Background: Resin-based composites (RBCs) provide excellent esthetics but the marginal micro-leakage in the proximal cavities remains a major concern. The aim of the present study was to assess the ability of various dental RBCs and techniques utilized for sealing deep dentin margin in class-II cavities. Methods: Box-cavities (class-II) on the distal and mesial surfaces of extracted (premolar) teeth were prepared with a gingival margin placed 1mm apical to the cemento-enamel junction. Teeth with prepared class II cavities were randomly divided into four study groups according to the type of restorative materials (conventional RBC; bulk-fill RBC; conventional RBC lined with flowable RBC and conventional RBC lined with resin-modified glass-ionomer-cement (GIC) as open sandwich-technique). Each group was further subdivided into a total-etch subgroup in which a separate etching step was performed before applying the bonding agent and a self-etch subgroup in which a self-etch adhesive system was used (n = 10). For each group, cavities were restored using the respective restorative materials and techniques, subjected to 1000 thermocycles, and placed in the methylene-blue dye. The specimen teeth were sectioned for further microscopic examination for micro-leakage. Results: The least dye penetration values were reported for group 4 (GIC) followed by the group Bulk-fill using the self-etch adhesive system (group 2b). The highest dye penetration was reported for the group Bulk-fill using the total-etch adhesive system (2a), followed by the group conventional RBC using the total-etch adhesive system). The total-etch adhesive system had significantly greater micro-leakage compared to the self-etch adhesive system (1a) (p = 0.026). Conclusions: The self-etch adhesive system significantly reduced the micro-leakage compared to the total-etch system. Bulk-fill RBC when bonded with the self-etch adhesive provided good marginal sealing ability comparable to open sandwich-technique using GIC.

SUMMARY Objective: To compare the 24-month clinical performance of two different resin composites in class II slot restorations. Methods and Materials: Thirty-seven patients having at least two approximal carious lesions were enrolled in the study. A total of 116 teeth (58 pairs) were restored with either a silorane-based composite (Filtek Silorane) and its self-etch adhesive (Silorane Adhesive System, 3M ESPE) or a methacrylate-based packable resin composite (X-tra Fil) and its self-etch adhesive (Futurabond NR, VOCO GmbH) according to the toss of a coin. The restorations were evaluated at baseline and at six-, 12-, and 24-month recalls by two calibrated examiners according to the modified US Public Health Service criteria. The comparison of the two restorative materials for each category was performed with the Pearson chi-square test. Within group differences of the materials at different recall times were compared using the Cochran Q and Friedman tests. Bonferroni-adjusted McNemar test was used when significant difference was found (p<0.05). Results: After 24 months, no statistically significant differences were found between the two restorative materials for the criteria evaluated. Conclusions: Both silorane- and methacrylate-based resin composites showed clinically acceptable performance in class II slot restorations after 24 months.

Background: Microleakage is defined as the passage of bacteria, fluid, molecules or ions between the cavity walls and restorative material. There are limited studies in the literature that have compared the microleakage of the newer restorative materials. Therefore, the aim of this study is to compare and evaluate microleakage in Class II cavity in primary molars restored with glass ionomer cement, zirconomer and cention N using stereomicroscope. Method: Standardized Class II cavities were prepared on the extracted primary molars All the prepared samples were divided into 3 experimental groups and were restored as follows: Group I- GIC (GC Universal Restorative); Group II- Zirconomer (SHOFU Inc., Kyoto, Japan) and Group III- Cention-N (Ivoclar Vivadent). The restored teeth were thermocycled, immersed in methylene blue dye and sectioned along the mesiodistal direction. The dye penetration at the occlusal surface and cervical surface was evaluated and compared using a stereo-microscope. Data was analysed using Krushal-Wallis test (Non-parametric ANOVA). Results: Among the three restorative materials, Cention N as compared to GIC and Zirconomer showed least microleakage at both the occlusal surface and cervical surface. Conclusion: Cention N a newer restorative material displayed lesser microleakage as compared to GIC and Zirconomer.

