Introduction: Epidemiological observations indicate that stewardesses are exposed to reproductive and endocrine system disturbances. The aim of the study was to assess of thyroid function disturbances as well as to identify factors affecting the thyroid function among stewardesses working both within one time zone and on long-distance flights. Material and methods: The cross-sectional study covered 103 women aged 23–46. The study group (I) was divided into two subgroups: group Ia comprising stewardesses flying within one flight zone and group Ib stewardesses working on long-distance flights. The control group (II) were women of reproductive age who sought medical assistance due to marital infertility in whom the male factor was found to be responsible for problems with conception in the course of the diagnostic process. The assessment included: age, BMI, menstrual cycle regularity, length of work, frequency of flying, thyroid stimulating hormone (TSH) concentration, level of free thyroxine (fT4), antibodies to thyroglobulin (aTG) and to thyroperoxidase (aTPO), prolactin concentration, sex hormone binding globulin (SHGB) concentration, total cholesterol, and low density lipoprotein (LDL) fraction. Descriptive methods and inferential statistics methods were used to compile the data. Results: The difference between the concentrations of TSH in the study group (2.59 mcIU/mL) and the control group (1.52 mcIU/mL) was statistically significant (p < 0.01). An elevated titer of thyroid antibodies (aTPO and/or aTG) was revealed in 46.3% of stewardesses and in 15.1% of patients from the control group (p < 0.001). Groups Ia and Ib in individual concentrations were not statistically significant. The frequency of occurrence of an elevated titer of thyroid antibodies depended on the length of work in the study group (p > 0.05). No statistically significant difference was found in patients spending up to 60 h a month flying and in patients spending more than 60 h flying, the percentage of the occurrence of thyroid antibodies was 50% and 43.5, respectively. Conclusions: The occurrence in stewardesses of a higher TSH concentration than in the control group can signify that stewardesses are burdened with a higher risk of the development of hypothyroidism in the future. The character of the work of stewardesses (frequency of flying as well as length of work) does not affect the immunological profile of the thyroid.

The work of flight attendants is associated with exposure to long-term stress, which may cause increased secretion of cortisol. The aim of the study is to determine the circadian rhythm of cortisol and to seek factors of potential influence on the secretion of cortisol in female flight attendants working within one time zone as well as on long-distance flights. The prospective study covers 103 women aged 23–46. The study group (I) was divided into two subgroups: group Ia, comprising female flight attendants flying within one flight zone, and group Ib, comprising female flight attendants working on long-distance flights. The control group (II) are women of reproductive age who sought medical assistance due to marital infertility in whom the male factor was found to be responsible for problems with conception in the course of the diagnostic process. The assessment included: age, BMI, menstrual cycle regularity, the length of service, the frequency of flying, diurnal profile of the secretion of cortisol, testosterone, estradiol, 17-OH progesterone, SHBG, androstenedione, and progesterone concentration. Descriptive methods and inferential statistics methods were used to compile the data. Comparing the profile of flight attendants from groups Ia and Ib shows that the curve flattened among women flying within one time zone. The secretion curve is also more flattened in women with less years worked and in flight attendants working less than 60 h per month. Due to the character of work, the female flights attendants do not have hypersecretion of cortisol. Frequency of flying and length of work affect the dysregulation of HPA axis.

