because multiple treatments exist that vary in terms of mode of administration, treatment schedules, and benefit–risk profiles. As part of the IMI PREFER project, we developed a protocol for an online preference survey for NSCLC patients exploring differences in preferences according to patient characteristics (preference heterogeneity). Moreover, this study will evaluate and compare the use of two different preference elicitation methods, the discrete choice experiment (DCE) and the swing weighting (SW) task. Finally, the study explores how demographic (i.e., age, gender, and educational level) and clinical (i.e., cancer stage and line of treatment) information, health literacy, health locus of control, and quality of life may influence or explain patient preferences and the usefulness of a digital interactive tool in providing information on preference elicitation tasks according to patients.Methods: An online survey will be implemented with the aim to recruit 510 NSCLC patients in Belgium and Italy. Participants will be randomized 50:50 to first receive either the DCE or the SW. The survey will also collect information on participants' disease-related status, health locus of control, health literacy, quality of life, and perception of the educational tool.Discussion: This protocol outlines methodological and practical steps to quantitatively elicit and study patient preferences for NSCLC treatment alternatives. Results from this study will increase the understanding of which treatment aspects are most valued by NSCLC patients to inform decision-making in drug development, regulatory approval, and reimbursement. Methodologically, the comparison between the DCE and the SW task will be valuable to gain information on how these preference methods perform against each other in eliciting patient preferences. Overall, this protocol may assist researchers, drug developers, and decision-makers in designing quantitative patient preferences into decision-making along the medical product life cycle.

Background: Non–small cell lung cancer (NSCLC) is the most common histologic type of lung cancer, accounting for 70–85% of all lung cancers. It has brought a heavy burden of disease and financial cost to families, society, and the nation of China. Patients have differing preferences for treatment because of their varying physical conditions and socioeconomic backgrounds, which ultimately affects the choice of treatment as well as treatment outcomes. For better and sustained health outcomes, it is vital to understand patients' preferences. We can then provide medical services to match these preferences and needs rather than basing treatment on our clinical viewpoints alone.Objectives: The aim of this study was to elicit patient preferences for treatment using a discrete-choice experiment and to explore the value/importance that patients place on the different attributes of treatment in order to provide a basis for clinical decision making and patient health management.Methods: The study was conducted with NSCLC patients from three typical hospitals in southwestern China. After identifying patient-relevant treatment attributes via literature review and qualitative semi structured interviews, a discrete-choice experiment (DCE) including seven patient-relevant attributes was conducted using a fractional factorial SAS design. The empiric data analyses of patients were performed using mixed logit models.Results: NSCLC patients (N = 202) completed a survey via a face-to-face interview. Among the seven attributes, the following were considered important: progression-free survival, disease control rate, cost, weakness/fatigue, and nausea/vomiting; mode of administration and rash were considered less important. A clear preference for an increase in progression-free survival and disease control rate was demonstrated. Compared with 5 months of progression-free survival, respondents were willing to pay more (19,860 RMB) for 11 months of progression-free survival (coef.: 0.687). Compared with a 60% rate of disease control, respondents were willing to pay more (19,940 RMB) for a 90% rate of disease control (coef.: 0.690).Conclusions: This study demonstrates the value of DCEs in determining patient preferences for the treatment of NSCLC. The results indicate that not only efficacy factors (such as progression-free survival and disease control rate) were considered but also other factors (such as side effects and treatment costs) and trade-offs between attributes were held to be important. These results are in accord with expectations and can provide evidence for more effective and efficient treatment results. Furthermore, the current results can increase benefits if the presented therapies can be designed, assessed, and chosen based on patient-oriented findings.

Abstract Background and aims To assess experts’ preference for sarcopenia outcomes. Methods A discrete-choice experiment was conducted among 37 experts (medical doctors and researchers) from different countries around the world. In the survey, they were repetitively asked to choose which one of two hypothetical patients suffering from sarcopenia deserves the most a treatment. The two hypothetical patients differed in five pre-selected sarcopenia outcomes: quality of life, mobility, domestic activities, fatigue and falls. A mixed logit panel model was used to estimate the relative importance of each attribute. Results All sarcopenia outcomes were shown to be significant, and thus, important for experts. Overall, the most important sarcopenia outcome was falls (27%) followed by domestic activities and mobility (24%), quality of life (15%) and fatigue (10%). Discussion and conclusion Compared to patient’s preferences, experts considered falls as a more important outcome of sarcopenia, while the outcomes fatigue and difficulties in domestic activities were considered as less important.