Background: Class II restoration with zirconia inlay is concerned by numerous studies about the luting coupling between zirconia inlay and teeth. The present study was performed to evaluate the microleakage of Class II zirconia inlayusing two different luting agents and compare to direct restoration using bulk fill composite. Aims: To evaluate the microleakage of Class II restorations using three different techniques. Materials and methods: The study was performed in laboratory with three groups. Each of thirty extracted human teeth was prepared a class II cavity with the same dimensions, then these teeth were randomly divided into 3 groups restored by 3 different approaches. Group 1: zirconia inlay cemented with self-etch resin cement (Multilink N); Group 2: zirconia inlay cemented with resin-modified glass ionomer cement (Fuji Plus); Group 3: direct composite restoration using bulk fill composite(Tetric N-Ceram Bulk Fill). All restorations were subjected to thermal cycling (100 cycles 50C – 55 0C), then immersed to 2% methylene blue solution for 24 hours. The microleakage determined by the extent of dye penetration along the gingival wall was assessed using two methods: quantitative and semi-quantitative method. Results: Among three types of restorations, group 1 demonstrated the significantly lower rate of leakage compared to the others, while group 2 and 3 showed no significant difference. Conclusion: Zirconia inlay restoration cemented with self-etch resin cement has least microleakage degree when compare to class II zirconia inlay restoration cemented with resin-modified glass ionomer cement and direct composite restoration using bulk fill composite. This study evaluated, in vitro, marginal microleakage in class II restorations confected with the glass ionomer cement Vitremer and with the composite resins Ariston pHc and P-60. The aims of the study were to assess the effect of thermocycling on those materials and to evaluate two methods utilized in the analysis of dye penetration. Sixty premolars divided in three groups were utilized; the teeth had proximal cavities whose cervical walls were located 1 mm below the cementoenamel junction. Half of the test specimens from each group underwent thermocycling; the other half remained in deionized water, at 37ºC. The specimens were immersed, for 24 hours, in a basic 0.5% fuchsin solution at 37ºC. For the analysis of microleakage, the specimens were sectioned in a mesio-distal direction, and the observation was carried out with the software Imagetools. The results were evaluated through the 2-way ANOVA and through the Tukey’s test. All groups presented marginal microleakage. The smallest values were obtained with Vitremer, followed by those obtained with the composite resins P-60 and Ariston pHc. There was no statistically significant difference caused by thermocycling, and the method of maximum infiltration was the best for detecting the extension of microleakage.

SUMMARY Bulk-fill composites have been introduced to facilitate the placement of deep direct resin composite restorations. This study aimed at analyzing the cervical marginal integrity of bulk-fill vs incremental and open-sandwich class II resin composite restorations after thermomechanical cycling using replica scanning electron microscopy (SEM) and ranking according to the World Dental Federation (FDI) criteria. Box-only class II cavities were prepared in 91 maxillary premolars with the gingival margin placed 1 mm above and below the cemento-enamel junction. Eighty-four premolars were divided into self-etch and total-etch groups, then subdivided into six restorative subgroups (n=7): 1-Tetric Ceram HB (TC) was used incrementally and in the open-sandwich technique with 2-Tetric EvoFlow (EF) and 3-Smart Dentin Replacement (SD). Bulk-fill restoratives were 4-SonicFill (SF), 5-Tetric N-Ceram Bulk Fill (TN), and 6-Tetric EvoCeram Bulk Fill (TE). In subgroups 1-5, Tetric N-Bond self-etch and Tetric N-Bond total-etch adhesives were used, whereas in subgroup 6, AdheSE self-etch and ExciTE F total etch were used. One more group (n=7) was restored with Filtek P90 Low Shrink Posterior Restorative (P9) only in combination with its self-etch P90 System Adhesive. Materials were manipulated and light cured (20 seconds, 1600 mW/cm2), and restorations were artificially aged by thermo-occlusal load cycling. Polyvinyl-siloxane impressions were taken and poured with epoxy resin. Resin replicas were examined by SEM (200×) for marginal sealing, and percentages of perfect margins were analyzed. Moreover, samples were examined using loupes (3.5×) and explorers and categorized according to the FDI criteria. Results were statistically analyzed (SEM by Kruskal-Wallis test and FDI by chi-square test) without significant differences in either the replica SEM groups (p=0.848) or the FDI criteria groups (p>0.05). The best SEM results at the enamel margin were in TC+EF/total-etch and SF/total-etch and at the cementum margins were in SF/total-etch and TE/self-etch, while the worst were in TC/self-etch at both margins. According to FDI criteria, the best was TE/total-etch at the enamel margin, and the poorest was P9/self-etch at the cementum margin. Groups did not differ significantly, and there was a strong correlation in results between replica SEM and FDI ranking.