Abstract Background Challenges of diagnosing and treating sepsis only seem more difficult as incidence increases, patients become older and sicker, and pathogenic organisms evolve. New understanding of inflammatory mediators and pathways, immunity, and genetic variability in this disease state suggests that the current definitions of SIRS, sepsis, severe sepsis, and septic shock are oversimplified. Objective The aim of our study is to evaluate the level of RDW, CRP and clinical scores "SOFA and APACHI" as markers in patients with sepsis and their levels on the outcome and resolution of sepsis in ICU. Methodology We conducted a prospective observational controlled study on 90 adult persons of both sex, 45 of them are adult patients and served as the study group (Group I), and the other 45 are healthy adult volunteers and served as the control group (Group II). The study group represented patients admitted to the ICU of Intensive Care Unit at Damanhour Medical National Institute who fulfilled the diagnostic criteria for sepsis, severe sepsis and septic shock on arrival to ICU according to the SCCM/ ESICM/ ACCP/ ATS/SIS International Sepsis Definitions Conference. Results. In the present study we found that CRP measured on admission was not a predictor of mortality, while that measured at day 5 and day 10 predicted mortality, where there was no statistically significant difference in CRP levels between survivors and non-survivors at the day of admission while there were statistically significant differences between survivors and non survivors according to CRP levels at day 5 (p = 0.001*) and at day 10 (p = 0.001*). It was found also that there were statistically significant differences between survivors and non-survivors according to RDW at day 1(p = 0.011*) and at day 5(p = 0.009*), at day 10 was found there was no statistically significant difference between survivors and non-survivors (p = 0.338). Conclusion RDW is a new promising cheap and readily available biomarker that can be able to diagnose patients with sepsis with accuracy comparable to CRP. Also, RDW at admission is able to predict mortality.

Background: the aim was to study the prevalence of abnormal thyroid function and thyroid autoimmunity in infertile women and in general population (control group) and to correlate the thyroid dysfunction with presence of anti-thyroid antibodies. It was a case control study. The study was carried out at Department of obstetrics and gynaecology, Kasturba Hospital, Delhi. Population of the study was fifty infertile women in whom other causes of infertility are excluded and fifty fertile women attending out- patient department with other complaints.Methods: All the women enrolled in the study were non pregnant, clinically euthyroid (with no signs and symptoms of thyroid disorders and no documented abnormal thyroid function test) and no known autoimmune disorder. Detailed history and a thorough general physical examination, including thyroid examination was done. Haemogram, blood group, fasting and post prandial blood sugar, VDRL, transabdominal ultrasound, TSH, free T3, free T4, and antithyroid per-oxidase antibody assay was done using the ELISA technique.Results: The mean age of the study group was found 28.38±2.45 years, 22% had abnormal thyroid function, 20% had antithyroid antibodies positive and the mean TSH of the study group was 4.61±1.72µIU/ml. The mean age of the control group was found to be 29.10±2.01 years, 12% had abnormal thyroid function, 10% had antithyroid antibodies positive and the mean TSH of the control group was 3.89±1.56µIU/ml.Conclusions: Thyroid dysfunction and anti-thyroid antibodies were more prevalent in patients with infertility. A statistically significant correlation was established between thyroid autoimmunity and thyroid dysfunction in infertile women in the study.

Abstract Background Migraine is a neurological disease associated with abnormal excitability in visual, somatosensory and motor cortex. Previous researches reported abnormalities in the auditory brainstem response (ABR) and in vestibular functions. These results are an indicator of impending auditory malfunction in Migraine and disruption of central auditory processing mechanisms. Objectives This study aimed to assess and compare central auditory processing (CAP) performance in migraine patients with and without dizziness and healthy controls. Subjects and method Sixty subjects aged between18 and 60 years divided into 3 groups; control group (twenty normal healthy subjects), study group I (twenty subject diagnosed with migraine) and study group II (twenty subjects diagnosed with vestibular migraine). They were subjected to a headache sheet, central auditory processing questionnaire and Psychophysical central auditory tests: including Arabic Speech Intelligibility In Noise (SPIN) test for adults, Arabic Dichotic digit test, Gap In Noise (GIN) test, Duration pattern test and Arabic memory tests. Results Statistically significant difference was found between the 2 study groups and control group regarding right dichotic (p = 0.004),right and left duration pattern tests (p = 0.01) with highly statistically significant difference(p &lt; 0.001) Gap In Noise (GIN) test, left dichotic digit test, right and left speech in noise. But there was no statistically significant difference between the two study groups regarding all tests. Conclusions Patients with migraine and vestibular migraine had poor performance in all central auditory tests when compared with control this mean that migraine lead to impaired central auditory processing in adults. Also, there was no significant difference between the 2 study groups regarding central auditory test results which may support that both migraine with and without dizziness have the same pathophysiology.

