This research proposes a strategy for applying support objects—equipment, tools, and even furniture/environment from which humans can receive reaction force through their hands—for fall prevention in the elderly. This paper presents an assessment example of support objects based on balance recovery characteristics and a discussion regarding their application according to the assessment results. The balance recovery characteristics depend on the direction in which reaction force can be obtained based on the shape of the support object and direction in which the force is easily exerted on the hand. Evaluation indices for assessing the height and shape of nonportable support objects and determining a position of the tip on the ground of a cane, a typical portable support object, in the anterior direction are briefly introduced based on the authors’ previous works. The strategy for the application of support objects utilizing the evaluation indices is proposed; better use of support objects, their locations, new-shaped ones, and support devices with a new design concept are discussed and introduced based on the values of the calculated indices according to the type/usage of the support objects.

Assessing the number of users defined by a set of specific usage attributes in a given usage contextual situation is not always an obvious task in a market segmentation process. Although new approaches in design and marketing seem to be more sensitive to the adequacy of a design concept with the usage scenarios, these methods do not systematically consider the various usage situations. The present article puts forward a methodology intending to build a usage scenarios space in which the input data is thoroughly collected and validated. This methodology is applied to the complex and multifactorial issue of falls among the elderly in the Metropolitan France. In this paper, numerous medical publications have been made to study influential factors of fall situations. However, even solution providers for fall prevention and teleassistance ignore the real situational coverage of their solutions. As a result, “usage scenarios space” is built using an appropriate segmentation of usage contexts (here, fall situations) and user characteristics. These data are used for a design oracle to predict (simulate) the various and multiple usage scenarios.

The aim of this retrospective study was to determine the prevalence of orthostatic hypotension (OH) among a convenience sample of older adults on two Acute Care of the Elderly (ACE) units of the ChristianaCare™ in Delaware. Another aim was to determine if subjects with documented OH experienced falls. Retrospective de-identified data was obtained from electronic medical records for the years 2015 to 2018. Among all patients who had valid first orthostatic vital sign (OVS) readings ( n = 7,745), 39.2% had orthostatic hypotension on the first reading. Among the patients, 42.8% were found to be hypotensive during OVS. Thirty-one (0.9%) of those with OH fell at some point during their stay. The odds ratio for falls in the presence of OH was 1.34 with a 95% confidence interval (0.82, 2.21), but a chi-square test failed to find significance ( p = .2494). The results could not determine if OVS should be mandatory in fall prevention protocols.

ABSTRACT One of hypothetical mechanisms related to cognition is exercise-induced IGF-1. Objective: The aim of this study was to analyze the effects of exercise on IGF-1 levels and cognition in the elderly. Methods: The article searches were conducted on Pubmed, Web of Science, PsycINFO and Scielo databases and reviewed according to PRISMA guidelines. The inclusion criteria were: [1] original articles published up to 2017; [2] samples including elderly; [3] protocols including physical exercise; [4] longitudinal studies having exercise as main outcome; [5] assessment of IGF-1; [6] cognition assessment. Results: Seven studies were included in this review. Three of the studies showed an exercise-induced increase in IGF-1; three found stable IGF-1 levels and one found a reduction in IGF-1; with and without improvement in cognition. Conclusion: Disparities in the type of physical exercise, protocols and samples under different conditions hinder the establishment of a consensus on IGF-1, cognition and physical exercise.

Abstract Background: The importance of fall prevention rehabilitations has been well recognized. Recently, telerehabilitation was developed, however, there have been no reports on the use of telerehabilitation with direct support from specialists for fall prevention among the elderly. We herein reported telerehabilitation by carers educated by our novel educational program.Methods: Nine elderly people in two nursing homes were enrolled using our original criteria. Carers are educated with our educational program using telelecture system. Telerehabilitation was performed by carers following the instruction from rehabilitation specialists in Asahikawa Medical University using the telemedicine system every 2-4 weeks for three months. Carers were assessed with our original questionnaire before and after the telelecture. Berg Balance Scale (BBS), Timed Up & Go test (TUG test), Hand-held dynamometer (HHD) and Mini-Mental State Examination (MMSE) were assessed before and after telerehabilitation.Results: The average number of times to perform telerehabilitation in all institutes was 4.7. Levels of understanding of carers were significantly increased after the telelecture. No adverse event occurred during the study period. Median BBSs before and three months after telerehabilitation were 43 and 49, respectively. Those of TUG test, right and left HHD and MMSE were 17.89 and 18.53, 7.95 and 11.55, 9.85 and 13.20, and 16 and 19, respectively. All results were improved after telerehabilitation.Conclusions: Our telerehabilitation program exhibited significant effects in elderly people as well as levels of understanding rehabilitation of caregivers in the facilities for the elderly people safely.Trial registration: University Hospital Medical Information Network Clinical Trials Registry(UMIN-CTR)UMIN000041439, August 17th, 2020

To assess the fall of risk among the elderly in selected urban area of Thrissur. Objectives a) To assess the fall risk among the elderly people in selected urban area of Kachery, Thrissur. b) To associate fall risk score with selected demographic variables. c) To correlate fall risk assessment questionnaire score and modified fall prevention checklist for personal risk factors score among elderly individuals. d) To teach the elderly people about fall prevention exercise. Methodology: Non-experimental descriptive research design is adopted in this study. We selected 60 samples through random sampling technique. Standardized fall risk assessment questionnaire built by national aging research institute and modified fall prevention checklist for personal risk factors built by Hamilton county was used to collect the data. Tool was administered by interview method for assessing fall risk. Fall preventive intervention module developed, validated and administered following data collection. The data collected were analyzed by using descriptive and inferential statistics. Result: The demographic profile of elderly people shows that 46.66% belongs to age group between 60-70 years and most of them 58.33% were females. The majority 73.33% of elderly person are at low risk for fall and 26.66% are at high risk for fall in questionnaire and 81.67% of elderly are low risk for fall, 18.33% are risk and there is no elderly person high risk for fall in checklist. The overall fall risk is high among elderly individual alone in home and low in elderly in nuclear families. The risk for fall among elderly based on previous history of fall shows that elderly with visual impairment those who don’t have previous knowledge about fall prevention and elderly age between 91-100 is high risk for fall. The risk for fall based on ability to perform ADL in elderly shows those who living alone in the home high risk for fall. There is significant association between score with selected variables like age, previous knowledge about fall prevention, elderly residing in their home alone. We found that there is perfect positive correlation r=1 between fall risk assessment, questionnaires and checklist score. Discussion: At the end of the study the investigator found that the risk for fall based on the assessment of fall history among elderly people shows that, there is significant association between age (p=0.0273) that is, age group between 91-100 years are high risk for fall. Previous knowledge about fall prevention shows that, elderly without previous knowledge about fall prevention is at high risk for fall (p= 0.03074). In sensory impairment that is, elderly having visual impairment (p=0.998) having risk for fall. The risk for fall among elderly people based on their ability to perform activities of daily living shows that, elderly residing in their home alone shows more risk for fall.

This paper investigated the status of the elderly population as well as the elderly market potential in china, and analyzed the importance of universal design on elderly electric scooters. We designed an elderly electric scooter under the guidance of universal design concept and ergonomics. Its wheelchair styling is a breakthrough which distinguishes it from the traditional electric scooter. The new scooter is safer, more fashionable and space-saving. We expect that the universal electric scooter can benefit the elderly and make the whole society concentrate on the old group.