Background : White Spot Lesion (WSL) is caused by Streptococcus Mutan which causes demineralization of teeth. One of the treatments of WSL is topical application of fluoride which aims to remineralize the teeth. However, if topical fluoride treatment does not give satisfactory results, alternative treatments can be made using restorative treatments, one of which is veneers. The procedure for applying veneers requires the application of an adhesive system. The adhesive system used to apply veneers is self-etch and total-etch. Several references state that the application of fluoride which aims to remineralize teeth can influence the adhesiv strength of the restorative material using self etch and total-etch techniques. Purpose : To determine the effect of composite resin adhesion strength between total-etch and self etch techniques on enamel after fluoride application. Review : Literature sources used in the preparation of the article through several databases with descriptions of the effects of fluoride application before total-etch and/or self etch administration. From the existing references, it was found that the application of fluoride before the total-etch system had a less significant effect on the adhesiv strength. Whereas in the self etch system, fluoride application has a significant effect on the adhesiv strength. Conclusion : More references say that the application of fluoride before the total-etch system has a better adhesive strength than the application of fluoride before the self etch system

Objective: To determine the three-year survival rate of Class II resin-modified glass-ionomer cement (RMGIC), Vitremer, restorations in primary molars and to compare these results with measurements of survival of Class II restorations of standard restorative materials. Study Design: Data on Class II restorations placed in primary molars during a six-year period were collected through a chart review and radiographic evaluation in the office of a board-certified pediatric dentist. A radiograph showing that the restoration was intact was required at least 3 years after placement to qualify as successful. If no radiograph existed, the restoration was excluded. If the restoration was not found to be intact radiographically or was charted as having been replaced before three years it was recorded as a failure. The results of this study were then compared to other standard restorative materials using normalized annual failure rates. Results: Of the 1,231 Class II resinmodified glass-ionomer cement restorations placed over six years 427 met the inclusion criteria. There was a 97.42% survival rate for a 3-year period equivalent to an annual failure rate of 0.86%. Conclusions: A novel approach comparing materials showed that in this study Vitremer compared very favorably to previously published success rates of other standard restorative materials (amalgam, composite, stainless steel crown, compomer) and other RMGIC studies.

This study was designed to evaluate microleakage that appeared on Resin-Modified Glass-Ionomer Cement (RMGIC) restorations. Sixty class V cavities () were cut on thirty extracted third molars, which were randomly allocated to three experimental groups. All the buccal cavities were pretreated with polyacrylic acid, whereas the lingual cavities were treated with three one-step Self-Etch adhesives, respectively, Xeno III (Dentsply Detrey GmbH, Konstanz, Germany), iBond exp (Heraeus Kulzer gmbH & Co. KG, Hanau, Germany), and Adper Prompt-L-Pop (3M ESPE AG, Dental products Seefeld, Germany). All cavities were completely filled with RMGIC, teeth were thermocycled for 800 cycles, and leakage was evaluated. Results were expressed as means standard deviations (SDs). Microleakage scores were analysed by means of generalized linear mixed models (GLMMs) assuming an ordinal logistic link function. All results were considered to be significant at the 5% critical level (). The results showed that bonding RMGIC to dentin with a Self-Etch adhesive rather than using polyacrylic acid did not influence microleakage scores (), except for one tested Self-Etch adhesive, namely, Xeno III (). Nevertheless, our results did not show any significant difference between the three tested Self-Etch adhesive systems. In conclusion, the pretreatment of dentin with Self-Etch adhesive system, before RMGIC filling, seems to be an alternative to the conventional Dentin Conditioner for the clinicians as suggested by our results (thermocycling) and others (microtensile tests).

Clinical Relevance The two-step etch&rinse and the two-step self-etch adhesive systems tested in this study demonstrated similar clinical performance in Class II cavities after two years.