Abstract Purpose This study was to assess the status of quality of life and explore the possible factors correlated with quality of life among non-small cell lung cancer (NSCLC) patients with skin adverse drug reactions under targeted therapy. Methods We performed a cross-sectional study including 536 NSCLC patients with skin adverse drug reactions by targeted therapy in cancer outpatient clinics of three hospitals in China between May 2020 and May 2021. And we collected data with structured questionnaires and identified the relationships among coping style, self-management and quality of life by Pearson correlation analysis and multiple linear regression algorithm. Results The total score of quality of life was 46±12.84 in 536 NSCLC patients with skin adverse drug reactions undergoing targeted therapy. In multiple linear regression analysis, we identified the significant factors associated with quality of life including age, education level, combination of medicine, Charlson Comorbidity Index (CCI), stages of disease, facing, yield, symptom management, daily activity management, psychological and emotional management, self-efficacy and self-management (P < 0.05). Conclusions NSCLC patients with skin adverse drug reactions undergoing targeted therapy generally had a compromised quality of life. And the critical factors that affected the status of quality of life were age, education level, co-morbidity, the combinatorial application of drugs and stage of disease, self-management and coping styles.

e16570 Background: The main objective of our study was to assess patient reported symptom burden and impact on quality of life in advanced non small cell lung cancer (NSCLC) patients in the United States. Methods: Patients with advanced (stage IIIB/IV) NSCLC (N=450) were recruited with informed consent in a nationwide (US) lung cancer study from Oct-Dec 2011. Patient reported symptoms were assessed using the Lung Cancer Symptom Scale (LCSS) on a 0-100 visual analogue scale and included six symptoms: fatigue, appetite loss, shortness of breath, cough, pain and blood in sputum. An average symptom burden index was calculated. Quality of life was assessed using the Functional Assessment of Cancer Therapy- Lung (FACT-L).Higher scores indicate higher symptom severity on the LCSS and better quality of life on the FACT-L. Correlation between the total FACT-L score and LCSS symptom burden index was assessed. A multivariate regression analysis was performed with FACT-L total score as the dependent variable and LCSS symptom scores as predictors controlling for age, gender, stage and performance status. Results: Majority of the patients were male (59%), Caucasian (74%), smokers/ex-smokers (78%) with an average age of 64 years. Proportion of patients reporting each lung cancer symptom was: Fatigue (100%), loss of appetite (97%), shortness of breath (95%), cough (93%), pain (92%) and blood in sputum (63%). The average (SD) symptom burden index was 42.3 (21.5).The mean± SD severity scores on symptoms were: fatigue (53.2±24.7), loss of appetite (48.1±25.8), cough (48.4±29.9), shortness of breath (44.7± 27), pain (39.7± 28.1) and blood in sputum (18.4±23.6). The average (SD) FACT-L score was 71.7 (25.3). A significant negative correlation was found between the LCSS symptom burden index and FACT-L scores (ρ= -0.82; p<0.001). Loss of appetite (β=-0.204; p<0.001), cough (β= -0.145; p<0.01), pain (β=-0.265; p<0.001), shortness of breath (β = -0.145; p<0.01), age (β= 0.217; p<0.05) and performance status (β = 0.283; p<0.001) were found to be significant predictors of quality of life. Conclusions: Cough, pain, shortness of breath and loss of appetite contribute to symptom burden and have a significant negative impact on quality of life in advanced NSCLC patients.

e19594 Background: Nutritional interventions have shown increased of energy intake but the effect of improvement in health related quality of life (HRQL) in non small cell lung cancer (NSCLC) patients is controversial. Eicosapentaenoic acid has been proposed to have anti-inflammatory, anticachectic and possibly could have antitumoral effects. Methods: Patients with advanced NSCLC naive to treatment were included and randomly assigned to receive diet and oral nutritional supplement enriched with EPA or only isocaloric diet. All patients received paclitaxel and cisplatin-carboplatin treatment. Weight, body composition measured with bioelectrical impedance analysis, dietary ingestion, inflammatory parameters and HRQL were assessed at baseline and after 1st and 2nd cycle. Response to chemotherapy and survival were evaluated. Results: Ninety-two patients were analyzed, (46 EPA, 46 control group). No basal differences between groups were found. Experimental group present significantly increased albumin levels, energy, proteins, lipids and carbohydrate intake, and reduced serum CPR, TNF-α and Neutrophyls/Lymphocytes index. Patients in experimental group presented minor prevalence of weight loss and increased 1.6 kg of lean body mass (LBM) compared with controls. In HRQL, experimental group presented an improvement in global and physical scale and anorexia, and regarding toxicity, fatigue and neuropathy diminished. There was no difference in response rate, progression free survival and overall survival between groups. Conclusions: EPA enriched supplementation provided a therapeutic advantage by increasing LBM and nutrients intake, improving HRQL, and reduce several toxicity parameters in NSCLC patients under chemotherapy. The study was registered with ClinicalTrials.gov, number NCT01048970.