Millions of people around the world are suffering from a destructive disease known as diabetes mellitus. In the present study hemoglobin concentration was estimated in female type 2 diabetes to observe the frequency of lowering hemoglobin level in these patients and to assess the incidence of anemia. This Descriptive type of cross sectional study was carried out in the Department of Physiology, Mymensingh Medical College, Mymensingh between the periods from July 2016 to June 2017. A total of 210 subjects were selected and were grouped as control group, Group I (70 healthy female of reproductive age, 21-45 years) and study group , Group II which was again subdivided to Group IIA (70 diabetic female of reproductive age , 21-45 years) and Group IIB (70 diabetic female of post-menopausal age, 45-70 years). Laboratory analysis of fasting serum glucose hemoglobin concentration was evaluated by the Cyanmethemoglobin (CMG) method. A comparison in hemoglobin concentration was done in between Group I and Group IIA and also in between Group IIA and Group IIB. The results were calculated and analyzed by using SPSS. For statistical analysis students unpaired 't' test was performed. The result showed that hemoglobin level was significantly decreased in the study group in comparison to the control group. It was concluded that type 2 diabetes is associated with decreased hemoglobin concentration leading to anemia. CBMJ 2020 January: Vol. 09 No. 01 P: 25-29

The objective: to study the efficacy of complex treatment of inflammatory diseases of the pelvic organs (vaginitis), occurring in conjunction with urinary tract infection (cystitis), with a combined medication of plant origin Tutukon produced by Miguel and Garriga, S. A. ("Grand medical Group AG", Spain/Switzerland) in women of reproductive age. Patients and methods. Were examined 60 women of reproductive age with the presence of vaginitis and associated cystitis. All the patients were divided into two clinical groups. In group I (control; n=30) treatment was carried out according to the standard scheme with the use of antimicrobial drugs, antispasmodics, local antiseptics for the sanitation of the vagina. In group II (study group; n=30) treatment was carried out according to the standard scheme with the inclusion of the drug Tutukon. All the patients were examined at 3-rd, 7-th, 10-th day of treatment and 30 days after discontinuation of therapy. Results. During the research was given the analysis of the dynamics of clinical and laboratory parameters in women of both groups before and after treatment. In patients of study group, the standard therapy which included Tutukon, the treatment was more effective than in patients of control group: a rapid onset of clinical effect, reducing the duration of the disease, a complete clinical response in 90% of cases versus 63%, alkalization of urine - pH 6.5±0.10, the absence of recurrence within 30 days after the end of therapy. Conclusion. Tutukon medicine is recommended for complex treatment of vaginitis, occurring in combination with cystitis in women of reproductive age. The article presents the comparative analysis of the state of the cervix in women with endometrial polyps and micropolyps. Patients and methods. The study involved 130 patients aged 18-35 years: 70 patients with endometrial polyps (group I), 30 patients with micropolyps (group II) and 30 patients of the control group (group III). Results. According to the anamnesis of women in the I group were significantly more frequent diseases of the cervix, which corrected physical surgery methods, in particular cryodestruction. In group II, the representatives of these indicators were similar to healthy. Normal colposcopic picture met significantly less frequently in patients and I, and II group. The differences in the incidence of HPV high oncogenic risk in all groups were not statistically significant. Conclusion. Destructive methods used in the detection of any changes in the cervix are often overly aggressive, form scars and contributing to inflamaciones process. In the chain of events leading to the formation of PE, cervical pathology and its correction can take the basic place. The aim of this in-vivo study was to evaluate/compare the clinical periodontal parameters in patients with true combined endo-perio lesions (EPL), treated with gutta-percha (GP) and mineral trioxide (MTA) as an obturation material alone and with addition of bone grafting in such lesions. 120 Saudi patients (mean age = 41yrs) diagnosed with true combined EPL participated in this study. Group I (control group, n = 30) was treated with conventional endodontic treatment using GP for obturation. Group II (n = 30) was treated with conventional endodontic treatment using MTA for obturation. Group III (n = 30) was treated with conventional endodontic treatment using GP for obturation + grafting procedure to fill the bony defect. Group IV (n = 30) was treated with conventional endodontic treatment using MTA for obturation + grafting procedure to fill the bony defect. Clinical parameters (Pocket depth (PD); Clinical attachment loss (CAL); keratinized tissue width (KTW); gingival phenotype (G.Ph.) and Cone Beam Computed Tomography Periapical Index (CBCTPAI)) were recorded and compared at baseline, 3, 6, 12 months’ interval. For the groups III and IV, CBCTPAI showed significant difference (p < 0.0001) with the other groups at 6 months and 1-year interval. The group with MTA + bone graft showed 76% and 90% patients with 0 score at 6 months and 1-year follow-up, respectively. Comparison of mean values of PD among study groups at 3 months, 6 months and 1 year showed significant difference at 3 months, whereas the mean PD values of subjects in GP + bone graft showed significantly higher PD values than other 3 groups (p = 0.025). Use of GP and MTA for root canal obturation along with periodontal therapy and bone augmentation helps in resolving complex endo-perio lesions. Bone grafting in addition to obturation with MTA was found to be the best treatment strategy in management of EPL cases and is recommended for clinicians who are treating EPL patients.

Abstract Introduction Good hearing in pilots, including central auditory skills, is critical for flight safety and the prevention of aircraft accidents. Pure tone audiometry alone may not be enough to assess hearing in the members of this population who, in addition to high noise levels, routinely face speech recognition tasks in non-ideal conditions. Objective To characterize the frequency-following response (FFR) of a group of military pilots compared with a control group. Methods Twenty military pilots in the Study Group and 20 non-pilot military personnel, not exposed to noise in their work, in the Control Group, all with normal hearing, aged between 30 and 40 years old, completed a questionnaire to assess their hearing habits, and their FFRs were measured with a /da/ syllable (duration 40 milliseconds, speed 10.9/s), at 80 dB NA in the right ear. All procedures were approved by the ethical committee of the institution. Statistical analysis was performed using the t-Student or Mann-Whitney tests for quantitative variables, and the Fisher or chi-squared tests for qualitative variables, and a value of p < 0.05 was considered to be statistically significant. Results There was no significant difference between the groups regarding auditory habits. In the FFR, wave amplitudes A (p = 0.01) and C (p = 0.04) were significantly lower in the Study Group. Conclusion Working as a military pilot can be a crucial factor in determining an individual's typical FFR pattern, demonstrated in the present study by statistically significant reductions in the amplitudes of the A and C waves.

AbstractFracture resistance of endodontically treated teeth restored with post. Aims: This study aims to compare the fracture resistance of endodontically treated teeth restored with resin fiber and stainless steel post. Commercially available prefabricated resin fiber post(Dentsply Maillefer Easy Post), prefabricated stainless steel post(Coltene/Whaledent Parapost) were used. Methods and Material: Forty five maxillary central incisors were obturated and divided into 3 groups: Control Group (Group I) without any post (n = 15), Resin Fiber Post Group (Group II) (n = 15) and Stainless Steel Post Group (Group III) (n = 15). In all Groups except control group, post space was prepared; a post was cemented, and a core build-up was provided. All the specimens were subjected to compressive force under a universal testing machine until fracture. Statistical analysis used: The results were analyzed using the variable analysis test (ANOVA). Results: One-way analysis of variance revealed significant difference among test groups. The control group demonstrated highest fracture resistance (925.2183 N), followed by the resin fiber post group (486.7265 N) and stainless steel post group (423.539N). Conclusions: Teeth restored with resin fiber post showed higher fracture resistance values than prefabricated stainless steel post.
